V těhotenství je anafylaktická reakce naštěstí sice relativně vzácná, její důsledky ale mohou vést až k závažnému postižení matky i plodu. Ve většině publikovaných případů těžké anafylaxe v těhotenství bylo těžké postižení dokonce spíše na straně plodu než matky. Vznik anafylaxe se většinou nedá předvídat, první symptomy jsou někdy netypické, pouze lokální, ale rychle mohou přecházet v generalizované postižení celého organismu. Ať už jsou zprostředkovány protilátkami typu IgE (pravá anafylaxe), nebo vznikají na jiném podkladě (anafylaktoidní reakce), klinický obraz bývá obdobný. Je třeba myslet na to, že i klinicky nenápadné počáteční symptomy mohou mít průběh závažný a život ohrožující. Každé opomenutí a prodleva v diagnostice a léčbě anafylaxe může vést k fatálnímu konci. O úspěšném výsledku léčby rozhoduje velkou měrou medicínská zkušenost a včasnost zásahu. Protože je toto téma v současné literatuře zmiňováno spíše jen výjimečně, je cílem tohoto článku shrnout problematiku anafylaxe v těhotenství z hlediska patofyziologie, klinického průběhu, diagnostiky i léčby., Anaphylaxis in pregnancy is a relatively rare reaction but it can lead to serious maternal and fetal impairment. In published cases of severe anaphylaxis in pregnancy there were even more severe disabilities in the fetus than the mother. Development of anaphylaxis is usually unpredictable, the first symptoms are sometimes atypical and only local, but can move quickly in a generalized impairment of the whole organism. Whatever they are mediated by IgE antibodies (right anaphylaxis), or are caused on another basis (anaphylactoid reactions), the clinical picture is similar. It should be kept in mind that even clinically inconspicuous initial symptoms may progress into serious and life-threatening state. Any omission and delay in diagnosis and treatment of anaphylaxis can lead to fatal end. A successful outcome is largely dependent on experience and timeliness of medical intervention. Ultrazvukové měření děložního hrdla představuje dobře zavedenou metodu predikce předčasného porodu. Nicméně sensitivita a pozitivní predikční hodnota ultrazvukové cervikometrie zůstává stále relativně neuspokojivá. Proto je snaha upřesnit predikci předčasného porodu využitím dalších metod v kombinaci s ultrazvukovou cervikometrií. Jedna skupina výzkumných prací se zabývá hodnocením různých parametrů z cervikovaginálního sekretu – nejvýznamnější představuje fetální fibronektin (fFN). Pozitivní test na fFN ukazuje relativně nízkou pozitivní predikční hodnotu stran předčasného porodu, nicméně výhodou je vysoká negativní predikční hodnota – bezmála 95%. Data z posledních studií ukazují užitečnost hodnocení fFN spolu s cervikometrií. Nedávné objevy ukázaly, že T regulační lymfocyty (CD4+CD25+CD127 - dále zkráceně Treg lymfocyty) mohou hrát důležitou roli v patofyziologii předčasného porodu. Cílem výzkumu na našem pracovišti je vyhodnotit vztah mezi hladinou Treg lymfocytů a ultrazvukovou délkou děložního hrdla k riziku předčasného porodu. Pilotní data z naší práce ukazují, že kombinované hodnocení uvedených parametrů představuje mnohem lepší možnost predikce předčasného porodu než hodnocení obou parametrů samostatně. Je však potřeba dalšího výzkumu, který bude mít za cíl vyhodnotit korelaci mezi sníženými hladinami Treg lymfocytů a předčasným porodem., Ultrasound measurement of the uterine cervix represents a well-established method in the prediction of preterm labour. However, the sensitivity and positive predictive value of ultrasound cervicometry still remain rather unsatisfactory. Hence, the effort to provide more specific predictions of premature labour using other applicable methods in combination with ultrasound cervicometry. One group of studies have focused on determining various parameters from cervicovaginal secretion - the most important one being fetal fibronectin (fFN). A positive fFN test in the birth canal shows a relatively low positive predictive value in terms of premature birth; however, it does provide the advantage of a high negative predictive value, which is about 95%. Data from latest studies show the usefulness of fFN in combination with cervicometry. Recent discoveries suggest that T regulatory lymphocytes (CD4+CD25+CD127 - Tregs) may play an important role in the pathophysiology of preterm labour. The purpose of our study was to assess the relationship between maternal circulating Tregs and the sonographically measured uterine cervix and the risk of preterm labour. We can show for the first time that the combined assessment of Treg cell count and cervical length provides a much better prediction of preterm delivery that either parameter used on its own. Toxoplasmóza