Crohnova choroba (CD) a ulcerózní kolitida (UC) jsou chronické střevní záněty (IBD) neznámého původu, v jejichž výskytu pozorujeme zřetelný západo-východní a severo-jižní gradient. Předpokládaná patogeneze zahrnuje působení dosud neidentifikovaných vnějších vlivů u geneticky predisponovaných jedinců za účasti některých složek střevní mikroflóry. V klinickém obraze obou nemocí se střídají období aktivity s fázemi klidovými; dominujícími symptomy jsou bolesti břicha a průjmy, typickým projevem UC jsou krvavé stolice. Nemocní s CD jsou často postiženi hnisavými komplikacemi v podobě abscesů a píštělí nitrobřišních nebo perianálních. Základem léčby je dlouhodobá medikamentózní terapie aminosalicyláty, kortikosteroidy, imunosupresivy nebo biologickou léčbou. Navzdory léčbě musí být značná část pacientů během života pro střevní zánět operována. Nejčastější komplikací obou nemocí je sideropenie a sideropenická anemie, nemocní jsou ohroženi také nežádoucími účinky podávaných medikamentů., Crohn’s disease (CD) and ulcerative colitis (UC) are chronic inflammatory bowel diseases (IBD) of unknown origin with a distinct west-east and north-south gradient in their geographic distribution. Unidentified external factors in combination with genetic predisposition and intestinal microbiome are supposed to cause both diseases. Periods of clinical activity and quiescent phases are typical of the disease course. Patients suffer from abdominal pain and diarrhoea, bloody stools usually occur in UC, while CD patients frequently develop purulent complications – intra-abdominal or perianal abscesses and fistulas. Patients have to use long-term medical treatment with aminosalicylates, corticosteroids, immunosuppressive drugs or biologic therapy. Despite medical therapy, many patients undergo surgery for their IBD. Těhotenství žen bez domova představuje specifický sociálně-zdravotní problém, který má řadu aspektů. Prvním z nich je minimální prevence otěhotnění plynoucí jednak z menší dostupnosti antikoncepčních prostředků z ekonomických důvodů, jednak z nízkého intelektu, případně z naděje, že těhotenství je cesta k vyřešení tíživé sociální situace za pomoci státu nebo dobročinných organizací. Vážným problémem je také značné zdravotní riziko pro těhotnou i pro plod plynoucí z nedostatečné intimní hygieny, z absence pravidelné gynekologické péče, z rizika nákazy sexuálně přenosnými chorobami, ze závislosti na návykových látkách i z malnutrice. Podobně je sociálně-zdravotně problematická i perinatální péče. Tato situace je demonstrována ilustrativní kazuistikou. The first one is minimum prevention of pregnancy as a result of a more difficult access to contraceptives for economic reasons, because of low intelligence or maybe a hope that pregnancy offers a way to solve a precarious social situation with the support of the state or charitable organizations. A serious problem is also a considerable health risk for the pregnant woman and fetus, resulting from inadequate intimate hygiene, the absence of regular gynecological care, risk of sexually transmitted diseases, addictions and malnutrition. Similarly problematic are the social and health aspects of perinatal care. The described state is illustrated by case studies. Po téměř deseti letech od zahájení screeningu kolorektálního karcinomu (KRCA) v České republice došlo ke změně jeho programu. Hlavní epidemiologické ukazatele KRCA, incidence a mortalita, pro nás zůstávají nepříznivé. Sekundární prevence KRCA zatím nenaplňuje očekávání. Úprava programu spočívá ve volbě vlastní screeningové metody a modifikaci jejího intervalu. Zvýšení adherence populace ke screeningu se očekává od nové alternativy, primární screeningové kolonoskopie. Kvalitativní změnu přináší imunochemický test na okultní krvácení, který se jak u praktických lékařů a gynekologů, tak u pacientů těší stále větší oblibě. Prohlubuje se spolupráce mezi jednotlivými odbornostmi participujícími na screeningu. Prostřednictvím médií i jiných marketingových akcí se zvyšuje informovanost české populace. Data o výkonech poskytovatelů za rok 2009 naznačí, zda screening v nové podobě je krokem správným směrem. Efekt screeningu ovšem budeme moci zhodnotit až s odstupem několika let., Almost ten years after the introduction of colorectal cancer (CRC) screening in the Czech Republic, there has been a change in the programme. The major epidemiological indicators of CRC, i. e. incidence and mortality, still remain unfavourable. Secondary prevention of CRC has not fulfilled the expectations so far. The change of the programme consists in choosing an individual screening method and modifying its interval. Primary screening colonoscopy, which is a new alternative, is expected to increase the adherence of the population to the screening. A qualitative change has been brought by the use of an immunochemical faecal occult blood test that is becoming increasingly popular with general practitioners and gynaecologists as well as patients. The cooperation among the individual disciplines involved in the screening programme has become more in-depth. Through the media and other marketing strategies, the awareness of the Czech population has been rising. The 2009 data on the procedures done by providers will suggest whether this new form of screening is a step in the right direction.
