Úvod: Prevalence rizikových faktorů aterosklerózy se posouvá do mladších věkových kategorií. U vybrané již preselektované skupiny dětí s obezitou, dyslipidemií a metabolickým syndromem jsme sledovali antropometrické a laboratorní ukazatele během měsíčního redukčního pobytu dětí v Léčebně dr. Filipa, s.r.o., v Poděbradech. Současně byly sledovány genetické parametry spolupodílející se na vzniku obezity. Soubor: V rámci projektu byly statisticky zpracovány výsledky u 350 obézních dětských pacientů ve věku 8–16 let, průměrného věku 13,7 ? 2,1 roku, průměrné výšky 163 ? 10,6 cm. Metody: Při vstupu a při ukončení hospitalizace byly změřeny antropometrické parametry (výška, váha, tloušťka kožních řas, obvody tělesných částí), změřen krevní tlak a pulz. Byl stanoven lipidogram a další metabolické ukazatele, izolována DNA ke stanovení polymorfizmů genů pro FTO a MC4R. Výsledky: Během pobytu dětí došlo ke statisticky významnému poklesu metabolických ukazatelů a redukci antropometrických hodnot. Současně jsme prokázali skutečnost, že u nositelů rizikových polymorfizmů asociovaných s obezitou byla lepší odezva na režimová a dietní opatření. Závěr: Identifikovat jedince s horší odezvou na režimová opatření bude velice důležité, jelikož u těchto jedinců bude nutné časněji zahájit farmakologickou léčbu s cílem zabránit rozvoji komplikací plynoucích z přítomnosti těchto rizikových faktorů., Objectives: The study was aimed to determine risk factors of atherosclerosis after one month lifestyle intervention in overweight/obese children and also FTO and MC4R gene variants associated with obesity. Design and Methods: 350 non-diabetic Czech children (age 13.7 ? 2.1 years, 163 ? 10.6 cm hight) was examined. Before and after 4 weeks of lifestyle intervention (comprising a reduction of energy intake), biochemical and anthropometrical measurements were performed. Results: The mean weight loss achieved was 6.2 ? 2.1 kg (P < 0.001). Significant associations between BMI decrease and FTO and MC4R variants were found. Carriers of the FTO GG genotype and/or MC4R CC genotype lost significantly more body weight in comparison to the non-carriers (P < 0.0009 for BMI and P < 0.002 for body weight). The differences remain significant after adjustment for sex age and baseline values (P = 0.004 for BMI and P = 0.01 for body weight). Conclusions: It is necessary to look for the risk individuals with wrong response to the regime intervention. BACKGROUND: Though mid-term survival rates of over 95% in several series have been published, there is still a paucity of related literature regarding the role of vertical stem instability in the osteointegration of fluted tapered stems. This paper presents a comprehensive and prospective assessment on short-term experiences with uncemented modular femoral stem in the treatment of defective femur during revision surgery of total hip replacement. MATERIALS AND METHODS: Clinical and radiological monitoring of 20 consecutive patients with implanted tapered fluted revision stem (Lima Corporate, Udine, Italy) was of 27 months in average (20-35 months). The average pre-operative Merle d'Aubigné and Postel method score was 6.3 points (3-10 points). The frequency of femur defects, classified according to Paprosky, was IIIA = 9 and IIIB = 11. RESULTS: During last follow-up, the Merle d'Aubigné and Postel hip score was on average 11.7 (6-16 points). Compared to post-operation radiograph, stem migration of 1.9 mm (0-11 mm) on average was found. This vertical stem migration was observed only when comparing hip radiographs immediately after surgery, and at 6 weeks post-surgery. The Paprosky IIIA defects group, presented a subsided stem by an average of 1.5 mm. In the group of Paprosky IIIB defects, the stem subsidence was on average 2.3 mm. All 20 patients in the study showed excellent osteointegration of the uncemented revision modular stem. CONCLUSIONS: This study found and excellent osteointegration of the Lima uncemented tapered fluted revision