Léčba gastrointestinálních stromálních nádorů (GIST) může být příkladem úspěšnosti multidisciplinárního přístupu k léčbě maligních onemocnění. Významného pokroku v léčbě metastatického GIST bylo dosaženo molekulárně cílenou léčbou imatinibem. Dostupnost účinné systémové léčby vedla i k rozšíření možností chirurgické léčby a v současné době je dokumentována účinnost imatinibu v adjuvantní i neadjuvantní indikaci. Klíčovým pro stanovení správného léčebného postupu a optimální výsledek léčby je úzká spolupráce chirurgického a interního onkologa i soustředění nemocných do center specializovaných na léčbu tohoto vzácného nádorového onemocnění., The therapy of gastrointestinal stromal tumors may serve as an example of success of multidisciplinary approach in the therapy of malignant disorders. A significant progress in the therapy of metastatic GIST was obtained with the use of molecular targeted therapy (imatinib). The advent of targeted therapy also resulted in improved possibilities of surgical intervention and imatinib is currently used in the adjuvant as well as neoadjuvant indication. Léčba karcinomu prostaty se v posledních letech stává stále složitější, převážně kvůli nástupu nových cytostatik, antiandrogenů a radiofarmak. Problém sekvenční léčby nebude pravděpodobně nikdy vyřešen, neboť se do léčebných schémat budou neustále zapojovat další preparáty. Vždy je nezbytné brát do úvahy preference pacienta, jeho výkonnostní stav a komorbidity a bohužel také i možnosti jednotlivých pracovišť. Skupiny pacientů vstupujících do klinických studií jsou pečlivě vybrané a neodpovídají realitě denní praxe. Multidisciplinární týmy za účasti urologa, klinického a radiačního onkologa a dalších odborností tak nabývají na jasném významu. V rámci konzilií je možné vybrat vhodnou léčebnou strategii individuálně pro každého pacienta, včetně možnosti účastnit se klinických zkoušek na pracovištích konzultantů. Abirateron a enzalutamid budou brzy schváleny i pro použití pro metastatický karcinom před aplikací chemoterapie s nespornou výhodou příznivého profilu nežádoucích účinků. The consultation can select an appropriate treatment strategy for each patient individually, including the possibility to participate in clinical trials at the respective departments.
