Carnosine is a dipeptide formed of the amino acids β-alanine and histidine. Only a limited number of studies have examined the effects of carnosine on sympathetic nerve activation and anxiety. The present study was undertaken to determine the dose-related effects of carnosine on anxiety in the elevated T-maze test (ETM) with electrodermal activity (EDA). Carnosine was injected in three groups of rats with doses of 10 (low dose), 100 (medium dose) and 1000 (high dose) mg/kg i.p. Physiological saline was injected in the sham group. The anxiety scores of the rats were measured with ETM 20 minutes after injection. Then, SCL was measured. The decreased number of entries into the open arm (NEOA), the percentage of time spent in the open arm (% TSOA) and higher EDA [shown by skin conductance level (SCL)] indicate higher anxiety. The NEOA and % TSOA were lower in the high-dose group than in the other groups. SCL was lower in the medium-dose carnosine group than in the high-dose carnosine and sham groups. SCL was higher in the high-dose group than in the medium-dose and sham groups. Our results suggest that high-dose carnosine produced anxiety-like effects as assessed in the SCL and ETM. Diabetická nefropatie postihuje 20–40 % pacientů s diabetem a je hlavní příčinou selhání ledvin. Diabetici s alterovanou funkcí ledvin mají omezené terapeutické možnosti vzhledem k riziku akumulace perorálních antidiabetik a jejich metabolitů při redukované glomerulární filtraci. Dobrá metabolická kontrola je důležitá v časných fázích nefropatie ke snížení rizika progrese a ve stadiu selhání ledvin redukuje riziko progrese aterosklerózy a zlepšuje životní prognózu. Metabolizmus inzulinu je při renálním selhání změněn, clearance inzulinu je prodloužena a zvyšuje se riziko hypoglykemií. Inzulinová analoga krátkodobě působící mají rychlejší absorpci a dlouhodobě působící nižší riziko hypoglykemií. Mohou tak pozitivně ovlivnit kompenzaci diabetu u pacientů s diabetem a renální dysfunkcí., The main cause of renal failure is diabetic nephropathy which affects 20–40 % of diabetic patients. Diabetics with altered renal function have restricted therapeutic options due to the risk of accumulation of oral antidiabetic drugs and of their metabolites at a reduced glomerular filtration rate. Good metabolic control is very important during the early phases of nephropathy for reducing the risk of progression and in the stage of renal failure reduces the risk of progression of atherosclerosis and improves life prognosis. Metabolism of insulin is changed during renal failure, clearance of insulin is prolonged, the risk of hypoglycemia increases. Short-acting insulin analogues have faster absorption and long-acting analogues have a lower risk of hypoglycemia. Inzulinovou rezistenci (IR) definujeme jako stav, při němž normální koncentrace volného plazmatického inzulinu vyvolává sníženou odpověď organizmu. V užším slova smyslu IR chápeme jako poruchu účinku inzulinu v cílové struktuře, která může vzniknout na kterékoli úrovni inzulinové signalizační kaskády. V klinických podmínkách ji obvykle definujeme jako poruchu účinku inzulinu v metabolizmu glukózy, i když platí, že porucha se může se týkat různých účinků inzulinu a různých buněčných struktur. Charakteristickým rysem IR svázané s metabolickým syndromem či diabetem 2. typu je defektní signalizace, která postihuje PI3-kinázovou větev inzulinové signalizační kaskády. Další účinky inzulinu, které závisí na signalizaci vedoucí přes Ras komplex a MAP-kinázu, nemusí být postiženy. Vlivem kompenzatorní hyperinzulinemie mohou být dokonce zvýšeny. Článek shrnuje některé novější poznatky týkající struktury a regulací inzulinové signalizační kaskády a rozebírá vybrané primární a sekundární příčiny IR, které zahrnují faktory genetické a epigenetické, regulační roli mikroRNA a metabolické, humorální a imunologické faktory. Detailní poznání příčin IR nabízí možnosti její racionální léčby. Ta se v současné době opírá o léčbu odstranitelných příčin IR, tj. důslednou kompenzaci diabetu, redukci hmotnosti, režimová opatření (dieta, fyzická aktivita), přehodnocení potřeby léčby kortikoidy, léčbu přidružených onemocnění, a případně podání metforminu či pioglitazonu. In the narrower sense we understand IR as the impairment of insulin action in the target structure which may arise at any level of the insulin signalling cascade. In the clinical conditions we usually define it as the impairment of insulin action in glucose metabolism, although it is true that the impairment may concern different effects of insulin and different cell structures. The characteristic feature of IR linked