Typ studie: retrospektivní. Cíl: Cílem studie bylo zjistit úspěšnost porodů u žen s císařským řezem v anamnéze na našem pracovišti. Rovněž bylo cílem určit rizikové faktory pro úspěšnost porodu po předchozím císařském řezu a míru rizika, kterou tyto pacientky podstupují. Metodika: Výzkumný soubor tvořilo 1 060 porodů, které na našem pracovišti proběhly ve sledovaném období. Z toho 103 žen mělo císařský řez v anamnéze a 70 z nich se pokusilo porodit vaginálně. Výsledky: Z 1 060 porodů bylo 19,2 % sekcí, ze 70 TOLAC (trial of labour after caesarean – pokus o spontánní porod po císařském řezu) bylo 54,3 % úspěšných (porodily vaginálně), ze 13 indukcí po sekci bylo úspěšných 23,1 %. Jako prediktivní faktor pro úspěšný TOLAC byl prokázán vaginální porod v anamnéze (p = 0,00), BMI < 35 (p = 0,04) a trend naznačil věk rodičky < 25 let (p = 0,05). Byl prokázán signifikantní rozdíl v krevní ztrátě (měřené odhadem) mezi pokusem o vaginální porod po sekci a bez sekce v anamnéze (p = 0,000), pokusem o vaginální porod po sekci a primární sekcí po sekci (p = 0,000) a mezi úspěšným a neúspěšným pokusem o vaginální porod po sekci (p = 0,000). Byl rovněž prokázán signifikantní rozdíl Apgar(ové) skóre v 1. a 5. minutě u dětí z úspěšných a neúspěšných pokusů o vaginální porod po sekci. Tyto výsledky byly dány do kontextu s doporučenými postupy z USA, Kanady, Spojeného království, Německa, Rakouska a Austrálie a Nového Zélandu. Závěr: Studie poskytla informace, se kterými je možné seznámit pacientku plánující pokus o vaginální porod po sekci na našem pracovišti. We also wanted to determine the predictive factors for successful vaginal birth and the amount of risk which the vaginal birth after caesarean brings to the patient. Metodics: There were 1,060 births in our study, that took place in our clinic at the specified time. Out of this set there were 103 women, who had caesarean delivery in medical history and 70 of those tried to deliver vaginally. Results: Out of 1,060 births there were 19.2 % caesarean sections, from 70 patients who tried to give birth vaginally after caesaren (TOLAC – trial of labour after caesarean) 54.3 % succeeded (the rest gave birth per sectionem caesaream), out of 13 induced labours after caesarean there were only 23.1 % successful. We determined following predictive factors for successful TOLAC: at least one vaginal birth in medical history (p = 0.00), BMI under 35 (p = 0.04) at the time of birth and age under 25 (p = 0.05). We found significant difference in the blood loss between following groups: TOLAC vs deliveries without caesarean in medical history (p = 0.000), TOLAC vs. elective repeat sections (p = 0.000) and successful vs. unsuccessful TOLAC (p = 0.000). We also found significant difference in Apgar score in first and fifth minute between the group of successful TOLAC and unsucessful TOLAC. These results were compared with the current guidelines in UK, USA, Canada, Australia and New Zealand, Germany and Austria. Conclusion: This study has brought us data, that should be provided to the patients who think about giving birth in our clinic and who have already caesarean section in medical history. Příčiny vzniku Parkinsonovy nemoci (PN) zůstávají po mnoho let předmětem živých diskuzí a intenzivních mezioborových výzkumů. Přibližně 5–10 % všech případů PN představují monogenní formy, které se projevují především u osob mladšího věku, zatímco většinu případů tvoří sporadická onemocnění multifaktoriálního rázu. Klíčovou molekulární událostí v rozvoji neurodegenerace při PN je porucha konformace malé vezikulární bílkoviny α‑synukleinu, jež iniciuje její fibrilizaci s tvorbou neurotoxických cytoplazmatických agregátů, Lewyho tělísek a Lewyho neuritů. Patologie α‑synukleinu při PN je podmíněna specifickým vzájemným působením faktorů prostředí, zvláštností genomu a systémového metabolizmu, což v souhrnu určuje charakter procesů buněčné detoxikace, fungování mitochondrií, synaptické transmise a endozomálního transportu. V přehledu jsou podrobně posouzeny možné exogenní a endogenní spouštěče patologického procesu při PN. Zvláštní pozornost při analýze rizikových faktorů náleží úloze různých neurotoxinů, prionové hypotéze rozvoje PN a rovněž současným představám o genetice familiárních a sporadických případů PN., Etiology of Parkinson’s disease (PD) for many years has been remaining the matter of active discussions