Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Úvod: Naše výsledky byly zhodnoceny za účelem posouzení bezpečnosti a efektivity endoskopické endonazální techniky v léčbě kraniofaryngeomů. <p align="">Materiál a metody: Od roku 2008 bylo endoskopicky endonazálně operováno14 pacientů s typickým supraselárním extraventrikulárním kraniofaryngeomem. Soubor se skládá z 11 mužů a 3 žen (věk 17–60 roků, průměrný věk 38 let). Jedenáct pacientů mělo předoperační deficit zorného pole v rozsahu od malého deficitu ve vnějším kvadrantu až k slepotě jednoho oka a těžkému deficitu druhého oka. Byla použita endoskopická endonazální technika čtyř rukou s peroperační 3,0 T MR. <p align="">Výsledky: V sedmi případech bylo dosaženo radikální resekce. Ve čtyřech případech byla provedena resekce subtotální, ve dvou případech resekce parciální. V jednom případě se jednalo o vypuštění kraniofaryngeomové cysty. V resekci po iMR bylo pokračováno ve třech případech. K úpravě zorného pole došlo v pěti případech. Pooperační diabetes insipidus se nově objevil ve třech případech. Reoperace pro únik mozkomíšního moku byla nezbytná ve třech případech. <p align="">Závěr: Endoskopické techniky v léčbě kraniofaryngeomu jsou bezpečné. Nicméně jsou spojeny s vyšším výskytem pooperačního úniku mozkomíšního moku ve srovnání s transkraniálními přístupy., Introduction: We reviewed our results in order to evaluate safety of endoscopic endonasal technique in the treatment of craniopharyngiomas. <p align="">Material and methods: Since 2008, endoscopic endonasal approach was used in 14 patients with typical suprasellar extraventricular craniopharyngioma. This cohort consisted of 11 males and three females (age 17 to 60 years, average age 38 years). Eleven patients had preoperative visual field deficit ranging from small deficit in the outer quadrant to blindness on one eye and a severe deficit of the second eye. Endoscopic endonasal four hands technique was used with intraoperative 3.0 T MRI. <p align="">Results: In seven cases, the radical resection was achieved. Subtotal resection was performed in four cases. The resection was partial in two cases. In one case, a cyst was drained. Resection after iMRI was performed in three cases. Visual field deficit improved in five cases. Postoperative diabetes insipidus developed in three cases. Reoperation for CSF leakage was necessary in three cases. <p align="">Conclusion: Endoscopic technique in the treatment of craniopharyngioma is safe. Úvod: Radiofrekvenční ablace (RFA) je etablovaná metoda využívaná k paliativnímu ošetření neresekabilních tumorů jater. Na našem pracovišti využíváme k ošetření metastáz kolorektálního karcinomu (CLM) otevřený či perkutánní přístup. Metoda: V retrospektivní studii (1/2001−1/2015) jsme vyhodnotili klinická data u pacientů podstupujících perkutánní nebo otevřenou RFA pro neresekabilní CLM. Cílem bylo zhodnotit faktory ovlivňující celkové přežití (OS), bezpříznakové období (NED) a nonablaci ložisek ve vztahu k velikosti a počtu ložisek, typu přístupu a typu použité sondy. Výsledky: RFA podstoupilo 147 pacientů s průměrným věkem 65 let. Celkem bylo provedeno 168 RFA výkonů. Celkové přežití bylo ovlivněno vysokým počtem cenzorů. Roční a tříleté OS bylo 96,3 % a 61 % bez statisticky významného rozdílu mezi perkutánním a otevřeným přístupem. NED bylo statisticky významně kratší u pacientů s perkutánním výkonem. NED nebylo ovlivněno ani velikostí, ani počtem ložisek. Vyšší riziko nonablace bylo pozorováno u větších ložisek, ale výsledek nebyl statisticky signifikantní. Perkutánní výkony měly kratší dobu hospitalizace a nižší počet komplikací. Závěr: RFA je alternativní metoda léčby neresekabilních CLM. Otevřený přístup měl v naší studii nižší riziko nonablace. Perkutánní výkony měly nižší riziko komplikací a kratší dobu hospitalizace., Introduction: Radiofrequency ablation (RFA) is a well-established method for palliative therapy of unresectable liver tumors. We use an open or percutaneous approach for the treatment of colorectal liver metastases (CLM). Method: Clinical data of patients undergoing percutaneous or open RFA for CLM between January 2001 and January 2015 were included in the retrospective study. We evaluated clinical factors for overall survival (OS), no evidence of disease (NED) and non-ablation in relation to tumor sizes and numbers, type of approach and type of used probes. Results: 147 patients underwent RFA for CLM in this time period. Mean age was 65 years. 