to the metabolic syndrome or Type 2 diabetes is defective signalling which affects PI3-kinase branch of insulin signalling cascade. Other insulin actions depending on the signalling through the Ras complex and MAP-kinase, may not be affected. Due to compensatory hyperinsulinemia they may be even increased. The article summarizes some recent findings regarding the structure and regulation of insulin signalling cascade and analyses selected primary and secondary causes of IR which include genetic and epigenetic factors, the microRNA regulation role, metabolic, humoral and immunological factors. The detailed knowledge of the causes of IR opens possibilities of its rational treatment. This is currently based on the treatment of curable causes of IR, i.e. consistent compensation of diabetes, weight reduction, regimen arrangements (diet, physical activity), re-assessment of the need to use corticosteroids in therapy, treatment of coexisting conditions and possibly administration of metformin or pioglitazone. Cíl: Ukázat na možnosti a bezpečnost IRE v léčbě nádorů. Metodika: Uvádíme soubor 23 nemocných s adenokarcinomem pankreatu, dvou nemocných se solitární jaterní metastázou kolorektálního karcinomu a jednoho adenokarcinomu solitární ledviny. Všichni byli léčeni pomocí IRE perkutánně pod CT kontrolou nebo při laparotomii s US asistencí. Výsledky: IRE je bezpečná metoda, 30denní mortalita nulová. Umožňuje destrukci nádorů v lokalitách, kde ostatní ablační metody selhávají. U neresekabilních nádorů pankreatu však v naší sestavě nepřinesla významné prodloužení života. Průměrná doba přežití ve skupině nádorů pankreatu je 10 měsíců. Závěr: IRE je bezpečná a užitečná ablační metoda zvláště pak nádorů umístěných u kritických struktur., Aim: To show the safety and clinical usefulness of IRE. Methods: A total of 23 pancreatic adeno-carcinomas, two liver mets of colorectal carcinoma and 1 renal adeno carcinoma have undergone IRE percutaneously under CT control or during laparotomy with US guidance. Results: IRE is safe method, 30 days mortality 0. There is no significant prolongation of survival in the group of unresectable pancreatic cancer. Survival in the group of pancreatic cancer is 10 months. Úvod: Ireverzibilní elektroporatizace (IRE) je relativně nová metoda tkáňové ablace. Hlavním atributem a největší výhodou oproti jiným metodám je mechanismus působení, který nevyužívá tepelné energie. V tomto souborném sdělení jsou uvedeny současné zkušenosti s experimentálním a klinickým použitím této metody. Cíleně se pak zabývá jejím použitím u lokálně pokročilého karcinomu pankreatu. Metoda: Podstatou mechanismu je vznik mikropórů v membráně buňky, které poruší nitrobuněčnou homeostázu, to vede k řízené smrti buňky – apoptóze. Absence tepelné denaturace bílkovinných struktur chrání cévy a vývody v okolí ablace. Tato vlastnost je klíčová pro použití této metody u pankreatické tkáně, kde pro komplikované anatomické poměry ostatní, termické metody selhávaly pro vysokou periprocedurální morbiditu a mortalitu. Preklinické a klinické studie: Schopnost šetřit cévní struktury a žlázové vývody byla ověřena celou řadou animálních experimentů. V klinické, humánní medicíně pak byla bezpečnost metody ověřena na tkáních jater, ledvin, plic a pankreatu. IRE v léčbě pokročilého karcinomu pankreatu: Největší zkušenosti s použitím ireverzibilní elektroporatizace jsou u lokálně pokročilého karcinomu pankreatu, kde klinické studie z posledních 5 let přinášejí povzbudivé výsledky. Závěr: Ireverzibilní elektroporatizace je bezpečnou metodou použitelnou ke zmenšení tumorózní masy u karcinomu pankreatu. Stanovení její efektivity v léčbě vyžaduje další studie., Introduction: Irreversible electroporation (IRE) is a quite novel method of tissue ablation. Its mechanism of action that does not use thermal energy is the most important feature of the method. Current experience with IRE in animal studies and in clinical practice are summarized in the paper. In particular, the paper is focused on using IRE in locally advanced pancreatic carcinoma. Method: The basic principle of IRE is that it causes micropores in the phospholipid membrane of cells. This leads to an impairment of cellular homeostasis and programmed cell death – apoptosis. Because of absence of protein denaturation this method spares tubular structures like vessels and ducts. This is the key feature that allows to use IRE in the pancreas where common thermic ablative procedures cannot be used for difficult anatomic circumstances and resulting injury of surrounding structures. Pre-clinical and clinical studies: The ability to spare vascular structures