modular stem in defective femur with a cortical bone segment present in the diaphyseal isthmus area. U 68letého pacienta s metastatickým nádorem ledviny, současně léčeného kortikoidy pro idiopatickou trombocytopenickou purpuru (ITP), bylo dosaženo podáním sunitinibu stabilizace stavu po dobu zatím 70 měsíců, bez nutnosti hospitalizace či ATB terapie, nebo krevních substitučních derivátů. Toxicita systémové léčby může být u pacientů s nádorem ledviny závažným a někdy i limitujícím problémem. U tohoto pacienta i při závažné komorbiditě podání sunitinibu vedlo k významnému klinickému účinku bez závažné toxicity., In a patient with metastatic renal cell carcinoma, concomitantly treated with corticoids because of idiopathic thrombocytopenic purpura (ITP), an unusually long survival of 70 months with minimal toxicity has been achieved using sunitinib. In patients with renal cell cancer toxicity of systemic therapy could be a serious and limiting factor. Karcinom prostaty jako jeden z nejčastějších zhoubných nádorů a příčina smrti u mužů představuje vysoce závažný medicínský, ale i společenský a ekonomický problém. Diagnostika a následná léčba – jak kurativní, tak především léčba pokročilých stadií, včetně jejich komplikací – je proces, který je velmi náročný. Jednak pro pacienty, protože zejména časná detekce karcinomu prostaty se týká obrovské cílové skupiny mužů, jednak pro zdravotnictví, protože stále rostoucí incidence a prevalence karcinomu prostaty klade na systém nesmírné nároky organizační, personální a v neposlední řadě i ekonomické. Není možno říci, jaká je optimální léčba, protože onemocnění představuje velmi heterogenní skupinu, ale je třeba se pokusit odpovědět na otázku, jaká je optimální léčba u konkrétního pacienta a zda vzhledem ke stadiu onemocnění je ta která léčebná modalita možná a hlavně smysluplná. Autor se zaměřil na karcinom prostaty vyšších stadií a rizikových skupin a hodnotí možnosti provedení a výsledky jednotlivých léčebných modalit – radikální operace a radioterapie., Prostate cancer – one of the most common malignant tumours and cause of death in men – represents very significant medical but also social and economic problem. Diagnostics and ensuing treatment (both curative treatment and treatment of advanced stages, including associated complications) is a very complex process, not only for patients (because early detection of prostate cancer affects large numbers of men) but also for healthcare systems, because continuously increasing incidence and prevalence have significant organizational, HR and last but not at least economic demands. Owing to the heterogeneous nature of prostate cancer the optimal therapy is not clear. But we can try to determine the best treatment in an individual patient and assess if it is feasible and justified considering the individual prostate cancer stage. The clinical outcome of 153 Graves' disease patients treated with a wide dose range of radioactive iodine-131 (RAI) was analyzed retrospectively. Six to nine months after the first dose of RAI 60 patients (39%) were hypothyroid (or rather thyroxine-substituted) and 26 (17%) were euthyroid, while 67 patients (44%) did not respond properly: in 32 (21%) their antithyroid drug (ATD) dose could be reduced but not withdrawn (partial response) and 35 (23%) remained hyperthyroid or the same dose of ATD was necessary (no response). The outcome did not correspond significantly to the administered activity of RAI (medians 259, 259, 222, and 259 MBq for hypothyroid, euthyroid, partial, and no response subgroups, respectively), or the activity retained in the gland at 24 h (medians 127, 105, 143, and 152 MBq). The effect was, however, clearly, and in a stepwise pattern, dependent on initial thyroid volume (17, 26, 33 and 35 ml, P < 0.001) or activity per gram tissue retained at 24 h (6.02, 4.95, 4.75, and 4.44 MBq/g, P = 0.002). Also, higher residual level of thyrotoxicosis at the time of RAI treatment was connected with worse outcome. The