Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Cíl: Cílem studie je představit dlouhodobé výsledky chirurgické léčby supratentoriálních kavernózních malformací mozku na Neurochirurgické klinice 1. LF UK a ÚVN‑VFN. <p align="">Soubor a metodika: Retrospektivně jsme do studie zařadili pacienty se supratentoriálním, kortiko‑subkortikálně uloženým kavernomem léčené v letech 2000–2012. Hodnotili jsme epidemiologická data pacientů, radiologický nález, klinický obraz onemocnění a operační výsledky včetně komplikací. <p align="">Výsledky: Iniciálním příznakem byl epileptický záchvat (u 49 % kavernomů), cefalea (22 %), fokální neurologický deficit (19 %) a 15 % bylo náhodně nalezených. Známky recentního krvácení na MR byly přítomny u 27 % pacientů. Operovali jsme celkem 145 pacientů s 158 kavernomy. Dvacet pět lézí bylo léčeno konzervativně. K chirurgické komplikaci došlo v 8 %. U dvou pacientů (1,4 %) se trvale zhoršil stav v přímé souvislosti s operací. Pooperační výskyt epileptického záchvatu byl signifikantně vyšší ve skupině s rannou infekcí či pooperačním hematomem (p < 0,05). <p align="">Závěr: Mikrochirurgická resekce hemisferálního kavernomu je relativně bezpečná operace s minimální morbiditou a mortalitou. Pooperační hematom nebo ranná infekce může mít epileptogenní potenciál., Aim: The aim of the study is to present surgical outcome of treatment of supratentorial cavernous malformation of the brain at the Department of Neurosurgery, Charles University and the Central Military Hospital in Prague. <p align="">Material and methods: We retrospectively enrolled patients diagnosed between 2000 and 2012 with supratentorial, cortico‑subcortically located cavernoma. We analysed epidemiological and radiological data, clinical presentation and surgical results including complications. <p align="">Results: Initial symptoms included epileptic seizure (49%), headache (22%) and focal neurological deficit (19%); 15% of cavernomas were found incidentally. Radiological signs of recent haemorrhage on MR scans were found in 27% patients. We performed surgery in 145 patients with 158 cavernous malformations. Twenty five lesions were treated conservatively. Surgical complications occurred in 8% of patients. One patient died and one had permanent neurological deficit attributable to surgery. Postoperative seizure rate was significantly higher in a group with wound infection or postoperative hematoma (p < 0.05). <p align="">Conclusion: Microsurgical resection of lobar cavernoma is relatively safe procedure with minimal morbidity and mortality. Úvod: Naše výsledky byly zhodnoceny za účelem posouzení bezpečnosti a efektivity endoskopické endonazální techniky v léčbě kraniofaryngeomů. <p align="">Materiál a metody: Od roku 2008 bylo endoskopicky endonazálně operováno14 pacientů s typickým supraselárním extraventrikulárním kraniofaryngeomem. Soubor se skládá z 11 mužů a 3 žen (věk 17–60 roků, průměrný věk 38 let). Jedenáct pacientů mělo předoperační deficit zorného pole v rozsahu od malého deficitu ve vnějším kvadrantu až k slepotě jednoho oka a těžkému deficitu druhého oka. Byla použita endoskopická endonazální technika čtyř rukou s peroperační 3,0 T MR. <p align="">Výsledky: V sedmi případech bylo dosaženo radikální resekce. Ve čtyřech případech byla provedena resekce subtotální, ve dvou případech resekce parciální. V jednom případě se jednalo o vypuštění kraniofaryngeomové cysty. V resekci po iMR bylo pokračováno ve třech případech. K úpravě zorného pole došlo v pěti případech. Pooperační diabetes insipidus se nově objevil ve třech případech. Reoperace pro únik mozkomíšního moku byla nezbytná ve třech případech. <p align="">Závěr: Endoskopické techniky v léčbě kraniofaryngeomu jsou bezpečné. Nicméně jsou spojeny s vyšším výskytem pooperačního úniku mozkomíšního moku ve srovnání s transkraniálními přístupy., Introduction: We reviewed our results in order to evaluate safety of endoscopic endonasal technique in the treatment of craniopharyngiomas. <p align="">Material and methods: Since 2008, endoscopic endonasal approach was used in 14 patients with typical suprasellar extraventricular craniopharyngioma. This cohort consisted of 11 males and three females (age 17 to 60 years, average age 38 years). Eleven patients had preoperative visual field deficit ranging from small deficit in the outer quadrant to blindness on one eye and a severe deficit of the second eye. Endoscopic endonasal four hands technique was used with intraoperative 3.0 T MRI. <p align="">Results: