Cíl studie: Klinicky afunkční adenomy (KAA) představují zhruba 25–30 % všech tumorů hypofýzy. Neurochirurgická operace nádoru bývá často neúspěšná a zůstává po ní nádorové reziduum. Účinek následné biologické léčby je závislý na přítomnosti transmembránových receptorů v nádorových buňkách. Naším cílem bylo určit expresní profil somatostatinových receptorů (SSTR1–5) a estrogenového receptoru 1 (ER1) u 69 vzorků KAA. Materiál a metody: Vzorky adenomů se uchovávaly v RNAlater Tissue Protect až do doby zpracování v laboratoři. RNA jsme izolovali Trizolem a přepsali do cDNA pomocí SuperScriptu III. Exprese jednotlivých receptorů byla stanovena metodou kvantitativní real time PCR. Získané výsledky jsme přepočítali na housekeepingový gen pro beta-glukuronidázu. Výsledky: U vyšetřovaného souboru 69 afunkčních adenomů byly kromě SSTR4 a SSTR5 exprimovány všechny zbývající vyšetřované receptory. Mediány relativních hodnot byly: 63,8 % pro SSTR1, 55,4 % pro SSTR2, 19,6 % pro SSTR3, 2,6 % pro SSTR4, 9,2 % pro SSTR5 a 75,4 % pro ER1. Závěr: Variabilní exprese jednotlivých podtypů SSTR a ER1 může ovlivnit účinnost biologické léčby KAA. Proto by určení expresního profilu sledovaných receptorů mělo předcházet zahájení biologické léčby KAA somatostatinovými analogy či modulátory estrogenových receptorů., Objective: Non-functioning pituitary adenomas (NFA) comprise about 25–30% of all pituitary tumors. Transsphenoidal neurosurgery, the treatment of choice, is often unsuccessful and frequently leaves tumor remnants. The effect of subsequent biological treatment is mediated by transmembrane receptors in the adenoma cells. Our aim was to determine the somatostatin (SSTR1–5) and estrogen receptor 1 (ER1) expression profile in 69 specimens of NFA. Material and Methods: Adenoma samples were submerged in RNAlater Tissue Protect and transported frozen to the laboratory. The RNA was isolated by Trizol Reagent and transcribed to cDNA by SuperScript III. The expression of the receptors was determined by quantitative real time PCR. Results were normalized to the beta-glucuronidase housekeeping gene. Results: With exceptions of SSTR4 and SSTR5, all other subtypes of receptors were expressed in the examined tumor specimens. The median relative quantification values were: 63.8% for SSTR1, 55.4% for SSTR2, 19.6% for SSTR3, 2.6% for SSTR4, 9.2% for SSTR5, and 75.4% for ER1. Conclusion: Variable expression of somatostatin and estrogen receptors could influence the effectiveness of the biological treatment. Chronické myeloproliferativní neoplázie (MPN) představují různorodou skupinu myeloidních neoplázií, jejichž molekulární podstata je charakterizovaná přítomností mutace JAK2V617F. Po tomto zásadním objevu bylo zjištěno několik dalších mutací, což jen zdůraznilo nečekanou molekulární složitost. Ústředním rysem MPN je deregulace dráhy JAK/STAT a i když vzbudila velký zájem vzhledem k možnosti cílené léčby inhibitory JAK2, dospělo se k závěru, že ve většině případů, ne-li ve všech, jsou mutace JAK2 sekundární mutační událostí. Další opakující se otázkou je postižení genů ovlivňujících epigenetickou kontrolu genové exprese a nově také sestřih (splicing) RNA. Většinu těchto mutací mají rovněž pacienti s myelodysplastickými syndromy. Studie zaměřené na složitou klonální hierarchii MPN svědčí o stavu genetické nestability, který by mohl být buď získaný, nebo dědičný. V tomto ohledu nám objevení specifického zárodečného haplotypu u JAK2 poskytlo vysvětlení jevu „familiárního clusteringu” MPN, přestože se na něm pravděpodobně podílejí i jiné, dosud neznámé haplotypy. Cílem tohoto přehledu je shrnout současné poznatky o molekulárních abnormalitách MPN a probrat jejich úlohu v diagnostice a prognóze., Chronic myeloproliferative neoplasms (MPNs) represent a heterogeneous group of myeloid neoplasia whose molecular asset is marked by the presence of a JAK2V617F mutation. Following this seminal discovery, several other mutations have been identified highlighting an unexpected molecular complexity. Deregulation of the JAK/STAT pathway is a central feature of MPNs and, although it has raised much interest for the possibility of targeted therapy with JAK2 inhibitors, it has been realized that JAK2 mutations are secondary mutational events in most, if not all, cases. Another recurrent theme is the involvement of genes intervening in the epigenetic control of gene expression and, more recently, in RNA splicing. Most of these mutations are shared by patients with myelodysplastic syndromes as well. Studies focusing on the complex clonal hierarchy of MPNs suggest a condition of genetic instability, that could be either acquired or inherited. At