dose-dependency of outcome was further analyzed. When our sample was divided into tertiles, according to the adjusted dose, the same modest success rates (47%) were seen in the lower and middle tertiles. However, doses higher than 5.88 MBq/g (the upper tertile) resulted in success rate of 75%. Finer division into decils has shown a threshold-like increase in cure rate between the 7th and the 8th decil. In the first 7 decils (doses ≤ 6 MBq/g) the complete response rate was 45 to 50%, in the 8th decil (6.0 to 7.8~MBq/g) it rose to 80% and was not further increased with increasing dose. Direct comparison of higher (> 6 MBq/g, cure rate 80%) and lower (≤ 6 MBq/g, cure rate 46%) doses gave highly significant difference (P < 0.001). With our dosing range we found a dose-dependent clinical outcome that suggests an optimum delivered dose near 6.5 MBq/g, resulting in successful treatment of ca 80% patients. and J. Čepková, J. Tromboembolická nemoc (TEN) je relativně častým onemocněním s akutním rizikem úmrtí a potenciálními dlouhodobými následky v podobě posttrombotického syndromu nebo chronické plicní hypertenze. Základním léčebným postupem zůstává antikoagulační léčba; trombolytická léčba a zavedení kaválního filtru jsou určeny pro vhodně indikované individuální stavy. V posledních letech se v léčbě TEN objevila nová přímá perorální antikoagulancia (NOAC) – inhibitory faktoru Xa nebo trombinu, která prokázala stejnou účinnost a lepší bezpečnost léčby ve srovnání s konvenčním postupem. V polovině roku 2014 jsou registrovány v České republice 3 z těchto léků – rivaroxaban (Xarelto?), dabigatran (Pradaxa?) a apixaban (Eliquis?). Mají srovnatelnou účinnost a bezpečnost, rozdílná jsou však schémata podávání. Rivaroxabanem a apixabanem lze léčit perorálně ihned od stanovení diagnózy žilní trombózy nebo hemodynamicky stabilní plicní embolie, dabigatranem po úvodní několikadenní aplikaci LMWH. Omezením nových léků je ekonomická náročnost léčby. Nedostupnost antidota je dočasná, vyvíjejí se jak pro dabigatran, tak pro oba xabany. Délka antikoagulační medikace po akutní fázi se odvíjí od přítomnosti řady faktorů vzniku trombózy, v kontextu individuálního rizika krvácení. Minimální délka antikoagulační léčby je 3 měsíce, častěji však léčíme 6–12 měsíců, u nejrizikovějších nemocných tzv. dlouhodobě. Dobré výsledky extenze antikoagulační medikace na zábranu rekurence TEN ve studiích s novými antikoagulancii mohou změnit zavedené zvyklosti dlouhodobé medikace nemocných s TEN., Thromboembolic disease (TD) is a relatively common disease with acute risk of death and potential long-term consequences in term of postthrombotic syndrome or chronic pulmonary hypertension. Anticoagulant therapy is the basic therapeutic procedure; thrombolytic therapy and the introduction cava filter are appropriately indicated for individual cases. In past few years, new direct oral anticoagulant drugs (NOAC) have occurred – Xa factor or thrombin inhibitors which have demonstrated the same efficacy and even higher safety in comparison to conventional treatment. In mid 2014, 3 drugs of this group are registered in Czech Republic – rivaroxaban (Xarelto?), dabigatran (Pradaxa?) and apixaban (Eliquis?). These drugs have comparable efficacy and safety but they differ in schedule of dose administration. Rivaroxaban and apixaban can be administered immediately after diagnosis of venous thrombosis or hemodynamically stable pulmonary embolism. LMWH application has to precede few days the administration of dabigatran. Limitation of new drugs is their price. Unavailability of antidotes is temporary because current researches continue to find one for dabigatran and another for both of xabans. Duration of anticoagulant treatment after acute phase depends on the presence of thrombosis risk factors and the individual bleeding risk. Minimal duration of anticoagulant therapy is 3 months, commonly 6–12 months and in high risk patients it is “long term” treatment. Spinální arteriovenózní malformace