Proces angiogeneze je zásadní v růstu a metastazování maligních nádorů a možnosti jeho ovlivnění jsou již několik let v popředí zájmu v onkologii. V současnosti jsou již v klinické praxi dostupná léčiva cílená na proces angiogeneze, která prokázala svůj přínos v řadě klinických studií, i když výzkum přináší stále nové detailní poznatky. Humanizovaná monoklonální protilátka proti VEGF bevacizumab je v léčbě solidních nádorů z těchto léků zatím nejúspěšnější a již patří do standardní léčby metastatického karcinomu kolorekta a nachází uplatnění v léčbě karcinomu prsu, ledvin a plic. Probíhají četné studie zkoumající uplatnění bevacizumabu u jiných solidních malignit. Nežádoucí účinky jsou dobře známy a jejich profil je poměrně příznivý. The most advanced in the treatment of solid tumors at present time is bevacizumab, a humanized monoclonal anti-VEGF antibody. It has become a part of standard treatment of metastatic colorectal cancer and is settling in the treatment of breast, kidney and lung cancers. To determine the role of bevacizumab in the treatment of other solid malignancies, there are many clinical trials ongoing. The toxicity profile has been well described and is quite favorable. Viac ako 50 % pacientov s malignitami je diagnostikovaných vo veku nad 65 rokov. Malígne ochorenia u seniorov predstavujú často náročné medicínske a psychosociálne problémy, ktoré ovplyvňujú možnosť optimálnej liečby. Títo pacienti bývajú často nedostatočne liečení. Navyše sú veľmi často vylúčení z účasti v klinických štúdiách, čo tiež neprispieva k náležitej stratégii liečby. Pre úspešne zvládnutie týchto ochorení je bezpodmienečne nutná náležitá stratégia a taktika liečby založená na znalosti rozsahu ochorenia, fyziologických rezerv, predpokladaného života pacientov, ale tiež analýzy nákladov a prínosu liečby., More than 50 % of cases of cancer are diagnosed in patients older than age 65. Elderly patients with cancer often present with medical and physiological challenges that make the selection of their optimal treatment daunting. Unfortunately as a result, these patients are often undertreated. Elderly are often excluded from participation in clinical trials and receive untested or inadeguate treatment. Targeted therapy, characterized by a low toxicity profile, are potential treatment option for elderly patients. Optimální léčba lokalizovaného karcinomu prostaty není přesně definována. Je možné použít radikální prostatektomii, zevní radioterapii, brachyterapii nebo v některých případech taktiku aktivního sledování. Intersticiální brachyterapie je jedním z přístupů, který se v posledních letech velmi rychle rozvíjí vlivem rozvoje ultrazvukem navigované technologie, nových radioizotopů a dokonalejšími plánovacími systémy. Je vynikající metodou k dodání vysoké dávky do prostaty při velmi nízkém procentu vážných komplikací. Brachyterapie může být provedena technikou trvalého zavedení radioizotopů do prostaty (permanentní aplikace) nebo technikou vysokodávkové dočasné brachyterapie. Obě metody je možné a v některých případech i vhodné kombinovat se zevní radioterapií. Nejlepší technika brachyterapie však není známá a její výběr závisí značně i na zkušenostech pracoviště. Kromě léčebných výsledků je velmi důležité znát i možné nežádoucí účinky léčby s ohledem na kvalitu života pacientů., The optimal treatment for localized prostate cancer has not been clearly defined. It is possible to use radical prostatectomy, external radiotherapy, brachytherapy or, in some cases, the strategy of active surveillance. In recent years, interstitial brachytherapy has been advancing rapidly due to the development of ultrasound-guided techniques, new radioisotopes and more advanced planning systems. It is an excellent method for high-dose delivery within the prostate with a very low rate of serious complications. Brachytherapy can be performed by permanent implantation of radioisotopes into the prostate or by using high-dose rate temporary brachytherapy. Both methods can and, in some cases, should