Cíl: Podle literatury dosahuje četnost komplikací kranioplastiky po dekompresivní kraniektomii i více než 30 %. Cílem studie je hodnocení četnosti, příčin a léčby komplikací kranioplastik po dekompresivní kraniektomii pro expanzivně se chovající supratentoriální ischemii nebo kraniocerebrální poranění a analýza důvodů neprovedení kranioplastiky u části nemocných po dekompresivní kraniektomii. Soubor a metodika: V retrospektivní studii nemocných po dekompresivní kraniektomii (2006–2014) byla kranioplastika provedena u 25 z 35 nemocných s expanzivní malacií (71,4 %) a u 33 z 68 zraněných s poúrazovou nitrolební hypertenzí (48,5 %). Výsledky: Nejčastější příčinou neprovedení kranioplastiky bylo úmrtí. U 20 % nemocných s mozkovou ischemií a 61,3 % zraněných byla kranioplastika provedena časně (do 10 týdnů). Četnost neurochirurgických komplikací byla u kranioplastik u nemocných s expanzivní malacií 12 % (explantace plastiky u 8 %) a u kranioplastik po kraniocerebrálním poranění dosahovala 9,1 % (explantace plastiky u 3 %). Hydrocefalus a subdurální kolekce byly před kranioplastikou přítomny u 6 a 9,1 % zraněných. Četnost hydrocefalu po kranioplastice byla u kraniotraumat 6 % a u mozkové ischemie 4 %. Závěry: Četnost komplikací kranioplastik, byť nižší než uvádí literatura, převyšuje hodnoty očekávané u elektivních operací. Subdurální kolekce a hydrocefalus nelze jednoznačně hodnotit jako komplikaci kranioplastiky, protože mohou být přítomny již před kranioplastikou v důsledku základního onemocnění., Aim: Reported incidence of cranioplasty complications after decompressive craniectomy may exceed 30%. The aim of the study was to analyse the incidence, causes and treatment options for neurosurgical complications of cranioplasty after decompressive craniectomy for expansive supratentorial ischaemic stroke and craniocerebral injury and to analyse the reasons for not performing cranioplasty in a subgroup of patients after decompressive craniectomy. Patients and methods: During a retrospective study (2006–2014) cranioplasty was performed in 25 of 35 patients after decompressive craniectomy for expansive supratentorial ischaemia (71.4%) and in 33 of 68 (48.5%) patients after decompressive craniectomy for posttraumatic intracranial hypertension. Results: Death was the most frequent reason for not performing cranioplasty after decompressive craniectomy. The frequency of early cranioplasties (< 10 weeks after decompressive craniectomy) was 20% in patients with ischaemic stroke and 61.3% in head-injured patients. The incidence of neurosurgical complications was 12% in patients after expansive brain ischaemia (cranioplasty explantation rate 8%). The incidence of cranioplasty complications in patients after craniocerebral injury was 9.1% (cranioplasty explantation rate 3%). Subdural collections and hydrocephalus were observed in 9.1% and 6% of patients after craniocerebral injury before cranioplasty, resp. Hydrocephalus requiring neurosurgical treatment was found in 6% of patients after craniocerebral injury and 4% of patients with brain ischaemia during the postcranioplasty period. Conclusions: Although the incidence of cranioplasty complications after decompressive craniectomy is lower than that reported in literature, it exceeds the rates expected for elective surgeries. Subdural collections and hydrocephalus cannot be unequivocally considered a complication of cranioplasty, because they are frequently observed before cranioplasty and may be caused by the disease requiring decompressive craniectomy. Úvod: Pectus excavatum je najčastejšou vrodenou deformitou hrudnej steny. Postihuje približne jedného zo 400 až 1000 novorodencov. Operačná korekcia sa stala metódou voľby u pacientov, ktorí spĺňajú indikačné kritéria. Najčastejšie využívanými technikami sú modifikovaná Ravitchova korekcia (HMRR) a minimálne invazívna korekcia pectus excavatum (MIRPE), pričom druhá menovaná je považovaná za metódou voľby u detských pacientov. V dospelej populácii je otázka najvhodnejšej techniky predmetom diskusie v odborných kruhoch. Metódy: Autori práce retrospektívne vyhodnotili súbor 60 dospelých pacientov, ktorí absolvovali MIRPE v období od októbra 2010 do marca 2015. Preferovanou technikou na pracovisku