The purpose of this study was to investigate the content neurospecific markers protein S-100 and neuroenolaza in blood serum and tear fluid of patients with ocular ischemic syndrome. Material and methods. We observed 43 patients aged 57 to 79 years, mean age 67.3 ± 2.7 years. Control group consisted of 11 volunteers without ophthalmic symptoms. The main group consisted of 32 patients with OIS. The neurospecific proteins S100 and NSE were investigated in blood serum and tear fluid. Results. The study found that in patients of the control group the content of protein were within the normal range: S -100 in the tear fluid – 0,0662 ± 0,00335 mkg/l, in the blood serum 0,0508 ± 0,00241 mkg/l. In patients of the main group the indicators of protein in the tear fluid were elevated in all patients - 3,12 ± 0,246 mkg/l ( p<0.005). The normal evels in blood serum of marker S-100 was in 30 patients - 0,0589 ± 0,00303 mkg/l, while, in 2 patients protein S-100 were raised and averaged 0,2175±0,00725 mkg/l. It was found that in patients of the control group content of protein NSE in the tear fluid and blood serum were within normal values - 15,86 ± 0,148 Ng/ml, 15,60 ± 0,202 Ng/ml respectively. In the main group the amount of protein NSE tended to increase in the tear fluid in 23 patients and averaged 33,012 ± 3,2626 Ng/ml (p<0.005), a significant decrease the quantity of protein was observed in 9 patients, which amounted to 5,166 ± 0,8301 Ng/ml. At normal levels in the blood serum protein NSE detected in 30 patients and averaged 14,48 ± 0,263 Ng/ml, whereas, in 2 patients there was a significant increase of content of protein NSE and was 27,47 ± 3,068 Ng/ml. Conclusions. Článek Lidské embryo v perspektivě bioetiky se podrobně věnuje současné bioetické diskuzi o morálním statusu lidského embrya. Nejdříve si klade otázku, zda je lidské embryo od okamžiku oplození individuálním jedincem lidského druhu a kriticky hodnotí některé námitky proti individualitě lidského embrya. Poté se zaměřuje na problematiku kritérií personální identity a jejich relevanci pro bioetickou diskuzi. Kriticky se vyrovnává rovněž s tzv. konstitucionalizmem. Metafyzickou reflexi uzavírá obhajoba animalismu. V poslední části článku se hodnotí etický relativismus, utilitarismus a předkládá se nárys etické teorie Johna M. Finnise. Závěr práce tvoří teze, že lidské embryo je od okamžiku svého vzniku lidskou osobou obdařenou inherentními, nezcizitelnými lidskými právy., The article The Human Embryo from a Bioethical Perspective devotes in detail to the contemporary bioethical discussion on the moral status of the human embryo. First, it rises an issue whether a human embryo is an individual being of a human kind since the moment of fertilization and it critically assesses some objections against individuality of the human embryo. Following that it focuses on criteria of personal identity and its relevance for bioethical discussion. It critically deals with the so called constitutionalism. Metaphysical reflection is concluded by a defense of animalism. The last part of the article evaluates ethical relativism, utilitarianism and brings in an outline of an ethical theory of John M. In rats with leukemia activity of liver mitochondrial enzymes are decreased at 6 months. Later, we noted compensatory activation of complexes II and III of the respiratory chain and transition to decompensation stage by final deadline. The activity of complex IV of the respiratory chain remains high. The article presents results of a study of the relationship 'locus of control health and subjective wellbeing. In a heterogeneous sample (N = 239) in the stage of youth (1930 years) shall be established: locus of control determines 11.56% of the changes in the assessment of health, changes in health contribute to 12.82% of the variation in assessment of happiness and 6.10% in the measurement of life satisfaction. Young people with an internal locus of control are healthier, happier and more satisfied with their lives than those with an external locus of control. In a study of cause effect relationship is established that internal locus of control determines a healthier lifestyle. Inadequate health care, more severe risk factors (stress, unhealthy diet, inadequate sleep, smoking) among young people with an external locus of control, outline prospects for greater vulnerability of this group of the population of the so called "Diseases of modern society." Cíl: Zhodnotit doporučení pro management bolesti (nefarmakologickou intervencí) v souvislosti s extrakcí hrudního drénu v rámci ošetřovatelské péče o nemocné s hrudní drenáží. Metodika: Systematickou analýzou s využitím Evidence based nursing (klíčových slov s využitím Booleovských operátorů AND, OR i NOT) bylo v elektronických databázích, dostupných pro uživatele Masarykovy Univerzity (SCOPUS, EBSCO a MEDLINE – PubMed) vyhledáno v rámci primární analýzy 148 zdrojů. Po kritickém rozboru a s využitím vylučovacích kritérií bylo analýze podrobeno celkem 27 relevantních studií