and intensive interdisciplinary studies. About 5–10% of all cases of PD are represented by monogenic forms manifesting predominantly in younger persons, while most cases of the disease are sporadic and have multifactorial nature. The key molecular event in the development of neurodegeneration in PD is the conformational change of a small vesicular protein α‑synuclein initiating its self‑fibrilization with forming neurotoxic cytoplasmic aggregates and Lewy bodies/neurites. Misfolding of α‑synuclein in PD is caused by specific interaction of environmental factors, genomic factors and characteristics of systemic metabolism, which, in combination, determines the processes of cell detoxication, mitochondrial functioning, synaptic transmission and endosomal transport. In this review, possible exogenous and endogenous triggers of pathological process in PD are discussed in detail. Among the risk factors, special consideration is given to the role of different neurotoxins, prion hypothesis of the development of PD, as well as up‑to‑date knowledge about genetics of familial and sporadic cases of PD. Mentální anorexie je psychické onemocnění projevující se jako multifaktoriální porucha příjmu potravy, která postihuje především adolescentní dívky a mladé ženy, vedoucí k hladovění a nadměrné ztrátě hmotnosti. Může mít za následek více komplikací, které ovlivňují různé orgánové systémy. Ty vedou k závažným poruchám s nejvyšší úmrtností mezi všemi mentálními nemocemi. Pokud jde o prevalenci jiných poruch příjmu potravy, jako je bulimie a záchvatovité přejídání, tyto poruchy příjmu potravy jsou méně nebezpečné pro většinu klinických případů, nicméně také obtížně léčitelné. V našem sdělení se zabýváme také nedávnými změnami klasifikace poruch příjmu potravy a nabízíme stručný přehled o klinickém obrazu a epidemiologii nejčastějších poruch příjmu potravy se zaměřením na možnosti a úskalí prevence v uvedeném kontextu., Anorexia nervosa is a mental dysfunction manifested as multifactor eating disorder. It mainly affects adolescent and young women and leads to starvation and excessive weight loss, with associated complications that affect a number of organ systems. Other eating disorders such as bulimia nervosa and binge eating are clinically less dangerous but equally difficult to manage. Po téměř deseti letech od zahájení screeningu kolorektálního karcinomu (KRCA) v České republice došlo ke změně jeho programu. Hlavní epidemiologické ukazatele KRCA, incidence a mortalita, pro nás zůstávají nepříznivé. Sekundární prevence KRCA zatím nenaplňuje očekávání. Úprava programu spočívá ve volbě vlastní screeningové metody a modifikaci jejího intervalu. Zvýšení adherence populace ke screeningu se očekává od nové alternativy, primární screeningové kolonoskopie. Kvalitativní změnu přináší imunochemický test na okultní krvácení, který se jak u praktických lékařů a gynekologů, tak u pacientů těší stále větší oblibě. Prohlubuje se spolupráce mezi jednotlivými odbornostmi participujícími na screeningu. Prostřednictvím médií i jiných marketingových akcí se zvyšuje informovanost české populace. Data o výkonech poskytovatelů za rok 2009 naznačí, zda screening v nové podobě je krokem správným směrem. Efekt screeningu ovšem budeme moci zhodnotit až s odstupem několika let., Almost ten years after the introduction of colorectal cancer (CRC) screening in the Czech Republic, there has been a change in the programme. The major epidemiological indicators of CRC, i. e. incidence and mortality, still remain unfavourable. Secondary prevention of CRC has not fulfilled the expectations so far. The change of the programme consists in choosing an individual screening method and modifying its interval. Primary screening colonoscopy, which is a new alternative, is expected to increase the adherence of the population to the screening. A qualitative change has been brought by the use of an immunochemical faecal occult blood test that is becoming increasingly popular with general practitioners and gynaecologists as well as patients. The cooperation among the individual disciplines involved in the screening programme has become more in-depth. Through the media and other marketing strategies, the awareness of the Czech population has been rising. The 2009 data on the procedures done by providers will suggest whether this new form of screening is a step in the right direction.