this regard, the discovery of a specific germline haplotype in JAK2 provided an explanation for the phenomenon of familial clustering of MPNs, although other still unknown haplotypes are likely involved. Gastrointestinální stromální nádory (GISTy) jsou mezenchymální nádory gastrointestinálního traktu (GI), pocházející z intestinálních Cajalových buněk převážně ze žaludku a tenkého střeva. V posledním desetiletí došlo k rozvoji cílené molekulární léčby mnoha solidních nádorů a s objevem onkogenních mutací kináz u GISTů došlo k rychlému pochopení molekulárně biologické povahy těchto nádorů a přispělo k zavedení inhibitorů tyrozinkináz do klinické praxe i u tohoto typu nádorů. Proto v současné době hraje významnou úlohu molekulární genotypizace těchto lézí, především z hlediska prognózy, léčebné odpovědi, respektive primární nebo sekundární rezistence na cílenou léčbu., Gastrointestinal stromal tumors (GISTs) are mesenchymal tumors of the gastrointestinal tract (GI) arising from the interstitial cells of Cajal, primarily of the stomach and small intestine. During the past decade came up to progress of molecular targeted therapies for many solid tumors and in association with discovery of onkogenic kinase mutations in GISTs to rapid evolution in understanding of molecular biology features of these tumors and to implementation tyrosine kinase inhibitors in clinical practice. Programy podpory zdraví vyžadují při korektním zpracování nejen popis samotného prvku intervence, který je nástrojem projektu, ale také korektní vyhodnocení, tj. monitoring a evaluaci programu. Monitoring a evaluace jsou součástí sofistikovaného procesu, který má své nepodkročitelné standardy a podmínky, jež je nutno respektovat. Před hodnocením projektu je důležité stanovit konkrétní kritéria hodnocení – podmínkou správně vybraných kritérií jsou vhodně zvolené indikátory hodnocení. Nevhodně stanovené indikátory, které nereflektují cíle projektu a nejsou k nim relevantní, jsou nejčastější chybou většiny programů podpory zdraví. Je záhodno plánovat evaluaci projektu vnější i vnitřní a vyjmenovat její předem předpokládané překážky. Zároveň je dobré navrhnout vnímání obsahu termínu "efektivita projektu" u každého konkrétního programu, případně predikovat zamýšlený poměr ceny projektu k jeho účinnosti. Návrhy programů podpory zdraví by v rámci plánovaného monitoringu a evaluace měly obsahovat jasně stanovený způsob prezentace průběhu a výsledků, včetně závěrečných informací plynoucích z výsledků monitoringu a evaluace. Problematika monitoringu a evaluace programů podpory zdraví, jejich proces a metody by se měly stát předmětem zkoumání a dalšího vývoje v oborech zabývajících se prevencí a podporou zdraví., For correct processing, health promotion programmes require not only the key element of intervention as a project instrument, but also proper assessment, which means programme monitoring and evaluation. Monitoring and evaluation should follow a sophisticated procedure with specific standards and conditions that need to be respected. Prior to evaluation of the project, it is important to determine specific evaluation criteria; amongst basic conditions of properly selected criteria are correctly selected indicators of such an evaluation. Improperly set indicators, that do not reflect the objectives of the project and are not relevant to them represent the most commonly enacted mistake in the majority of health promotion programmes. It is advisable to plan both internal and external evaluation and to list anticipated obstacles and barriers in advance, in order to define the concept of "project effectiveness" for each specific programme, and attempt to predict a project "cost/effect" ratio. Proposals of health promotion programmes, as part of planned monitoring and evaluation, should include clearly defined methods for presenting the process, course and results of the programme, including the final information derived from the results of monitoring and evaluation. Cieľ: Cieľom práce bolo zistiť výskyt komplikácií v závislosti od veku a pohlavia pacientov, od dĺžky zavedenia stimulačnej elektródy, od zvolenej cesty prístupu zavedenia stimulácie u pacientov hospitalizovaných na Oddelení arytmií a koronárnej jednotky v Univerzitnej nemocnici Martin. Metodika: Pre zber potrebných údajov sme použili obsahovú analýzu zdravotnej dokumentácie, nemocničný informačný systém, pozorovanie na základe vypracovaného záznamového hárku. Údaje sme analyzovali pomocou deskriptívnej a induktívnej štatistiky. V sledovanom súbore bolo 38 pacientov od 58 rokov do 93 rokov. Pacienti boli akútne prijatí na Jednotku intenzívnej starostlivosti za účelom