Concha bullosa (CB) is among the most common anatomic variations of sinonasal anatomy. Although usually asymptomatic, CB can occasionally cause nasal obstruction or headache. Obstructions within the mucociliary transport system can develop into a mucocele or mucopyocele. A 48-year-old female, with a history of progressive headache and nasal obstruction, was referred to our department. Paranasal sinus tomography revealed a nasal mass in the left nasal cavity resembling a mucopyocele in the middle turbinate. Under general anesthesia, the purulent material was aspirated, and the lateral part of the left turbinate was resected. Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. Apendicitida je nejčastější břišní náhlá příhoda. Diagnóza může být ihned jasná u pacientů s klasickými příznaky a znaky. Atypické příznaky mohou výrazně ztížit stanovení diagnózy a zpozdit léčbu. Klasickým příznakem je bolest. Dále může být nevolnost, zvracení a nechutenství. Vyšetření břicha prokáže lokalizovanou bolestivost a rigiditu břišního svalstva v pravém dolním kvadrantu. Laboratorní vyšetření obvykle prokáže leukocytózu s posunem doleva a zvýšené hodnoty C reaktivního proteinu. Ke stanovení diagnózy výrazně pomohou nativní snímek břicha, ultrasonografické nebo CT vyšetření. V roce 1889 byla apendektomie akceptována jako standardní léčba, protože zachraňovala životy, a od té doby platí dictum: odstranění zánětlivě změněného apendixu je nezbytné!, Appendicitis is the most common abdominal emergency. While the clinical diagnosis may be easy in patients who present with classic signs and symptoms. Atypical presentations may result in diagnostic embarrassment and delay in treatment. Typical sign is abdominal pain. Furthermore, it can be nausea, vomiting and anorexia. Abdominal examination reveals localised tenderness and muscular rigidity in the right lower abdominal quadrant. Laboratory data usually reveal an elevated leukocytosis with a left shift and elevated C-reactive protein. To establish the diagnosis greatly help native abdominal X-ray, ultrasound or CT. Úvod: Infekčná endokarditída u pacienta po transplantácii obličky je závažná infekčná komplikácia, ktorá zvyšuje riziko straty štepu ako aj mortalitu pacientov. Najdôležitejším predisponujúcim faktorom je imunosupresívna liečba – predovšetkým indukčná imunosupresia. Materiál a popis prípadu: V transplantačnom centre Martin podstúpilo v období 12 rokov transplantáciu obličky 250 pacientov. Z tohto súboru pacientov bolo 5 pacientov (2 %) po náhrade chlopne. Prezentujeme prípad pacienta po transplantácii obličky s rozvojom endokarditídy bioprotézy aortálnej chlopne mesiac po úspešnej transplantácii obličky. Diagnostika endokarditídy štandardnými postupmi (transtorakálne echokardiografické vyšetrenie, transezofageálne echokardiografické vyšetrenie, hemokultúry) bola neúspešná. Endokarditídu sme diagnostikovali raritne až pomocou PET-CT vyšetrenia s následnou zmenou antibiotickej liečby a úspešným zvládnutím tejto potransplantačnej komplikácie. Záver: Endokarditída po transplantácii obličky je vážna komplikácia, ktorá výrazne zhoršuje mortalitu príjemcov. Riziko rozvoja infekčnej endokarditídy po transplantácii zvyšuje aj indukcia, a to predovšetkým antitymocytárny globulín. Diagnostika iba pomocou PET-CT vyšetrenia je raritná, ale v prípade nášho popísaného prípadu, zásadne zmenila prístup k pacientovi a viedla k úspešnej liečbe. Kľúčové slová: endokarditída – indukcia – PET-CT – transplantácia obličky, Introduction: Infective endocarditis in a patient after kidney transplantation is a serious infective complication which increases the risk of loss of the graft and also the mortality of patients. The most important predisposing factor is the immunosuppressive therapy – mainly induction immunosuppression. Material and case description: 250 patients underwent kidney transplantation throughout the period of 12 years in the Transplant Center Martin. This set of patients included 5 patients (2 %) after heart valve replacement. We present the case of a patient after kidney transplantation with development of endocarditis of the bioprosthesis of the aortic valve one month after successful kidney transplantation. Diagnostics of endocarditis by standard procedures (examination by transthoracic echocardiogram, transesophageal echocardiography, hemocultures) was unsuccessful. We rarely diagnosed endocarditis only by PET-CT examination with a consequent change of the antibiotic treatment and successful managing of this post-transplant complication. Conclusion: Endocarditis after kidney transplantation is a serious complication which significantly worsens the mortality of patients. The risk of development of infective endocarditis after transplantation is also increased by induction, mainly by antithymocyte globulin. Diagnostics only by PET-CT examination is rare; however, in this case it fundamentally changed the approach to the patient and led to a successful treatment.