168 RFAs were performed in total. OS was influenced by a high number of censors. OS for the first and third years was 93.6% and 61% with no statistical differences between the percutaneous and open approach. NED was significantly shorter in patients with the percutaneous approach. NED was not influenced neither by size nor number of the lesions. A higher risk of non-ablation was observed as statistically significant in patients with percutaneous RFA. A higher, although not statistically significant, risk of non-ablation was also observed for larger metastases. Patients with percutaneous RFA showed a shorter stay in the hospital and fewer complications. Conclusion: RFA is an alternative approach to the treatment of unresectable CLM. In our study the open approach was associated with a lower risk of non-ablation. Percutaneous RFA showed a lower risk of complications and a shorter stay in the hospital., and J. Brůha, V. Liška, R. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet. Úvod: Kompletní resekce je obecně uznávaný standard léčby GII ependymomů IV. komory u dospělých, zatímco role adjuvantní radioterapie zůstává nejistá. Účelem předkládané studie je zhodnotit lokální kontrolu onemocnění a klinický stav pacientů s tímto onemocněním po chirurgické terapii. Materiál a metody: Retrospektivně jsme zhodnotili 15 pacientů (sedm mužů a osm žen) s histologicky verifikovaným GII ependymomem IV. komory v časovém období leden 2001–prosinec 2011. Výsledky: Kompletní resekce tumoru byla dosažena u 12 pacientů, u žádného z nich nebyla indikována adjuvantní radioterapie. Medián dalšího sledování byl 51 měsíců (min.–max.; 11–123 měsíců). Ke grafické recidivě došlo u jednoho pacienta, ten byl reoperován s časovým odstupem 51 měsíců od prvního zákroku. Nový neurologický deficit po operaci byl zaznamenán ve dvou případech. U dvou ze tří pacientů po subtotální resekci došlo ke grafické progresi rezidua po 11 a 16 měsících od první operace, jeden z nich byl reoperován s odstupem 27 měsíců. Adjuvantní radioterapie byla indikována u obou pacientů po reoperaci. Závěr: Dle našich dat je kompletní resekce efektivní a bezpečná léčba GII ependymomů IV. komory u dospělých. Následná radioterapie by měla být rezervována pro případy subtotální resekce s následnou progresí rezidua tumoru a reoperace., Introduction: Surgical resection is a generally accepted standard treatment modality in GII posterior fossa ependymomas in adults, while the role of adjuvant radiotherapy remains unclear. The purpose of this study was to evaluate local control of tumor growth and clinical outcome of patients after surgical treatment of GII fourth ventricle ependymomas. Material and methods: We conducted a retrospective study of 15 adult patients (seven males and eight females) with histologically confirmed GII fourth ventricular ependymoma between January 2001 and December 2011. Results: Gross total resection was achieved in 12 patients, none of them received adjuvant treatment after the initial surgery. Median follow-up after GTR was 51 months (min. – max.; 11–123 months). One patient showed radiographic signs of tumor recurrence and was re-operated 51 months after the initial surgery. Remaining patients were without any signs of tumor recurrence. After the surgery, a new neurological deficit occurred in two patients. Two of the three patients after subtotal tumor resection showed residual mass progression after 11 and 16 months postoperatively, one of them was re-operated 27 months after the initial surgery. Adjuvant radiotherapy was indicated in both re-operated patients. Conclusion: According to our data, gross total resection is effective and safe treatment of GII fourth ventricle ependymomas. Adjuvant radiotherapy should be reserved for the cases where complete removal of tumor mass is not possible