Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. V letech 1959–2006 bylo u českých mužů evidováno 81 884 nových karcinomů prostaty (KP) s nárůstem z 12,3 na 96,1 na 100 tis. mužů. Úmrtnost se zvýšila z 21,3 na 27,2 na 100 tis. mužů. Podíl KP ze všech nádorů se zvýšil u incidence ze 7,1 % na 13,2 %, u mortality z 6,7 % na 8,9 %. V letech 1989–2005 vzrostla prevalence KP z 98 na 408,5 na 100 tisíc mužů a dosáhla téměř 20 500 žijících. Z 11 183 nemocných s vícečetnými KP bylo evidováno 4 608 primárních a 6 575 následných případů, představujících 17,4 % ze všech evidovaných KP v letech 1976–2005. Z primárních KP bylo léčeno 89,7 % duplicit a 10,3 % multiplicit. Počet primárních KP vzrostl na 282 v roce 1996 a pak klesal, následných KP trvale rostl na 705 v roce 2005. Téměř polovina KP se vyskytla ve věku 70–79 let a třetina ve věku 50–69 let. Poměr 1 115 (21,3 %) synchronních a 4 121 (78,7 %) metachronních následných novotvarů byl 1:3,7 s průměrnou dobou vzniku následného novotvaru 4,1 roku. Z 5 236 následných novotvarů po primárních KP byly nepočetnější nádory kůže 28,6 %, trávicí 26,7 %, močové 17,8 % a respirační 10,9 % soustavy. Z 5 613 novotvarů před KP jako druhou diagnózou, bylo 35,3 % nádorů kůže, 21 % trávicí, 19,6 % močové a 8,7 % respirační soustavy. Z nemocných s primárním KP k 17. 10. 2007 žilo 22,4 % a zemřelo 77,6 %, s následným KP žilo 31,6 % a zemřelo 68,4 % mužů. Vzhledem k výskytu KP ve spojení s dalšími novotvary není u české populace zatím dostatečná intervence ke zdravému životnímu stylu také vzhledem k očekávanému počtu 42 tisíc mužů, žijících s KP v roce 2015., In Czech men there were registered 81,884 new prostate cancers (PC) during 1959–2006, increased from 12.3 to 96.1 per 100,000 men. The mortality increased from 21.3 to 27.2 per 100,000 men. The share of PC of all cancers increased from 7,1 % to 13.2 % in incidence and from 6.7 % to 8.9 % in mortality. The prevalence of PC increased from 98 to 408.5 per 100,000 men in 1989–2005 and reached nearly 20,500 survived. Of 11,183 patients with PC were recorded 4,608 primary and 6,575 subsequent cases, represented 17.4 % of all registered PC in 1976–2005. There were treated 89.7 % duplicities and 10.3 % multiplicities of all primary PC. The most frequent from 5,236 subsequent neoplasms after primary PC there were 28,6 % cancers of skin, 26,7 % gastrointestinal cancers, 17,8 % of urinary and 10,9 % in respiratory tract. The most frequent from 5,613 neoplasms before PC as the second diagnosis there were 35,3 % cancers of skin, 21 % gastrointestinal, 19,6 % urinary and 8,7 % respiratory tract. From patients with primary PC survived 22,4 % and died 77,6 %, from subsequent PC survived 31,6 % and died 68,4 % men up to 17. 10. 2007. Z 1,043 milionu nových nádorů děložního hrdla, těla a vaječníků ve světě v roce 2008 uvádí databáze Globocan 2008 (IARC) 31 % ve vyspělých a 69 % v méně vyspělých zemích; z 489 tisíc zemřelých uvádí 27 % ve vyspělých a 73 % v méně vyspělých zemích. Do roku 2030 mohou nová onemocnění ve světě vzrůst o 550 tisíc, tj. 53 %, a úmrtí o 303 tisíc, tj. 62 %. V méně vyspělých zemích může být nárůst o 65 % na 1,19 milionu nových případů, o 80 % na 639 tisíc úmrtí, ve vyspělých zemích o 19 % na 382 tisíc nových případů, o 29 % na 168 tisíc úmrtí. Z odhadu pětileté prevalence 3,203 milionu případů ve světě se v roce 2008 vyskytlo 67 % v méně vyspělých a 33 % ve vyspělých zemích včetně 677 tisíc v Evropě. Porovnáním hrubé incidence 40 zemí Evropy byly ženy ČR první u nádoru děložního těla, deváté u nádorů vaječníků a jedenácté u nádoru děložního hrdla. Of the estimated five-year prevalence of 3.203 million cases worldwide there were 67 % in less, and 33 % in more developed countries which included Europe with 677 thousand cases in 2008. On comparison of crude incidence rates of 40 European countries, Czech females ranked alarming first in uterine, ninth in ovarian and eleventh in cervical cancer. Česká republika patří celosvětově mezi země s nejvyšším výskytem kolorektálního karcinomu (KRK), jehož incidence však mezi českými kraji není distribuována rovnoměrně. Ve všech epidemiologických ukazatelích (incidence, mortalita i prevalence) tohoto maligního onemocnění dominuje Plzeňský kraj. Vzhledem k prodlužování průměrné délky života vzrostl v posledním desetiletí podíl KRK jako druhého metachronního primárního maligního onemocnění. V roce 2000 byl zahájen Národní program screeningu KRK, který t. č. nabízí asymptomatickým jedincům starším 50 let možnost výběru ze dvou screeningových metod (testy na okultní krvácení a primární screeningová kolonoskopie). Od roku 2006 bylo v rámci screeningu kolonoskopicky vyšetřeno 57 255 osob, u 17 760 (31,0 %) z nich byly diagnostikovány a odstraněny adenomové polypy a u 2 665 (4,7 %) byl nalezen karcinom., The Czech Republic belongs to the countries with highest colorectal cancer (CRC) incidence worldwide which differs in regions of this country. The Pilsner region stands out in all epidemiologic features (incidence, mortality, prevalence). The proportion of CRC as the second metachronous primary malignant disease has increased in past ten years, due to the elongation of life expectancy. The National colorectal cancer screening programme was launched in year 2000. Nowadays two screening methods (fecal occult glood tests and primary screening colonoscopy) are offered to asymptomatic individuals aged over 57 255 people have been examined by colonoscopy since year 2006.