autorov bola „short-bar“ technika s pravostrannou torakoskopickou asistenciou. Získané údaje vyhodnotili s pomocou štatistického softvéru SPSS? verzia 21.0.0 (IBM, USA). Platnosť hypotéz a potvrdenie štatisticky významných vzťahov overili štatistickými testami. Klinické a demografické premenné sme vyhodnotili Studentovým t-testom pre parametrické dáta a Mann- Whitneyovým U-testom pre dáta neparametrické. Závislosť kvalitatívnych znakov boli otestované po zadaní do kontingenčných tabuliek pomocou Pearsonovho χ2 testu dobrej zhody. Za hladinu štatistickej významnosti sme zvolili p<0,05. Výsledky: V sledovanom období podstúpilo operačnú korekciu 45 mužov a 15 žien. Medián veku v súbore bol 24 rokov, bez štatisticky signifikantného rozdielu medzi mužmi (25 rokov) a ženami (23 rokov, p=0,368). Medián Hallerovho indexu v súbore bol 3,93, pričom mierne vyšší bol u ženských pacientov (p= 0,285). Asymetrická deformita bola prítomná u 20 pacientov (33 %), bez signifikantného rozdielu medzi pohlaviami (p=0,206). Medián dĺžky operačného výkonu v celom súbore pacientov bol 73 minút, pričom trvanie korekcie u mužov bolo mierne dlhšie (80 vs. 70 min, p=0,312). Dve dlahy sme implantovali 27 pacientom. Nezaznamenali sme pritom štatisticky signifikantný rozdiel medzi pohlaviami (p=0,454). Komplikácie boli prítomné u 15 pacientov (25 %), pričom sme nepotvrdili štatisticky signifikantný rozdiel podľa typu pohlavia (p=0,863), počtu implantovaných dláh (p=0,178) a tvaru deformity (p= 0,527). Najčastejšou komplikáciou boli recidivujúce ranové serómy u 6 pacientov (10 %). Medián pooperačnej hospitalizácie bol 5 dní, pričom sme nezaznamenali rozdiel medzi pohlaviami (p=0,653) a pacientami s rôznym počtom implantovaných dláh (p=0,600). Rehospitalizácia s potrebou ďaľšej chirurgickej intervencie bola indikovaná u 5 pacientov (8,3 %). Príčinou bola alergia na chróm (1×), ranová infekcia (1x) a recidivujúci seróm (3x) s potrebou V.A.C.? terapie (KCI, USA). Záver: MIRPE je možné ponúknuť ako bezpečnú a efektívnu techniku pri korekcii pectus excavatum v dospelej populácii aj na pracovisku bez predchádzajúcich skúseností s týmto výkonom u detských pacientov., Introduction: Pectus excavatum is the most common congenital chest wall deformity. Aproximatelly 1 out of 400 to 1000 newborns are affected by this diagnosis. Surgical correction is indicated in patients that fulfil the indication criteria. The highly modified Ravitch correction (HMRR) and minimally invasive pectus excavatum repair (MIRPE) are by far the most popular methods of correction. MIRPE has been established as the method of choice amongst children. Feasibility of the minimally invasive approach in the funnel chest correction in adult population still remains controversial. Methods: The authors present a retrospective analysis of adult patients treated by MIRPE in their institution. Sixty adult patients with pectus excavatum underwent MIRPE from October 2010 to March 2015. Various parameters were evaluated retrospectively. The effectiveness and safety of both techniques were evaluated with the use of statistical software SPSS? version 21.0 (IBM, USA). Data were evaluated using Student´s t-test or Mann-Whitney U-test for normally and non-normally distributed data, respectively. The comparison of categorical variables between the groups was evaluated by Pearson´s chi-square test. A p value of less than 0.05 was considered statistically significant. Results: 45 men and 15 women underwent MIRPE during the period. Median age in the group was 23 years without any statistically significant difference between males and females (p=0.386). Median Haller index was 3.93 and was slightly higher in women (p= 0.285). Asymmetric deformities were present in 20 patients (33.3%) without any statistically significant difference between the sexes (p=0.206). Median duration of the surgical correction was 73 minutes and was longer in males (p=0.312). Two bars were implanted in 27 patients with no significant difference between the sexes (p=0.454). Complication rate was 25% (15/60) without any significant difference based on the number of implanted bars (p=0.178), sex (p=0.863) or the presence of an asymmetric deformity (p=0.527). Wound seromas with 10% (6/60) were the most