Cíl: Autoři prezentují tříleté klinické a radiologické výsledky po implantaci interspinózní rozpěrky pro laterální lumbální stenózu páteřního kanálu. Soubor a metodika: Po dobu tří let sledovali 42 pacientů s 62 implantovanými interspinózními náhradami. Indikací k implantaci byla jednostranná nebo oboustranná laterální stenóza páteřního kanálu s degenerací meziobratlové ploténky. Pacienti byli vyšetřeni podle algoritmu, který zahrnoval: neurologické vyšetření, RTG – prosté a dynamické snímky, MR vyšetření a v případě kontraindikace MR bylo provedeno CT vyšetření. Klinický nález byl hodnocen pomocí vizuální analogové škály (VAS), Oswestry Disability Indexem (ODI) a Odom kritériemi. Na RTG snímcích byl hodnocen rozsah pohybů, distrakce v oblasti zadní hrany meziobratlové ploténky a postavení implantátu. Po šesti měsících a po dvou letech bylo provedeno pooperační MR vyšetření a byla hodnocena distrakce foramen intervertebrale v předozadním (AP) a kraniokaudálním rozměru a míra degenerace meziobratlové ploténky ve srovnání s předoperačním nálezem. Výsledky: Po implantaci interspinózní rozpěrky do bederní páteře došlo ve tříletém sledovacím období k poklesu skóre ve VAS škále v průměru o 2,7 bodů a ODI o 24,5 %. Byla prokázána distrakce v oblasti zadní části disku a distrakce v AP rozměru foramen intervertebrale. Závěr: Po zavedení interspinózní rozpěrky došlo ke zlepšení ODI a VAS parametrů, byla zjištěna statisticky významná distrakce v oblasti zadní části meziobratlové ploténky a AP rozměru foramen intervertebrale. Nebyly zaznamenány žádné peri‑ či pooperační komplikace., Aim: The authors present clinical and radiological results of a 3‑year follow‑up of patients after implantation of interspinous spacers for lateral stenosis. Cohort and methodology: We followed 42 patients with 62 implanted interspinous spacers for a period of three years. The indication for implantation was unilateral or bilateral lateral stenosis with degenerative disc disease. Patients were examined according to an algorithm involving: neurological examination, X‑ray – plain and dynamic images, MRI, patients contraindicated for MRI underwent CT examination. Clinical findings were assessed using a VAS score, ODI and Odom criteria. X‑rays were performed and the range of motion, distraction in ventral and dorsal areas of the intervertebral disc and foramen intervertebrale and the position of the implant were evaluated. Six months and two years post‑surgery, patients underwent postoperative MRI examination and intervertebral foramen distraction in anteroposterior and craniocaudal diameter, and the assessment was compared with preoperative findings. Results: Implantation of interspinous spacers in the lumbar spine in a 3-year follow‑up period was followed by the mean improvement of 2.7 points on VAS score, and of 24.5% on ODI. Statistically significant difference in the distraction of the intervertebral foramen in the dorsal part of the intervertebral disc was proven by pre‑ and post‑operative MRI. Conclusion: Implantation of a spacer led to improved ODI and VAS parameters, distraction in the region of the intervertebral foramen, especially in the AP diameter, and in the dorsal part of an intervertebral disc. V terapii meningeomů na prvním místě přetrvává chirurgická léčba. Cílem chirurgické léčby je úplné odstranění nádoru, které často znamená úplné vyléčení pacienta. Prognózu pacienta určuje především radikalita resekce, dále také anatomické umístění a histologický typ nádoru dle WHO klasifikace. Některé meningeomy, zejm. meningeomy baze lební, které často utiskují nebo obrůstají velké cévy a hlavové nervy, a dále potom meningeomy, které histologicky odpovídají WHO st. III, jsou velmi obtížně léčitelné. Nezanedbatelným faktorem je i celkový stav a interní