63letý pacient, hypertonik, celoživotní kuřák, byl na kliniku TRN FN Plzeň přijat pro tumor pravé plíce zjištěný pro bolesti v pravém hemitoraxu. Na základě stážování byla stanovena diagnóza adenokarcinomu pravé plíce s generalizací do obou plic a levé nadledviny, st. IV. Byla zahájena chemoterapie Paclitaxel (200 mg/m2) – Carboplatina (AUC) – Avastin (15 mg/kg) v rámci studie SAIL firmy Roche s velmi dobrou tolerancí. Po 3. cyklu chemoterapie byla potvrzena parciální remise dle CT hrudníku, dále pokračováno ve stejném schématu chemoterapie s Avastinem do 6. cyklu. Kontrolní CT hrudníku prokázalo další regresi tumoru. Vzhledem k velmi dobrému efektu podávané léčby bylo dále pokračováno v monoterapii Avastinem v třítýdenních intervalech zatím do 22 cyklů. Po 22 měsících léčby s Avastinem bylo dosaženo trvající parciální remise nálezu dle PET/CT trupu, pacient je klinicky zcela bez potíží, neomezen v běžném životě, tedy s velmi dobrou kvalitou života (KS 90 bodů), bez vedlejších nežádoucích účinků Avastinu. Chemoterapie již nemá potenciál pro další prodlužování přežití, je tedy potřebné hledání nových léčebných strategií. Avastin jako inhibitor angiogeneze vede poprvé k prodloužení celkového přežití nad 12 měsíců. Významné prodloužení přežití bez progrese onemocnění (PFS) umožňuje delší přežití bez zhoršení symptomů., A 63-year-old hypertensive patient, a life-long smoker, presented to the Department of Tuberculosis and Respiratory Disease of the Plzeň Teaching Hospital with a right-lung tumour detected due to pain in the right hemithorax. Based on staging, a diagnosis of stage IV right lung adenocarcinoma with generalization into both lungs and the left adrenal gland was made. Chemotherapy with Paclitaxel (200 mg/m2) – Carboplatin (AUC) – Avastin (15 mg/kg) was initiated within the SAIL study of the Roche company with very good tolerance. Following the third chemotherapy cycle, partial remission was confirmed based on CT of the chest; the same chemotherapy regimen with Avastin was continued for six cycles. Follow-up CT of the chest revealed further tumour regression. Given the very good effect of the treatment administered, monotherapy with Avastin was continued in three-week intervals for 22 cycles so far. Following 22 months of Avastin treatment, sustained partial remission was achieved according to PET/CT of the trunk; the patient is clinically free of complaints, without limitations in everyday life, thus with a very good quality of life (KS of 90) and free of adverse effects of Avastin. Chemotherapy has no more potential for further extension of survival; therefore, new therapeutic strategies have to be looked for. As an angiogenesis inhibitor, Avastin leads to an extension of overall survival beyond 12 months. Kolorektální karcinom patří ve vyspělých zemích mezi nejčastější nádorová onemocnění a Česká republika je celosvětově na špici v jeho výskytu. Roční incidence u nás přesahuje 7 000 osob ročně. Primárně je 20 % pacientů diagnostikováno ve stadiu IV a téměř u poloviny pacientů se v průběhu onemocnění vyskytnou metastázy. V posledních 15 letech onkologická léčba metastického onemocnění prodělala významný vývoj. Standardem se stala terapie fluorouracilem, jehož modulace leucovorinem vedla k prodloužení mediánu přežití u infúzních režimů až k 15 měsícům (symptomatická terapie 6–8 měsíců). Kombinace fluorouracilu s oxaliplatinou nebo irinotecanem vede k dalšímu prodloužení k 20 měsícům. Následné přidání biologické léčby (bevacizumab, cetuximab, panitumumab) posouvá medián nad 20 měsíců. Cetuximab je monoklonální chimérická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován pro léčbu pacientů s generalizovaným kolorektálním karcinomem s EGFR pozitivitou a K-ras wild type v monoterapii nebo v kombinaci s chemoterapií po selhání terapie irinotecanem a oxaliplatinou. Ve 2 registračních studiích (EMR 62 202–007-BOND 1, IMCL CP02–9 923) vedl ke klinickému benefitu u více jak poloviny nemocných. Ve studii publikované Cunninghamem a spolupracovníky byli pacienti zařazeni do ramene s cetuximabem a irinotecanem anebo do ramene s cetuximabem samotným (PR byla dokumentována u 23 % pacientů, SD u 33 % léčených pacientů). Medián přežití byl prodloužen z 6,9 měsíců na 8,6 měsíců. Evropská léková agentura (EMEA) doporučila cetuximab k léčbě metastického kolorektálního karcinomu (s expresí EGFR, K-ras wild type) v kombinaci s chemoterapií do I. linie