be combined with external radiotherapy. However, the best technique for brachytherapy has not been determined and the selection largely depends on the expertise of the staff. The problem of prophylaxis of incompetence of bronchial stump and bronchial fistula is many sided. Notwithstanding the development of new technologies, perfection of surgical technique, its frequency still remains on high level that persistently requests the search of new ways for resolving this problem, which preserved its topicality till nowadays. We have found that: The most efficiency of BFS is achieved during treatment of bronchial fistulas with diameter up to 3 mm and with effectiveness at 83.2% patients after rightside PE and at 92.3% patients after leftside PE. In case of BPF with diameter more than 3 mm the number of satisfactory results of BFS reaches only 33.3%. but, it is necessary to mention that among patients with BPF sized over 3 mm after leftside PE the effectiveness of BFS significantly exceeds as general index (50.0% against 33.3%) so after rightside PE (50.0% against 25.0%)., F. Gastrointestinální stromální tumory (GIST) jsou nejčastějším mezenchymálním nádorem trávicího traktu s incidencí kolem 7–19 případů na 1 milion. Vyskytují se v celém zažívacím traktu, s predilekční lokalizací v žaludku (65 %), tenkém střevě (25 %), tlustém střevě a konečníku (5–10 %) a výjimečně v jícnu a jiných lokalizacích (5 %). Incidence v České republice není přesně známa, ale odhaduje se na 300–350 nových případů za rok. Jejich léčba je primárně chirurgická. Na rozdíl od zhoubných epiteliálních nádorů GIT, které mají vysokou tendenci k metastazování a vyžadují kromě radikálního odstranění i velmi radikální spádovou lymfadenektomii, chirurgická léčba GIST je ve většině případů omezena na radikální odstranění nádoru do zdravé tkáně. Rozsáhlá ložiska někdy vyžadují multiorgánové resekce, ale základní podmínkou úspěchu je jejich odstranění bez porušení celistvosti. Způsoby chirurgické léčby jsou dány zkušenostmi pracoviště a zahrnují jak klasický otevřený přístup, tak stále častěji používané miniinvazivní laparoskopické metody. Prognóza onemocnění se odvíjí od radikality operace a maligního potenciálu, který je dán velikostí nádoru a mitotickým indexem. Adjuvantní léčba GIST je v kompetenci specializovaného onkologického oddělení a ve většině případů spočívá v podávání molekulárně cílené biologické terapie. Zavedení cílené biologické léčby v podobě imatinib mesylátu představuje obrovský pokrok s dramatickým zlepšením výsledků přežívání i pokročilých a metastatických forem onemocnění. V současné době jsou k dispozici i další tyrozinkinázové inhibitory jako sunitinib malát, sorafenib, nilotinib, masitinib aj. Ačkoli jsou silné indicie proto, že by neoadjuvantní léčba mohla mít přínos v léčbě pokročilých stadií GIST, k dispozici zatím nejsou „evidence based“ údaje, které by opravňovaly její použití. V současné době probíhají v Německu a Kanadě studie fáze 2., které na tyto otázky přinesou odpověď., Gastrointestinal stromal tumours (GISTs) are the most common gastrointestinal mesenchymal tumours with an incidence of about 7–19 cases per one million population. They occur in the whole gastrointestinal tract; the predilection sites include the stomach (65 %), small intestine (25 %), large intestine and rectum (5–10 %) and rarely the oesophagus and other sites (5 %). The incidence in the Czech Republic is not exactly known but it is estimated to be 300–350 new cases per year. The treatment is primarily surgical. Unlike malignant epithelial GIT tumours that are very likely to metastasize and require, in addition to being removed radically, a very radical sentinel lymphadenectomy, the surgical treatment for GIST is mostly restricted to radical resection of the tumour from healthy tissue. Extensive foci sometimes require multiorgan resection but success is mainly determined by their removal without disrupting integrity. The methods of surgical treatment depend on the experience of the workplace and