Concha bullosa (CB) is among the most common anatomic variations of sinonasal anatomy. Although usually asymptomatic, CB can occasionally cause nasal obstruction or headache. Obstructions within the mucociliary transport system can develop into a mucocele or mucopyocele. A 48-year-old female, with a history of progressive headache and nasal obstruction, was referred to our department. Paranasal sinus tomography revealed a nasal mass in the left nasal cavity resembling a mucopyocele in the middle turbinate. Under general anesthesia, the purulent material was aspirated, and the lateral part of the left turbinate was resected. Mucopyoceles are common within the paranasal sinuses, but uncommon with CB; thus, they should be considered in patients with a large hyperemic nasal mass. and K. Sari, Z. K. Gencer, Y. Kantekin
Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. Východiska: Monoklonální anti-EGFR protilátka cetuximab představuje cílenou biologickou léčbu, která se stává standardem personalizované medicíny nemocných s metastazujícím kolorektálním karcinomem. Stupeň kožního postižení je nejvýznamnějším prediktivním faktorem účinnosti cetuximabu. Formou klinické kazuistiky je prezentována možná léčba kožní toxicity cetuximabu. Klinická kazuistika: Autoři popisují případ 57leté pacientky s metastazujícím kolorektálním karcinomem, která byla léčena paliativní biochemoterapií ve schématu cetuximab+FOLFIRI. Šest týdnů od zahájení terapie došlo k progresi kožního nálezu a rozvoji papulopustulózního exantému III. stupně. Včasné zahájení lokální léčby na bázi aplikace krému s 0,1% vitaminem K1 v kombinaci se systémovou antibiotickou léčbou a antihistaminiky umožnila nemocné pokračovat v biologické léčbě. Závěr: Včasná dermatologická léčba nemocných s projevy kožní toxicity při terapii monoklonální protilátkou cetuximab umožní pokračování onkologické léčby bez přerušení tak, aby pacienti mohli mít z této léčby co největší užitek., Background: Cetuximab, a monoclonal anti-EGFR antibody, represents aimed biological treatment that becomes a standard of personalized medicine with metastatic colorectal carcinoma patients. Level of skin toxicity is the most significant predictive factor of cetuximab effectiveness. Possible dermal toxicity treatment with cetuximab application is presented in the form of a clinical case report. Case: Authors describe a case of a 57-year-old woman-patient suffering from metastatic colorectal carcinoma who was treated through palliative bio-chemotherapy in the schedule: cetuximab+FOLFIRI. Progression of the dermal disease and papulopustulous progress of the grade III exanthema were detected in a six-week-long period from the therapy initiation. Timely initiation of local treatment on the basis of cream with 0.1 % vitamin K1 application in combination with systematic antibiotic treatment and antihistamines application enabled the patient to continue with biological treatment. Lyellův syndrom (syndrom toxické epidermální nekrolýzy) je raritně se vyskytující onemocnění, které poprvé popsal skotský lékař Alan Lyell v roce 1956. Je charakterizováno rozsáhlou kožní a slizniční nekrolýzou postihující minimálně 30 % tělesného povrchu a systémovými projevy. Podle četnosti výskytu se jedná o extrémně vzácnou nozologickou jednotku s incidencí 0,5–1,5/1 milion obyvatel a rok. Leyllův syndrom je považován za toxoalergickou reakci, spouštěčem je nejčastěji některé léčivo. Syndrom je spojen s vysokou letalitou (knižně udávanou v rozmezí 30–90 %). Neodkladná lokální i celková léčba na jednotkách intenzivní péče nebo na specializovaném pracovišti může zlepšit celkově špatnou prognózu pacientů. V naší kazuistice popisujeme vzácný případ pacienta s rozvojem Lyellova syndromu po práci s běžným fungicidním prostředkem, který je častou součástí vybavení zahrádkářů a zemědělců. Klíčová slova: bis-dithiokarbamidany – Leyllův syndrom – toxická epidermální nekrolýza, Leyll’s syndrome (syndrome of toxic epidermal necrolysis) is a rare disease, firstly described by Scottish doctor of medicine Allan Lyell in 1956. It is characterized by huge skin and mucosa necrolysis, which affects at least 30 % of body surface, and systemic symptoms. According to the frequency of the occurrence it is an extremely rare condition, with an incidence of 0.5–2 cases per million residents per year. Leyll’s syndrome is considered as a toxoallergic reaction, triggered mostly by some medication and it is associated with a very high mortality rate (in the literature reported between 30 to 90 %). Adequate and timely local and systemic treatment at the Intensive Care Unit or at the specialized clinic can improve the overall poor prognosis of the patients. In our case report we describe a very rare case of the Lyell’s syndrome after exposure to the antifungal organosulfur compound, which is widely used by the homegardners and farmers.
