Tyto poruchy zahrnují vysokou frekvenci stolice, urgenci, evakuační poruchy, inkontinenci a sexuální a urologické dysfunkce. Samostatně jsou v přehledu rozebrány sexuální a močové dysfunkce., The most important quality parameters of rectal cancer surgery are oncological radicality, postoperative complications, recurrence rate and survival. Rectal dysfunction following low anterior resection occurs in up to 25–50% of patients. Despite its high frequency, however, it does not receive enough attention. Rectal dysfunction after surgery includes frequent bowel movements, urgency, incomplete evacuation, incontinence or sexual and urinary dysfunctions. The complex of symptoms is collectively referred to as the low anterior resection syndrome – LARS. In this review, we discuss the alterations in anorectal physiology after low anterior resection, the etiology and risk factors of LARS, different types of neorectal reservoir construction, and various options for prevention and treatment of LARS. Furthermore, sexual and urinary dysfunction is briefly reviewed., and P. Kocián, J. Hoch
Léčba kolorektálního karcinomu (KRK) vyžaduje multidisciplinární přístup s účastí gastroenterologa, chirurga, patologa, onkologa a radiologa. V případě metastatického KRK jsou možnosti léčby jednotné, použití kombinace chirurgické a cílené systémové léčby. Každá léčebná modalita KRK je zatížená rizikem komplikací. Proto je nezbytné s každým pacientem probrat prospěch a rizika navrhované léčby. Prezentujeme dvě kazuistiky pacientů s KRK, u kterých léčba vedla k dosažení remise i za cenu významných komplikací., Treatment of colorectal cancer (CRC) requires a multidisciplinary approach involving a gastroenterologist, surgeon, pathologist, oncologist and a radiologist. Treatment options depend on the stage of the disease, the overall condition of the patient and the molecular characteristics of the tumor. In the context of preoperative care, treatment procedures for cancer of the rectum is slightly different from the treatment of the colon cancer. In the case of metastatic CRC, there are unified treatment options, using a combination of surgical and targeted systemic therapy. Each treatment modality is at risk of complications. Therefore it is necessary to discuss the benefits and risks of the proposed treatment with each patient. We are presenting two cases of patients with CRC, where the treatment led to a remission, even at the cost of significant complications., Miroslav Zavoral, Tomáš Grega, Štěpán Suchánek, and Literatura
