V predkladanej štúdii autori ponúkajú prehľad problematiky zdravia rómskej populácie na Slovensku spolu s faktormi ktoré ho ovplyvňujú. Rozoberajú demografický vývoj Rómov v spojitosti s reprodukčným a populačným zdravím. Autori predkladajú analýzu zdravia Rómov založenú na výsledkoch parciálnych prieskumov a samohodnotení zdravotného stavu samotnými Rómami. Článok stručne mapuje oblasti najvýraznejších problémov zdravotného stavu rómskeho etnika na Slovensku. Informuje čitateľa o známych príčinách tohto stavu. Zlepšenie situácie v oblasti zdravia vidia autori v transkultúrnom prístupe k Rómom zo strany zdravotníckych pracovníkov a majoritnej spoločnosti a v realizácii efektívnej edukácie zameranej na podporu a ochranu zdravia., In the present study, the authors provide an overview of the health problems of the Roma population in Slovakia together with influencing factors and analyse factors. The demographic development of the Roma in conjunction with reproductive population health. The authors also present an analysis of the health of the Roma based on the partial results of surveys and self-assessed health status. The article briefly charts the area that is most outstanding in health problems of the Roma ethnic group in Slovakia. It informs the reader about the known causes of this condition. Koincidence výskytu zhoubného nádoru a gravidity není běžná situace, ale není vzácná. Nejedná se již o kazuistické případy, ale o soubory. Frekvence malignit v těhotenství je závislá na kvalitě sběru dat a zkoumané populaci. Jen velmi přibližně lze říci, že v našich euroatlantických podmínkách zdravotnické péče se jeden zhoubný nádor vyskytuje cca na 1 000 porodů. Vzhledem k jejich malé frekvenci a skutečnosti, že klinické příznaky tumoru jsou často připisovány těhotenským změnám, nebo jimi maskovány, dochází k pozdější diagnóze tumoru u těhotné než u negravidní ženy. Průměrný věk těhotných se zvyšuje. Vznik tumorů je v pokročilejším věku. Lze tudíž v budoucnu předpokládat nárůst této koincidence. V onkologii pak může vzniknout oblast (zatím naštěstí ještě ne subspecializace) onkotěhotenství (oncopregnancy). Nejčastější nádory v těhotenství jsou maligní melanom, karcinom děložního hrdla, lymfomy, karcinom prsu, ovariální karcinom a leukemie. Screeningově se u nás v těhotenství vyšetřuje pouze děložní hrdlo (cervikální cytologie, kolposkopie). Diagnóza ostatních nádorů je závislá na autoobservaci ženy a diagnostickém důvtipu ambulantního gynekologa. Diagnóza v těhotenství se neliší od netěhotné ženy, ale je v některých oblastech ztížená. Tumor markery těhotné jsou zvýšeny stejně jako denzita prsu. Těhotenské změny na cervixu ztěžují jak cytologickou, tak kolposkopickou a histologickou diagnostiku. Ultrazvukové změny na ovariu mohou být zavádějící. Léčba těhotné je v mnoha ohledech problematická, nicméně musí být stejně účinná jak u netěhotné. Žena nesmí být penalizována za to, že je těhotná. Léčba těhotné musí být individualizovaná. Musí se vzít v potaz jak typ a stadium zhoubného nádoru, tak přání těhotné v pokračování těhotenství a rizika, modifikace léčby, nebo jejího odložení do viability plodu, potažmo do jeho ukončení. Operační léčba je indikována z diagnostických, stagingových a léčebných důvodů. Pokud se netýká reprodukčních orgánů těhotné, je většinou dobře tolerována jak matkou, tak plodem. Již neplatí doporučení neoperovat v prvním trimestru. Diagnostické dávky ozáření jsou minimální a neovlivňují vývoj plodu. Nicméně tam, kde je to možné je vhodné dát přednost UZ a MRI před rtg a CT. Léčebné ozáření může způsobit výrazné poškození plodu ionizujícím zářením. Poškození je ovlivněno dávkou, typem tkáně a velikostí ozařovaného pole. Jde o buněčnou smrt, kancerogenezi, genetické ovlivnění, hypotrofii plodu, vznik mikrocefalie a mentální retardaci. Léčebné ozařování břicha je v těhotenství kontraindikováno. Chemoterapie je v těhotenství používána jako adjuvantní terapie. Vzhledem k negativnímu vlivu chemoterapie na plod jako jsou nitroděložní úmrtí, malformace, růstová retardace, vznik malignit v budoucnosti a dalších je obecná tendence se chemoterapii v těhotenství vyhnout, nebo ji alespoň posunout za první trimestr. Vzhledem k vysoké koncentraci chemoterapeutik v mateřském mléku, je kojení v průběhu chemoterapie kontraindikováno., The coincidence of malignant disease and pregnangy is not a current situation, but is not rare. These cases are no more casuistic, but at present, there are groups of these patients published. The frequency of malignit diseases in pregnancy depends of the quality of data collections in a defined population. Only very approximatively we can state, that in our euroatlantic conditions and medical care, one malignat disease is diagnosed in 1 000 pregnants. Because of such a low frequency and the fact, that the symptoms of a malignant disease are often attribuated to pregnancy changes, or are covered by them, the diagnosis is done in pregnants later than in non pregnant women. The mean age of pregnant women becomes higher. Tumor growth is characteristic for higher age. Thus we can suppose in the future the rise of the coincidence ot these two states. The most frequent tumors in pregnancy are malignant melanoma, breast, cervical and ovarian cancers, lymphomas and leukemia. There is only one screening method in pregnancy: the cervical cancer by means of PAP smears and colposcopy. The diagnostics of other tumors depends on autodiagnosis of the women or on the diagnostic skills of the obstetrician. The oncologic diagnosis in pregnancy doesn´t differ from non pregnant wonem. In some organs and tumors it is more difficult in pregnancy. Tumor markers are elevated in pregnancy, the breast density is increased and the pregnancy induces cervical canges similar to neoplasia. The oncologic treatment of pregnant poses a lot of problems, but must be as efficace as in non pregnant state. The treatment must be tailored for each patient. We must take in an account the type and the stage of the tumor, the desire of the women to continue the pregnancy, the risks and the modification of the treatment and the posibility of it´s postponing after the delivery or at least the viability of the fetus. The surgery is diagnostic, staging and curative. If it doesn´t touch reproductive organs of the patient, it is well tolerated by the woman as by the foetus. The surgery may take place even in the first trimester. Diagnostic ionisation doses are minimal and have no negative impact on foetal developpment. Newertheless we prefer US and MRI to rtg and CT. Radiotherapy may cause severe foetal dammage. This is influenced by the dosis, type of the tissue and the magnitude of the field influenced by the ionisation. It may cause the cellular death, cancerogenesis and genetic influence, foetal hypotrophia, microcephalia and mental retardation. Abdominal radiotherapy in pregnancy is contraindicated. In pregnancy only adiuvant chemotherapy is used. Due to it´s negative side effects on the foetus like intrauterine death, malformations, growth retardation, the malignant growth in the future, there is a tendency if possible to eliminate chemotherapy during the pregnancy or at least to apply it after the first trimester.
