Cíl: Cílem práce bylo provést komplexní zhodnocení faktorů ovlivňujících úspěšnost zachování sluchu při mikrochirurgické léčbě vestibulárního schwannomu (VS). Soubor a metodika: Prospektivně jsme hodnotili soubor 81 pacientů, kteří v období 2008–2010 podstoupili radikální exstirpaci VS retrosigmoidním‑transmeatálním přístupem a v době zákroku měli zachovaný sluch. Hodnotili jsme faktory stran pacienta, nádoru, audiometrické parametry a technické aspekty operace. Parametry byly statisticky vyhodnoceny včetně regresivní analýzy nezávislých proměnných. Výsledky: Sluch se podařilo zachovat u 17 pacientů (tzv. užitečný sluch: devět; tzv. neužitečný sluch: osm). U dvou pacientů jsme zaznamenali zlepšení z neužitečné úrovně na sluch užitečný. Velikost VS byla ve skupině se zachovaným sluchem menší (p < 0,001), převažovaly tak nádory nižších stadií (p = 0,001). Hloubka zasahování do vnitřního zvukovodu nevykazovala vztah k úspěšnosti zachování sluchu. K významným faktorům ovlivňujícím výsledek patřila předoperační úroveň sluchu (p < 0,001) včetně typu odpovědi v BERA (p = 0,03). Užití intraoperační BERA bylo pro výsledek klíčové (p < 0,001). Úspěšnost zachování BERA však s výsledkem nekorelovala. Ke klíčovým faktorům ovlivňujícím uchování sluchu patřila úspěšná identifikace kochleárního nervu (p < 0,001), užití endoskopické techniky (p < 0,001), měkká konzistence tumoru (p = 0,05) a absence adhezí k neurovaskulárním strukturám (p < 0,001). Krvácivost ani přítomnost cystické komponenty úspěšnost uchování sluchu neovlivnily. Závěry: Mimo zkušenost chirurgického týmu patří k hlavním faktorům ovlivňujícím uchování sluchu velikost nádoru, úroveň sluchu před operací a možnost užití intraoperačního monitoringu sluchu. Klíčová je identifikace kochleární porce n. VIII. Z tohoto hlediska se jeví přínosné užití endoskopické techniky. K významným intraoperačním faktorům pak náleží měkká konzistence tumoru a absence adhezí k neurovaskulárním strukturám., Aim: The aim of the study was to conduct a comprehensive analysis of the factors influencing the success rate of hearing preservation in patients undergoing microsurgical removal of vestibular schwannoma (VS). Materials and methods: A total of 81 consecutive patients with serviceable hearing underwent surgical removal of unilateral vestibular schwannoma via retrosigmoid-transmeatal approach between 2008 and 2010. Patient, tumor and audiometric parameters together with intraoperative findings were prospectively analyzed. Data were statistically processed including a regression analysis of independent variables. Results: Serviceable hearing was preserved in 17 patients (useful hearing in nine and non-useful in eight cases). We observed improvement of hearing from non-useful to useful level in two patients. Average size of VS was smaller in the preserved hearing group (p < 0.001) where lower stage tumors prevailed (p = 0.001). Extent of internal auditory canal filling did not correlate with the result. Preoperative hearing level (p < 0.001) and the type of brainstem auditory evoked response (p = 0.03) arose as significant prognostic factors. Employment of intraoperative BERA was crucial for the outcome (p < 0.001) but success rate of its preservation was not correlated with the result. Identification of the cochlear nerve (p < 0.001), employment of endoscopic technique (p < 0.001), soft consistency of the tumor (p = 0.05) and the lack of adhesions to the neurovascular structures (p < 0.001) proved to be crucial factors affecting the preservation of hearing. Neither hypervascularity nor the presence of cystic component affected outcome. Conclusions: Apart from the experience of the surgical team, size of the tumor, preoperative hearing level and possibility of intraoperative hearing monitoring represented the main factors for hearing preservation. Early identification of the cochlear portion of the CN VIII is key. For this, the use of endoscopic technique seems to be beneficial. Soft consistency of tumor and lack of adhesions to the neurovascular structures represent crucial intraoperative factors. Protinádorová farmakoterapie je vedena dominantně parenterální formou, a proto je volba adekvátního žilního vstupu v onkologii zásadním tématem, nabývajícím na významu s rozšiřováním možností druhů žilních katétrů, které jsou k dispozici. Periferní žilní systém je navíc v některých případech při dlouhodobé léčbě výrazně zatěžován, neboť celá řada látek má nevyhovující vlastnosti pro periferní aplikaci – hlavně pH, osmolalitu a chemickou strukturu. Permanentní centrální venózní vstup nabízí šetření periferního žilního systému, je