Ovarian cancer is the fifth most common malignancy in the world's female population and with the highest lethality index among gynecological tumors. The prognosis of metastatic disease is usually poor, especially in platinum-resistant cases. There are several options for the treatment of metastatic disease resistant to platinum derivates (e.g. paclitaxel, topotecan and pegylated liposomal doxorubicin), all of which are considered equipotent. Pegylated liposomal doxorubicin (PLD) is a liposomal form of the anthracycline antibiotic doxorubicin. It is characterized by more convenient pharmacokinetics and a different toxicity profile. Cardiotoxicity, the major adverse effect of conventional doxorubicin, is reduced in PLD as well as hematotoxicity, alopecia, nausea and vomiting. Skin toxicity and mucositis, however, emerge as serious issues since they represent dose and schedule-limiting toxicities. The pharmacokinetics of PLD (prolonged biological half-life and preferential distribution into tumor tissue) provide new possibilities to address these toxicity issues. The extracorporeal elimination of circulating liposomes after PLD saturation in the tumor tissue represents a novel and potent strategy to diminish drug toxicity. Difuzní B-velkobuněčný lymfom (DLBCL) představuje agresivní lymfoidní neoplazii, která patří díky moderním léčebným přístupům k vysoce kurabilním nádorovým onemocněním. Ale i v éře imunochemoterapie existuje významná skupina pacientů, kteří jsou primárně refrakterní na podávanou terapii, nebo po jejím ukončení relabují. Výsledek časného PET/CT vyšetření (ePET/CT) v průběhu terapie může být cenným ukazatelem citlivosti nádoru na podávanou léčbu. Možnosti hodnocení ePET/CT jsou dnes poměrně široké a zahrnují jak vizuální popis, tak také různé metody semikvantitivního posouzení léčebné odpovědi. I přes množství dosud získaných dat a řadu slibných předběžných výsledků zůstává role ePET/CT stále předmětem klinického výzkumu a jeho výsledek zatím nemůže být standardně používaným vodítkem pro rozhodování o další léčbě nemocného., Diffuse large B-cell lymphoma (DLBCL) is an aggressive lymphoid neoplasia, and for modern therapeutic approaches is a highly curable cancer. The assessment of ePET/CT today are quite broad and include both visual description, as well as different methods semiquantitative assessment of treatment response. Cíle studie: Cílem této studie je sledování vzájemného vztahu plasmatických hladin imatinibu s jeho denní dávkou a časovým intervalem od poslední užité dávky u pacientů s chronickou myeloidní leukemií na terapii imatinibem. Chronická myeloidní leukemie (CML) je myeloproliferativní onemocnění charakteristické přítomností chromozomu Philadelphia a leukemického fúzního genu BCR-ABL vedoucího k expresi konstitutivně aktivní tyrosinkinázy zodpovědné za maligní transformaci. Největší úspěšnost v léčbě byla zaznamenána prostřednictvím cílených inhibitorů vzniklého onkoproteinu, proto je dnes imatinib používán jako lék první linie. Terapeutické monitorování plasmatických hladin imatinibu u pacientů s CML je důležitým nástrojem k optimalizaci a individualizaci léčby. Typ studie: Klinická Materiál a metody: V naší studii jsme analyzovali soubor 1790 vzorků od 168 pacientů s CML na terapii imatinibem. Veškerá měření byla provedena metodou kapalinové chromatografie ve spojení s tandemovou hmotnostní spektrometrií v rámci rutinního vyšetřování plasmatických hladin tyrosinkinázových inhibitorů. Denní dávka imatinibu s pohybovala v rozmezí 100 až 800 mg; vzorky byly odebrány 1–120 hodin od poslední užité dávky. Výsledky a závěr: Analýza výsledků odhalila významnou korelaci plasmatické hladiny imatinibu s jeho denní dávkou (R2 = 0,075) a časovým intervalem od poslední užité dávky (R2 = 0,177). Z celého souboru bylo vybráno 616 vzorků odebraných 24±4 hodiny od poslední dávky od 111 pacientů s denní dávkou 400 mg imatinibu a byla stanovena inter- (48 %) a intraindividuální (36 %) variabilita., Objective: The aim of the study is monitoring of correlation of imatinib plasma levels with daily dose and sampling time in patients with chronic myeloid leukemia on imatinib therapy.
