Fyziologická gravidita je sprevádzaná zmenami v lokálnej a systémovej koagulácii a fibrinolýze. Hemostáza zasahuje do procesu implantácie blastocysty a placentácie. Udržiava optimálne prekrvenie placenty, zabezpečuje rýchle a adekvátne zrážanie počas pôrodu. Mnohé koagulačné proteíny sú exprimované na začiatku embryonálneho vývoja a hrajú úlohu mimo koaguláciu v proliferácii a diferenciácii buniek. Mechanizmus regulácie v placente nie je dostatočne chápaný. Poruchy hemostázy sa podieľajú na peripartálnom krvácaní, gestačnom tromboembolizme a považujú sa za rizikový faktor neúspešnej reprodukcie a tehotenských komplikácií., Physiological pregnancy is accompanied by changes in local and systemic coagulation and fibrinolysis. Haemostasis interferes with the implantation of the blastocyst and placentation. Maintain optimal blood flow placenta, ensures prompt and adequate clotting during the labour. Many coagulation proteins are expressed early in embryonic development and play roles outside of coagulation in cells proliferation and differentiation. Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. Cieľ: Cieľom práce bolo validizovať defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré defi nujúce charakteristiky sú sestrami považované za hlavné a ktoré za vedľajšie. Metódy: Na hodnotenie defi nujúcich charakteristík ošetrovateľskej diagnózy Defi cit vedomostí bol zvolený Fehringov model validity diagnostického obsahu. Výsledky: Za hlavné defi nujúce charakteristiky boli označené: „slovné vyjadrenie problému“ a „chybné, nepresné dodržiavanie inštrukcií“. Za vedľajšie defi nujúce charakteristiky boli označené: „chybné, nepresné výsledky testu“ a „neprimerané správanie“. Charakteristika „prehnané správanie“ dosiahla vážené skóre menej ako 0,5. Záver: Nie všetky defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí uvedené v klasifi kačnom systéme NANDA-International sú pre vybraný súbor sestier rovnako významné pre jej stanovenie., Aim: The aim of the study was to validate the nursing diagnosis Deficient Knowledge by a chosen sample of Slovak nurses – experts, i. e. to find out which defining characteristics are considered major and minor. Methods: The Fehring’s Diagnostic Content Validity Model was used to evaluate the defi ning characteristics of nursing diagnosis Deficient Knowledge. Results: Th e following defi ning characteristics were marked as major: “verbalization of the problem”, “inaccurate compliance with instructions”. The following characteristics were marked as minor: “poor and inaccurate performance in a test” and “inappropriate behaviour”. The defining characteristic “exaggerated behaviour” had the weighted ratio less than 0.5. Cieľ: Cieľom práce bolo zistiť, aký vplyv má bolesť na vybrané dimenzie ţivota pacientov v domácom prostredí, medzi ktoré patrí nálada, vzťah k iným ľuďom, radosť zo ţivota, spánok. Metodika: Súbor tvorilo 144 respondentov (91 ţien a 53 muţov) v podmienkach domáceho prostredia vo veku od 29 do 97 rokov v siedmich samosprávnych krajoch Slovenska. Zber údajov prebiehal v období november 2008 aţ január 2009. V rámci realizácie zberu údajov bol pouţitý štandardizovaný dotazník BPI (Brief Pain Inventory). Výsledky boli vyhodnotené jednorozmernou deskriptívnou štatistikou. Výsledky: Prítomnosť bolesti v skúmanej vzorke negatívne ovplyvňuje náladu, spánok a radosť zo ţivota viac u muţov ako u ţien. U ţien viac ovplyvňuje bolesť vzťah k iným ľuďom ako u muţov. Štatistickým spracovaním nebola zistená signifikantná súvislosť medzi premennými: nálada – pohlavie (p = 0,387), vzťah k iným ľuďom – pohlavie (p = 0,876), spánok – pohlavie (p = 0,065), radosť zo ţivota – pohlavie (p = 0,238). Záver: Bolesť výrazne ovplyvňuje vybrané dimenzie ţivota pacientov v domácom prostredí, čo vedie k širokej škále negatívnych emocionálnych reakcií a prejavov, ktoré môţu zintenzívňovať bolesť. Pravidelné hodnotenie vplyvu bolesti na jednotlivé dimenzie ţivota