and residual tumor is growing, or after repeated resection. Cíl: Cílem práce bylo provést komplexní zhodnocení faktorů ovlivňujících úspěšnost zachování sluchu při mikrochirurgické léčbě vestibulárního schwannomu (VS). Soubor a metodika: Prospektivně jsme hodnotili soubor 81 pacientů, kteří v období 2008–2010 podstoupili radikální exstirpaci VS retrosigmoidním‑transmeatálním přístupem a v době zákroku měli zachovaný sluch. Hodnotili jsme faktory stran pacienta, nádoru, audiometrické parametry a technické aspekty operace. Parametry byly statisticky vyhodnoceny včetně regresivní analýzy nezávislých proměnných. Výsledky: Sluch se podařilo zachovat u 17 pacientů (tzv. užitečný sluch: devět; tzv. neužitečný sluch: osm). U dvou pacientů jsme zaznamenali zlepšení z neužitečné úrovně na sluch užitečný. Velikost VS byla ve skupině se zachovaným sluchem menší (p < 0,001), převažovaly tak nádory nižších stadií (p = 0,001). Hloubka zasahování do vnitřního zvukovodu nevykazovala vztah k úspěšnosti zachování sluchu. K významným faktorům ovlivňujícím výsledek patřila předoperační úroveň sluchu (p < 0,001) včetně typu odpovědi v BERA (p = 0,03). Užití intraoperační BERA bylo pro výsledek klíčové (p < 0,001). Úspěšnost zachování BERA však s výsledkem nekorelovala. Ke klíčovým faktorům ovlivňujícím uchování sluchu patřila úspěšná identifikace kochleárního nervu (p < 0,001), užití endoskopické techniky (p < 0,001), měkká konzistence tumoru (p = 0,05) a absence adhezí k neurovaskulárním strukturám (p < 0,001). Krvácivost ani přítomnost cystické komponenty úspěšnost uchování sluchu neovlivnily. Závěry: Mimo zkušenost chirurgického týmu patří k hlavním faktorům ovlivňujícím uchování sluchu velikost nádoru, úroveň sluchu před operací a možnost užití intraoperačního monitoringu sluchu. Klíčová je identifikace kochleární porce n. VIII. Z tohoto hlediska se jeví přínosné užití endoskopické techniky. K významným intraoperačním faktorům pak náleží měkká konzistence tumoru a absence adhezí k neurovaskulárním strukturám., Aim: The aim of the study was to conduct a comprehensive analysis of the factors influencing the success rate of hearing preservation in patients undergoing microsurgical removal of vestibular schwannoma (VS). Materials and methods: A total of 81 consecutive patients with serviceable hearing underwent surgical removal of unilateral vestibular schwannoma via retrosigmoid-transmeatal approach between 2008 and 2010. Patient, tumor and audiometric parameters together with intraoperative findings were prospectively analyzed. Data were statistically processed including a regression analysis of independent variables. Results: Serviceable hearing was preserved in 17 patients (useful hearing in nine and non-useful in eight cases). We observed improvement of hearing from non-useful to useful level in two patients. Average size of VS was smaller in the preserved hearing group (p < 0.001) where lower stage tumors prevailed (p = 0.001). Extent of internal auditory canal filling did not correlate with the result. Preoperative hearing level (p < 0.001) and the type of brainstem auditory evoked response (p = 0.03) arose as significant prognostic factors. Employment of intraoperative BERA was crucial for the outcome (p < 0.001) but success rate of its preservation was not correlated with the result. Identification of the cochlear nerve (p < 0.001), employment of endoscopic technique (p < 0.001), soft consistency of the tumor (p = 0.05) and the lack of adhesions to the neurovascular structures (p < 0.001) proved to be crucial factors affecting the preservation of hearing. Neither hypervascularity nor the presence of cystic component affected outcome. Conclusions: Apart from the experience of the surgical team, size of the tumor, preoperative hearing level and possibility of intraoperative hearing monitoring represented the main factors for hearing preservation. Early identification of the cochlear portion of the CN VIII is key. For this, the use of endoscopic technique seems to be beneficial. Soft consistency of tumor and lack of adhesions to the neurovascular structures represent crucial intraoperative factors.
