Biguanidy jsou léky určené pro terapii hyperglykemie u diabetu 2. typu. V poslední době se ukazuje, že nejčastěji předepisovaný biguanid metformin vykazuje také protinádorový účinek. Jeho možné využití v protinádorové léčbě podporují epidemiologické, preklinické a klinické studie. Příznivé ovlivnění karcinomu prsu a tlustého střeva by mohlo souviset s primární schopností metforminu snižovat hladinu cirkulujícího inzulinu, ale metformin má nepochybně i přímý cytostatický efekt, který se uskutečňuje prostřednictvím inhibice mTOR (mammalian target of rapamycin) signální dráhy a blokádou syntézy proteinů. Využití metforminu v protinádorové léčbě nebo chemoprevenci nádorů ale ukáže teprve budoucnost., Metformin, an oral biguanide antidiabetic drug, is the most widely prescribed agent for treatment of type 2 diabetes. The drug mainly works by lowering glucose production by the liver, and thus lowering fasting blood glucose. Metformin acts as an insulin sensitizer, which leads to inhibition of gluconeogenesis in the liver, and has been found to be beneficial especially in overweight patients with diabetes. Metformin use has been postulated to contribute to a reduced risk of pancreatic, prostate, ovarian, and breast cancers. Metformin‘ s mechanism of action was examined recently and determined to be initiated by metformins role as an adenosine 5‘-mono-phosphate– activated protein kinase (AMPK) activator through a tumor suppressor protein kinase known as LKB-1, which regulates AMPK levels. Cíl: Cílem studie je stanovit prevalenci chronického selhání ledvin u 101 pacientů s monoklonální gamapatií neurčeného významu (MGUS). Typ studie: retrospektivní studie. Materiál a metody: Porovnali jsme glomerulární filtraci odhadovanou ze sérového kreatininu a cystatinu C a hodnotili jsme korelaci mezi koncentrací volných lehkých řetězců v séru a funkcí ledvin. U všech pacientů jsme provedli elektroforézu pro - teinů séra s denzitometrickou kvantifikací monoklonálního imunoglobulinu a změřili jsme koncentraci kreatininu, cystatinu C, volných lehkých řetězců kappa a lambda, β 2 mikroglobulinu. Výsledky: Chronické selhání ledvin jsme nalezli u 38,6 % pacientů na základě odhadu ze sérového cystatinu C a u 34,6 % pacientů na základě odhadu ze sérového kreatininu. B2mikroglobulin má největší počet významných korelací; cystatin C (0,90), CysC GFR (-0,69), Kreatinin (0,70), MDRD GFR (-0,60), MIg koncentrace (0,24), κ FLC (0,22) λ FLC (0,34) a věk (0,40). Tento velký počet korelací lze vysvětlit tím, že β 2mikroglobulin odráží jak zátěž organismu maligními buňkami, tak pokles glomerulární filtrace. Závěr: Chronické selhání ledvin jsme nalezli u 38,6 % pacientů při odhadu glomerulární filtrace z cystatinu C a u 34,6 % při odhadu ze sérového kreatininu. Třída monoklonálního imunoglobulinu může mít vliv na výsledky odhadu glomerulární filtrace ze sérového cystatinu C, odhad glomerulární filtrace ze sérového kreatininu není ovlivněn třídou imunoglobulinu. Diabetici neměli statisticky signifikantně nižší glomerulární filtraci než pacienti bez diabetu mellitu., Background: Renal function is more frequently decreased in patients with monoclonal gammopathy. 10 % of patients with multiple myeloma present with acute renal failure and renal function is included in CRAB criteria for the diagnosis of multiple myeloma. Aim: The aim of this study is to determine the prevalence of chronic renal failure (CRF), defined as a glomerular filtration rate (GFR) below 1.0 ml/s/1.73m 2 , in 101 patients with monoclonal gammopathy of undetermined significance (MGUS). Design: retrospective study Material and methods: We compared GFR estimates from serum creatinine (MDRD GFR) and serum cystatin C (CysC GFR) and looked at the correlation between free light chains (FLC) concentration and renal markers. We performed serum protein electrophoresis (SPE) with quantification of monoclonal intact