Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. Léčba kolorektálního karcinomu (KRK) vyžaduje multidisciplinární přístup s účastí gastroenterologa, chirurga, patologa, onkologa a radiologa. Možnosti léčby jsou závislé na stadiu nemoci, celkovém stavu pacienta a také molekulární charakteristice nádoru. V rámci předoperační péče se léčebné postupy u karcinomu konečníku mírně liší od léčby karcinomu tlustého střeva. V případě metastatického KRK jsou možnosti léčby jednotné, použití kombinace chirurgické a cílené systémové léčby. Každá léčebná modalita KRK je zatížená rizikem komplikací. Proto je nezbytné s každým pacientem probrat prospěch a rizika navrhované léčby. Prezentujeme dvě kazuistiky pacientů s KRK, u kterých léčba vedla k dosažení remise i za cenu významných komplikací., Treatment of colorectal cancer (CRC) requires a multidisciplinary approach involving a gastroenterologist, surgeon, pathologist, oncologist and a radiologist. In the context of preoperative care, treatment procedures for cancer of the rectum is slightly different from the treatment of the colon cancer. In the case of metastatic CRC, there are unified treatment options, using a combination of surgical and targeted systemic therapy. Each treatment modality is at risk of complications. Therefore it is necessary to discuss the benefits and risks of the proposed treatment with each patient. Význam péče o výživu je neoddiskutovatelný. Malnutrice provází vysoké procento onkologicky nemocných, může zásadně zhoršit výsledky onkologické léčby a ovlivňuje kvalitu života pacienta, proto její léčba i prevence musí být součástí podpůrné péče v onkologii. Stav výživy pacientů je nutné sledovat v průběhu celého onemocnění a jeho léčby. V článku je předvedena kazuistika padesátiletého pacienta s druhou recidivou karcinomu hrany jazyka, který díky podpůrné péči, jejíž součástí je i péče o výživu, absolvoval reiradiaci kombinovanou s cetuximabem a nyní je bez známek choroby. Kazuistika je také dokladem vývoje nutriční péče na našem ústavu v průběhu několika let., The significance of nutritional care is undeniable. Malnutrition is seen in a high proportion of cancer patients, may substantially deteriorate the outcome of oncological treatment, and affects the quality of life of the patient; therefore, its treatment and prevention must be a part of supportive care in oncology. The nutritional status of patients needs to be monitored throughout the disease course and treatment. The paper presents a case report of a fifty-year-old patient with a second recurrence of carcinoma of the margin of the tongue who owing to supportive care, a part of which is nutritional care, underwent reirradiation combined with cetuximab and is currently free of symptoms of the disease.
