Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Metodika: Retrospektivní monocentrická observační studie sledující výskyt a charakteristiky infekční endokarditidy v definované spádové oblasti za roky 2009–2013. Zařazeni byli pouze pacienti s prokázanou a léčenou infekční endokarditidou a pacienti, u nichž byla diagnóza stanovena až pitvou. Výsledky: Ve sledovaném období bylo zachyceno celkem 39 případů infekční endokarditidy, což odpovídá průměrné incidenci 7,8 případů na 100 000 obyvatel ročně. Medián věku pacientů v době diagnózy byl 67 let. V souboru nemocných výrazně převažovali muži, kteří byli postiženi 3krát častěji než ženy. Přibližně stejně četně byla postižena mitrální (36 %) a aortální (33 %) chlopeň a elektroda kardiostimulačního systému (31 %). Ve srovnání s referenční prací bylo zachyceno více infekcí u pacientů s kardiostimulátory. Dvě třetiny infekcí byly způsobeny stafylokoky, což je proti pilotní studii výrazný nárůst. Naopak mortalita se podstatně neměnila. Závěr: Charakteristiky infekční endokarditidy se vyvíjejí, hospitalizační mortalita pacientů s IE je vysoká, kolem 30 %. Samo onemocnění IE je z hlediska roční úmrtnosti špatným prognostickým faktorem. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. Úvod: Kardiovaskulární (KV) onemocnění patří celosvětově k hlavním příčinám úmrtí. Časné odhalení rizikových faktorů a zvýšeného celkového KV rizika může být významným pomocníkem při snižování morbidity i mortality. Za optimální věk pro hodnocení KV rizika v populaci se doporučuje věk 40 let u mužů a 50 let u žen. V rámci prezentovaného průřezového šetření byl zhodnocen KV rizikový profil osob v těchto věkových kategoriích při vyšetření všeobecným praktickým lékařem. Metodika: Do průzkumu bylo zařazeno 1 812 osob, z toho mužů v průměrném věku 40 a žen v průměrném věku 50 let. U každého vyšetřeného byla zjištěna a do protokolu studie zaznamenána rodinná, osobní a farmakologická anamnéza, abúzus, fyzická aktivita. Byly změřeny základní antropometrické údaje, tepová frekvence, krevní tlak, zjištěny hodnoty sérového lipidogramu a glykemie. Vyšetřující lékaři zaznamenávali přítomnost nově zjištěných rizik KV onemocnění a stanovili celkové KV riziko dle tabulek SCORE. Bylo hodnoceno procento nemocných s již existujícím sledovaným onemocněním, procento zastoupení těch, kteří měli dobře kontrolované rizikové faktory, a dále procento pacientů, kterým byl nově zjištěn některý ze sledovaných rizikových faktorů. Hypotéza vztahu mezi pozitivní rodinnou anamnézou a výskytem onemocnění byla testována za použití Pearsonova testu. Výsledky: Bylo zařazeno 961 mužů průměrného věku 42,9 ? 4,7 let a 851 žen průměrného věku 51,2 ? 3,6 let. V pásmu nadváhy se nacházelo 49 % mužů a 31 % žen, obézních bylo 32 % mužů a 31 % žen. Aktivně kouřilo 36 % mužů a 22 % žen. Prevalence diabetu 2. typu byla shodně 11 % u obou pohlaví. Arteriální hypertenze byla zjištěna u 43 % mužů a 45 % žen a dyslipidemie u 39 % mužů a 41 % žen. Farmakologicky léčeno pro kteroukoli z uvedených chorob bylo 48 % osob, z nich pouze 8 % dosahovalo dobré kontroly všech identifikovaných rizik. Nově byl zjištěn diabetes u 3 % mužů i žen, arteriální hypertenze u 7 % mužů a 5 % žen a dyslipidemie u 20 % mužů i žen. 62 % mužů a 65 % žen doporučili participující lékaři nefarmakologickou léčbu. U 53 % mužů a 51 % žen byla zahájena nová farmakoterapie. U mužů i u žen šlo nejčastěji o antihypertenzní medikaci ze skupiny ACE-inhibitorů (29 % u mužů a 24 % u žen) a hypolipidemikum ze skupiny statinů (29 % u mužů a 27 % u žen). Z analýzy vztahu mezi pozitivní rodinnou anamnézou a výskytem onemocnění u dané osoby bylo zjištěno, že tato souvislost existuje pro arteriální hypertenzi, diabetes mellitus 2. typu i dyslipidemii. Závěr: Provedené šetření dokumentuje vysokou prevalenci rizikových faktorů kardiovaskulárních onemocnění v populaci 40letých mužů a 50letých žen. Vysoké zastoupení nově zjištěných rizikových faktorů i nadpoloviční podíl osob, u nichž byla v rámci provedeného šetření zahájena farmakoterapie, jasně dokazují nezbytnost