and ducts was confirmed in many animal studies. Subsequently, IRE was safely utilized also in human liver, pancreas, lung and kidneys. IRE in the treatment of advanced pancreatic cancer: Most experience with IRE ablation has been gathered for locally advanced pancreatic carcinoma where clinical studies published in the recent 5 years have provided encouraging results. OBJECTIVE: The aim of the present study was to investigate the effect of allergic rhinitis on the success of the operation in chronic otitis surgery by using score for allergic rhinitis (SFAR). MATERIALS AND METHODS: In the present study; 121 patients, who underwent type 1 tympanoplasty were examined retrospectively. SFAR of all patients were recorded. The graft success rates of 26 patients with allergic rhinitis (AR) and 95 patients with no allergic rhinitis group (NAR) were compared. RESULTS: While the graft success rate in NAR group was 89.5%, this rate was 80.8% in the AR group. However, the difference between groups was not statistically significant (p = 0.311). CONCLUSION: These findings suggest that allergic rhinitis decreases the graft success rate of the pathologies occurring in eustachian tube, middle ear and mastoid although statistically significant difference wasn't found. Cíl: Cílem studie bylo zhodnotit přínos elektrofyziologického vyšetření v diferenciální diagnostice lumbální spinální stenózy (LS) a diabetické polyneuropatie (DPN). Soubor a metodika: Elektrofyziologické vyšetření bylo provedeno u 68 pacientů s klinicky symptomatickou LS, u 28 pacientů s DPN a 32 zdravých dobrovolníků. Výsledky: Vyšetřené elektrofyziologické parametry z horních končetin (latence F vlny n. ulnaris a amplituda senzitivního neurogramu n. radialis), všechny hodnocené latence z dolních končetin (F vlny n. tibialis, H reflexu m. soleus, spinální latence MEP) a amplituda senzitivního neurogramu n. suralis signifikantně odlišovaly pacienty s LS a DPN. ROC analýza nicméně prokázala pouze dva elektrofyziologické parametry jako efektivní nezávislé faktory pro odlišení pacientů s LS a DPN: latenci ulnarisové F vlny (optimální diskriminační hodnota je 24,2 ms, senzitivita 82,7 % a specificita 63,9 %) a amplitudu senzitivního neurogramu n. radialis (optimální diskriminační hodnota je 10,5 ?V, senzitivita 75,5 % a specificita 58,2 %). Vícerozměrná diskriminační analýza poskytla kanonické skóre s nejsilnější prediktivní hodnotou. Závěry: Elektrofyziologické vyšetření z horních končetin je užitečné v diferenciální diagnostice LS a DPN, nicméně nejlepší diskriminační efekt vykazuje kanonické skóre, které zahrnuje několik elektrofyziologických parametrů. Methods: Electrophysiological parameters were examined in 68 patients with clinically symptomatic LS, in 28 patients with DPN, and in a group of 32 healthy volunteers. Results: Electrophysiological parameters evaluated from the upper extremities (F-wave latency of the ulnar nerve and radial SNAP amplitude), all the latencies measured in the lower extremities (tibial F-wave, soleus H-reflex and spinal latency of MEP), and the sural SNAP amplitude contributed significantly to distinguishing the LS from DPN patients. ROC analysis, however, disclosed only two electrophysiological parameters as effective in independent discrimination between the LS and DPN patient groups: ulnar F-wave latency (cut-off point at 24.2 ms, sensitivity 82.7% and specificity 63.9%); and radial SNAP amplitude (cut-off point at 10.5 ?V, sensitivity 75.5%, specificity 58.2%). Multivariate discrimination provided a canonical score with the most powerful predictive value of all. Poúrazový hydrocefalus způsobený poruchami cirkulace mozkomíšního moku často komplikuje klinický průběh a proces léčby po kraniocerebrálním poranění. Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. I když k rozvoji hydrocefalu po míšním poranění dochází výjimečně, tato možnost by měla být zvážena při pozdním zhoršení klinického stavu nemocného po poranění krční míchy, zvláště při přítomnosti anatomických anomálií mokového prostoru a poúrazovém subarachnoidálním krvácení., Posttraumatic hydrocephalus caused by cerebrospinal fluid circulation disturbances frequently complicates the clinical course and treatment after craniocerebral injury. Hydrocephalus complicating spinal cord injury is only exceptionally reported. The paper presents two cases of complete cervical spinal cord injury with subsequent development of hydrocephalus. The analysis of both cases and literature data confirmed the dominant role of non-spinal factors in the development of hydrocephalus after spinal cord injury. Despite the exceptional occurrence of