léčby na základě studie CRYSTAL. Letos v dubnu byly publikovány v JCO výsledky této studie, která potvrdila efekt cetuximabu v I. linii léčby K-ras wild type karcinomů kolorekta s efektem prodloužení přežití (medián OS = 23,5 versus 20,0 měsíců; hazard ratio (HR) = 0,796; P = 0,0093), PFS (medián PFS = 9,9 versus 8,4 měsíců; HR = 0,696; P = 0,0012) a četnost léčebných odpovědí (RR) (RR = 57,3 % versus 39,7 %; odds ratio = 2,069; P < 0,001) ve srovnání se samotným FOLFIRI., Colorectal cancer in third-line treatment Colorectal cancer is among the most common malignancies in the developed countries and the Czech Republic is the leading country in terms of its incidence worldwide. The annual incidence in the Czech Republic exceeds 7 000 persons. Primarily, 20 % of patients are diagnosed with stage IV and nearly half of the patients develop metastases during the course of the disease. In the past 15 years, oncological treatment for metastatic disease has made significant progress. Treatment with fluorouracil has become a standard; its modulation with leucovorin has resulted in a prolonged median survival in infusion regimens of up to 15 months (6–8 months in symptomatic treatment). A combination of fluorouracil with oxaliplatin or irinotecan leads to a further extension up to 20 months. A subsequent addition of biological therapy (bevacizumab, cetuximab, panitumumab) pushes the median beyond 20 months. Cetuximab is a chimeric IgG1 monoclonal antibody which targets the extracellular domain of the EGFR. In the Czech Republic, it is registered for the treatment of patients with generalized colorectal cancer with EGFR positivity and K-ras wild type after a failed treatment with irinotecan. In two registration studies, it resulted in a clinical benefit in more than half of the patients (PR was documented in 23 % patients; SD in 33 % of the patients treated). The median survival increased from 6.9 months to 8.6 months. Pro indikaci spinocelulárních karcinomů hlavy a krku je v rámci cílené léčby zaregistrován pouze jediný přípravek a sice cetuximab, jehož nižší toxicita je nespornou výhodou oproti chemoterapii. Díky vysokému počtu dosažených kompletních remisí u lokoregionálně pokročilých onemocnění v našem souboru máme zkušenosti s toutu léčbou velmi dobré. Současně s klinickým sledováním se rozvíjí i výzkum prediktivních markerů., Cetuximab whose lower toxicity is a clear benefit compared to chemotherapy is the only agent registered for the targeted therapy of squamous cell head and neck cancers. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%). Autoři prezentují sestavu 76 nemocných radikálně operovaných pro karcinom hlavy pankreatu a analyzují pooperační komplikace a přežívání. Materiál a metodika: V období 1/2006–9/2010 podstoupilo 76 nemocných potenciálně kurativní hemipankreatoduodenektomii pro adenokarcinom hlavy pankreatu. Pacienti jsou prospektivně sledováni, diskutovány jsou komplikace a přežití. Výsledky: 30denní pooperační mortalita je 2,6 %. Na základě TNM klasifikace 11 % mělo stadium I, 78 % stadium II a 3 % stadium III. Nejzávažnější komplikací byla insuficience pankreatojejunální anastomózy (6,5 %) a opožděná evakuace žaludku (11,8 %). Ve stadiu I přežívá 1., 2., 3. a 4. rok 82 %, 52 %, 35 % a 35 % operovaných. Závěr: Resekce hlavy pankreatu je bezpečná operace s nízkou mortalitou, ale neuspokojivým dlouhodobým přežíváním. Toto je zřejmě způsobeno agresivní biologickou aktivitou nádoru a pozdní diagnostikou., The authors refer seventy six patients radically operated for pancreatic head carcinoma and analyzed postoperative complications and survival. Material and Methods: During 1/2006–9/2010 76 patients underwent potentially curative hemipancreato-duodenectomy for adenocarcinoma of the head of pancreas. The patients are prospectively followed, complications and survival are discussed. Results: The 30-days postoperative mortality was 2,6 %. According to TNM classification 11 % of patients had stage I, 78 % stage II and 3 % stage III. Most severe complications were leak from pancreato-jejunal anastomosis (6,5 %) and delayed gastric emptying (11.8 %). The survival analyses showed 1,2,3 and 4 years survival as follows: 82 %, 52 %, 35 %, 35 % respectively. Conclusion: Resection of the pancreatic head is a safe procedure with low mortality but disappointing long-term survival. Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%).
