Cíl: Prezentovat neobvyklý případ 49leté ženy s pozitivní anamnézou metastazujícího maligního melanomu, u které se při 18F-FDG PET/CT vyšetření po léčbě zobrazil enchondrom v hlavičce fibuly napodobující solitární metastázu maligního melanomu. Metodika: Pacientka v rámci kontroly efektu léčby absolvovala celotělové PET/ CT zobrazení, a to 60 minut po intravenózní aplikaci radiofarmaka 18F-FDG o aktivitě 386 MBq. Akviziční čas PET skenu byl 2,5 minuty na 15 cm projekci, celkem 14 projekcí. Kontrastní (i.v. + per os kontrast) CT sken byl proveden ve venózní fázi. Výsledky: 18F-FDG PET/CT prokázalo osteolytické ložisko s jemným sklerotickým lemem v hlavičce levé fibuly s zvýšenou akumulaci 18F-FDG. Vzhledem k pozitivní anamnéze metastazujícího maligního melanomu bylo vysloveno podezření na solitární metastázu a doporučena biopsie. Následně pacientka podstoupila resekci hlavičky fibuly a histologické vyšetření zjistilo přítomnost enchondromu. Závěr: Enchondrom může vykazovat zvýšenou ložiskovou akumulací 18F-FDG v kosti, a tudíž má schopnost napodobovat solitární metastázu maligního melanomu při PET/CT zobrazení a tím způsobit potenciální obtíže při hodnocení nálezu., Aim: To present a rare case report of the fibular head enchondroma mimics a solitary malignant melanoma metastasis on 18F-FDG PET/CT imaging. Method: The 49-year-old woman with positive history of the metastatic malignant melanoma after treatment underwent 18F-FDG PET/CT. The whole body PET/CT scan was obtained 60 minutes after intravenous injection of 18F-FDG (386 MBq). The PET scan acquisition time was 2.5 minutes per bed position for whole body imaging and fourteen bed positions were necessary. Contrast-enhanced CT (venous phase) provided both full CT evaluation (including intravenous and oral contrast) and PET attenuation correction. Results: The examination revealed 18F-FDG avid bone lytic lesion with the mild sclerotic border in the left fibular head. The PET/CT finding was suspected for solitary melanoma metastasis. The patient underwent surgery and enchondroma was proven by histopathology. Conclusion: Benign enchondroma shows increased 18F-FDG uptake in bone lesion and can mimic solitary malignant melanoma metastasis on PET/CT imaging and can cause potential pitfall in the PET/ CT evaluation., Jiří Doležal, Martin Slanina, and Literatura
Cíl: Posoudit vlastní zkušenosti s prováděním PET/CT s fluorocholinem (18F-FCH-PET/CT) u karcinomu prostaty. Metodika: Byla provedena analýza indikací a výsledků 172 vyšetření 18F-FCH-PET/ CT u 139 mužů, průměrný věk nemocných byl 62,5 v rozpětí 48-81 let. Věnovali jsme se výskytu metastáz, vztahu hladiny PSA u biochemického relaps a přítomnosti nádorové tkáně po radikální léčbě a také porovnání bi-optického nálezu s nálezem PET/CT. Výsledky: U nemocných s hladinou PSA 2-5 ng/1 jsme nalezli metastázy nebo lokální rekurenci v 77,4 %, u hladin nad 5 ng/1 ve 100 %. Při porovnání nálezů biopsie s nálezy PET/CT jsme dosáhli senzitivity 96 %, specificity 83,3 %. Závěr: 18F-FCH-PET/CT je, jak ukazují naše výsledky, efektivním nástrojem v posuzování stagingu i restagingu karcinomu prostaty., Aim: To evaluate our own experience with fluorocholine 18F-FCH-PET/CT in prostatic carcinoma. Method: the analysis of the indications was performed in sample of 172 18F-FCH-PET/CT in 139 male patients, with mean age of 62.5 years in range 48-81 years. We evaluated number and site of metastases, the relation of PSA level in biochemical relaps with the presence of FCH accumulation in