zaměřených na ošetřovatelskou péči o osoby s hrudní drenáží obecně. Podrobná druhostupňová analýza zahrnovala 7 relevantních zdrojů zaměřených na management bolesti u osob s hrudním drénem a zejména jeho extrakcí. Výsledky: Na základě kritické analýzy relevantních zdrojů nelze jednoznačně doporučit jednotnou strategii pro management bolesti, v rámci nefarmakologických intervencí, v souvislosti s extrakcí hrudního drénu. Nejčastěji je doporučován multimodální přístup analgetizace. Závěr: V dostupných relevantních zdrojích existují doporučení pro nefarmakologické ovlivnění bolesti v souvislosti s extrakcí hrudního drénu, která však nelze generalizovat s ohledem na sílu důkazu. Jedním z doposud nedostatečně ověřených postupů je užití fyzikální terapie, zejména aplikace chladu preemptivně. Dostupná doporučení nelze považovat za silné důkazy, ale navrhujeme jejich využití v klinické studii v podmínkách klinické ošetřovatelské praxe v České republice., Aim: To evaluate recommendations for pain management (by non-pharmacological intervention) in relation to the extraction of chest tube in the nursing care of patients with thoracic drainage. Methods: Through systematic analysis, using Evidence Based Nursing (keywords using Boolean operators AND, OR and NOT) the total of 148 sources were found in primary search in the electronic databases available to the users of Masaryk University (SCOPUS, EBSCO, and MEDLINE - PubMed). After a critical analysis with the use of exclusion criteria, the total of 27 relevant studies on nursing care for people with thoracic drainage in general were subjected to analysis. Detailed two-phase analysis included seven relevant sources focused on pain management in patients with chest drains, and especially its extraction. Results: On the basis of the critical analysis of relevant sources we cannot clearly recommend a unified strategy for pain management within non-pharmacological interventions in relation to the extraction of a chest tube. Using a multimodal analgesic approach is most frequently recommended. Conclusion: There are recommendations for non-pharmacological pain affecting in relation to chest tube extraction, but we cannot generalize them with regard to the strength of evidence. One of the insufficiently proven practices is the use of physical therapy, especially the pre-emptive application of cold. In a review of the book " Medical law and ethics " by Jonathan Herring, I made it my goal to familiarize the reader with the issues of medical law and ethics, which author discuss in his work . The book should be a valuable source of knowledge for both, experts and laymen, because the author dealt with many topics from different angles, that raise a number of legal and ethical issues (the issue of euthanasia , research on humans , cloning , sterilization , etc. .). Význam dnešního medicínského práva neustále stoupá, ale oblasti transplantologie není věnován dostatek pozornosti. Samotná nevědomost laické veřejnosti není tolik alarmující jako letitý nezájem a ignorace odborné veřejnosti našimi zákonodárci. Zákon č. 285/2002 Sb., o darování, odběrech a transplantacích tkání a orgánů a o změně některých zákonů nabyl, podle části osmé tohoto zákona, účinnosti dnem 1. září 2002. Od tohoto roku upozorňují lékaři, jak ústně tak i písemně, na velmi problematické ustanovení § 11 odst. 4, transplantačního zákona. Ani dnes, v roce 2011, však nedošlo k žádné změně v jeho znění, a to ani v podobě potenciálního řešení uzavřít mezinárodní smlouvu, které by alespoň z části mohlo přispět ke zlepšení situace. Vhodných orgánů k transplantacím není dostatek a na česká transplantační centra mohou být pak v případě odebrání orgánů od zemřelých cizinců podávány žaloby na porušení zákona, třebaže je v čekací listině několik urgentních čekatelů na transplantaci. O protizákonné jednání by se bohužel jednalo i v těch případech, kdy sama rodina zesnulého vznese dotaz, či požádá o možný odběr vhodných orgánů od svého zesnulého příbuzného, a tak jsou lékaři nuceni jejich přání odmítnout. Ve chvíli, kdy pak ve zdravotnickém zařízení čeká na složitý operační zákrok a záchranu života několik pacientů a lékařům se zároveň dostane svolení od příbuzných potenciálního dárce, dostávají se odborníci do situace, kdy by měli postupovat podle zákona, ovšem na úkor bioetiky a základních lidských práv a svobod. Takové počínání by bylo, dle mého názoru, nejen v rozporu s lékařskou etikou, ale zejména s nejvýše postavenými vnitrostátními zákony jako je Ústava České republiky a Listina základních práv a svobod. Na poli mezinárodního práva by se pak jednalo o přesah principů vyplývajících z Úmluvy na ochranu lidských práv a důstojnosti lidské bytosti v souvislosti s aplikací biologie a medicíny, tedy Úmluvy o lidských právech a biomedicíně, kterou je Česká republika od 1. října 2001 vázána. Ačkoliv je zcela nepochybné, že se transplantačním chirurgům vždy nepodaří zachránit život všem svým pacientům, je důležité, aby se lidé o medicínsko-právní problematiku začali, ve