komorbidity. Autoři uvádějí nejčastější typy obtížně chirurgicky řešitelných meningeomů a popisují zkušenosti svého pracoviště s chirurgickým odstraněním, další léčbou a sledováním pacientů s těmito nádory., Surgical treatment is still considered as the method of choice for the treatment of meningiomas. The goal of treatment is to completely surgically remove the tumor; complete excision often results in a permanent cure of the patient. The patient prognosis is mainly determined by the completeness of the resection but anatomical location and histological type of the tumor according to the WHO classification also play an important role. Some meningiomas, especially the skull base meningiomas, often involving large vessels and cranial nerves, as well as WHO grade III meningiomas are very difficult to treat. Patient general health status and internal comorbidities represent another very important factor. The authors describe the most common types of surgically difficult-to-treat meningiomas and describe their experience with surgical removal and additional treatment and surveillance of patients with these tumors. Úvod: Tumory thalamu jsou relativně vzácné léze rostoucí ve vysoce funkčně důležité části mozku. Jsou častější v pediatrické populaci. Jejich chirurgické odstranění je náročné s rizikem značné morbidity. Relativně benigní charakter mnoha těchto lézí vybízí k pokusu o jejich radikální odstranění. Chirurgický přístup musí být velice pečlivě plánován, obvykle s použitím moderních diagnostických metod, jako je DTI (Diffusion Tensor Imaging). Lokalizace a projekce tumoru v thalamu hraje při výběru přístupu důležitou roli. Materiál a metodika: Od roku 2005 do roku 2014 bylo na Neurochirurgické klinice FN Brno operováno celkem 13 pacientů s tumorem thalamu, průměrný věk 20,6 roku (1–64 let). Jednalo se o 10 pacientů mužského a tři ženského pohlaví. Další 15letý pacient s pilocytárním astrocytomem je observován. Transkortikální přístup byl použit 7×, transkalózní přístup 4×, transsylvijský přístup 2× a supracerebellární infratentoriální 1×. Výsledky: Totální resekce (GTR) byla provedena v šesti případech, subtotální resekce ve čtyřech případech a parciální resekce ve třech případech. Jednalo se o osm pilocytárních astrocytomů, jeden subependymální obrovskobuněčný astrocytom, jeden difúzní astrocytom GII a tři glioblastomy. Dva starší pacienti s glioblastomem zemřeli šest a devět měsíců po operaci. Ostatní pacienti přežívají se střední délkou sledování 5 let. Neurologický stav po operaci byl dobrý kromě jednoho staršího pacienta s glioblastomem, u kterého se rozvinula těžká hemiparéza a fatická porucha. Závěr: Pokud je zvolen správný chirurgický přístup, thalamické tumory mohou být celkem bezpečně radikálně odstraněny. Volba přístupu je dána projekcí tumoru. Menší tumory, které nedosahují povrchu thalamu, mohou být observovány nebo bioptovány, pokud máme podezření na maligní povahu léze. Následná onkologická léčba je vyhrazena pro vysokostupňové gliomy., Introduction: Thalamic tumours are relatively rare lesions growing in a highly functional part of the brain. They are more frequent in paediatric population. Their surgery is challenging and associated with a risk of significant morbidity. Relatively benign nature of many of these tumours suggests that radical resection should be attempted. The approach has to be very carefully planned, sometimes with help of modern diagnostic methods such as DTI. Location and projection of the tumour in the thalamus plays an important role in selecting an approach. Material and methods: During 2005–2014 we operated on 13 patients with thalamic tumours, mean age 20.6 years (1–64 years). These were 10 males and three females. Another 15-year-old male patient is being observed. Transcortical