realizácie dočasnej kardiostimulácie, ktorá bola užívaná v priemere 5,28 ± 2,33 dní. Výsledky: Na základe spracovania údajov sme u 17 pacientov zistili výskyt komplikácií, pričom najčastejšou komplikáciou bola lokálna infekcia. Štatistickou analýzou sme zaznamenali významné vzťahy medzi dĺžkou zavedenia stimulačnej elektródy a výskytom lokálnej infekcie, kde nám vznikla silná signifikantná korelácia r(S) = 0,518 na hladine významnosti p = 0,001 a rovnako pri výskyte hematómu sme zaznamenali koreláciu r(S) = 0,329 na hladine významnosti p = 0,023. Záver: Analýzou získaných údajov sme identifikovali najčastejšie vyskytujúce sa komplikácie v spojitosti s dočasnou kardiostimuláciou. Najčastejšie sme zaznamenali výskyt lokálnej infekcie, ktorý súvisí s následnou ošetrovateľskou starostlivosťou, ale aj s významnými prediktormi, ktoré môžu do značnej miery ovplyvniť ich vývoj., Aim: The aim of our study was to determinate the incidence of complications depending on the age and gender of patients, duration of implantation of stimulation electrode, of selected access point of implementation stimulation in patients hospitalized at the Arrhythmia unit and Coronary care unit in University hospital in Martin. Methods: For the collection of required data we have used content analysis of medical records, hospital information system, surveillance based on own designed sheet. The data were analysed using descriptive and inductive statistic. The sample consisted of 38 patients in the range from 58 to 93 years of age. Patients were acutely admitted to ICU with the aim of realization of temporary cardiac pacing, which lasted 5.28 ± 2.33 days in average. Results: On the basis of processed data, we found incidence of complications in 17 patients, where the most frequent complication was local infection. Using statistical analysis we found a significant relationship between duration of implementation of stimulation electrode and incidence of local infection, which resulted in strong significant correlation (r(S) = 0.518, p = 0.001) and similarly, we noted correlation in the incidence of haematoma (r(S) = 0.329, p = 0.023). Conclusion: Using analysis of the obtained data we have identified the most frequently occurring complications in conjunction with the temporary cardiac pacing. Cíl: Cílem studie bylo zjistit potencionální riziko hypertenze u dětí ve věku 14 – 15 let dvojím standardním měřením krevního tlaku a zmapovat moţný vztah mezi hodnotami krevního tlaku dětí a jejich vybranými sociodemografickými faktory a tělesnými parametry. Metodika: Výzkumný soubor tvořilo 143 ţáků 8. a 9. tříd základních škol v Ostravě zapojených do projektu Zdravá škola. Data byla získána měřením fyziologických funkcí a antropometrických dat a doplňujícími otázkami na vybrané rizikové faktory kardiovaskulárních chorob. Výsledky: Měření krevního tlaku (TK) proběhla opakovaně, při druhém měření byla prokázána odlišnost naměřených hodnot TK mezi pohlavími, kdy u chlapců byly hodnoty TK vyšší neţ u děvčat. Bylo potvrzeno, ţe na hodnoty TK má vliv pohybová aktivita, a to zejména u dětí s vysokými hodnotami BMI. U normálních hodnot BMI korelace mezi hodnotou krevního tlaku a pohybovou aktivitou nebyla statisticky významná. Z rizikové skupiny 14 dětí s rodinnou predispozicí hypertenze bylo evidováno 29 % dětí s průměrnou hodnotou TK v pásmu pro 95. percentil (hypertenzní hodnoty) vzhledem k danému věku, pohlaví a výšce. Závěr: Výskyt vysokého TK byl četnější u dětí s nadváhou a obezitou. Pohybová aktivita ovlivňovala hodnotu krevního tlaku, zvláště pak právě u dětí s nadváhou a obezitou, kdy hodnoty TK u těchto dětí, které necvičily, byly vyšší neţ u dětí cvičících. Pravidelným měřením a správným zhodnocením TK můţe sestra včas zachytit děti a adolescenty s rizikovými parametry., Aim: The aim of the thesis is to examine potential risk of hypertension in children aged 14 - 15 years by two standard measurements of blood pressure and to find out possible relation between values of blood pressure in children and their selected socio-demographic factors and physical parameters. Methods: The research was carried out on 143 pupils from elementary schools in Ostrava, which participate in the project „Zdravá škola“(Healthy School). The data were obtained from measuring of physiological functions, survey of anthropometric data and further questions focused on particular risk factors for cardiovascular diseases. Results: The second measurement revealed a difference in values of blood pressure between sexes, when boys had higher blood pressure than girls. The research confirmed that blood pressure is influenced by physical activity, especially in