časného screeningu rizikových faktorů. Genetická determinace jednotlivých kardiovaskulárních rizikových faktorů odrážející se v rodinné anamnéze je významnou komponentou celkového rizika oběhových onemocnění, a musí být aktivně zjišťována a zohledňována při úvaze o volbě intenzity preventivních strategií., Background: Cardiovascular (CV) disease belongs to the most important mortality causes worldwide. Early identification of risk factors and increased CV risk may help decrease of morbidity and mortality. An optimal age for CV risk factors screening ages of 40 in males and of 50 in females have been identified. CV risk profile of persons in these age categories has been examined by general practitioners and recorded for the purposes of this cross-sectional survey. Methods: 1812 persons, males at average age of 40 years and females at average age of 50 years, were included into the survey. In each of the examined family and personal history including pharmacological were recorded into the study protocol as well as abusus and physical activity patterns. Basic anthropometrical parameters, e.g. heart rate, blood pressure, body mass index and laboratory measures including blood lipids and glycaemia. The participating physicians recorded newly identified risk factors CV disease and determined the global CVD risk according to the SCORE charts. Percentage of those already treated for any of the main CVD risk factors and, among these, also attaining the treatment goals. The percentage of patients with a newly identified CV risk factor was calculated. We also tested the hypothesis of a relationship between positive family history of any of the followed risk factors and its risk in the examined probands using Pearson´s test. Results: 961 males, average age of 42.9 ? 4.7 years, and 851 females , average age of 51.2 ? 3.6 years, were enrolled into the study. 49% of males and 31% of females were overweight and 32% men and 31% of women were obese. There were 36% of smokers among men and 22% among women. The prevalence of type 2 diabetes was 11% in males and also in females. Arterial hypertension was diagnosed in 43% of males and 45% of females while dyslipidemia was present in 39% of males and 41% of females. Pharmacological treatment of any of the above mentioned diseases was used in 48% of the probands, however, only 7% of them attained treatment goals of blood pressure, LDL-cholesterol and glycaemia. Type 2 diabetes was newly identified in 3% of both males and females, arterial hypertension in 8% of males and 5% of females and dyslipidemia was newly detected in 20% of probands of both genders. Non-pharmacological treatment was recommended to 62% of male and to 65% of female participants, respectively. Pharmacological treatment was initiated in 53% of males and 51% of females. In both genders this was mostly antihypertensive treatment with ACE inhibitors (29% of males and 24% of females) and lipid lowering therapy with a statin (29% of males, 27% females). The analysis of relationship between the positive family history of any of the followed diseases and their presence in the probands examined revealed significant increases of the risk for arterial hypertension, type 2 diabetes mellitus and dyslipidemia. Conclusion: The survey following CV risk profile in a cohort of 40 years old men and 50 years old women showed high prevalence of CV risk factors in these age categories in the Czech probands. The high frequency of modifiable risk factors and the need to initiate pharmacological treatment in more than one half of the examined population documents the need of early detection of risk. Cíl: Lehké hypoglykemie ovlivňují zvládání diabetu a produktivitu pacientů i kvalitu jejich života. Údaje o výskytu hypoglykemií v běžném životě pacientů s diabetem 2. typu léčených inzulinovými analogy přitom nejsou dostatečné. Cílem průzkumu provedeného v České republice v rámci mezinárodního projektu GAPP2™ (Global Attitudes of Patient and Physicians) bylo ověřit incidenci lehkých hypoglykemií a jejich důsledky u pacientů