approach was used 7×, transcallosal 4×, transsylvian 2× and supracerebellar infratentorial 1×. Results: Gross total resection was achieved in six cases, subtotal in four and partial in three. There were eight pilocytic astrocytomas, one subependymal giant cell astrocytoma, one diffuse astrocytoma G II and three glioblastomas. Two older patients with glioblastomas died six and nine month after operation. All other patiens are still alive with the mean follow-up 5 years. Neurological status after the operation was good except for one older patient with glioblastoma who developed severe hemiparesis and aphasia. Conclusion: Thalamic tumours might be safely radically resected if correct approach is used. The choice of approach is based on the projection of the tumour. Smaller tumours that are not reach thalamic surface might be followed up or biopsied if there is a likelihood of their malignant nature. Oncological treatment should be reserved to malignant tumours. Úvod: V roce 2016 to bude 60 let od doby, kdy Nissen poprvé publikoval svoji operační metodu 360° fundoplikace kolem distálního jícnu a její výsledný efekt. V současnosti je Nissenova fundoplikace pravděpodobně nejčastěji používanou metodou k chirurgické léčbě gastroezofageálního refluxu (GER) a hiátové hernie (HH). Incidence HH je v západní populaci mezi 25–50 %. Současná prevalence GER je v Evropě kolem 4–10 %, v Severní Americe 5–7 %, ale v některých studiích je tato prevalence udávána dokonce kolem 10–20 %. Metody: Na Chirurgické klinice 2. LF UK a ÚVN bylo v období 2010 až 2015 provedeno 157 laparoskopických operací pro hiátovou hernii nebo refluxní nemoc jícnu (GERD). Většina nemocných byla indikována pro symptomatickou HH s/bez GERD, jen minimum nemocných bylo indikováno pro GERD bez HH. V souboru bylo 73 mužů a 84 žen s průměrným věkem 53,9 roku v rozmezí 20–88 let. Z tohoto počtu mělo 11 nemocných syndrom hrudního žaludku. U 109 nemocných (69,4 %) byla provedena 360° fundoplikace dle Nissena (NFP) a u 48 nemocných (30,6 %) byla provedena 270° fundoplikace dle Toupeta (TFP). Po operaci byli nemocní ambulantně kontrolováni v intervalu 3 měsíce, 6 měsíců a 12 měsíců a dále po roce. Prospektivně byly sledovány příznaky pyrózy, dysfagie, možnosti či nemožnosti odříhnutí, meteorismu nebo výskyt recidivy, získaná data byla zkoumána retrospektivně. Pro vyhodnocení získaných dat byla použita metoda deskriptivní statistiky a explorační jednosměrné analýzy. Výsledky: V období 2010 až 2015 bylo provedeno 157 laparoskopických operací pro hiátovou hernii nebo GERD. V souboru se nevyskytlo úmrtí ve vztahu k provedené operaci. Tři měsíce po operaci došlo jak u pacientů po NFP, tak u pacientů po TFP k výraznému poklesu až vymizení symptomů refluxu, příznaků pyrózy a epigastrické bolesti. Pooperační dysfagické obtíže vymizely ve skupině po NFP do 4–7 týdnů, u nemocných po TFP do 3–5 týdnů. V intervalu 2–5 let po operaci došlo celkem u 36 (22,9 %) nemocných k recidivě obtíží. U 22 nemocných (14 po NFP, 8 po TFP), u kterých byla prokázána endoskopicky nebo RTG vyšetřením recidiva refluxu bez recidivy kýly, byla zahájena léčba inhibitory protonové pumpy s velmi dobrým efektem. U 14 nemocných s anatomickou recidivou hiátové hernie s/bez refluxu (8 po NFP, 6 po TFP) byla provedena laparoskopicky rehiátoplastika a refundoplikace dle Nissena. Závěr: Laparoskopická chirurgická antirefluxní léčba GERD a HH je bezpečná metoda s velmi nízkou perioperační morbiditou a s minimální mortalitou. NFP nebo TFP ve stejné míře dlouhodobě obnovují mechanickou bariéru proti regurgitaci žaludečního obsahu do jícnu se signifikantním efektem na vymizení příznaků GERD a HH a s výrazným zlepšením kvality života nemocných. U obézních nemocných je před operací nutná redukce hmotnosti. U morbidně