In seven cases, the radical resection was achieved. Subtotal resection was performed in four cases. The resection was partial in two cases. In one case, a cyst was drained. Resection after iMRI was performed in three cases. Visual field deficit improved in five cases. Postoperative diabetes insipidus developed in three cases. Reoperation for CSF leakage was necessary in three cases. <p align="">Conclusion: Endoscopic technique in the treatment of craniopharyngioma is safe. Cíl: Podle literatury dosahuje četnost komplikací kranioplastiky po dekompresivní kraniektomii i více než 30 %. Cílem studie je hodnocení četnosti, příčin a léčby komplikací kranioplastik po dekompresivní kraniektomii pro expanzivně se chovající supratentoriální ischemii nebo kraniocerebrální poranění a analýza důvodů neprovedení kranioplastiky u části nemocných po dekompresivní kraniektomii. Soubor a metodika: V retrospektivní studii nemocných po dekompresivní kraniektomii (2006–2014) byla kranioplastika provedena u 25 z 35 nemocných s expanzivní malacií (71,4 %) a u 33 z 68 zraněných s poúrazovou nitrolební hypertenzí (48,5 %). Výsledky: Nejčastější příčinou neprovedení kranioplastiky bylo úmrtí. U 20 % nemocných s mozkovou ischemií a 61,3 % zraněných byla kranioplastika provedena časně (do 10 týdnů). Četnost neurochirurgických komplikací byla u kranioplastik u nemocných s expanzivní malacií 12 % (explantace plastiky u 8 %) a u kranioplastik po kraniocerebrálním poranění dosahovala 9,1 % (explantace plastiky u 3 %). Hydrocefalus a subdurální kolekce byly před kranioplastikou přítomny u 6 a 9,1 % zraněných. Četnost hydrocefalu po kranioplastice byla u kraniotraumat 6 % a u mozkové ischemie 4 %. Závěry: Četnost komplikací kranioplastik, byť nižší než uvádí literatura, převyšuje hodnoty očekávané u elektivních operací. Subdurální kolekce a hydrocefalus nelze jednoznačně hodnotit jako komplikaci kranioplastiky, protože mohou být přítomny již před kranioplastikou v důsledku základního onemocnění., Aim: Reported incidence of cranioplasty complications after decompressive craniectomy may exceed 30%. The aim of the study was to analyse the incidence, causes and treatment options for neurosurgical complications of cranioplasty after decompressive craniectomy for expansive supratentorial ischaemic stroke and craniocerebral injury and to analyse the reasons for not performing cranioplasty in a subgroup of patients after decompressive craniectomy. Patients and methods: During a retrospective study (2006–2014) cranioplasty was performed in 25 of 35 patients after decompressive craniectomy for expansive supratentorial ischaemia (71.4%) and in 33 of 68 (48.5%) patients after decompressive craniectomy for posttraumatic intracranial hypertension. Results: Death was the most frequent reason for not performing cranioplasty after decompressive craniectomy. The frequency of early cranioplasties (< 10 weeks after decompressive craniectomy) was 20% in patients with ischaemic stroke and 61.3% in head-injured patients. The incidence of neurosurgical complications was 12% in patients after expansive brain ischaemia (cranioplasty explantation rate 8%). The incidence of cranioplasty complications in patients after craniocerebral injury was 9.1% (cranioplasty explantation rate 3%). Subdural collections and hydrocephalus were observed in 9.1% and 6% of patients after craniocerebral injury before cranioplasty, resp. Hydrocephalus requiring neurosurgical treatment was found in 6% of patients after craniocerebral injury and 4% of patients with brain ischaemia during the postcranioplasty period. Conclusions: Although the incidence of cranioplasty complications after decompressive craniectomy is lower than that reported in literature, it exceeds the rates expected for elective surgeries. Subdural collections and hydrocephalus cannot be unequivocally considered a complication of cranioplasty, because they are frequently observed before cranioplasty and may be caused by the disease requiring decompressive craniectomy. Úvod: Radiofrekvenční ablace (RFA) je etablovaná metoda využívaná k paliativnímu ošetření neresekabilních tumorů jater. Na našem pracovišti využíváme k ošetření metastáz kolorektálního karcinomu (CLM) otevřený či perkutánní přístup. Metoda: V retrospektivní studii (1/2001−1/2015) jsme vyhodnotili klinická data u pacientů podstupujících perkutánní nebo otevřenou RFA pro neresekabilní CLM. Cílem bylo zhodnotit faktory ovlivňující celkové přežití (OS), bezpříznakové období (NED) a