s diabetem 2. typu léčených inzulinovými analogy. Metodika: Projekt GAPP2™ – Globální přístupy pacientů a lékařů (Global Attitudes of Patients and Physicians) je mezinárodní průřezovou studií provedenou on-line prostřednictvím internetu formou dotazníku u pacientů s diabetem 2. typu léčených inzulinovými analogy a u lékařů, kteří tyto pacienty ošetřují. Průzkum proběhl ve dvou vlnách celkem v 17 zemích světa. Do první vlny, která byla dokončena počátkem roku 2012, bylo zahrnuto 6 zemí, do druhé, dokončené v roce 2014, pak dalších 11 zemí včetně České republiky. Průzkum měl za cíl získat data o pohledech pacientů a lékařů na některé aspekty inzulinové léčby diabetu 2. typu a o přetrvávajících problémech v této oblasti v reálné každodenní praxi. V jedné části výzkumu byly sledovány lehké hypoglykemie při léčbě inzulinovými analogy. Byla zkoumána četnost výskytu lehkých hypoglykemií, reakce pacientů i lékařů na tyto hypoglykemie (úprava inzulinové léčby, selfmonitoring glykemie) a dále byl sledován dopad lehkých hypoglykemií na kvalitu života pacientů z pohledu pacientů i zdravotníků. Výsledky: Výsledky průzkumu ukázaly, že lehké hypoglykemie jsou u pacientů léčených inzulinovými analogy v ČR běžné a že pacienti v ČR se hypoglykemií obávají více a cítí se jimi v běžném životě více omezeni než pacienti v ostatních zemích průzkumu GAPP2™. Pacienti se snaží vyhýbat nočním hypoglykemiím i za cenu snížení nebo vynechání předepsané dávky inzulinu. Dále z výzkumu vyplývá, že lékaři nejsou o frekvenci a závažnosti hypoglykemií ze strany pacientů dostatečně informováni. Závěr: Lehké hypoglykemie jsou v ČR stejně běžné jako v ostatních zemích průzkumu GAPP2™, čeští pacienti se jich však obávají více, přičemž tento fakt zůstává často zdravotníkům skryt. Zvýšená obava pacientů z hypoglykemie ukazuje nepokrytý prostor v rámci edukace, který je třeba věnovat poučení, jak hypoglykemiím předcházet a jak je zvládat. Zvláštní důraz je přitom nutno klást na noční hypoglykemie. Zdravotníci by měli problematiku hypoglykemií s pacienty více probírat. Je důležité motivovat pacienty, aby zdravotníky o frekvenci a závažnosti hypoglykemií dostatečně informovali., Objective: Mild hypoglycaemia affects diabetes management, productivity and quality of life of patients. Data are scarce about the incidence of mild hypoglycaemia in daily life of type 2 diabetic patients treated with insulin analogues. The aim was to focus on the incidence of mild hypoglycaemia and its consequences in the international GAPP2™ survey (Global Attitudes of Patient and Physicians) conducted also in the Czech Republic. Methods: The GAPP2 ™ project – Global Attitudes of Patients and Physicians is an international cross-sectional study conducted online via Internet in a questionnaire form dedicated to type 2 diabetic patients treated with insulin analogues and physicians who treat these patients. The survey was realized in two steps including 17 countries. The first step was completed by six countries in 2012. The second step was terminated by additional eleven countries including the Czech Republic in 2014. The survey was designed to obtain data on some aspects of insulin therapy and persistent issues in daily practice from the views of patients and physicians. One part of survey was dedicated to mild hypoglycaemia occurring in treatment with insulin analogues. The incidence of mild hypoglycaemia and the reaction of patients and physicians on hypoglycaemia (including modification of therapy, self-monitoring) were investigated. Moreover, there was investigated the impact of mild hypoglycaemia on quality of life from the view of patients and physicians. Results: The results of the survey have shown that mild hypoglycaemia is common in patients with insulin analogues in the Czech Republic. In addition, patients in the Czech Republic are more afraid from hypoglycaemia and feel more limited in daily life in comparison to other countries in GAPP2 ™ survey. Patients try to avoid nocturnal hypoglycaemia through reduction or omission of prescribed dose of