obézních nemocných s HH indikovaných k bariatricko-metabolické operaci je doporučováno provést hiátoplastiku současně s bariatrickým výkonem., Introduction: In 2016 60 years will have elapsed from the first publication by Nissen about his surgical method of 360° fundoplication around distal esophagus and its final effect. Nissen’s fundoplication is currently the most common surgical method for the gastroesophageal reflux (GER) and hiatal hernia (HH) probably. Incidence of HH ranges between 25–50% in the western population. Current prevalence of GER ranges between 4–10% in Europe and 5–7% in North America, but some studies report even 10–20%. Methods: From 2010 to 2015, 157 laparoscopic procedures for HH or gastroesophageal reflux disease (GERD) were done at the Department of Surgery, the Second Faculty of Medicine, Charles University and Central Military Hospital in Prague. Most of the patients were operated because of symptomatic HH with/without GERD. Only very few patients were operated only for GERD without HH. The group of the patients included 73 men and 84 women with the mean age 53.9 years in the range 20–88 years. 11 patients of the group were operated because of the upside-down stomach syndrome. 109 (69.4%) patients underwent Nissen’s 360° fundoplication (NFP) and 48 (30.6%) patients underwent Toupet’s 270° fundoplication (TFP). The patients were followed after 3 month, 6 month, 12 month and then once a year after the surgery. Symptoms such as heartburn, dysphagia, belching, bloating or recurrence of HH or GERD were collected prospectively and analyzed retrospectively. Methods of descriptive statistics and explorative unidirectional analysis were used. Results: From 2010 to 2015, 157 laparoscopic procedures were done for HH or GERD. No death occurred in the group of the patients in connection with the surgery. Similar rates of considerable improvement or resolution of the symptoms such as the reflux, heartburn, and epigastric pain were observed after three months from the surgery among patients after NFP or TFP. Postoperative dysphagia resolved in 4–7 weeks in the group after NFP, and in 3–5 weeks in the group after TFP. Disease recurrence was observed in 36 (22.9%) patients in 2 to 5 years after the surgery. Recurrence of only the gastroesophageal reflux without HH was observed in 22 patients (14 after NFP; 8 after TFP), shown by endoscopy or x-ray assessment, with a satisfactory response to PPI medication. Laparoscopic rehiatoplasty and refundoplication according to Nissen were performed in 14 patients with HH recurrence with/without the reflux (8 after NFP; 6 after TFP). Conclusion: Laparoscopic antireflux surgery of GERD and HH is safe and is associated with very low perioperative morbidity and minimal morbidity. Both NFP and TFP have a similar effect on restoration of the mechanical gastroesophageal barrier against gastric reflux, with significant effect on resolution of GERD and HH symptoms and with a marked improvement of the patient’s quality of life. As regards morbid obese patients, weight reduction is needed before the surgery. Úvod: Perkutánní punkční cholecystostomie je považována za výkon v nouzi při selhávání konzervativní léčby těžké akutní cholecystitidy u starých a kriticky nemocných pacientů. Otázkou je, nakolik je v současnosti toto řešení aktuální či obsoletní. Metody: Retrospektivně byla analyzována data pacientů, kteří podstoupili v období 1/2010−1/2015 punkční cholecystostomii pod kontrolou počítačové tomografie (CT). Sledována byla data pacientů, úspěšnost a komplikace výkonu, krátkodobé a dlouhodobé výsledky. Výsledky: Punkční cholecystostomie pod CT navigací byla na Chirurgické klinice 2. LF UK a FNM za sledované období provedena u 30 pacientů. V souboru bylo 21 žen (70 %), 9 mužů (30 %), průměrný věk 78 let (SD?12,3), medián 82 let (34–93 let). Indikací výkonu byla těžká akutní cholecystitida, respektive empyém žlučníku nereagující na konzervativní léčbu u rizikových pacientů aktuálně neúnosných pro operační výkon. Primárně byla punkce úspěšná u 23 pacientů (77 %), selhala u 7 pacientů (23 %) s nutností opakované punkce u jedné pacientky a urgentní cholecystektomie u 6 nemocných. Průměrná doba hospitalizace byla 16,5 dne (SD?8,2), medián 15 dnů (7–49 dnů). Celková 30 denní mortalita byla 17 %, mortalita v souvislosti s primární indikací k výkonu byla 10 %. Recidivu onemocnění jsme zachytili u 3 pacientů (10 %). U jednoho byla řešena opakovanou drenáží, u druhého konzervativní terapií a u třetího akutní cholecystektomií. U jednoho pacienta (3 %) byla provedena odložená laparoskopická cholecystektomie bez komplikací. Závěr: Perkutánní cholecystostomie pod CT kontrolou u pacientů neúnosných k operaci představuje efektivní a bezpečnou terapeutickou možnost., Introduction: Percutaneous cholecystostomy is considered to be an emergency treatment option when conservative treatment of acute cholecystitis fails in elderly and critically ill patients. The question is: to what extent is this technique still up-to-date or obsolete. Methods: We retrospectively reviewed data of patients who underwent a computer tomography (CT) guided percutaneous cholecystostomy between 1/2010−1/2015. We analyzed the patient data, the success rate, complications of the procedure, short- and long-term outcomes. Results: 30 patients undergoing CT-guided percutaneous cholecystostomy at the Department of Surgery, 2nd Faculty of Medicine, Charles University in Prague and Motol University Hospital during the study period were enrolled. The study group included 21 females (70%) and 9 males (30%) with mean age of 78 years (SD?12.3), median 82 years (range 34−93 years). Percutaneous cholecystostomy was indicated for patients with severe cholecystitis/empyema of the gallbladder not responding to conservative therapy who were poor candidates for operative cholecystectomy. Of these, 23 patients (77%) were successfully treated with initial percutaneous cholecystostomy whereas 7 patients (23%) experienced treatment failure – one was subsequently successfully treated with repeated percutaneous cholecystostomy and six underwent emergency cholecystectomy. The mean length of stay was 16.5 days (SD ?8.2), median 15 days (7–49 days). The total 30-day mortality was 17%, and indication-related mortality was 10%. Three patients (10%) had a recurrence. One patient required repeated percutaneous drainage, the second recovered on conservative treatment and the third patient underwent acute cholecystectomy. Only one patient (3%) underwent delayed laparoscopic cholecystectomy without complications. V kazuistike opisujeme prípad 49-ročného muža od detstva morbídne obézneho (BMI 40 kg/m2), s artériovou hypertenziou (asi od 15 rokov, liečená od roku 2004), dyslipidémiou (od roku 2006), diabetes mellitus 2. typu (od roku 2006, na inzulinoterapii od roku 2008) a fajčením (do roku 2011, 20 cigariet denne). Liečba: 16 druhov liekov, 8 na hypertenziu, statín, liečba diabetu, aspirín, alopurinol. V roku 2010 (ako 45-ročný) hospitalizovaný na našej klinike pre dyspnoe a bolesti na hrudníku s hodnotou krvného tlaku 180/110 mm Hg (zistené srdcové zlyhávanie s ejekčnou frakciou ĽK 33 %, vo funkčnej triede NYHA II, echokardiograficky: ľavá predsieň: 46 mm, rozmer ľavej komory v diastole: 70 mm, interventrikulárne septum: 12 mm, hypokinéza septa, dopplerovská ultrasonografia artérií dolných končatín (kalcifikácie, difúzne aterosklerotické zmeny, neprítomná stenóza), CT koronárna angiografia (signifikantná stenóza ľavej koronárnej artérie). Zahájená liečba perorálny