pacientov prispieva veľkou mierou taktieţ ku kvalitnej komunikácií medzi sestrou pracujúcou v domácom prostredí a pacientom zaloţenou na vzájomnej dôvere., Aim: The aim of our work was to determine the impact on pain in selected domains of life of patients in home care in terms of their mood, relationship to other people, joy of life and sleep. Methods: 144 respondents (91 women and 53 men aged from 29 to 97) in home care were randomly selected to make up the sample in 7 Slovak municipalities. The data collection was gathered from November 2008 to January 2009 using brief Pain Inventory. The results were evaluated by one-dimensional descriptive statistics. Results: The presence of pain among the respondents negatively influences the spirits, sleep and joy of life; more among men than women. Pain affects relationship to other people more among women than men. Statistical analysis showed no significant correlation between variables: spirits - sex (p = 0.387), relationship to other people – sex (p = 0.876), sleep – sex (p = 0.065), the joy of life - sex (p = 0.238). Conclusion: Pain has a significant impact in selected domains of patient`s life in home care leading to a wide range of negative emotions and behavior that could intensify the pain. This research paper presents the results of ovarian reserve estimation for 125 women with the Polycystic Ovary Syndrome (PCOS) who have undergone various methods of surgical treatment resection of the ovaries, thermokauterisation and drilling by laser (HoYag). Ovarian reserve was estimated according to the amount of antral follicles, level of follitropin and Müllerian inhibiting substance (MIS), also named antiMüllerian hormone (AMH). Blood flow in ovarian tissue was also examined after various methods of surgical treatment. Lichen sclerosus (LS) je onemocnění neznámé etiologie, postihující především postmenopauzální ženy. LS patří mezi nejčastější non-neoplastické epiteliální změny na vulvě, má typický makroskopický vzhled a klinicky je charakterizován intenzivním pruritem, případně dyspareunií. Mezi rizika LS patří tendence k jizvení a vznik dlaždicového karcinomu v terénu LS. Léčba LS je obyčejně dlouhodobá, opakovaná a jejím základem je lokální aplikace silného kortikosteroidu. Při adekvátní konzervativní léčbě je chirurgické řešení indikováno zřídka. Pacientky s LS je vhodné sledovat na specializovaném pracovišti., Lichen sclerosus (LS) is a disease of unknown ethiology, affecting mainly postmenopausal women. LS belongs to the most frequent non-neoplastic epithelial disorders of the vulva, it has a typical macroscopic pattern, and it is characterized with an intensive pruritus or dyspareunia. Patients with LS have a risk of scarring of external genitalia and risk of developing squamous cell carcinoma of the vulva. Treatment of LS is usually long-term, repeated, and it is based on local potent corticosteroids. Surgical treatment is rarely indicated in the management of LS. V loňském roce vyšel v Časopise zdravotnického práva a bioetiky článek Skalpel a duše (č. 1/2013, autorka Andrea Baršová), který se zabýval proměnami vztahu společnosti a práva k transsexualitě. O několik měsíců později přinesla světová média zprávu o „uznání třetího pohlaví“ v Německu. Novinka s obsahem citovaného článku souvisí. Stejná linie myšlení, která vedla k upuštění od nucených zásahů do těla u transsexuálů, má zde svoji paralelu v přístupu k intersexuálním lidem. Předkládaný příspěvek shrnuje dosavadní přístupy k intersexualitě a seznamuje s nejnovějším vývojem v Německu, zemi s výrazným hnutím intersexuálních lidí, a na mezinárodní úrovni. Malá změna v německém právním řádu, a sice možnost neuvést v matriční knize narození pohlaví dítěte, má zatím k uznání jiného pohlaví daleko. Zpráva Německé etické rady z roku 2012, která německé zákonodárce ke změně vedla, a probuzený zájem mezinárodních lidskoprávních organizací o toto téma jsou však zřetelnými signály