Mnohočetný myelom (MM) a solitární plazmocytom (SP) jsou nejčastější primární nádory páteře. Léčba MM a SP páteře je komplexní proces, který zahrnuje léčbu kauzální s cílem potlačit nádorový klon a dále léčbu podpůrnou, jejíž součástí je i chirurgická terapie. Operace přispívají k lepší kvalitě života a nabývají na významu a smysluplnosti zejm. díky zlepšené prognóze onemocnění. Navrhli jsme algoritmus operační léčby MM a SP a ten jsme dále prospektivně ověřovali a jeho vyhodnocení bylo cílem této studie. Pacienti byli operováni v průběhu 10 let, minimální doba sledování byl jeden rok. Soubor tvořilo 32 pacientů (20 mužů a 12 žen) s průměrným věkem 61 let. Operováno bylo celkem 61 obratlů. Důvodem k operaci byly projevy útlaku míchy a progredující neurologický deficit (hodnoceno Frankelovou škálou) nebo na konzervativní léčbu nereagující axiální bolest (hodnoceno VAS; Visual Analogue Scale) páteře. Provedena byla některá z těchto operací: vertebroplastika (VP), laminektomie, transpedikulární fixace, somatektomie a jejich kombinace, nebo okcipito‑cervikální fixace. Pooperační VAS a Frankelova škála byly vyhodnoceny za jeden rok, skiagramy a MR jsme provedli a zhodnotili každý jeden rok. U žádného pacienta nenastala lokální recidiva tumoru ani selhání stabilizace na operovaném úseku v celém sledovaném období. Průměrný předoperační VAS byl 6,8 a jeden rok po operaci se průměrný VAS snížil na 1,1. Všechny operační výkony vedly nejen ke kontrole bolesti, ale také k prevenci nebo zlepšení neurologického postižení po celou dobu sledování. Hodnotili jsme náš postup – algoritmus operací, míru radikality a jejich načasování jako optimální pro MM i SP., Multiple myeloma (MM) and solitary plasmacytoma (SP) are the most frequent primary tumors of the spine. Management of the spinal MM and SP is a complex process involving causal treatment to suppress the tumor clone, as well as supportive therapy, including surgery and radiotherapy. Surgery should be considered because of its favourable effect on disease prognosis. We proposed a surgical treatment algorithm in patients with spinal MM and SP and the aim of this article is to present prospective evaluation of this algorithm. Patients undergoing surgical treatment during the past 10 years were included in this study, with the minimum follow-up of one year. A total of 32 patients (20 males and 12 females) with the mean age of 61 years were included. Surgery of 61 vertebrae was performed. The procedures were indicated for progressing neurological deficit (Frankel score) and for axial spinal pain (VAS classification). We performed the following procedures: vertebroplasty, laminectomy, transpedicular fixation, somatectomy, and their combination, or occipito-cervical fixation. Frankel score and VAS was assessed one year after the surgery and X-ray and spinal MRI was performed every year. No local relapses of the tumor or stabilization failure were detected. The mean preoperative VAS was 6.8 and improved to 1.1 one year after the surgery. During the follow up period, we observed positive effect of surgery on pain control and on prevention or improvement of neurological dysfunction. The authors concluded that all surgical procedures, the extent of resection and timing were adequate in all subjects. Onemocnění z depozice lehkých řetězců (light chain deposition disease – LCDD) patří mezi vzácná systémová onemocnění způsobená monoklonální proliferací terminálně diferencovaných B-lymfocytů s produkcí volných lehkých řetězců imunoglobulinu a jejich ukládáním především v ledvinách či v dalších tkáních. Náplní sdělení je poukázat na úskalí diagnostiky této klinické jednotky a na příkladu série 4 nemocných s LCDD předložit důkazy o vysoké účinnosti léčby s bortezomibem z pohledu hematologické i orgánové terapeutické odezvy. Z našeho hodnocení vyplývá, že léčba bortezomibem je u LCDD velmi rychlá a účinná, bývá však při intenzivním dávkovacím schématu doprovázena řadou nežádoucích účinků. Z nich je nejzávažnější periferní neuropatie, ohrožující i případné setrvání nemocného v léčbě. Méně intenzivní režimy (tzv. bortezomib weekly) představují alternativu s nižším výskytem nežádoucích účinků. Vysokodávkovaná chemoterapie s podporou autologní transplantace krvetvorných buněk je u vhodných kandidátů doporučeným a relativně bezpečným postupem. Orgánová léčebná odezva se významně opožďuje za hematologickou odezvou a zůstává otázkou, zdali je postižení tkání depozity lehkých řetězců plně reverzibilní. The aim of our study is to show the pitfalls of the diagnostics, and to demonstrate the effect of bortezomib-based therapy on a series of 4 patients with LCDD, from the point of hematological and organ therapeutic response. We include that bortezomib based treatment provides rapid and effective hematological response. It