furosemid 40 mg denne. V máji roku 2011 hospitalizovaný pre akútny koronárny syndróm: akútny NSTEMI spodnej steny (koronarografia: 2-cievne postihnutie, realizovaná PKI, implantovaný DES – ramus circumflexus, paroxyzmus fibrilácie predsiení, funkčná trieda NYHA III, ľavokomorová ejekčná frakcia: 30 %, pľúcna hypertenzia). V roku 2012 realizovaná renálna denervácia pre rezistentnú hypertenziu, implantovaný karotický stent pre stenózu karotickej artérie, prítomná diabetická nefropatia (KDOQI 3. stupňa, GF 40 ml/min). V auguste roku 2014 hospitalizovaný na našej klinike pre pľúcny edém, kardiogénny šok, akútnu ischémiu pravého predkolenia pri periférnej embolizácii, prítomný atriálny flutter, zhoršenie renálnych parametrov, echokardiograficky: ľavá predsieň: 55 mm, rozmer ľavej komory: 75 mm, akinéza septa a zadnej steny, oklúzia artérií pravej dolnej končatiny (vzhľadom na vážny stav pacienta kontraindikovaná angiochirurgická intervencia, zvažovaná amputácia končatiny z vitálnej indikácie), pacient zomrel po 4 dňoch hospitalizácie na intenzívnej jednotke po neúspešnej liečbe. Kombinácia ochorení diabetes, hypertenzia a ischemická choroba srdca je častá a prognosticky vážna. Diabetes zvyšuje kardiovaskulárnu morbiditu a mortalitu, a preto by sme mali hľadať diabetes u všetkých kardiovaskulárnych pacientov., The case study describes a case of 49-year-old man with morbid obesity since childhood (BMI 40 kg/m2), arterial hypertension (approx. since aged 15, treated since 2004), dyslipidemia (since 2006), type 2 diabetes mellitus (since 2006, on insulin therapy since 2008) and smoking (until 2011, 20 cigarettes a day). Treatment: 16 types of medication, 8 for hypertension, statin, therapy for diabetes, aspirin, allopurinol. In 2010 (when aged 45) hospitalized in our clinic with dyspnoea and chest pain with a high pressure reading of 180/110 mm Hg (identified symptoms of heart failure with LV ejection fraction of 33 %, in NYHA II functional class, echocardiographically: left atrium: 46 mm, left ventricular chamber size in diastole: 70 mm, interventricular septum: 12 mm, septal hypokinesis, Doppler ultrasonography of lower limb arteries (calcification, diffuse atherosclerotic changes, absent stenosis), CT coronary angiography (significant stenosis of the left coronary artery). Treatment started with 40 mg oral dose of furosemide daily. In May 2011 he was hospitalized with an acute coronary syndrome: acute NSTEMI of the inferior wall (coronarography: 2-vascular problems, implemented PKI, implanted DES – ramus circumflexus, paroxysmal atrial fibrillation, NYHA III functional class, left ventricular ejection fraction: 30 %, pulmonary hypertension). In 2012 renal denervation for resistant hypertension was carried out, carotid stent implanted for stenosis of the carotid artery, presence of diabetic nephropathy (KDOQI stage 3, GF 40 ml/min). In August 2014 admitted to our clinic with pulmonary oedema, cardiogenic shock, acute ischemia of the right calf with peripheral embolisation, presence of atrial flutter, impairment of renal parameters, echocardiographically: left atrium: 55 mm, left ventricle size: 75 mm, akinesis of the septum and posterior wall, occlusion of the right leg arteries (given the patient‘s serious state angio-surgical intervention was contraindicated, vitally indicated leg amputation considered), the patient died after 4 days of hospitalization in an intensive care unit after unsuccessful treatment. A combination of diabetes, hypertension and ischemic heart disease is frequent and prognostically serious. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. CONCLUSION: Since the introduction of ART, the frequency of oral mucosal lesions is minimal in patients with HIV infection. and M. Šembera, V. Radochová, R. Slezák