tumorous tissue and finally we correlate the findings of biopsy with results of PET/CT. Results: IN patients with PSA level 2-5 ng/1 we found in cases of biochemical relapse the accumulating tumorous tissue of local recurrence or metastases in 77.4%, in those patients with PSA level above 5 ng/1 in 100% of cases. In comparison of biopsies with PET/CT we found sensitivity 96% and specificity 83.3%. Cíl: posoudit možnosti hodnocení N-stagingu karcinomu prsu pomocí 18F-FDG-PET/CT. Metodika: V souboru 26 žen vyšetřených před operací prsu pomocí 18F-FDG-PET/CT byl porovnán výkon zobrazení pomocí dvou přístrojů se tříprstencovým detektorem PET (15 žen) a se čtyřprstencovým detektorem PET (11 žen), kdy ve druhé skupině bylo použito techniky rekonstrukce obrazu time-of-flight a point spread function a současně snížení dávky aplikovaného radiofarmaka na 2, 5 MBq/kg ve srovnání s 4 MBq/kg. Výsledky: V celém souboru bylo dosaženo pro hodnocení uzlinových metastáz v axile senzitivity 85%, specificity 65%, u nemocných vyšetřených pomocí zařízení s time-of-flight a point spread function bylo dosaženo významně lepších výsledků se senzitivitou 100 % a specificitou 80 %. Závěr: Použitím rekonstrukce obrazu time-of-flight a point spread function dosahuje lepších výsledků v hodnocení uzlinových axilárních metastáz i za současného snížení dávky aplikovaného radiofarmaka na 2,5 MBq/kg., Aim: to evaluate the possibilities of the N--staging assesment of the breast carcinoma using 18F-FDG-PET/CT. Method: In 26 women examined before surgery using 18F-FDG-PET/CT, the performance was compared when the three-ring PET system was used (15 women) and four-ring PET system with image reconstruction using time-ofiflight and point spread function even with the radiopharmaceuticals dose reduction to 2.5 MBq/kg from 4 MBq/kg. Results: The sensitivity to detect axillary lmph nodes metastases reached 85%, specificity of 65% in the whole sample, in patients examined by the system enabling the reconstruction time-of-flight and point spread function, the sensitivity was improved significantly to 100% with specificity of 85%. Difuzní B-velkobuněčný lymfom (DLBCL) představuje agresivní lymfoidní neoplazii, která patří díky moderním léčebným přístupům k vysoce kurabilním nádorovým onemocněním. Ale i v éře imunochemoterapie existuje významná skupina pacientů, kteří jsou primárně refrakterní na podávanou terapii, nebo po jejím ukončení relabují. Výsledek časného PET/CT vyšetření (ePET/CT) v průběhu terapie může být cenným ukazatelem citlivosti nádoru na podávanou léčbu. Možnosti hodnocení ePET/CT jsou dnes poměrně široké a zahrnují jak vizuální popis, tak také různé metody semikvantitivního posouzení léčebné odpovědi. I přes množství dosud získaných dat a řadu slibných předběžných výsledků zůstává role ePET/CT stále předmětem klinického výzkumu a jeho výsledek zatím nemůže být standardně používaným vodítkem pro rozhodování o další léčbě nemocného., Diffuse large B-cell lymphoma (DLBCL) is an aggressive lymphoid neoplasia, and for modern therapeutic approaches is a highly curable cancer. In the era of immunochemotherapy there is still a significant group of patients who are primarily refractory to a therapy or relapse after treatment. Result early PET/CT (ePET/CT) during therapy can be a valuable indicator of tumor sensitivity to a given treatment. The assessment of ePET/CT today are quite broad and include both visual description, as well as different methods semiquantitative assessment of treatment response.
