Cíl. Posoudit stupeň korelace výsledků chemické analýzy urolitiázy pomocí výpočetní tomografie s duální energií záření (DECT) s laboratorní chemickou analýzou konkrementů a zhodnotit přínos metody DECT pro klinickou praxi. Metoda. Celkem 67 pacientů bylo vyšetřeno pomocí DECT s požadavkem na chemickou analýzu již známé či suspektní urolitiázy. K analýze chemického složení konkrementů pomocí DECT jsme využili simultánní expozici 140 kV a 80 kV. Získaná data byla analyzována pomocí programu Syngo Dual Energy Siemens v modulu Kidney Stones. U 30 pacientů byl následně proveden urologický výkon s extrakcí litiázy a jejím chemickým rozborem. V tomto souboru jsme pak korelovali výslednou chemickou skladbu urolitiázy získanou oběma metodami. Výsledky. V souboru 30 pacientů se podařilo prokázat u konkrementů velikosti nad 3 mm 100% senzitivitu a 100% specificitu DECT analýzy. V případě velmi drobné urolitiázy (tj. pod 3 mm) je přesnost detekce 100%, ale schopnost diferenciace na jednotlivé typy urolitiázy snížená na 97 %. Závěr. Metoda DECT analýzy urolitiázy vykazuje u konkrementů nad velikost 3 mm 100% shodu výsledků v porovnání s chemickou laboratorní analýzou močových konkrementů a má tedy velký přínos pro klinickou praxi, zejména v možnosti včasného zahájení vhodného terapeutického postupu., Aim. To assess the degree of correlation between the results of chemical analysis of urolithiasis using dual energy computed tomography (DECT) with laboratory chemical analysis of stone, and to evaluate the contribution of this method to clinical practice. Method. Total 67 patients underwent DECT in order to performe the chemical analysis of already known or suspected urolithiasis. To analyze the chemical composition of the stone using DECT, we used the simultaneous exposure to 140 kV and 80 kV. The obtained data were analyzed using software Syngo Siemens Dual Energy in program "Kidney Stones". The method of the analysis was based on a material decomposition algorithm. In 30 patients, the extraction ofthe stone followed and the chemical and crystalograpic analysis was made. The results of virtual chemical analysis and laboratory findings were compared. Results. In the stones greater than 3 mm, sensitivity and specificity reached 100% using DECT analysis. In the case of very small stone disease (ie. less than 3 mm), the detection reached 100% accuracy, but the ability to differentiate the correct type of stone disease decreased to 97 %. Conclusion. DECT analysis of urolithiasis shows the stones of size 3 mm with 100% match results in comparison with laboratory chemical analysis of urinary stones and therefore has great benefit for clinical practice, especially early in choosing the appropriate therapeutic procedure., Bednářová A, Ferda J, Kreuzberg B, Klečka J, Hora M., and Literatura 6
Cíl: Hlavním cílem studie je popsat transverzální a longitudinální mobilitu nervus medianus u pacientů se syndromem karpálního tunelu před otevřenou dekompresí karpálního tunelu a s odstupem po ní. Materiál a metodika: V období od března 2007 do června 2009 bylo do studie zahrnuto celkem 72 pacientů se syndromem karpálního tunelu. Vyšetření pohybu nervu bylo provedeno neinvazivně pomocí ultrazvuku před operací a několik měsíců po operaci. Transverzální pohyb byl měřen v úrovni os pisiforme a podélný pohyb byl měřen na volární straně předloktí ve vzdálenosti 8–11 cm proximálně od zápěstí. Dekomprese karpálního tunelu byla provedena standardní otevřenou technikou. Výsledky: Průměrná transverzální amplituda nervu byla v našem vzorku 1,82 mm předoperačně a 1,76 mm pooperačně a rozdíly nebyly statisticky významné (p > 0,05). Průměrná amplituda longitudinálního pohybu nervus medianus byla 3,74 mm předoperačně a 3,89 mm pooperačně a rozdíl nebylo možno hodnotit jako statisticky významný (p > 0,05). Závěr: Po otevřené dekompresi karpálního tunelu nedochází ke statisticky významným změnám v transverzální a ani v longitudinální mobilitě nervus medianus. Otevřená dekomprese u pacientů se syndromem karpálního tunelu nemá vliv na schopnost nervus medianus přizpůsobovat se pohybu okolních struktur., Aim: The main objective of the study was to describe transverse and longitudinal movement of the median nerve in patients with the carpal tunnel syndrome