svém vlastním zájmu, více zajímat a nestaly se pouhým davem, jež slepě následuje některá ustanovení popírající etiku a základní lidská práva. Pro všechny tyto důvody se domnívám, že by bylo vhodné co nejdříve učinit kroky vedoucí ke změně transplantačního zákona. Na vybranou se zákonodárcům nabízejí dvě možná řešení, jak tuto kolizní situaci vyřešit. Prvním z nich, a dle mého názoru i vhodnějším, je zrušení ustanovení §11 odst. 4, o odběrech tkání a orgánů od mrtvých cizinců. Druhou možností by pak mohlo být uzavření mezinárodní smlouvy, která by speciálně danou problematiku upravovala. Transplantologie je velice perspektivním oborem, ale také citlivým a snadno zneužitelným v mnoha ohledech. Ať už se jedná o výše zmiňovanou problematiku odběru od mrtvých cizích státních příslušníků, nebo o velmi rozšířenou transplantační turistiku a obchodování s orgány, je nutné učinit mnohé změny a zabezpečení v legislativě. Zdravý člověk si jistě nepřipouští, že by se právě jeho mohla tato problematika jakkoliv dotknout. Je však nutné si uvědomit, že v takovýchto chvílích se nejedná pouze o něj samotného, a to ať už se vyskytne v roli čekatele, nebo případného dárce., Over the last years, the importance of the medical law has been increasing, however to the issue of transplantations among public has not been paid the sufficient attention. But this ignorance is not as alarming as a longtime lack of interest of Czech lawmakers. The Act No. 285/2002 – concerning a donation, removal and transplantation of the tissue and organs (Transplantation Act) – came into being on September 1, 2002. Since that time, physicians have referred to the contradictory provision of § 11, section 4 and also to the fact that this act is not seen as a contribution to a better clearance but rather as an increasing physicians’ uncertainty. Until now, lawmakers have not adjusted its wording, even the form of the international agreement, which would help us to solve this situation. There is no sufficient number of suitable organs and tissues for transplantations. In case of an organs removal from deceased foreigners, Czech Transplantation Centers are sued for law-breaking, although there are candidates for transplantation on a waiting list. It would be considered as an unlawful conduct even in those cases, when family ask for taking suitable organs from their deceased relatives, so that physicians are forced to reject their request. When patients wait for the surgical intervention and donor’s relatives agree with transplantation, medical facilities face the dilemma of following the law but at the expense of bioethics and human rights. This acting would be in a contradiction of the medical ethics and acts, such as the Constitution of Czech Republic and the Charter of Fundamental Rights and Basic Freedoms. In the context of the international law based on the Convention for the Protection of Human Rights and Dignity of the Human Being with regard to the Application of Biology and Medicine – Convention on Human Rights and Biomedicine, there would be an overlap of their principles which Czech Republic is obliged to follow from October 1, 2001. There is no doubt that physicians are not able to save each patient, but it is important to support people to be interested in medical and law issues. I presume that it would be appropriate to take action to change the Transplantation Act as soon as it is possible. From my point of view, there are two ways to solve this situation: to abandon the § 11, section 4 (perhaps the most acceptable), or to enter into the international treaty, which regulates this topic. Transplantology is a very promising field of medicine, but it is exploitable in many ways. Whether it is an issue of taking organs from deceased foreigner, or well-known “transplant tourism” and organ trade, it is necessary to have pillars in the legislative. A healthy person will not admit that this issue might be concerning him in any way. Introduction and Objectives: One of the characteristic changes of tumor formation is accumulation of genetic disorders in mitochondrial and nuclear genome. Mitochondrial disorders, from its side, are responsible for failure of metabolism, apoptosis, cell growth, formation of reactive oxygen species, etc. Overprpoduction of reactive oxygen species (ROS) significantly impacts the respiration chain enzymes and entirely the antioxidant system of mitochondria. Finally this may become a favorable condition for normal cells transformation. The purpose of the presented work was to study the mitochondrial defects and to establish their role in prostate cancer development. Results: Experimental results demonstrate significant increase of the activity of mitochondrial succinate dehydrogenaze (complex II) of the malignant epithelial cells of prostate, and slight changes in cytochrome oxydase (complex IV) activity. Also significant activation of the antioxidant system (glutathione-dependant system) of mitochondria in prostate malignant epithelial cells was revealed.