je jedna z nejčastějších zoonóz u nás. Patří mezi onemocnění, která mohou při primoinfekci v graviditě ohrozit vývoj plodu či způsobit mnohočetná poškození, často manifestovaná až po narození dítěte. Závažnost postižení, jeho variabilita a pravděpodobnost vzniku závisí na době, kdy se těhotná žena nakazila. V ČR máme k dispozici několik diagnostických metod. Základem je sérologické vyšetření (protilátky celkové i jednotlivých tříd, avidita IgG, komparativní imunoblot), prováděné z krve či plodové vody. Možný je i přímý průkaz DNA Toxoplasma gondii pomocí PCR z krve, plodové vody či po porodu z placenty. Prenatálně se provádí izolační pokus na myši (inokulací plodové vody). Pomocnou metodou je také ultrazvuk či MRI v průběhu těhotenství, se zaměřením na případnou fetopatii. Po narození je dítě z rizikové gravdity podrobně vyšetřeno a dlouhodobě sledováno. Terapii lze podat prenatálně i postnatálně, o jejím efektu se vedou diskuze. Základními preparáty jsou spiramycin, pyrimethamin a sulfadiazin (doplněny kyselinou folinovou). V článku jsou probrány indikace k jednotlivým vyšetřením a doporučeny nejvhodnější postupy v diagnostice, léčbě a také prevenci tohoto onemocnění u těhotných. Text se podrobně věnuje i interpretaci možných výsledků a obsahuje vyjádření k preventivnímu vyšetření těhotných žen u nás., Toxoplasmosis is one of the most common zoonoses in the Czech Republic. Toxoplasma infection during pregnancy can affect fetal development and cause multiple damages that often do not manifest until after the birth. The severity of pediatric disability, its variability and the probability of occurrence depend on the time of maternal infection. Serological diagnosis of toxoplasmosis in pregnant women is performed in two steps: first, total anti-Toxoplasma antibodies are detected, most commonly by the complement fixation test (CFT) combined with IgG ELISA. In the case of a negative result, the woman can be retested 1-3 months later. Low CFT titers show latent infection with a minimal risk of primary infection during pregnancy. When CFT titers of 1:64 or higher are obtained, the second step follows. Positivity in the IgM and IgA tests and low IgG avidity can confirm a recent infection. The diagnosis of fetal infection is based on the detection of anti-Toxoplasma antibodies, and of IgM antibodies in particular, and/or on the detection of Toxoplasma in amniotic fluid by PCR or the mouse test. Other helpful methods are fetal ultrasound or MRI focusing on fetopathy. Each child born from a high-risk pregnancy is thoroughly examined and followed up on a long-term basis. The detection of neonatal anti-Toxoplasma IgM antibodies and a neonatal IgG profile differing from the maternal IgG profile in comparative Western blot are the major markers of congenital infection. Toxoplasma infection can be treated prenatally and postnatally and the therapeutic outcomes are still being considered. The first-line options for congenital toxoplasmosis are spiramycin, pyrimethamine and sulfadiazine with folinic acid. The article discusses the indications for screening and recommends best practices for the diagnosis, treatment and prevention of toxoplasmosis during pregnancy. Trofoblastická nemoc představuje velmi heterogenní skupinu benigních i vysoce maligních stavů. Hlavní společnou charakteristikou je nízká incidence onemocnění v populaci, která může vést k diagnostickým rozpakům a závažným terapeutickým chybám. Současným cílem specializovaných center v Evropě i ve zbytku světa je stanovit jasná kritéria, která budou zárukou adekvátní léčby a porovnatelných výsledků. Obezita a nadváha se stávají velkým medicínským problémem současnosti. Komplikují a zhoršují prognózu mnoha situací včetně těhotenství. Tuková tkáň není jen pasivní zásobárna energie, ale produkuje též hormony a cytokiny, kterými ovlivňuje činnost celého organizmu. Obezita negativně působí nejen přímo na matku a její reprodukční zdraví, ale působí i na zdravotní stav a vývoj dítěte. Kromě dobře známých komplikací se začínají diskutovat i další otázky jako například neuropsychický vývoj dítěte nebo vliv na vyšší riziko rozvoje astma, Obesity and overweight are becoming a major problem in medicine. Obesity impairs the prognosis of many conditions, including pregnancy. Adipose tissue is not only a passive reservoir of energy but also produces hormones and cytokines, which interfere with the regulation of the whole organism. Obesity adversely affects not only the mother and her reproductive health but also the health and the development of the child. In addition to well-known complication issues such as neuropsychological development of the child or a higher risk of astma are now discussed.