jsou skupina vzácných onemocnění, jejichž klinické projevy mohou být velmi rozmanité. Optimální léčebný postup je nutno vždy volit individuálně a pacienty směřovat do specializovaných center. Cílem sdělení je uvést přehlednou klasifikaci SAVM, jejich diferenciální diagnostiku a terapii a dále prezentovat dvě kazuistiky. První kazuistika se zmiňuje o případu 27letého muže, u kterého se rozvinuly silné bolesti bederní páteře s dominující meningeální symptomatikou v klinickém obraze. Na MR hrudního úseku páteře (Th11/12) byla nalezena AVM zásobovaná hypertrofickou radikulopiální tepnou. Na specializovaném pracovišti byla provedena úspěšná embolizace AV zkratu. Druhá kazuistika uvádí případ 41leté ženy, které vznikla při lyžování náhlá prudká bolest za krkem. V neurologickém obraze došlo k rozvoji inkompletní léze míšní s kvadruparézou s levostrannou akcentací do těžkého stupně s hranicí čití ve výši C1. Na provedeném MR a MRAG krční míchy a následně na DSA vertebrálních tepen byla detekována AVM zásobovaná levou arteria spinalis posterior. Terapeuticky byla indikována elektivní endovaskulární embolizace a pacientka byla ošetřena na specializovaném pracovišti ve Francii., Spinal arteriovenous malformations are a group of rare diseases with highly diverse clinical manifestations. The optimal therapeutic practice should always be selected individually and patients should be sent to specialized centres. The objective of this paper is to provide a structured classification of spinal AVMs, their differential diagnosis and therapy, and to present two case reports. The first report is concerned with a case of a 27-year-old man who developed severe pain in the lumbar spine with predominantly meningeal symptomatology. An AVM fed by a hypertrophic radiculopial artery was found on the thoracic spine (Th11/12) MR. A specialized centre performed successful embolization of an arteriovenous shunt. The second report presents a case of a 41-year-old woman, who felt sudden sharp pain in the neck while skiing. Neurologically, she developed an incomplete spinal cord lesion with quadriparesis with left-sided accentuation of severe intensity and sensation threshold at C1. The MR and MRAG of the cervical spinal cord and the following DSA of vertebral artery detected AVM fed by the left posterior spinal artery. The Purpose of this study is to demonstrate the effectiveness of combined treatment for III AB stage of nonsmallcell lung cancer. Nonsmallcell lung cancer spread form treatment is disputable. Operation is not final solution. The combined treatment is being tested (in clinical trials) in this study. This article argues that a combination of treatment is a better option than that of a specific treatment. Introduction: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease characterized by the development of osteoproductive changes in the spine which could possibly result in ankylosis. Treatment with tumour necrosis factor alpha (TNFα) inhibitors has proved to be an important step forward in the treatment of this disease, but for the time being it is not clear whether it favourably influences radiographic progression of the disease. Vascular endothelial growth factor most probably plays a role in the development of osteoproductive changes and recently its predictive influence on radiographic progression has been demonstrated. Bone morphogenic protein 2 (BMP-2) participates in the regulation of bone proliferation and its increased serum level has been demonstrated in patients with advanced AS and correlated with the degree of radiographic changes. Aim: The study aims to evaluate the VEGF and BMP-2 levels in patients with ankylosing spondylitis and how these levels relate to the concurrent treatment with TNFα inhibitors. Methods: Sera were evaluated from patients at the Rheumatologic Clinic of the Hradec Králové Faculty Hospital who fulfilled the modified New York Criteria for AS (n = 55). In these patients, the parameters of the activity of the disease (BASDAI = Bath Ankylosing Spondylitis Disease Activity Index, CRP = C-reactive protein) and the concurrent therapy (TNFα inhibitors, n = 21, vs. non-anti TNFα, n = 34) were recorded. The levels of VEGF and BMP-2 were analyzed using the ELISA method. Results: In patients treated with TNFα inhibitors, a significantly lower VEGF level was found when compared to untreated patients (140.3 (109.4; 262.2) vs. 261 (172.4; 396.6) pg/ml; p = 0.02). No difference was found between BMP-2 levels in both groups (treated vs. untreated patients) (254.8 (2301; 267.3) vs. 261.1 (248.6; 273.5) pg/ml; p = 0.24). A correlation analysis did not reveal any relationship between VEG F and BMP-2 (r = 0.057; p = 0.68). Serum levels of VEGF correlated with serum levels of CRP (r = 0.56; p = 0.00001) and the BASDAI value (r = 0.33; p = 0.015). Conclusion: Significantly lower VEGF levels were found in patients treated with TNFα inhibitors versus the untreated patients. These findings are in harmony with some hitherto published analyses and may give evidence of a favourable effect of TNFα inhibitors on radiographic progression. Kontinuální tvorba výpotků jakékoliv etiologie zásadním způsobem ovlivňuje kvalitu života nemocných. Akumulace tekutiny v pleurální dutině postupně komprimuje plicní parenchym. Prvním příznakem bývá progrese námahové dušnosti, později se přidává i klidová dušnost. S nárůstem výpotku dochází ke kompresi plicního parenchymu s rozvojem plicní atelektázy. Nemocný je ohrožen vznikem zánětového onemocnění plic v nevzdušném plicním parenchymu. U diagnózy maligního onemocnění je riziko zánětových komplikací výrazně vyšší vzhledem k předchozí chemoterapii či radioterapii, která zásadním způsobem snižuje imunitu. Opakovaná tvorba výpotku se výrazně podílí na rozvoji terminálního stavu nádorové kachexie. Efektivní metodou léčby je chemická pleurodéza, která se užívá u vybraného spektra nemocných. Principem je navození aseptické pleuritidy. Nejefektivnější cestou aplikace léčiva je videotorakoskopie. V souboru bylo 47 nemocných, 29 (61,7 %) mužů a 18 (38,3 %) žen, s cytologicky prokázaným recidivujícím maligním pleurálním výpotkem. Věkové rozmezí 42–80 let, Ø věk 65 let. Cílem práce bylo objektivizovat průběh celkových a lokálních zánětových změn po talkáži. Efekt léčby nebyl ovlivněn základním typem nádorového onemocnění. U obou skupin nebyly významně odlišné pooperační komplikace, což dokládá bezpečnost zvolené metody a podporuje správně zvolený algoritmus léčby., Continuous pleural effusion production of any aetiology can significantly affect the quality of patients life. Chronic effusion accumulation in the pleural cavity can lead to lung parenchyma compression. The first symptom of this entity is a progressive exertional dyspnea, which can later turn in dyspnea at rest. With the increase of the pleural effusion volume, pulmonary atelectasis can develop due to chronic parenchycha compression. The patient is at risk of inflamatory complications from the territory of non-ventilated parenchyma. Patients with a diagnosis of malignant disease have this risk considerably higher due to their previous chemotherapy or radiation, which substantially affect the immunity system. Repeated pleural effusion formation can significantly take part in the development of the terminal status of cancer cachexia. An effective method of palliative treatment is a chemical pleurodesis, which is used in selected patients. The principle is inducing aseptic inflammation. The most effective way of application is via videothoracoscopy. In a group of 47 patients, there were 29 (61.7 %) men and 18 (38.3 %) women with cytologically diagnosed recurrent malignant pleural. The age ranged between 42 and 80 with average age of 65 years. The aim was to assess the course of local and systemic inflammatory changes after talc application. The effect of treatment was not influenced by the type of malignancy . In both groups there was no significant difference in postoperative complications.