insulin. Furthermore, research has observed that physicians are poorly informed by patients about incidence and severity of hypoglycaemia. Conclusion: The incidence of mild hypoglycaemia is in the Czech Republic as common as in other countries involved in the GAPP2 ™ survey. However, the Czech patients are more afraid and this fact is often hidden before physicians. Increased fear of hypoglycaemia suggests inadequate education in terms of how to avoid and manage hypoglycaemia. Particular emphasis should be placed on nocturnal hypoglycaemia. Physicians should discuss more with patients about issue of hypoglycaemia. It is important to motivate patients to share the information with physicians about incidence and severity of hypoglycaemia., and Martin Prázný
V letech 1959–2008 počet zhoubných novotvarů centrální nervové soustavy (ZN CNS) z dat registru nádorů ČR dosáhl 26 548, z toho 14 263 (53,7 %) u mužů a 12 285 (46,3 %) u žen, v roce 2008 představující u mužů 412 (8,1/100 tis.), u žen 418 (7,9/100 tis.) onemocnění. Jejich prevalence v letech 1989–2005 vzrostla z 1169 na 2 565 žijících případů s očekávaným počtem 3 430 v roce 2015. Z 962 vícečetných ZN CNS, evidovaných v letech 1976–2005, bylo 150 primárních onemocnění u mužů a 162 u žen, 327 následných ZN CNS u mužů a 323 u žen tj. 5,1 % ze všech nově hlášených u mužů a 6,2 % u žen. Za toto období vzrostl v pětiletých intervalech počet primárních případů dvakrát, následných téměř třicetkrát. U primárních a následných ZN CNS převažovaly ve věku do 49 let ženy nad muži, v 50–69 letech muži nad ženami. Ze 14 krajů se vyskytlo 47 % ZN CNS ve čtyřech krajích (Moravskoslezský, Jihomoravský, Praha, Středočeský). Pouze 6,2 % multiplicit bylo evidováno v časných a 7,4 % v pokročilých klinických stadiích; 86,4 % stadií nebylo nahlášeno. Průměrný interval mezi 150 primárními ZN CNS a 170 jinými následnými novotvary u mužů byl pět let, mezi 162 primárními a 170 následnými u žen byl šest let, ze kterých byly u obou pohlaví nejpočetnější následné karcinomy trávicího ústrojí a kůže. Před 327 následnými ZN CNS bylo u mužů 390 primárních novotvarů, u žen před 323 následnými 362 primárních, ze kterých byly nejpočetnější karcinomy kůže, rodidel, prsů, prostaty a trávicího ústrojí. V říjnu 2007 žilo v době analýzy z 962 ZN CNS jen 91 a zemřelo 871 nemocných s převahou žijících žen nad muži., The number of malignant neoplasms of central nervous system (MN CNS), based in the Czech Cancer Registry, reached 26,548 diseases, of which there were 14,263 (53.7 %) of males and 12,285 (46.3 %) of females in 1959–2008, presented 412 (8.1 per 100,000 males) and 418 (7.9 per 100,000 females) diseases in 2008. Their prevalence increased from 1,169 to 2,565 survived cases between 1989–2005, with 3,430 cases expected in 2015. A total of 962 registered multiple MN CNS there were 150 primary diseases in males and 162 in females, 327 subsequent MN CNS in males and 323 in females, i. e. 5.1 % in males and 6.2 % in females of all newly registered MN CNS in period 1976–2005. In this period the number of primary cases increased twice, of subsequent ones nearly 30 times during five periods. The primary and subsequent MN CNS prevailed in the age group till 49 years in females over males, in the age group 50–69 years in males over females. A total of 14 Czech regions there were distributed 47 % of all multiple MN CNS in the four regions (Northern and Southern Moravia, Prague, Central Bohemia). Only 6.2 % multiplicities were registered in early and 7.4 % in advanced clinical stages; 86.4 % stages were unregistered. The average interval between 150 primary MN CNS and 170 other subsequent neoplasms was five years in males, between 162 primary and 170 subsequent cases six years in females, of which the most frequent were following digestive and skin carcinomas in both sex. In 327 subsequent MN CNS preceded 390 primary cancers in males, in 323 subsequent were 362 primary cases in females, of which the most frequent were carcinomas of skin, female genital and breast, prostate and digestive tract.
