We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Cieľ: Cieľom práce bolo zistiť výskyt komplikácií v závislosti od veku a pohlavia pacientov, od dĺžky zavedenia stimulačnej elektródy, od zvolenej cesty prístupu zavedenia stimulácie u pacientov hospitalizovaných na Oddelení arytmií a koronárnej jednotky v Univerzitnej nemocnici Martin. Metodika: Pre zber potrebných údajov sme použili obsahovú analýzu zdravotnej dokumentácie, nemocničný informačný systém, pozorovanie na základe vypracovaného záznamového hárku. Údaje sme analyzovali pomocou deskriptívnej a induktívnej štatistiky. V sledovanom súbore bolo 38 pacientov od 58 rokov do 93 rokov. Pacienti boli akútne prijatí na Jednotku intenzívnej starostlivosti za účelom realizácie dočasnej kardiostimulácie, ktorá bola užívaná v priemere 5,28 ± 2,33 dní. Výsledky: Na základe spracovania údajov sme u 17 pacientov zistili výskyt komplikácií, pričom najčastejšou komplikáciou bola lokálna infekcia. Štatistickou analýzou sme zaznamenali významné vzťahy medzi dĺžkou zavedenia stimulačnej elektródy a výskytom lokálnej infekcie, kde nám vznikla silná signifikantná korelácia r(S) = 0,518 na hladine významnosti p = 0,001 a rovnako pri výskyte hematómu sme zaznamenali koreláciu r(S) = 0,329 na hladine významnosti p = 0,023. Záver: Analýzou získaných údajov sme identifikovali najčastejšie vyskytujúce sa komplikácie v spojitosti s dočasnou kardiostimuláciou. Najčastejšie sme zaznamenali výskyt lokálnej infekcie, ktorý súvisí s následnou ošetrovateľskou starostlivosťou, ale aj s významnými prediktormi, ktoré môžu do značnej miery ovplyvniť ich vývoj., Aim: The aim of our study was to determinate the incidence of complications depending on the age and gender of patients, duration of implantation of stimulation electrode, of selected access point of implementation stimulation in patients hospitalized at the Arrhythmia unit and Coronary care unit in University hospital in Martin. Methods: For the collection of required data we have used content analysis of medical records, hospital information system, surveillance based on own designed sheet. The data were analysed using descriptive and inductive statistic. The sample consisted of 38 patients in the range from 58 to 93 years of age. Patients were acutely admitted to ICU with the aim of realization of temporary cardiac pacing, which lasted 5.28 ± 2.33 days in average. Results: On the basis of processed data, we found incidence of complications in 17 patients, where the most frequent complication was local infection. Using statistical analysis we found a significant relationship between duration of implementation of stimulation electrode and incidence of local infection, which resulted in strong significant correlation (r(S) = 0.518, p = 0.001) and similarly, we noted correlation in the incidence of haematoma (r(S) = 0.329, p = 0.023). Conclusion: Using analysis of the obtained data we have identified the most frequently occurring complications in conjunction with the temporary cardiac pacing. Lyellův syndrom (syndrom toxické epidermální nekrolýzy) je raritně se vyskytující onemocnění, které poprvé popsal skotský lékař Alan Lyell v roce 1956. Je charakterizováno rozsáhlou kožní a slizniční nekrolýzou postihující minimálně 30 % tělesného povrchu a systémovými projevy. Podle četnosti výskytu se jedná o extrémně vzácnou nozologickou jednotku s incidencí 0,5–1,5/1 milion obyvatel a rok. Leyllův syndrom je považován za toxoalergickou reakci, spouštěčem je nejčastěji některé léčivo. Syndrom je spojen s vysokou letalitou (knižně udávanou v rozmezí 30–90 %). Neodkladná lokální i celková léčba na jednotkách intenzivní péče nebo na specializovaném pracovišti může zlepšit celkově špatnou prognózu pacientů. V naší kazuistice popisujeme vzácný případ pacienta s rozvojem Lyellova syndromu po práci s běžným fungicidním prostředkem, který je častou součástí vybavení zahrádkářů a zemědělců. Klíčová slova: bis-dithiokarbamidany – Leyllův syndrom – toxická epidermální nekrolýza, Leyll’s syndrome (syndrome of toxic epidermal necrolysis) is a rare disease, firstly described by Scottish doctor of medicine Allan Lyell in 1956. It is characterized by huge skin and mucosa necrolysis, which affects at least 30 % of body surface, and systemic symptoms. According to the frequency of the occurrence it is an extremely rare condition, with an incidence of 0.5–2 cases per million residents per year. Leyll’s syndrome is considered as a toxoallergic reaction, triggered mostly by some medication and it is associated with a very high mortality rate (in the literature reported between 30 to 90 %). Adequate and timely local and systemic treatment at the Intensive Care Unit or at the specialized clinic can improve the overall poor prognosis of the