nastupující paradigmatické změny v přístupu k intersexuálním lidem v naší kultuře., A year ago, the article by Andrea Baršová Skalpel a duše (Scalpel and the Soul), which addressed recent transformation of societal and legal attitudes towards transsexuals, was published by the Journal of Medical Law and Bioethics (1/2013). A few months later, media worldwide released news on the recognition of third sex in Germany. This development is linked to the published article. The same line of thinking, which led to abandonment of enforced medical interferences in the bodies of transsexuals (in order to recognise their gender change), has here its parallel in current attitudes towards intersex people. This article sums up existing approaches to intersex individuals and discusses newest developments in Germany, a country with strong intersex movement, and on the international level. A small change in the German law, namely an option to leave open the entry for sex in birth registers, is far from legal recognition of other sex as such. Cieľ: Príspevok má design prierezovej štúdie, s parciálnym cieľom testovať psychometrické vlastnosti nástroja kvality ţivota (Caregiver Quality of Life Index Cancer) opatrovateľov v starostlivosti o zomierajúcich z hľadiska formy poskytovanej starostlivosti. Metodika: Výskumný súbor tvorilo 263 neprofesionálnych opatrovateľov, poskytujúcich starostlivosť zomierajúcemu v podmienkach domácej, kombinovanej a paliatívnej/hospicovej starostlivosti v troch krajoch Slovenskej republiky (Prešovský, Ţilinský, Banskobystrický). Psychometrické vlastnosti nástroja pouţitého v našej štúdii boli testované na základe analýzy spoľahlivosti, korelačnej a faktorovej analýzy. Výsledky: Faktorovou analýzou sa v našom výskume podarilo potvrdiť 4-faktorovú štruktúru nástroja CQOLC (záťaţ/narušenie ţivotného štýlu; sociálna opora; pozitívna adaptácia; finančná situácia). Extrahované faktory na základe odpovedí respondentov v našom výskumnom súbore nie sú úplne identické s originálnou verziou škály CQOLC. Najlepšiu konštruktovú validitu a vnútornú konzistenciu škály CQOLC sme potvrdili u respondentov, ktorí poskytujú domácu starostlivosť zomierajúcemu príbuznému. Záver: Psychometrickou analýzou dotazníkov sme zistili, ţe reliabilita a konštruktová validita nami pouţitého nástroja je porovnateľná s originálnou verziou, napriek odchýlkam v lokalizácii niektorých poloţiek. Nástroj CQOLC je pouţiteľný na posúdenie kvality ţivota neprofesionálnych opatrovateľov v kontexte starostlivosti o zomierajúcich v domácom prostredí., Aim: The paper has the design of the cross-sectional study. The aim of the study was to test the psychometric properties of the quality of life (Caregiver Quality of Life Index of Cancer) the caregivers who provide care of the dying patients in terms of forms of care. Methods: The sample of our research consisted of 263 non-professional caregivers, providing home care of the dying, combined care and palliative care in three districts of the Slovak Republic (Prešovský, Ţilinský, Banskobystrický). Psychometric properties of the instrument used in our study were tested on the basis of the analysis of the reliability, of the correlation analysis and factor analysis. Results: Factor analysis of questionnaire CQOLC in our research confirmed the fourfactor structure of the instrument CQOLC (burden/lifestyle disruption; social support; a positive adaptation; the financial concerns). Extracted the factors on the basis of the answers of the respondents in our research are not completely identical with the original version CQOLC. The best construct validity and reliability of the CQOLC we have confirmed with the caregivers, who provide a home care of the dying patients. Conclusion: Psychometric analysis of the questionnaires we found that, the reliability and construct validity is comparable to the original version of CQOLC, despite variations in the location of some items.
