Zlatým standardem léčby karcinomu endometria je chirurgická léčba. Adjuvantní radioterapie ve formě brachyterapie či zevní radioterapie je indikována u středně a vysoce rizikového karcinomu, kde snižuje zejména výskyt lokoregionální recidivy. U vysoce rizikového karcinomu jsou však pacientky ohroženy i generalizací onemocnění, což je důvodem pro zvažování zařazení adjuvantní chemoterapie do léčebného schématu. Chemoterapie může být použita současně s radioterapií jako konkomitantní chemoradioterapie nebo sekvenčně po skončení radioterapie. Článek popisuje výsledky prací, které použily adjuvantní chemoterapii. Výsledky nejsou zatím zcela jednoznačné, na závěry velkých studií stále čekáme. Gastrointestinální stromální nádory jsou nejčastější mezenchymální nádory gastrointestinálního traktu. Onkogenní aktivující mutace receptorových tyrozinkináz KIT (75–80 %) a PDGFRA (10 %) mají zásadní význam v molekulární patogenezi GISTu. Kompletní chirurgická resekce je standardem v léčbě lokalizovaných GISTů, avšak u třetiny pacientů dochází k recidivě do 2 let po resekci. Novější léčebné strategie a zavedení cílené léčby přináší signifikantně delší přežití pacientů s GISTem. Imatinib, inhibitor receptorových tyrozinkináz, a jeho zavedení v léčbě GISTu je nejvýznamnějším krokem a má význam v léčbě 1. linie neresekabilních nebo pokročilých GISTů, rovněž v léčbě adjuvantní po resekci GISTu s vysokým rizikem rekurence. Sunitinib je indikován v léčbě druhé linie. Regorafenib byl nedávno schválen pro léčbu třetí linie pacientů s progresí nebo intolerancí léčby imatinibem a sunitinibem. Oncogenic activating mutations in the receptor tyrosine kinases, KIT (75–80 %) and platelet- derived growth factor receptor alpha (PDGFRA, 10 %) play a crucial role in the molecular pathogenesis of GIST. Complete surgical resection is the standard treatment for localized GISTs, however, recurrence is observed in approximately a third of patients 2 years following resection. Better treatment strategies and targeted therapies have improved survival rates for patients with GIST significantly. Imatinib, the receptor tyrosine kinase inhibitor, has revolutionized the therapeutic landscape for GIST patients and remains the mainstay first-line clinical option for both unresectable and advanced GISTs, as well as for adjuvant treatment after resection of high risk GISTs. Sunitinib is indicated as second-line therapy. Regorafenib has been approved recently as third-line treatment of patients who progressed on or are intolerant to prior imatinib and sunitinib.