before and with delay after open decompression of the carpal tunnel. Materials and Methods: From March 2007 to July 2009, the study enrolled a total of 72 patients with the carpal tunnel syndrome. The examination of nerve movement was performed noninvasively by ultrasound before and several months after the surgery. Transverse nerve amplitude was measured at the level of the pisiform bone. Longitudinal movement was measured on the forearm at a distance of 8–11 cm proximal to the wrist. The standard open surgery was performed. Results: The mean of maximum transverse amplitude of the median nerve in our sample was 1.82 mm preoperatively and 1.76 mm postoperatively, and the difference was not statistically significant (p > 0.05). The average amplitude of the longitudinal movement of the median nerve was 3.74 mm preoperatively and 3.89 mm postoperatively, and the difference was not considered statistically significant (p > 0.05). Conclusions: There were no statistically significant changes in transverse or longitudinal movement of the median nerve after open carpal tunnel decompression. Open decompression does not affect biomechanical parameters of the median nerve in carpal tunnel syndrome patients. Cíl. Hlavním cílem práce je stanovení závislosti tíže klinických příznaků na ultrazvukovém nálezu u pacientů s podezřením na celiakální sprue (CS), tedy stanovení senzitivity a specificity ultrazvuku, dále pak posouzení velikosti sleziny u pacientů s CS. Metoda. Do souboru bylo zahrnuto celkem 134 pacientů, kteří byli prospektivně sledováni. Z toho u 68 pacientů byla potvrzena diagnóza CS jak na základě sérologických výsledků protilátek, tak enterobiopticky U dalších 41 pacientů byla CS potvrzena pouze protilátkově. U zbývajících 25 pacientů se nakonec prokázal primární izolovaný deficit laktázy U 66 pacientů s biopticky verifikovanou CS byla pomocí ultrazvuku změřena velikost sleziny. K tomuto souboru byl vytvořen srovnávací soubor 66 zdravých pacientů, kterým byla stejným způsobem změřena velikost sleziny. Výsledky. Znatelný byl jednoznačný trend prokazující, že pacienti s těžšími klinickými příznaky mají také nález na ultrazvuku vyššího stupně. Tento trend je významný hlavně u příznaků vyššího stupně. Dále bylo prokázáno, že existuje statisticky významný rozdíl (Fisher exact test p < 0,001) ve velikosti sleziny u pacientů s potvrzenou celiakií a kontrolním souborem zdravých pacientů. Pacienti s verifikovanou CS mají statisticky významně častěji zmenšenou slezinu (75,8 %) než kontrolní soubor zdravých lidí (25,8 %). Závěr. Ultrazvuk se jeví jako perspektivní metoda jak v diagnostice nemoci, tak pro sledování pacientů s již prokázanou chorobou. Prokazatelně existuje určitá souvislost mezi CS a velikostí sleziny., Aim. The main objective of this study is to determine the severity of clinical signs depending on ultrasonographic findings in patients with suspected celiac disease (CD), thus determining the sensitivity and specificity of ultrasonography Furthermore, assessment of the size of the spleen in patients with CD. Method. The group included a total of 134 patients, they were prospectively monitored. Of those, 68 patients were confirmed diagnosis of CD, both based on the results of serological antibodies and enterobiopsy A further 41 patients had only the positivity of specific antibodies. In the remaining 25 patients eventually showed isolated primary lactase deficiency. In 66 patients with biopsy-verified CD was measured spleen size using ultrasonography. This file was created by a comparative group of 66 healthy patients who have been equally measured the spleen size. Results. Noticeable was a clear trend showing that patients with more severe clinical symptoms have also more severe ultrasound findings. This trend is significant mainly at the higher level of symptoms. It was also shown that there is a statistically significant difference (Fisher exact test p < 0.001) in spleen size in patients with confirmed celiac disease and a control group of healthy patients. Patients with verified CD have statistically significantly reduced spleen (75.8%) than the control group of patients (25.8%). Conclusion. Ultrasound appears to be promising method in the detection of the disease and also in monitoring these patients.
