Léčba kolorektálního karcinomu (KRK) vyžaduje multidisciplinární přístup s účastí gastroenterologa, chirurga, patologa, onkologa a radiologa. V případě metastatického KRK jsou možnosti léčby jednotné, použití kombinace chirurgické a cílené systémové léčby. Každá léčebná modalita KRK je zatížená rizikem komplikací. Proto je nezbytné s každým pacientem probrat prospěch a rizika navrhované léčby. Prezentujeme dvě kazuistiky pacientů s KRK, u kterých léčba vedla k dosažení remise i za cenu významných komplikací., Treatment of colorectal cancer (CRC) requires a multidisciplinary approach involving a gastroenterologist, surgeon, pathologist, oncologist and a radiologist. Treatment options depend on the stage of the disease, the overall condition of the patient and the molecular characteristics of the tumor. In the context of preoperative care, treatment procedures for cancer of the rectum is slightly different from the treatment of the colon cancer. In the case of metastatic CRC, there are unified treatment options, using a combination of surgical and targeted systemic therapy. Each treatment modality is at risk of complications. Therefore it is necessary to discuss the benefits and risks of the proposed treatment with each patient. We are presenting two cases of patients with CRC, where the treatment led to a remission, even at the cost of significant complications., Miroslav Zavoral, Tomáš Grega, Štěpán Suchánek, and Literatura
Stručně jsou také uvedeny vlastní zkušenosti s léčbou 25 pacientů ozářených pomocí HDR brachyterapie., Penile cancer is a rare tumour, however it has serious psychological and social consequences. The incidence of penile cancer increases with reaching with maximum in the sixth decade of life. The aetiological and epidemiological penile cancer risk factors are well recognized. Among them human papilloma virus infection, namely types HPV 16 and 18 is an important. Due to its low incidence about 1/100 000, there is limited experience with penile cancer therapy in most of urology or oncology departments. Randomised prospective studies are not available. Reports about surgical and other oncological therapy describe small studies with patients treated during long period. Early surgical treatment of penile lesions, and if possible of nodal metastases, plays an essential role in curing the disease and its prognosis. The review article highlights the importance of precise diagnosis and staging for the stratifi cation of the patients and selection appropriate therapy according to risk and prognostic factors. The role of contemporary radiotherapy and chemotherapy in multimodal treatment of penile cancer is discussed. Additionally, personal experience with HDR brachytherapy in the treatment of 25 patients is discussed., Hana Stankušová, and Literatura
Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. Stereotactic treatment irradiation has the important role in the treatment of brain tumors., Hana Doleželová, Petr Pospíšil, Pavel Šlampa, Irena Čoupková, Jan Garčič, Pavel Fadrus, Tomáš Svoboda, Iveta Selingerová, Ivana Horová, and Literatura