common complication. Median postoperative length of stay was 5 days without any significant difference between males and females (p=0.653) and patients with different number of bars implanted (p=0.600). The need for rehospitalisation and reoperation was indicated in 5 patients (8.3%). Allergy to chromium led to bar extraction in 1 patient and 4 patients were treated by V.A.C.? therapy (KCI, USA) because of wound abscess (1) and seroma (3). Conclusion: Minimally invasive pectus excavatum repair is an effective surgical technique for the treatment of pectus excavatum in adult patients. Úvod: Radiofrekvenční ablace (RFA) je etablovaná metoda využívaná k paliativnímu ošetření neresekabilních tumorů jater. Na našem pracovišti využíváme k ošetření metastáz kolorektálního karcinomu (CLM) otevřený či perkutánní přístup. Metoda: V retrospektivní studii (1/2001−1/2015) jsme vyhodnotili klinická data u pacientů podstupujících perkutánní nebo otevřenou RFA pro neresekabilní CLM. Cílem bylo zhodnotit faktory ovlivňující celkové přežití (OS), bezpříznakové období (NED) a nonablaci ložisek ve vztahu k velikosti a počtu ložisek, typu přístupu a typu použité sondy. Výsledky: RFA podstoupilo 147 pacientů s průměrným věkem 65 let. Celkem bylo provedeno 168 RFA výkonů. Celkové přežití bylo ovlivněno vysokým počtem cenzorů. Roční a tříleté OS bylo 96,3 % a 61 % bez statisticky významného rozdílu mezi perkutánním a otevřeným přístupem. NED bylo statisticky významně kratší u pacientů s perkutánním výkonem. NED nebylo ovlivněno ani velikostí, ani počtem ložisek. Vyšší riziko nonablace bylo pozorováno u větších ložisek, ale výsledek nebyl statisticky signifikantní. Perkutánní výkony měly kratší dobu hospitalizace a nižší počet komplikací. Závěr: RFA je alternativní metoda léčby neresekabilních CLM. Otevřený přístup měl v naší studii nižší riziko nonablace. Perkutánní výkony měly nižší riziko komplikací a kratší dobu hospitalizace., Introduction: Radiofrequency ablation (RFA) is a well-established method for palliative therapy of unresectable liver tumors. We use an open or percutaneous approach for the treatment of colorectal liver metastases (CLM). Method: Clinical data of patients undergoing percutaneous or open RFA for CLM between January 2001 and January 2015 were included in the retrospective study. We evaluated clinical factors for overall survival (OS), no evidence of disease (NED) and non-ablation in relation to tumor sizes and numbers, type of approach and type of used probes. Results: 147 patients underwent RFA for CLM in this time period. Mean age was 65 years. 168 RFAs were performed in total. OS was influenced by a high number of censors. OS for the first and third years was 93.6% and 61% with no statistical differences between the percutaneous and open approach. NED was significantly shorter in patients with the percutaneous approach. NED was not influenced neither by size nor number of the lesions. A higher risk of non-ablation was observed as statistically significant in patients with percutaneous RFA. A higher, although not statistically significant, risk of non-ablation was also observed for larger metastases. Patients with percutaneous RFA showed a shorter stay in the hospital and fewer complications. Conclusion: RFA is an alternative approach to the treatment of unresectable CLM. In our study the open approach was associated with a lower risk of non-ablation. Percutaneous RFA showed a lower risk of complications and a shorter stay in the hospital., and J. Brůha, V. Liška, R. Úvod: Zhodnocení možnosti miniinvazivního odběru žilního graftu (Endoscopic Vein Harvest = EVH) za účelem cévně-chirurgické revaskularizace infrainguinálního tepenného systému končetiny. Zhodnocení našich časných výsledků a zkušeností s uvedenou metodou. Metoda: V časovém období duben 2012 až březen 2015 podstoupilo 16 pacientů endoskopický odběr velké safény (VSM) v rámci implantace infrainguinálního bypassu. Sedm (43,7 %) pacientů bylo indikováno k operaci pro obraz kritické končetinové ischemie (Rutherford kategorie 5). 