patients. In our case report we describe a very rare case of the Lyell’s syndrome after exposure to the antifungal organosulfur compound, which is widely used by the homegardners and farmers. Úvod: Cílem studie bylo hodnocení významu Pneumonia Severity Index (PSI) pro predikci 30denní mortality pacientů s komunitní pneumonií (CAP) hospitalizovaných na jednotce intenzivní pneumologické péče (JIP). U pacientů byly hodnoceny komorbidity, komplikace, fyzikální, laboratorní, RTG a mikrobiologické nálezy a jejich vztah k prognóze nemocných. Pacienti a metody: Do studie bylo zařazeno 197 pacientů hospitalizovaných pro CAP na JIP Kliniky plicních nemocí a TBC LF UP a FN Olomouc v letech 2008–2012. U všech pacientů byla hodnocena riziková kritéria dle systému PSI. Výsledky: V celém souboru pacientů s CAP byla průměrná hodnota PSI 115,4 ? SD 30,4 bodů. Z celkového počtu 197 pacientů zemřelo 29 nemocných (14,7 %). Při srovnání skupin zemřelých a přeživších pacientů byly prokázány statisticky významné rozdíly v PSI (průměr ? SD: 137,4; 26,1 vs 111,7 ? 29,6 bodů; p < 0,0001), věku pacientů (průměr ? SD: 76,3 ? 12,9 vs 65,5 ? 14,7 let; p < 0,0001) a výskytu onemocnění srdce (86,2 % vs 67,3 %; p = 0,04) s nejvyšším podílem ICHS (58,6 % vs 38,7 %; p = 0,04). Dále při hodnocení fyzikálních a laboratorních nálezů byla prokázána u zemřelých pacientů oproti přeživším významně vyšší frekvence výskytu tachykardie nad 90/min (51,7 % vs 27,4 %; p = 0,01), tachypnoe > 30/min (37,9 % vs 13,7 %; p = 0,001) a acidózy s pH < 7,35 (27,6 % vs 8,9 %; p = 0,004). Korelační analýza neprokázala statisticky významnou závislost mezi PSI a délkou hospitalizace u přeživších pacientů. U pacientů s infekcí Staphylococcus sp. a Klebsiella pneumoniae byla zjištěna statisticky signifikantně delší doba hospitalizace. Hodnocení souvislosti ostatních sledovaných komorbidit, fyzikálních, laboratorních nálezů a patogenů s mortalitou neprokázalo významné rozdíly. Závěr: Naše studie prokázala, že PSI je významným prediktorem 30denní mortality pacientů s CAP na JIP, ale nekoreluje s délkou hospitalizace ani s nálezem jednotlivých patogenů. Nezávislými negativními prognostickými faktory u pacientů s CAP jsou věk, výskyt nemocí srdce (zejména ICHS), tachykardie, tachypnoe a acidóza. Výskyt infekce Staphylococcus sp. a Klebsiella pneumoniae se významně podílejí na prodloužení doby hospitalizace. Všechny tyto faktory mohou upozorňovat na potřebu zvýšené péče o pacienty s CAP., Introduction: The aim of this study was to evaluate the prognostic value of Pneumonia Severity Index (PSI) for prediction of 30-day mortality in patients admitted to intensive care unit (ICU) for community-acquired pneumonia (CAP). In patients with CAP, comorbidities, complications, and physical, laboratory, radiological and microbiological findings were evaluated relative to their prognosis. Patient and methods: In the study, 197 patients with CAP, hospitalised at ICU of Department of Respiratory Medicine, University Hospital Olomouc between 2008 and 2012, were enrolled. Risk factors according to PSI were assessed in all patients. Results: In the studied cohort of patients with CAP, mean values of PSI were 115.4 ? SD 30.4 points. Overall, 29 patients (14.7 %) deceased. When comparing deceased and survived patients, statistically significant differences were found in PSI (mean ? SD: 137.4 ? 26.1 vs 111.7 ? 29.6 points, p < 0.0001), age (mean ? SD: 76.3 ? 12.9 vs 65.5 ? 14.7 years, p < 0.0001), incidence of heart diseases (86.2 % vs 67.3 %, p = 0.04) and ischaemic heart disease (58.6 % vs 38.7 %, p = 0.04). Assessment of physical and laboratory findings showed that deceased patients had significantly increased incidence of tachycardia above 90/min (51.7 % vs 27.4 %, p = 0.01), tachypnoe above 30/min (37.9 % vs 13.7 %, p = 0.001) and acidosis with pH < 7.35 (27.6 % vs 8.9 %, p = 0.004) comparing to survived patients. No significant correlation between PSI and the length of hospitalisation in survived patients was observed. In patients with Staphylococcus sp. and Klebsiella pneumoniae infection, longer hospitalisation period was observed. Comparison of other parameters such as comorbidities, physical and laboratory findings, and pathogens showed no significant differences when comparing deceased to survived patients. Conclusion: Our study showed that PSI represents an important predictor of 30-day mortality in patients with CAP at ICU, but does not correlate neither with the length of hospitalisation nor with particular pathogens. Independent negative prognostic factors in CAP were age, incidence of heart diseases (most importantly ischaemic heart disease), tachycardia, tachypnoe and acidosis. Staphylococcus sp. and Klebsiella pneumoniae infection led to longer hospitalisation period.