prevencí paravazace a lze jej využívat ve většině případů k odběrům krve. Kromě portů a tunelizovaných centrálních kanyl zažívá renesanci inzerce periferně implantovaných centrálních katétrů (PICC), které splňují kritéria permanentního centrálního vstupu. Uvádíme indikační kritéria pro implantaci jednotlivých druhů vstupů s přihlédnutím k vlastnostem pacienta, charakteru a délce podávané léčby. V rámci Společnosti pro porty a permanentní katétry jsme definovali několik oblastí onkologické péče, kdy můžeme preferovat inzerci PICC před ostatními druhy žilních katetrů., The choice of adequate venous access device is a key issue in oncology since anticancer drug therapy is administered via the parenteral route predominantly. An increasing importance of this topic has been observed with the expansion of various types of venous catheters available. In some cases, long-term therapy places a heavy burden on the peripheral venous system because many substances have unsatisfactory properties for peripheral administration – mainly the pH, osmolality and chemical structure. A long-term central venous access device offers a way to spare the peripheral venous system, prevents extravasation, and can be used for blood sampling in most cases. In addition to ports and tunneled venous access devices, there is a renaissance in insertion of peripherally inserted central catheters (PICCs) that meet the criteria for a permanent central venous access device. The indication criteria for the insertion of various kinds of devices are presented, taking into account the patient characteristics as well as the type and duration of the treatment administered. Bortezomib (Velcade) je lék ze skupiny inhibitorů proteasomu. Představuje zcela nový a jedinečný léčebný postup v léčbě mnohočetného myelomu (MM). Tato práce přináší retrospektivní analýzu léčebné účinnosti a nežádoucích účinků u nemocných s MM léčených v referenčních centrech CMG v České republice. Celkem bylo hodnoceno 424 nemocných MM léčených bortezomibem od června 2004 do června 2010. Medián délky sledování byl 12,4 měsíců. Průměrný věk nemocných byl 66 let (34,1–86,5). Jednalo se především o nemocné s relabujícím MM. Celkem 48,3 % (205/424) nemocných mělo bortezomib v rámci druhé léčebné linie. Léčebnou odpověď bylo možné hodnotit u 83,3 % (353/424) nemocných. Léčebné odpovědi (? 50% pokles MIg) bylo dosaženo u 52,7 % (186/353), z toho bylo u 14,2 % (50/353) nemocných dosaženo kompletní remise. Nejzávažnější toxicitou byla periferní neuropatie stupně 3 a vyšší u 14 % (49/358) nemocných. Trombocytopenie stupně 3 a vyšší byla u 28,8 % (103/357) nemocných. Infekční komplikace stupně 2 a vyšší byly u 33,6 % (121/359) nemocných. V průběhu léčby bortezomibem nedošlo v našem souboru nemocných ke zvýšení frekvence výskytu trombo embolických komplikací. Mimořádný přínos bortezomibu v klinické praxi je dán rychlým nástupem léčebné odpovědi a vysokým počtem dosažených kompletních remisí. Většina jeho nežádoucích účinků se zpravidla daří zvládat pomocí vhodných profylaktických opatření, případně krátkodobým přerušením a následným snížením dávky podle platných doporučení., Bortezomib (Velcade) is a representative of class of medicines called proteasome inhibitors. This drug represents quite new and unique medical treatment of the patients with multiple myeloma (MM). This article presents a retrospective analysis of effectiveness and adverse effects in patients with multiple myeloma treated at reference centers of CMG in the Czech Republic. A total 424 patients with multiple myeloma treated with bortezomib from June 2004 to June 2010 were evaluated. The median follow-up was 12,4 months. The mean age of patients was 66 year (34,1–86,5) and majority of them were patients with relapsed multiple myeloma. Of the total 424 patients, 205 patients had bortezomib as second line therapy (48,3 %). Assessment of terapeutic response was possible in 83,3 % of treated patients (353/424). Therapeutic response (defined as a ? 50 % decrease in the serum MIG) was achieved in 52,7 % (186/353). Among mentioned responses, complete response was obtained in 14,2 % (50/353). The most severe toxicity observed was peripheral neuropathy, grade 3 or higher was developed in 14 % (49/358). There was recorded a thrombocytopenia grade 3 or higher in 28,8 % (103/357), further occurance of infections grade 2 or higher in 33,6 % (121/359) and eventually we could observe increased number of thromboembolic disease as well. Rapid onset of therapeutic responses and high percentage of obtained complete responses are the main reasons of extraordinary clinical benefit of bortezomib.
