S prodlužujícím se průměrným věkem dožití čelíme nyní u starších pacientů pandemii osteoporózy. Základní preventivní opatření by měla začínat již v dětství snahou o dosažení maximální vrcholové kostní hmoty. U pacientů s involučním snížením BMD je základním opatřením adekvátní saturace kalciem a vitaminem D. Problematika nedostatečné saturace vitaminem D je komplexní a kromě efektů skeletálních zahrnuje i celou řadu efektů extraskeletálních, především vliv na svalovou sílu. U pacientů ve vyšších decenniích je porušena tvorba i vstřebávání vitaminu D a chronická „subklinická" hyposaturace tímto působkem je nesmírně častá. Přitom je základním patogenetickým mechanizmem involuční osteoporózy mírná sekundární hyperparatyreóza, způsobená tímto stavem. Zatím jedinou skupinou antiresorpčních preparátů, která je schopna zvýšit BMD u pacientů s involuční, senilní porózou a snížit riziko zlomenin nejen na páteři, ale i na kritickém krčku femoru jsou bisfosfonáty.