include both the classic open approach technique as well as the more increasingly used mini-invasive laparoscopic procedures. The disease prognosis depends on the radicality of surgery and the malignant potential which is determined by the size of the tumour and mitotic index. Adjuvant treatment for GIST is provided by a specialized oncological centre and, in most cases, consists in administering molecular-targeted biological therapy. The introduction of targeted biological therapy using imatinib mesylate represents a tremendous progress with a dramatic improvement in survival outcomes for both advanced and metastatic forms of the disease. Currently, other tyrosine kinase inhibitors, such as sunitinib malate, sorafenib, nilotinib or masitinib, are available. Although there are strong suggestions that neoadjuvant therapy might be beneficial in treating advanced stages of GIST, no evidence-based data are available to date that would warrant its use. Péče o nemocné se sarkomy měkkých tkání či sarkomy dutiny břišní a retroperitonea, kterých je celkem ročně v České republice asi 350, patří pouze omezenému počtu pracovišť, která jsou schopna a ochotna zajistit komplexní a kvalifikovanou zobrazovací a histopatologickou diagnostiku, širokou víceoborovou škálu operačních výkonů v různých anatomických lokalitách, včetně rekonstrukčních přístupů, zevní radioterapii i intersticiální brachyradioterapii, systémovou i intraarteriální chemoterapii, bedlivou dispenzarizaci nemocných a hodnocení dlouhodobých výsledků vlastní léčebné péče. Výsledky chaotické a náhodné péče o nemocné se sarkomy nelze účinně vylepšit ani novými diagnostickými a léčebnými metodami., Management of patients with soft tissue sarcomas elsewhere in the body (annual incidence in the Czech Republic about 350 cases) should be concentrated only to limited number of hospitals and teams which are able to provide comprehensive diagnostic services, including specialized histopathology, multidisciplinary spectrum of surgical operations, external radiotherapy and interstitial brachyradiotherapy, systemic and regional chemotherapy, careful long-term follow-up of patients and self-evaluation of therapeutic outcomes. Autoři prezentují sestavu 76 nemocných radikálně operovaných pro karcinom hlavy pankreatu a analyzují pooperační komplikace a přežívání. Materiál a metodika: V období 1/2006–9/2010 podstoupilo 76 nemocných potenciálně kurativní hemipankreatoduodenektomii pro adenokarcinom hlavy pankreatu. Pacienti jsou prospektivně sledováni, diskutovány jsou komplikace a přežití. Výsledky: 30denní pooperační mortalita je 2,6 %. Na základě TNM klasifikace 11 % mělo stadium I, 78 % stadium II a 3 % stadium III. Nejzávažnější komplikací byla insuficience pankreatojejunální anastomózy (6,5 %) a opožděná evakuace žaludku (11,8 %). Ve stadiu I přežívá 1., 2., 3. a 4. rok 82 %, 52 %, 35 % a 35 % operovaných. Závěr: Resekce hlavy pankreatu je bezpečná operace s nízkou mortalitou, ale neuspokojivým dlouhodobým přežíváním. Toto je zřejmě způsobeno agresivní biologickou aktivitou nádoru a pozdní diagnostikou., The authors refer seventy six patients radically operated for pancreatic head carcinoma and analyzed postoperative complications and survival. Material and Methods: During 1/2006–9/2010 76 patients underwent potentially curative hemipancreato-duodenectomy for adenocarcinoma of the head of pancreas. The patients are prospectively followed, complications and survival are discussed. Results: The 30-days postoperative mortality was 2,6 %. According to TNM classification 11 % of patients had stage I, 78 % stage II and 3 % stage III. Most severe complications were leak from pancreato-jejunal anastomosis (6,5 %) and delayed gastric emptying (11.8 %). The survival analyses showed 1,2,3 and 4 years survival as follows: 82 %, 52 %, 35 %, 35 % respectively. Conclusion: Resection of the pancreatic head is a safe procedure with low mortality but disappointing long-term survival. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted.
