Úvod: Jedním z onemocnění, které je potenciálním rizikovým stavem pro vznik chronické pankreatitidy, je pankreatitida akutní. Materiál: Z celkového počtu 231 osob, které bylo sledovaných s diagnózou chronické pankreatitidy, 56 nemocných bylo iniciálně léčeno s akutní pankreatitidou (24,2 %). V intervalu 12–24 měsíců od prvé ataky akutní pankreatitidy jejich stav postupně progredoval do obrazu pankreatitidy chronické. Osoby zařazené do studie po 1. atace akutní pankreatitidy abstinovaly, během období jejich sledování nebyla recidiva akutní pankreatitidy prokázána. Výsledky: V etiologii akutní pankreatitidy převládal alkohol (55,3 %), biliární etiologie byl prokázána v 35,7 %. Dle revidované Atlantské klasifikace byla u 69,6 % nemocných stanovena pankreatitida těžká, ostatní nemocní splnili kritérium formy intermediární, nemocní s lehkou formou nebyli zařazeni. Závěr: Významnými přítomnými rizikovými faktory bylo kouření a obezita a u 18 %, resp. u 25,8 % nemocných byl zjištěn pankreatogenní diabetes mellitus. Přitom 88,1 % nemocných trpících akutní pankreatitidou byli kuřáci. Největší počet osob s chronickou pankreatitidou po atace pankreatitidy akutní byl v produktivním věku, tj. 25–50 let. Do chronické pankreatitidy nepřechází pouze akutní pankreatitida alkoholická, ale nalezli jsme tento přechod i u pankreatitidy biliární etiologie., Introduction: One of the diseases involving a potential risk of developing chronic pancreatitis is acute pancreatitis. Material: Of the overall number of 231 individuals followed with a diagnosis of chronic pancreatitis, 56 patients were initially treated for acute pancreatitis (24.2 %). Results: The etiology of acute pancreatitis identified alcohol as the predominant cause (55.3 %), biliary etiology was proven in 35.7 %. According to the revised Atlanta classification, severe pancreatitis was established in 69.6 % of the patients, the others met the criterion for intermediate form, those with the light form were not included. Conclusion: Significant risk factors present among the patients were smoking, obesity and 18 %, resp. 25.8 % had pancreatogenous diabetes mellitus identified. 88.1 % of the patients with acute pancreatitis were smokers. The majority of individuals with chronic pancreatitis following an attack of acute pancreatitis were of a productive age from 25 to 50 years. Příspěvek seznamuje čtenáře s hlavními závěry výzkumného projektu, který byl realizován v rámci diplomové práce. Autorka čtenářům představuje základní teoretické modely citové vazby v romantickém vztahu. Kvantitativní výzkum byl realizován na vzorku 258 respondentů z České republiky. Výsledky potvrzují předchozí zahraniční studie, podle kterých jsou jedinci s jistou citovou vazbou spokojenější než jedinci s nejistou citovou vazbou. Faktory citové vazby se ukázaly jako lepší prediktory spokojenosti ve vztahu než věk, pohlaví nebo druh vztahu respondenta (partnerský vztah / manželství)., The article informs the readers about main conclusions of the research, what was realized in pursuance of the thesis. The author acquaints the readers with the main theoretical models of attachment in romantic relationship. Quantitative research was conducted on a sample of 258 respondents from Czech Republic. The results confirm the former foreign researches of attachment, according to which more securely attached individuals are more satisfied in a relationship than those less securely attached. Assaultive acts committed by people with a mental illness is a major public health issue that affects patients with their families, law enforcement authorities, and the public at large. Failure to provide treatment is in fact a major predictor of assaultive acts in patients with schizophrenia living in the community. Considering that the indigenous ethnic groups of Central Asia have similar sociocultural characteristics, these factors may be reflected in individuals with schizophrenia who have committed serious assaultive acts in Uzbekistan. Objectives: The aim of the work was to identify the sociocultural and clinical characteristics of schizophrenic representatives of indigenous ethnic groups of Central Asia who have committed violent crimes in Uzbekistan and have been found insane in regard to their offence, and to compare these subjects to ones belonging to the other ethnic groups. Material and methods: The data were collected in 2010–2013 in the Tashkent High Security Psychiatric Hospital via face-to-face interviews and also from the patients’ charts and from forensic psychiatric examination statements. Results: The sample consisted of 201 individuals. The sample was 90.1 percent (n = 181) male, with a predominance of the paranoid schizophrenia subtype according to the ICD-10 criteria. Of the subjects, 174 ones (86.6%) were representatives of the indigenous ethnic groups of Central Asia, and 27 ones (13.4%) were representatives of other ethnic groups. The duration of illness among the subjects belonging to the indigenous ethnic groups of Central Asia was less than in the other group; the individuals were rarely referred to psychiatric care because of the popularity of alternative medicine and the stigma attached to mental illness. A positive correlation between violence and various psychotic symptoms, such as delusions, hallucinations, and thought disorder, has also been demonstrated in this group. Žilní a