9 (56,3 %) pacientů bylo indikováno k výkonu pro těžké intermitentní klaudikace (Rutherford kategorie 3). Průměrný věk pacientů byl 59,9 roku, mužů bylo 14 (87,5 %), ženy 2 (12,5 %). Selekce pacientů k endoskopickému odběru VSM probíhala na základě klinického a sonografického zhodnocení její vhodnosti. U pacientů byla sledována jejich předoperační anamnestická data, klinické nálezy, peroperační a pooperační průběh včetně komplikací. Pacienti byli prospektivně sledováni v časových intervalech 3, 6, 12, 18, 24 měsíců a následně ? 1 rok od operačního výkonu. Byla vyhodnocena průchodnost rekonstrukcí pomocí Kaplanovy-Meierovy metody. Statistická analýza souboru byla provedena s použitím softwaru IBM SPSS Statistics 21.0. Výsledky: U všech pacientů byla odebraná VSM použita k provedení proximálního (10; 62,5 %) nebo distálního (6; 37,5 %) femoropopliteálního bypassu (FPB). Jeden pacient, v časné fázi zavádění metody podstoupil po endoskopické disekci VSM konverzi na otevřený odběr. U 2 pacientů (12,5 %) pooperačně vznikla chirurgická ranná infekce (SSI) Szilagyi gr. II (1 pacient s dokončeným EVH – lokalizace tříslo, 1 pacientka po konverzi EVH – lokalizace tříslo a odběrová rána VSM). Průměrná doba sledování pacientů v souboru činila 10,2 měsíce. Primární, primární asistovaná a sekundární průchodnost po 1. roce sledování byla 82,0 %, 93,8 % a 100,0 %; po 2. roce sledování byla rovněž 82,0 %, 93,8 % a 100,0 %. Amputation-free survival po 1. i 2. roce sledování byl 100,0 %. Žádný pacient v průběhu studie nezemřel – letalita 0,0 %. Závěr: Endoskopický odběr VSM představuje miniinvazivní alternativu ke klasickému otevřenému odběru VSM u pacientů, kteří podstupují infrainguinální bypass. Naše časné výsledky femoropopliteálních bypassů s použitím metody EVH vykazují srovnatelné výsledky průchodností oproti rekonstrukcím s tradičním otevřeným odběrem VSM. Je současně patrná nízká incidence chirurgických ranných infekcí u takto provedených bypassů., Introduction: This study was undertaken to determine the feasibility of endoscopic vein harvest (EVH) for infrainguinal arterial bypass surgery. We describe our initial experience and early results of bypasses done using this minimally invasive approach. Method: From April 2012 to March 2015, 16 patients underwent 16 femoropopliteal bypass operations with great saphenous vein (GSV) being harvested by endoscopic technique. The indication for intervention was critical limb ischemia (Rutherford category “5”) in 7 patients (43.7%) and severe intermittent claudication (Rutherford category “3”) in 9 patients (56.3%). There were 14 male (87.5%) and 2 female (12.5%) patients, with a mean age of 59.9 years. Selection of patients for EVH was based on clinical and duplex ultrasound appearance of GSV. Only patients with adequate GSV were considered for EVH. We collected data regarding patients’ demographics, history, clinical findings, operative procedures and postoperative recovery including complications. Patients were followed at 3, 6, 12, 18 and 24 months postoperatively and yearly thereafter. Patencies were analyzed by Kaplan-Meier method. Statistical analysis was performed using IBM SPSS Statistics 21.0 software (IBM Corp, Armonk, NY). Results: Endoscopically harvested GSV was utilised for formation of proximal (10; 62.5%) or distal (6; 37.5%) femoropopliteal bypass. One patient underwent conversion to open harvest after endoscopic dissection of the vein. It occurred early in our experience. All other GSV harvests were accomplished endoscopically. 2 patients (12.5%) developed postoperative surgical site infection (SSI) Szilagyi gr. II (1 patient after successful EVH – location: groin; 1 patient after conversion of EVH to open vein harvest – location: groin and vein harvest incision). Mean follow-up was 10.2 months (range 0.3 to 27.0 months). At 1 and 2 years, primary patency was 82.0% and 82.0%, assisted primary patency was 93.8% and 93.8%, and secondary patency was 100.0% and 100.0%. At 1 and 2 years, amputation-free survival was 100.0% and 100.0%. No patient died within the study period (mortality 0.0%). Conclusions: Endoscopic harvest of GSV is a minimally invasive alternative to a standard open harvest of GSV. It is a feasible option for patients undergoing infrainguinal arterial bypass. In our early experience, patencies of EVH femoropopliteal bypasses are comparable to those achieved using traditional open vein harvest technique.
