The aim of the study was to establish the frequency of hypovitaminosis D in children with type 1 diabetes mellitus (T1D), its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy. 58 children with T1D at the age 9–19 years were enrolled to the study. Vitamin D concentration less than 30 ng/ml was considered as insufficient. 37 children (63.79%) had vitamin D level under 30 ng/ml, from these 19 subjects (32.7%) had vitamin D level under 20 ng/ml and 2 subjects (3.44%) under 10 ng/ml. Children with vitamin D deficiency had significantly lower magnesium concentration and lower Z score of lumbar spine (−1.34 ± 1.24 vs. −0.30 ± 1.21, p = 0.01) compared to diabetics with sufficient vitamin D concentration. No significant difference was found in parameters calcium, phosphorus or glycosylated hemoglobin. Patients with diabetic nephropathy (n = 18) showed no significant difference in vitamin D, glycosylated hemoglobin or Z score of lumbar spine compared to the patients without nephropathy (n = 40). Subjects with nephropathy had significantly longer diabetes duration, significantly higher cholesterol and triacylglycerol concentration. Cíl: Cílem práce je zhodnotit endovaskulární léčbu iatrogenních poranění renálních tepen. Metodika: Od listopadu 2005 do listopadu 2013 jsme na našem pracovišti léčili jedenáct pacientů s iatrogenním poraněním renálních tepen. Jednalo se o pět žen a šest mužů ve věku od 45 do 71 let, věkový průměr byl 57, 8 roků. K iatrogennímu poranění došlo u šesti pacientů při perkutánní extrakci konkrementu, u dvou nemocných při parciální nefrektomii, u jedné nemocné při perkutánní nefrostomii a u dvou pacientů při biopsii transplantované ledviny. U sedmi pacientů bylo příčinou hematurie pseudoaneuryzma, u tří nemocných pseudoaneuryzma s arteriovenózní pištěli a u jednoho nemocného arteriovenózní pištěl. Vždy byly poraněny periferní větve renální tepny. U všech nemocných byly k embolizaci použity spirály. Výsledky: Technická úspěšnost v našem souboru byla 100%. U všech nemocných došlo k zástavě krvácení a u nemocných s transplantovanou ledvinou se zlepšila funkce. Nedošlo k žádné závažné komplikaci. Závěr: Endovaskulární léčba iatrogenních poranění renálních tepen je jednoduchá, bezpečná a velmi účinná., Aim: To evaluate the outcome of endovascular treatment of iatrogenic injury of renal arteries. Method: From November 2005 to November 2013 we treated 11 patients with iatrogenic injury of renal arteries. There were 5 women and 6 men, aged from 45 to 71; the average age was 57.8 years. Cause of injury was percutaneous extraction of concrements in 6 patients, partial nephrectomy in 2 patients and biopsy of transplant kidney in 2 patients. All patients had hematuria. Source of hematuria was pseudoaneurysm in 7 patients, pseudoaenurysm and arteriovenous fistula in 3 patients and arteriovenous fistula in one patient. Peripheral branches were injured in all patients. Renal injury was managed by coil embolization in all cases. Results: Technical success was achieved in 100%. Clinical success (stop hematuria, hemodynamic stability, improved function of transplant kidneys) was 100%. No major complication was found. Kolorektální karcinom postihuje asi 1 % nemocných s idiopatickým střevním zánětem (ISZ), ale na jejich mortalitě se podílí 10–20 %. Kolorektální karcinom se u těchto nemocných manifestuje v nižším věku než sporadický kolorektální karcinom. Molekulárně-genetické změny při ISZ-asociovaném karcinomu jsou podobné jako při sporadickém kolorektálním karcinomu, ale jejich časový sled a frekvence výskytu se liší. Kolorektální karcinom při ISZ je často multifokální a více agresivní. Rizikovými faktory jsou: trvání choroby, rozsah postižení střeva, začátek choroby ve věku do 30 let a její trvání ve věku nad 60 let, pozitivní rodinná anamnéza sporadického kolorektálního karcinomu, endoskopická a histologická aktivita zánětu a současná primární sklerozující cholangitida. Prevencí kolorektálního karcinomu při dlouhotrvajícím ISZ je dispenzární program, jehož hlavními prostředky jsou opakované kolonoskopie, převážně v intervalu 1–2 let, etážové biopsie a diagnostika dysplazie, tj. nádorové přeměny epitelu bez postižení slizniční proprie. Potvrzení dysplazie indikuje chirurgickou terapii (proktokolektomie a ileoanální pouch). Moderní endoskopické techniky umožňují častější záchyt dysplazie a snížení počtu biopsií. Další přínos lze očekávat od zavedení biomarkerů molekulární genetiky., Colorectal cancer (CRC) originates in approximately one per cent of subjects with idiopathic bowel disease (IBD). IBD-associated CRC is accompanied by similar genetic markers as sporadic CRC, but their order and frequency are different. IBD-associated cancer is frequently multifocal and more aggressive, The risk factors include: duration and extent of colitis, beginning of IBD at an early age and ongoing disease at a higher age (above 60 years), endoscopic and histological severity of inflammation, family history of sporadic CRC, and co- -existent primary sclerosing cholangitis. Intraluminální (mechanický) ileus tenkého střeva může být způsoben také fytobezoárem. V tomto případě se jednalo o fytobe-zoár, který byl zapřený v rozšířeném Meckelově divertiklu. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Cílem článku je prezentovat současná doporučení k léčbě karcinomu močového měchýře po iniciální transuretrální resekci tumoru. Adjuvantní intravezikální léčba chemoterapií a imunoterapií prokazatelně snižuje recidivy karcinomů a v případě imunoterapie při udržovací dlouhodobé léčbě dokonce i oddaluje progresi onemocnění. Karcinom prostaty patří mezi nejčastější nádorová onemocnění u mužů. V časném a lokalizovaném stadiu je léčitelný, určité procento pacientů však relabuje nebo se diagnóza stanoví až v pokročilejších stadiích, kde jsou léčebné možnosti limitované. Imunoterapie je kromě klasické hormonální terapie, radioterapie a chemoterapie další perspektivní léčebnou modalitou v těchto stadiích. Vznik nádorového onemocnění je mnohastupňový proces komplexní povahy, v němž samotné nádorové buňky představují heterogenní populaci zasazenou do kontextu mikroprostředí nenádorový ch buněk a tkání tvořících složitý nádorový „ekosystém“. Imunitní systém je schopen kontrolovat růst nádorových buněk v raných stadiích vzniku nádoru. S pokročilým růstem nádoru jsou imunitní mechanizmy víceméně paralyzovány a mnohdy na proliferaci nádorových buněk mají paradoxně podpůrný účinek.
