AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). INTRODUCTION: The splenium of the corpus callosum has a specific structure of blood supply with a tendency towards blood-brain barrier breakdown, intramyelinic edema, and damage due to hypoxia or toxins. Signs and symptoms of reversible syndrome of the splenium of the corpus callosum typically include disorientation, confusion, impaired consciousness, and epileptic seizures. CASE REPORT: A previously healthy 32-year-old man suffered from weakness, headache, and fever. Subsequently, he developed apathy, ataxia, and inability to walk, and therefore was admitted to the hospital. Cerebrospinal fluid showed protein elevation (0.9 g/l) and pleocytosis (232/1 ul). A brain MRI showed hyperintense lesions in the middle of the corpus callosum. The patient was treated with antibiotics, and subsequently, in combination with steroids. Two months later, the hyperintense lesions in the splenium and the basal ganglia had disappeared. Almost seven months since his hospitalization in the Department of Neurology, the patient has returned to his previous employment. He now does not exhibit any mental changes, an optic edema and urological problems have improved. In addition, he is now actively engaged in sports. CONCLUSION: We have described a case of a 32-year-old man with confusion, ataxia, and inability to stand and walk. The man developed a febrile meningeal syndrome and a hyperintense lesion of the splenium, which lasted for two months. Neurological changes, optic nerve edema, and urinary retention have resolved over the course of seven months. Rozpor mezi vědeckou evidencí a zdravotnickou praxí vede k nižší úrovni zdraví v populaci, a patří proto dnes k jedné z největších výzev v oblasti veřejného zdraví. Na tuto výzvu reaguje relativně nový a v České a Slovenské republice i dosud neznámý vědecký obor, který se nazývá implementační věda (implementation science). Cílem tohoto článku je představit základní východiska a poznatky implementační vědy českému a slovenskému publiku. Článek vychází z analýzy základních koncepčních článků publikovaných zejména v časopise Implementation Science. Obsah tvoří definice implementační vědy; popis implementačního procesu včetně identifikace jeho aktérů, kontextu a bariér; a popis modelů používaných v procesu implementace. Článek je doplněn o příklady z praxe relevantní pro českou a slovenskou neurologii a psychiatrii. Implementační věda přináší systematické poznatky a metody, které umožňují snižovat mezeru mezi vědeckou evidencí a praxí ve zdravotnictví., The conflict between scientific evidence and health care practice is associated with decrease in population health and currently represents one of the most urgent public health challenges. This challenge is being addressed by a relatively new scientific branch of Implementation Science, not yet known in the Czech Republic and Slovakia. The aim of this paper is to introduce the foundations of the Implementation Science to the Czech and Slovak audiences. The paper is based on a review of fundamental articles published mostly in the Implementation Science journal. The paper provides definitions of the Implementation Science, namely description of an implementation process including its actors, context and barriers as well as models applicable to implementation practice. The paper is complemented with practical examples related to the Czech and Slovak practice in neurology and psychiatry. The Implementation Science produces knowledge and methods to reduce the gap between scientific evidence and health care practice. Středoevropská klíšťová meningoencefalitida může být provázena myeloradikulitidou asi u 5 % nemocných. Autoři referují o 48leté ženě, která byla nejprve hospitalizována pro meningitidu, u níž nebyl určen původce, a to i přes rozsáhlé odběry na neurotropní infekce (včetně klíšťové meningoencefalitidy). Po zlepšení klinického stavu byla propuštěna domů. Po jednom měsíci byla znovu přijata na neurologické oddělení s těžkou tetraparézou, bolestmi svalů a bez poruch čití. Na podkladě klinického vyšetření a EMG byla stanovena diagnóza meningoradikulitidy s postižením motorických neuronů a předních kořenů. MRI nález potvrdil tuto naši diagnózu a nakonec se zjistily i zvýšené titry protilátek proti klíšťové meningoencefalitidě, ačkoliv byla tato žena řádně očkována. O dva měsíce později s podpůrnou medikamentózní léčbou a s intenzivní rehabilitací začala nemocná chodit v chodítku., Tick-born Central European encephalitis may be accompanied by myeloradiculitis symptoms in up to 5% of patients. We report a case of a 48-year old woman, who was at first hospitalized for unspecified meningitis, when all common neuroinfections were excluded – including tick encephalitis – and after improvement she was discharged home. One month later she was admitted at the department of neurology with severe tetraparesis, muscular pain, without sensory deficit. Diagnosis of myelopolyradiculitis with only motor neurons and anterior roots lesion was done according to clinical findings and EMG. MRI findings confirmed our diagnosis and titer antibodies against tick-encephalitis were considerably positive, though the woman was properly vaccinated. Diagnostika poruch čichu po úrazech hlavy patří mezi méně časté případy, které jsou lékaři