Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Objective: The study aimed to evaluate plasma adiponectin (ADP), adipocyte fatty acid binding protein (A-FABP) and fibroblast growth factor 21 (FGF 21) levels as potential predictors of severity of acute pancreatitis (AP) in day 4 after admission. Simultaneously, the classical proinflammatory makers were analysed as well. Settings: Department of Clinical Biochemistry and 2nd Department of Internal Medicine, Faculty Hospital Olomouc, I. P. Study design: The study was conducted in subjects with acute pancreatitis (n = 84, 37 females, 47 males). The analyses were performed in the groups according to the mild/severe classification of AP, and partly in the computed-tomography severity index (CTSI) score subgroups. Results: From adipokines, only FGF 21 tended to be higher in the severe AP subgroup in day 4. The multiple regression analysis revealed a positive association of A-FABP with procalcitonin (PCT), whilst FGF 21 was associated positively with ADP and negatively with C-reactive protein (CRP) in patients with the severe AP. The receiver-operator characteristics (ROC) analysis confirmed that not adipokines, but only CRP and interleukin 6 (IL-6) were suitable as potential predictors of the disease severity. The cut-off values were established for both parameters: 100 mg/L for CRP and 37 ng/L for IL-6, with negative predictive values (NPV) 96 % and 92 %, and positive predictive values (PPV) 39 % and 29 %, respectively. Conclusion: The role of ADP, A-FABP and FGF 21 has been limited in a prediction of the disease severity in day 4 after admission, while CRP and IL-6 might be useful to exclude a severe AP., Cíle studie: Studie byla zaměřena na vyhodnocení sérových hladin adiponektinu (ADP), adipocytárního proteinu vázajícího mastné kyseliny (A-FABP) a fibroblastového růstového faktoru 21 jako potenciálních prediktorů závažnosti akutní pankreatitidy (AP) 4. den po přijetí. Laboratorní markery byly měřeny v podskupinách podle klasifikace lehká/ těžká AP, a částečně také v podskupinách vytvořených na základě CTSI (computed-tomography severity index) skóre. Výsledky: Ze sledovaných adipokinů měl u pacientů s těžkou AP pouze FGF 21 tendenci k vyšším hladinám v den 4 po přijetí. Za pomoci mnohočetné regresní analýzy byla zjištěna pozitivní asociace A-FABP s prokalcitoninem (PCT), zatímco FGF 21 asocioval u těžké AP pozitivně s ADP a negativně s C-reaktivním proteinem (CRP). ROC (receiver-operator characteristics) analýza potvrdila, že pouze CRP a interleukin 6 (IL-6) jsou využitelné jako potenciální prediktory závažnosti onemocnění. Pro oba parametry byly stanoveny cut-off hodnoty: 100 mg/l pro CRP a 37 ng/l pro IL-6, s negativními prediktivními hodnotami (NPV) 96 % a 92 %, a s pozitivními prediktivními hodnotami (PPV) 39 % a 29 %. Cíle: Tato studie měla za cíl zjištění diagnostické přesnosti české verze souboru tří otázek ke stanovení diagnózy syndromu neklidných nohou (Restless Legs Syndrome, RLS), které se používají v epidemiologických studiích prováděných dotazníkovou cestou bez asistence investigátora. Originální anglická verze dotazníku byla publikována v roce 2003 Mezinárodní skupinou pro výzkum RLS (International RLS study group) společně s hlavními a vedlejšími diagnostickými kritérii pro klinické nebo telefonické stanovení diagnózy RLS. Soubor a metody: Sada otázek byla přeložena do češtiny a pak zaslepeně zpět do angličtiny, aby nedošlo k posunu významu. Ženy přicházející ke kontrole na Gynekologickou kliniku 1. LF UK a VFN v Praze v 34.–38. týdnu těhotenství byly požádány o vyplnění výše zmíněného dotazníku. Následně lékař vycvičený v diagnostice RLS pacientky kontaktoval na jimi zadaném telefonním čísle k standardnímu diagnostickému rozhovoru k ověření či vyloučení diagnózy RLS. Výsledky obou metod byly porovnávány a byla stanovena senzitivita, specificita, pozitivní a negativní prediktivní hodnoty. Výsledky: Dotazníkovou studii podstoupilo celkem 776 těhotných žen, ve věku 18–49 let. Celkem bylo validováno 199 dotazníků, u 65 osobní pohovor potvrdil diagnózu RLS. Pozitivita všech tří otázek má 89% specificitu, ale jen přibližně 78% pozitivní prediktivní hodnotu a senzitivitu. Za použití pouze první otázky je senzitivita 94 % a negativní prediktivní hodnota 96 %. Ze všech vyšetřených žen na všechny tři otázky odpovědělo pozitivně 28,0 % (95% konfidenční interval je 24,9–31,2 %). Závěr: Studie validovala českou verzi otázek a prokázala jejich relativně vysokou diagnostickou přesnost., Aims: The aim of this study was to estimate diagnostic accuracy of the Czech version of a self-administered minimum question set to diagnose the Restless Legs Syndrome (RLS). The wording of the three questions was published in 2003 by the International RLS study group together with the diagnostic criteria. Material and methods: The question set was translated into Czech, followed by blind translation back to English. Women in 36th–38th week of pregnancy were asked to complete the questionnaire. Subsequently, the women were contacted over a telephone by a physician trained in diagnosing RLS and a standard telephone interview to establish or exclude the diagnosis of RLS was performed. The results of both methods were compared to estimate sensitivity, specificity, positive and negative predictive values of the minimum question set. Results: We surveyed 776 pregnant women (18–49 years old) who came to a prenatal outpatient clinic to consult an obstetrician at the third trimester (36th–38th week of pregnancy). In total, 199 questionnaires were validated; personal interview confirmed the diagnosis of RLS in 65 of them. Positive response to all three questions yields specificity of 89% but only about 78% sensitivity and positive predictive value. Positive response to the first question only has sensitivity of 94% and negative predictive value of 96%. The prevalence of RLS during pregnancy estimated using the three-question questionnaire was 28% (95% confidence interval from 24.9 to 31.2%).