nonablaci ložisek ve vztahu k velikosti a počtu ložisek, typu přístupu a typu použité sondy. Výsledky: RFA podstoupilo 147 pacientů s průměrným věkem 65 let. Celkem bylo provedeno 168 RFA výkonů. Celkové přežití bylo ovlivněno vysokým počtem cenzorů. Roční a tříleté OS bylo 96,3 % a 61 % bez statisticky významného rozdílu mezi perkutánním a otevřeným přístupem. NED bylo statisticky významně kratší u pacientů s perkutánním výkonem. NED nebylo ovlivněno ani velikostí, ani počtem ložisek. Vyšší riziko nonablace bylo pozorováno u větších ložisek, ale výsledek nebyl statisticky signifikantní. Perkutánní výkony měly kratší dobu hospitalizace a nižší počet komplikací. Závěr: RFA je alternativní metoda léčby neresekabilních CLM. Otevřený přístup měl v naší studii nižší riziko nonablace. Perkutánní výkony měly nižší riziko komplikací a kratší dobu hospitalizace., Introduction: Radiofrequency ablation (RFA) is a well-established method for palliative therapy of unresectable liver tumors. We use an open or percutaneous approach for the treatment of colorectal liver metastases (CLM). Method: Clinical data of patients undergoing percutaneous or open RFA for CLM between January 2001 and January 2015 were included in the retrospective study. We evaluated clinical factors for overall survival (OS), no evidence of disease (NED) and non-ablation in relation to tumor sizes and numbers, type of approach and type of used probes. Results: 147 patients underwent RFA for CLM in this time period. Mean age was 65 years. 168 RFAs were performed in total. OS was influenced by a high number of censors. OS for the first and third years was 93.6% and 61% with no statistical differences between the percutaneous and open approach. NED was significantly shorter in patients with the percutaneous approach. NED was not influenced neither by size nor number of the lesions. A higher risk of non-ablation was observed as statistically significant in patients with percutaneous RFA. A higher, although not statistically significant, risk of non-ablation was also observed for larger metastases. Patients with percutaneous RFA showed a shorter stay in the hospital and fewer complications. Conclusion: RFA is an alternative approach to the treatment of unresectable CLM. Cíl: Cílem naší práce bylo rozšířit a zobecnit výsledky resekcí inzulárních gliomů. Navazujeme na náš pilotní již publikovaný soubor (2009 – osm pacientů), jenž jsme rozšířili na 20 operací. <p align="">Soubor a metodika: V období 2007–2013 jsme provedli 20 operací gliomů se signifikantním objemem nádoru v inzule. Operace byly prováděny v celkové anestezii při monitorování motorických evokovaných potenciálů. Radikalita operací byla hodnocena pomocí manuální počítačové volumetrie prostřednictvím software StealthDTI (Medtronic, Minneaopolis, USA). <p align="">Výsledky: U jedné pacientky po resekci multiformního glioblastomu došlo po operaci k prokrvácení rezidua s následnou hemiplegií, míra závažných komplikací činí 5 %. Během 19 operací jsme dosáhli průměrné 83% radikality resekce (43–100 %). V průběhu 15 operací neenhancujících nádorů (LGG a AA) jsme dosáhli v T2 vážení průměrné 80% redukce objemu nádoru. <p align="">Závěr: Gliomy inzulární oblasti jsou indikovány k chirurgickému odstranění za podobných indikačních kritérií jako gliomy v jiných oblastech mozku. Nezbytná je detailní znalost anatomie inzulární oblasti včetně schopnosti manipulace s vitálními cévními strukturami., Aim: We present the results of surgical resection in a group of patients with insular gliomas, we refer to our pilot study (2009 – eight patients) and include 20 surgeries performed until 2013. <p align="">Groups and methods: From 2007 to 2013 we performed 20 surgeries for gliomas with a significant invasion of the insular region; all the surgeries were conducted under general anaesthesia with motor evoked potential monitoring. The extent of resection was evaluated by means of manual computerized volumetry (software Stealth DTI, Medtronic, Minneapolis, USA). <p align="">Results: In one patient after glioblastoma multiforme resection, we observed postoperative hematoma in the tumour remnant with consequent hemiplegia, the rate of major complications was 5%. The mean extent of resection in 19 surgeries was 83% (43–100%). In 15 non-enhancing tumours (LGG and AA), we obtained the mean tumour volumetric reduction of 80%. <p align="">Conclusions: The same indication criteria for surgical resection apply to insular gliomas as to gliomas in other regions.