arteriální onemocnění zřejmě mohou mít řadu společných rizikových faktorů. Z hlediska patofyziologického se zřejmě jedná o podobný mechanizmus poškození cévní stěny, na jehož počátku jsou subklinické zánětlivé změny. Mechanizmus vzniku život ohrožujících trombotických příhod může být také podobný u obou skupin onemocnění, a to zejména selháním regulačních vlastností endotelu. Výsledky dosud publikovaných klinických a epidemiologických studií však zatím jednoznačné důkazy o společných faktorech vzniku a komplikací tepenných a žilních onemocnění nepřinesly. V našich studiích jsme zjistili častější výskyt nižšího poměru tlaků kotník/paže u žen s chronickým žilním onemocněním dolních končetin, nenalezli jsme však přímou souvislost mezi známkami preklinické aterosklerózy a výskytem žilních trombóz u pacientů s trombofilními stavy; u této skupiny však jsme však nalezli souvislost mezi tromboembolickými příhodami a hypertenzí. Tepenná i žilní onemocnění mohou tedy být příznivě ovlivněna již zavedenými léčebnými postupy zaměřenými na prevenci kardiovaskulárních onemocnění aterosklerotického původu. Důkazem možného společného ovlivnění tepenných a žilních onemocnění farmaky jsou výsledky rozsáhlé klinické studie, ve které došlo k významnému poklesu nejen kardiovaskulárních příhod, ale i tromboembolických žilních příhod při podávání rosuvastatinu. From the pathophysiological point of view a similar triggering mechanism was proposed for atherosclerosis and venous disease: subclinical inflammation. Life-threatening thrombotic events may also go through similar pathways in both entities and the culprit is probably dysfunctional endothelial cell in the vessel wall. In available clinical and population based studies, however, unequivocal data are presented regarding association between arterial and venous diseases and their risk factors. In our studies, we found a higher prevalence of lower ankle brachial index in women with chronic venous disease of the lower extremities At the same time, nevertheless we found no strong evidence of a direct link between preclinical atherosclerosis and the occurrence of venous thrombosis in patients with thrombophilias; in the latter group, however, we found a link between hypertension and thromboembolic events. Arterial and venous disease may thus be favorably managed by already well-established and available tools used in prevention of atherosclerotic cardiovascular disease. Evidence of a possible impact of pharmacotherapy on both arterial and venous disease stems from a large clinical study in which treatment with hypolipemic drug, rosuvastatin, significantly decreased not only incidence of cardiovascular events but also of venous thromboembolic events. Úvod: PARADIGM HF srovnávala inhibitor angiotenzinového receptoru a inhibitor neprilysinu LCZ696 s enalaprilem u nemocných se srdečním selháním a sníženou ejekční frakcí. Metodika: Jednalo se o dvojitě slepou studii u 8 442 nemocných se srdečním selháním NYHA klasifikace II, III a IV a ejekční frakcí pod 40 %, kteří dostávali LCZ696 (2 × 200 mg) nebo enalapril (2 × 10 mg) přidané ke standardní medikaci. Primární cíl byl smíšený – kardiovaskulární mortalita a hospitalizace pro první srdeční selhání. Studie byla naplánována tak, aby odhalila rozdíl v kardiovaskulární mortalitě. Výsledky: Studie byla ukončena předčasně podle předepsaných pravidel při průměrné době sledování 27 měsíců pro jasný prospěch z léčby LCZ696. V době ukončení se primární cíl vyskytl u 914 nemocných (21,8 %) ve LCZ696 skupině a u 1 117 nemocných (26,5 %) ve skupině léčené enalaprilem (HR LCZ696 skupina, 0,80; 95% CI, 0,73–0,87; p < 0,001). Celkem 711 nemocných (17,0 %) léčených LCZ696 a 835 nemocných (19,8 %) léčených enalaprilem zemřelo (HR pro úmrtí z jakékoli příčiny 0,84; 95% CI, 0,76–0,93; p < 0,001); z těchto nemocných 558 (13,3 %) a 693 (16,5 %) zemřelo z kardiovaskulární příčiny (HR 0,80; 95% CI, 0,71–0,89; p < 0,001). LCZ696 ve srovnání s enalaprilem snižoval riziko hospitalizace pro srdeční selhání o 21 % (p < 0,001) a snižoval symptomy srdečního selhání (p = 0,001). Ve skupině léčené LCZ696 bylo více hypotenzí a nezávažných angioedémů, ale menší výskyt renálního selhání, hyperkalemie a kašle než ve skupině léčené enalaprilem. Závěr: LCZ696 byl účinnější než enalapril ve snížení rizika kardiovaskulárního úmrtí a hospitalizací pro srdeční selhání. Methods: In this double‑blind trial, 8442 patients with class II, III, or IV heart failure and an ejection fraction of 40% or less were assigned to receive either LCZ696 (at a dose of 200 mg twice daily) or enalapril (at a dose of 10 mg twice daily), in addition to the standard therapy. There were two primary outcomes – mortality from cardiovascular causes and hospitalisation for heart failure, but the trial was designed to detect the differences in the rates of mortality from cardiovascular causes. Results: In line with the pre‑specified rules, the trial was stopped early, after a median follow‑up of 27 months, because the boundary indicating a clear benefit achieved with LCZ696 had been exceeded. At the time of study closure, the primary outcome had occurred in 914 patients (21.8%) in the LCZ696 group and 1117 patients (26.5%) in the enalapril group (hazard ratio in the LCZ696 group, 0.80; 95% confidence interval [CI], 0.73 to 0.87; P < 0.001). Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary.