hydrocephalus after spinal cord injury, this diagnosis should be considered in cases of delayed deterioration of a patient with cervical spinal cord injury, particularly if cerebrospinal fluid space abnormalities and posttraumatic subarachnoid haemorrhage are present., and J. Přítomnost kardiovaskulárního onemocnění a zejména ischemické choroby srdeční u pacientů s diabetes mellitus 1. typu vede ke zkrácení očekávané délky života. Hyperglykemie, která zvyšuje oxidativní stres, je považována za klíčový faktor v jejich etiopatogenezi. Kalcifikace v koronárním řečišti, přítomnost srdeční autonomní neuropatie a diabetické nefropatie představuje stavy, které jsou významně asociovány s manifestací ischemické choroby srdeční. Rovněž hypoglykemické stavy významně zvyšují kardiovaskulární riziko. Na podkladě výsledků klinických studií existují důkazy o tom, že dobrá kompenzace diabetes mellitus 1. typu od doby její manifestace snižuje významně výskyt a progresi makrovaskulárních komplikací, včetně ischemické choroby srdeční. Korekce hypertenze a poruchy lipidového metabolizmu je rovněž spojena s příznivým účinkem., The presence of cardiovascular disease, and especially coronary artery disease, in patients with type 1 diabetes mellitus impairs life expectancy. Hyperglycaemia, which increases oxidative stress, is the key pathophysiological factor. Coronary artery calcifications, the presence of cardiac autonomic neuropathy and diabetic nephropathy are significantly associated with coronary artery disease manifestations. Likewise, the hypoglycaemic episodes significantly increase the cardiovascular risk. Based on the clinical trial outcomes, there is evidence showing that good compensation of type 1 diabetes from the time of its manifestation can significantly reduce the occurrence and progression of macrovascular complications, including coronary artery disease. Úvod: Cílem článku je ukázat experimentální a klinické výsledky odpovědi ledvin na teplou a studenou ischemii. Porovnává různé operační postupy a možnosti ovlivnění ledvinné ischemie při resekcích ledvin. Článek je souhrnem aktuálních literárních údajů. Výsledky: Existují tři hlavní mechanizmy ischemického poškození ledvin – cévní mechanizmus s přetrvávající vazokonstrikcí a abnormální reakcí endoteliálních buněk, tubulární obstrukce se zpětným tokem moči a reperfuzní poškození. Spor trvá o maximální toleranci teplé ischemie (WI), která může být ovlivněna hlavně chirurgickou technikou. Závěr: Pokud je předpokládáno při operaci využití ischemie, nádor by měl být odstraněn v co nejkratší možné době. Obecně je doporučován čas kratší než 25 minut teplé ischemie bez ohledu na typ chirurgického přístupu. Pokud je možnost provedení resekce v tomto čase ohrožena, je třeba začít včas s chlazením ledviny. Studená ischemie (cold ischemia – CI) může být dle závislosti na způsobu chlazení tolerována až do doby dvou hodin (autotransplantace). K možným druhům chlazení při resekčních výkonech patří metoda in situ studené arteriální perfuze, povrchové chlazení ledovou tříští, retrográdní perfuze dutého systému nebo ex situ studená arteriální perfuze s autotransplantací. Technika metod dosažení CI závisí na předoperačním hodnocení nálezu, chirurgické technice (otevřená, laparoskopická nebo robotická operace) a zvyklostech pracoviště., Introduction: The objective of this literature review was to summarize clinical and experimental evidence of the renal responses to warm and cold ischemia. The study compared different surgical procedures and options for dealing with renal ischemia during partial nephrectomy. The article is a summary of the current literature data. Results: There are three main mechanisms of ischemic renal injury – vascular, persistent vasoconstriction with an abnormal endothelial cell compensatory response, and tubular obstruction, with backflow of urine, and injury due to reperfusion. Controversy regarding the maximal kidney tolerability to warm ischemia continues. This communication summarizes literary data regarding available surgical techniques used to diminish the effects of warm ischemia. Conclusion: If ischemia is required, the tumour should be removed within 25 minutes of warm ischemia, regardless of the surgical approach. If this longer time of resection is expected, we have to start immediately with cold ischemia. Cold ischemia, depending on the cooling method, can be tolerated for up to 2 hours (autotransplantation). The cold ischemia technique includes in situ cold arterial perfusion, ice slush placed around the kidney, retrograde caliceal perfusion or ex situ cold arterial perfusion with autotransplantation.