neurologie a otorinolaryngologie nuceni řešit. Odborník ORL musí vyloučit především konduktivní poruchy čichu (synechie dutiny nosní, zánět vedlejších nosních dutin ? VDN), které jsou potenciálně léčitelné konzervativními a chirurgickými postupy. Měl by být stanoven stupeň postižení čichu a případně odhalena simulace. V námi uváděných kazuistikách popisujeme 3 případy poúrazových poruch čichu. První pacient byl léčen pro laterobazální poranění. S měsíčním odstupem od úrazu u něj byla potvrzena porucha čichu subjektivní olfaktometrií. Kontrolní vyšetření čichu provedené za 6 měsíců prokázalo hyposmii a simulace poruchy čichu byla vyloučena. Na základě tohoto vyšetření byl podán návrh na ztížení společenského uplatnění. Druhý pacient se dostavil pro poruchu čichu, kterou dával do souvislosti s prací v chemickém provozu. Po opakovaných dotazech na úraz hlavy a na základě provedených zobrazovacích vyšetření (především magnetické rezonance) byla etiologie poruchy čichu uzavřena jako poúrazová. Třetí kazuistika popisuje vznik anosmie po úrazu hlavy v okcipitální krajině. V článku se zabýváme diagnostikou jednotlivých příčin poruch čichu a vyšetřovacími metodami čichu., Jan Vodička, K. Po dva roky trvajících bolestech bederní páteře s narůstajícími klaudikacemi dolních končetin byl u 61leté ženy diagnostikován rozsáhlý tumor v oblasti těla L3 a retroperitonea. Nádor byl totálně odstraněn a provedena náhrada těla L3 a stabilizace. Po operaci přetrvávaly výrazné bolesti, oslabení dolních končetin a instabilita stoje a chůze. Po léčebné rehabilitaci došlo k vymizení bolesti, ústupu nestability i parézy levé dolní končetiny. Vývoj pooperačního stavu dokumentuje stabilometrické vyšetření., After 2-year period of persistent pain of lumbar spine with increasing intensity of lower extremities claudication a vast tumor of L3 vertebra extending to the retroperitoneum was diagnosed in a 61-year old woman. The total L3 vertebrectomy as well as complete tumor removal has been performed and situation was reconstructed and stabilized. Pronounced pain, lower extremities paresis and instability persisted after the surgery. The rehabilitation therapy led to the relief of the pain improvement of standing and gait instability and paresis. Stiff-person syndrome (SPS) is a rare disorder characterized by muscle stiffness and painful spasms. Misdiagnosis may occur due to the fact that the clinical picture of SPS is often atypical. The main pathophysiologic mechanism underlying the development of SPS is insufficient inhibition at the cortical and spinal levels. There is good evidence for a primary autoimmune etiology. A 61-year-old man was admitted to a neurological department due to muscle hypertonia with episodic attacks of painful spasms predominantly affecting axial muscles. The symptoms developed shortly after tickborne meningoencephalitis. Electromyography (EMG) revealed signs of continuous motor unit activity. Antibodies against glutamate decarboxylase (anti-GAD) were highly elevated. We present a case of a man who developed clinically severe anti-GAD positive SPS, provoked by tick-borne encephalitis. After therapeutic plasma exchange (TPE) a rapid, temporary improvement of the clinical and neurophysiological findings was noted. Only after being placed on long-term immunosuppression did the patient achieve stable recovery. Na 3 kazuistikách je demonstrován současný pohled na diagnostiku a léčbu trombózy esovitého splavu. Trombóza vzniká na infekčním nebo neinfekčním podkladě. Příčinou infekční trombózy je akutní mastoiditida, častá u dětí obvykle spojená s hyperkoagulačním stavem nebo dlouhodobý chronický středoušní zánět s cholesteatomem, především u dospělých s poruchou imunity nebo diabetes mellitus. Neinfekční trombóza vzniká především na podkladě poruchy koagulace a postihuje zvláště mladé ženy, kuřačky, gravidní nebo ženy užívající hormonální antikoncepci. Léčba zánětlivých otogenních trombóz esovitého splavu je založena na podávání antibiotik a chirurgické sanaci spánkové kosti; podání antikoagulační léčby a chirurgické vybavení trombu není jednoznačné. Naopak antikoagulační terapie je klíčová u neinfekčních trombóz splavu. U všech nemocných s trombózou splavu je třeba cíleně pátrat po přítomnosti vrozeného či získaného trombofilního stavu., Three case studies demonstrate current views on the diagnosing and treatment of thrombosis of the sigmoid sinus. The factors causing thrombosis are of infectious and non-infectious nature. The cause of infectious thrombosis is acute mastoiditis which is frequent in children and is usually linked with a state of hypercoagulation, or a long-term chronic middle ear inflammation with cholesteatoma, especially in adult patients with an immune system disorder or diabetes mellitus. Non-infectious thrombosis primarily develops as a result of disturbed coagulation and prevailingly affects young women, female smokers, pregnant women or women taking hormonal contraception. The treatment of inflammatory otogenous thrombosis of sigmoid sinus is based on the administration of antibiotics and surgical sanitation of the temporal bone; administration of anticoagulation medication and surgical treatment of the thrombus is still an issue. On the other hand, anticoagulation therapy is a key element in non-infectious sigmoid sinus thromboses.