children with high value of BMI. In children with lower values of BMI, the correlation between the value of blood pressure and physical activity was not statistically relevant. From a high-risk group of 14 children with genetic predisposition, 29% of children had average value of blood pressure in range of 95. percentile (hypertension) given age, sex and height. Conclusion: The occurrence of high blood pressure is common in children who suffer from overweight and obesity. The value of blood pressure is influenced by physical activity, especially in overweight and obese children, when value of blood pressure in the children who did not practice, were higher in children than exercisers. This is an experimental study of lung morphology in modeling sepsis on a background of severe purulentinflammatory disease of soft tissues (necrotizing fasciitis) by original authors' method. The study showed that early stages of sepsis (13 days) were characterized by manifestations of changes in lung tissue as vascular response, in the second period (the 7th day) by appearance and growth of nonobstructive microatelectases, whereas the third period (the 14th day) was characterized by progression of purulentnecrotic processes in soft tissues, development of surgical sepsis and acute respiratory distress syndrome. Cíl: Výzkum se zaměřuje na zjištění míry syndromu vyhoření, motivačních faktorů dle Herzbergovy dvoufaktorové teorie a jejich vzájemného vztahu u všeobecných sester pracujících ve Fakultní nemocnici Olomouc. Cílem výzkumu bylo poznání osobních priorit faktorů pracovního prostředí všeobecných sester a stupně jejich saturace zaměstnavatelem. Vzájemnou komparací zjištěného pak přehledně ukázat největší divergence mezi přáními a realitou všeobecných sester v pracovním prostředí a jejich souvislostí se syndromem vyhoření. Metody: Pro účely výzkumu byl zvolen kvantitativní přístup. Ke sběru dat byly použity dva dotazníky: standardizovaný dotazník MBI (Maslach Burnout Inventory) a nestandardizovaný dotazník Motivačních faktorů vytvořený na principech Herzbergovy dvoufaktorové motivační teorie a po diskuzích s manažery nemocnic. Výsledky: Z výsledků výzkumu vyplynulo, že existuje statisticky významný vztah mezi mírou syndromu vyhoření a mírou neuspokojení faktorů pracovního prostředí dle Herzbergovy teorie. Nepotvrdil se předpoklad o existenci signifi kantního negativního vztahu mezi mírou syndromu vyhoření a spokojeností s fi nanční odměnou. Na žebříčku preferencí mají nejvyšší pracovní hodnotu faktory mzda, péče o pacienty a jistota pracovního místa. Mezi nejvíce divergující faktory patřila mzda a jistota pracovního místa. Výsledky potvrzují protektivní vliv saturace faktorů pracovního prostředí dle Herzbergovy teorie na výskyt syndromu vyhoření. Závěr: Výzkum prokázal význam saturace faktorů pracovního prostředí jako protektivního faktoru ve vztahu k syndromu vyhoření. Potvrdilo se, že se zvyšující se mírou saturace se snižuje míra vyhoření u všeobecných sester pracujících ve Fakultní nemocnici Olomouc., Aim: Th e research deals with the rate of burnout syndrome, motivation factors of Herzberg’s two-factor motivation theory and their mutual relation in general nurses working in the University Hospital Olomouc. Th e aim was to identify personal priorities among hygiene factors with general nurses and the degree of their saturation by the employer. Comparison of the fi ndings clearly shows the biggest divergence between wishes and reality of the working environment of general nurses and the relation to burnout syndrome. Methods: A quantitative approach was chosen for the research. To collect data, two questionnaires were used: a standardised MBI (Maslach Burnout inventory) and a non-standardised Motivation factors questionnaire, which was on the principles of Herzberg’s motivation theory aft er several discussions with hospital management. Results: Th e result of the survey research revealed an existing, statistically signifi cant relation between the burnout syndrome rate and the degree of dissatisfaction with hygiene factors according to Herzberg’s theory. Th e assumption about the existence of a signifi cant negative relation between burnout syndrome rate and salary was not confi rmed. Factors of salary care for the patient and job security occupy the top levels on the preference list. Th e most diverging factors include salary and job security. Th e results confi rm the protective eff ect the saturation of Herzberg’s hygiene factors has on the burnout syndrome rate. Conclusion: Th e survey research confi rmed the importance of saturating hygiene factors as a protective factor against burnout syndrome.