immunoglobulin (MIg) and measured serum creatinine, cysta - tin C, κ and λ FLC and β 2microglobulin in all patients. Results: We found CRF in 38.6% patients using estimation from serum cystatin C and 34.6 % using estimation from serum creatinine. B2microglobulin has the highest number of significant correlations; cystatin C (0.90), CysC GFR (-0.69), crea - tinine (0.70), MDRD GFR (-0.60), MIg concentration (0.24), κ FLC (0.22) λ FLC (0.34) and age (0.40). The best correlation results may be explained by the fact that β 2microglobulin reflects both malignant cells burden and renal function. Conclusions: We found CRF in 38.6 % MGUS patients using estimation from serum cystatin C and in 34.6 % using estima - tion from serum creatinine. The class of monoclonal immunoglobulin may influence results of glomerular filtration estimated from cystatin C but it does not influence GFR estimated from creatinine. C-reaktivní protein (CRP) je citlivý marker zánětu, jehož hladiny se zvyšují u jedinců s diabetem a metabolickým syndromem v souvislosti s rozvojem subklinického zánětu. Ukazují se možnosti využití stanovení hladin CRP jako indikátoru potenciálního rizika onemocnění metabolickým syndromem nebo aterosklerózou na podkladě rozvoje inzulínové rezistence. Práce ověřuje koncentrace C-reaktivního proteinu u vybraných skupin jedinců a jejich korelace s parametry typickými pro inzulínovou rezistenci. Byly studovány skupiny zdravých jedinců (A, n=126), jedinců s obezitou (B, n=213) a jedinců s diagnózou metabolického syndromu (C, n=79). Jedinci s obezitou ve skupině B mají parametry svědčící pro počínající inzulínovou rezistenci: hypertenzi, hyperglykemii, QUICKI (0,305), nižší adiponektinemii a vyšší aFABP ve srovnání se skupinou zdravých. Jedinci ve skupině C mají diagnózu metabolického syndromu, čemuž odpovídají markery inzulínové rezistence QUICKI (0,293), více se snižující adiponektinemie (7,3 mg/l) a vysoké aFABP (35,3 mg/l). Koncentrace CRP je ve skupině zdravých A nejnižší (0,75 mg/l), ve skupině obézních B se zvyšuje (2,65 mg/l) a ve skupině C s metabolickým syndromem je nejvyšší (3,62 mg/l). Největší korelace je mezi CRP a aFABP (0,46) a dále pak mezi aFABP a Body Mass Index (BMI) (0,57). Na patofyziologii rozvoje inzulínové rezistence se podílí subklinická zánětlivá reakce, vyjádřená hlavně ve viscerální tukové tkáni. U obézních jedinců byly stanoveny koncentrace CRP odlišné od zdravých jedinců, které se přibližují k hladině 3 mg/l označované za rizikovou. Jedinci s metabolickým syndromem vykazovali hodnoty CRP rizikové. Přítomnost obezity v asociaci se zvýšením hladiny CRP je považováno za významný rizikový faktor postupného rozvoje metabolického syndromu., C-reactive protein (CRP) is a sensitive marker of inflammation. It has been shown to be potentially useful as an indicator for predicting the risk of metabolic syndrome, based on the development of insulin resistance. The study verified CRP levels in selected groups of individuals and their correlations with parameters typical for insulin resistance. The following groups were studied: healthy individuals (A, n=126), obese individuals (B, n=213) and patients diagnosed with metabolic syndrome (C, n=79). Obese persons in Group B had parameters suggestive of early insulin resistance: hypertension, hyperglycaemia, QUICKI (0.305), lower adiponectin and higher aFABP levels as compared with the healthy subjects. Group C individuals were diagnosed with metabolic syndrome, as evidenced by the QUICKI markers for insulin resistance (0.293), much lower adiponectin (7.3 mg/L) and high aFABP levels (35.3 mg/L). CRP concentrations were lowest in Group A healthy individuals (0.75 mg/L), higher in Group B obese subjects (2.65 mg/L) and highest in Group C patients with metabolic syndrome (3.62 mg/L). The highest correlations were between CRP and aFABP (0.46) and between aFABP and Body Mass Index (0.57).