is, however, often accompanied by adverse events, especially within intensive treatment schedules. The most serious adverse effects includes peripheral neuropathy, which might be dose or treatment-limiting. Less intensive regimens („bortezomib weekly“) suggest an alternative with expectation of lower incidence of adverse effects. Autologous stem cell transplantation is a recommended and relatively safe approach in convenient candidates. Organ response is significantly delayed after hematological response, and organ damage by light chain deposits might not be fully reversible. Náplní sdělení je, na pozadí rozboru diagnostiky a léčby vlastních tří nemocných, podání současných poznatků o poměrně vzácném a v běžné klinické praxi opomíjeném onemocnění, vyznačujícím se depozicí lehkého řetězce imunoglobulinu v tkáních životně důležitých orgánů. Onemocnění z depozice lehkých řetězců imunoglobulinu postihuje především ledviny a vede k progresivní poruše funkce s konečným orgánovým selháním. Kromě etiopatogeneze, odlišení idiopatické formy od formy provázející mnohočetný myelom a vzácně i jiné B-lymfoproliferativní stavy, je nastíněna problematika klinických projevů, diagnostiky, diferenciální diagnostiky a terapie včetně přínosu biologické léčby bortezomibem a vysokodávkované chemoterapie s podporou autologních kmenových buněk, vedoucí k podstatnému zlepšení kvality i celkové délky života nemocných s touto prognosticky stále závažnou nemocí., The aim of the paper within the background of the diagnostics and treatment of three patients is the summary of recent knowledge about a relatively rare disease which is characterized by the deposition of immunoglobulin light chains in vital tissues and organs, especially the kidneys, leading to progressive deterioration of their function with end stage organ failure. Bortezomib (Velcade) je lék ze skupiny inhibitorů proteasomu. Představuje zcela nový a jedinečný léčebný postup v léčbě mnohočetného myelomu (MM). Tato práce přináší retrospektivní analýzu léčebné účinnosti a nežádoucích účinků u nemocných s MM léčených v referenčních centrech CMG v České republice. Celkem bylo hodnoceno 424 nemocných MM léčených bortezomibem od června 2004 do června 2010. Medián délky sledování byl 12,4 měsíců. Průměrný věk nemocných byl 66 let (34,1–86,5). Jednalo se především o nemocné s relabujícím MM. Celkem 48,3 % (205/424) nemocných mělo bortezomib v rámci druhé léčebné linie. Léčebnou odpověď bylo možné hodnotit u 83,3 % (353/424) nemocných. Léčebné odpovědi (? 50% pokles MIg) bylo dosaženo u 52,7 % (186/353), z toho bylo u 14,2 % (50/353) nemocných dosaženo kompletní remise. Nejzávažnější toxicitou byla periferní neuropatie stupně 3 a vyšší u 14 % (49/358) nemocných. Trombocytopenie stupně 3 a vyšší byla u 28,8 % (103/357) nemocných. Infekční komplikace stupně 2 a vyšší byly u 33,6 % (121/359) nemocných. V průběhu léčby bortezomibem nedošlo v našem souboru nemocných ke zvýšení frekvence výskytu trombo embolických komplikací. Mimořádný přínos bortezomibu v klinické praxi je dán rychlým nástupem léčebné odpovědi a vysokým počtem dosažených kompletních remisí. Většina jeho nežádoucích účinků se zpravidla daří zvládat pomocí vhodných profylaktických opatření, případně krátkodobým přerušením a následným snížením dávky podle platných doporučení., Bortezomib (Velcade) is a representative of class of medicines called proteasome inhibitors. This drug represents quite new and unique medical treatment of the patients with multiple myeloma (MM). This article presents a retrospective analysis of effectiveness and adverse effects in patients with multiple myeloma treated at reference centers of CMG in the Czech Republic. A total 424 patients with multiple myeloma treated with bortezomib from June 2004 to June 2010 were evaluated. The median follow-up was 12,4 months. The mean age of patients was 66 year (34,1–86,5) and majority of them were patients with relapsed multiple myeloma. Of the total 424 patients, 205 patients had bortezomib as second line therapy (48,3 %). Assessment of terapeutic response was possible in 83,3 % of treated patients (353/424). Therapeutic response (defined as a ? 50 % decrease in the serum MIG) was achieved in 52,7 % (186/353). Among mentioned responses, complete response was obtained in 14,2 % (50/353). The most severe toxicity observed was peripheral neuropathy, grade 3 or higher was developed in 14 % (49/358). There was recorded a thrombocytopenia grade 3 or higher in 28,8 % (103/357), further occurance of infections grade 2 or higher in 33,6 % (121/359) and eventually we could observe increased number of thromboembolic disease as well. Rapid onset of therapeutic responses and high percentage of obtained complete responses are the main reasons of extraordinary clinical benefit of bortezomib.
