A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Activated hepatic stellate cells (HSC) are a major source offibrous proteins in cirrhotic liver. Inducing or accelerating their apoptosis is a potential way of liver fibrosis treatment. Extracellular matrix (ECM) surrounding cells in tissue affects their differentiation, migration, proliferation and function. Type I collagen is the main ECM component in fibrotic liver. We have examined how this protein modifies apoptosis of normal rat HSC induced by gliotoxin, cycloheximide and cytochalasin D in vitro and spontaneous apoptosis of HSC isolated from CCl4-damaged liver. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Cíl studie: Na základě soudobých studií podat přehled o hlavních biologických funkcích lipoproteinů o vysoké hustotě (HDL), zejména ve vztahu k reverznímu transportu cholesterolu a k jejich protizánětlivému působení. Popsat pravděpodobné mechanismy vzniku dysfunkčních prozánětlivých HDL a uvést některé parametry asociované s tímto procesem. Podat přehled o laboratorních metodách stanovení kvantitativních, strukturních a funkčních vlastností HDL, včetně moderních metod založených na hmotové spektrometrii. Typ studie: přehledový článek Závěr: V případě systémového zánětu a/nebo oxidačního stresu dochází ke vzniku dysfunkčních HDL akumulujících oxidanty. Probíhá strukturní modifikace apolipoproteinu AI, což vede mimo jiné k inhibici reverzního transportu cholesterolu a tvorbě prozánětlivých HDL. Na ztrátě příznivých vlastností HDL se podílí modifikace proteinové i lipidové složky HDL. Izolované měření HDL cholesterolu nemusí u některých syndromů bezvýhradně korelovat s mírou kardiovaskulárního rizika a v některých situacích je vhodné stanovit markery mapující strukturu a funkci HDL., Objective: On the basis of recent studies to make an overview concerning the crucial biological functions of high density lipoproteins (HDL), with emphasis on the role in reverse cholesterol transport and their antiinflammatory traits. The aim was to describe probable mechanisms of dysfunctional proinflammatory HDL formation with introduction some of associated parameters. To present laboratory methods for determination of quantitative, structural and functional qualities of HDL, including advanced mass spectrometry techniques. Study design: review Conclusion: In the case of systemic inflammation and/or oxidative stress, the formation of dysfunctional HDL accumulating oxidants takes place, including apo AI structural modifications. This process can lead to the inhibition of reverse cholesterol transport and proinflammatory HDL generation, among others. Loss of beneficial qualities results from modification of both lipid and protein components of HDL. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Cíl. Ischemické cévní mozkové příhody (iCMP) jsou stále jednou z nejčastějších příčin mortality a morbidity postihující i mladší populaci v produktivním věku. Klíčovým faktorem ovlivňujícím prognózu po iCMP je včasná rekanalizace uzavřené mozkové tepny s obnovením perfuze v postižené mozkové tkáni. Cílem práce bylo zhodnocení bezpečnosti a efektivity kombinované revaskularizační terapie sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie u pacientů s akutní iCMP a uzávěrem mozkové tepny. Metoda. Do souboru pacientů byli zařazeni všichni po sobě jdoucí pacienti s akutní iCMP a prokázaným uzávěrem střední mozkové tepny v úseku M1-2 nebo distální části arteria carotis interna nebo uzávěrem arteria basilaris, a to v období od června 2010 do dubna 2012. Všichni nemocní byli léčeni kombinovanou revaskularizační terapií, tzn. iniciální intravenózní trombolýzou a přímo navazující mechanickou trombektomií systémem Solitaire. Iniciální stupeň neurologického deficitu byl stanoven pomocí NIHSS skóre. Výsledný klinický stav byl zhodnocen po 90 dnech pomocí modifikované Rankinovy škály (mRS). Výsledky. Do souboru bylo podle kritérií zařazeno 50 pacientů (průměrný věk 66, 8 ? 14,6 roku) se vstupním mediánem NIHSS skóre 18,0 (8-32). Rekanalizace (TICI 2A, B a 3) bylo dosaženo u 94 % pacientů, kompletní rekanalizace (TICI 3) u 72% pacientů. Průměrný rekanalizační čas (čas od začátku symptomů do dosažení maximální rekanalizace) byl v našem souboru 244,2 ? 87,9 minut. Symptomatické intracerebrální krvácení se vyskytlo u 6 % pacientů. Medián mRS po 90 dnech byl v našem souboru 1, 60 % pacientů mělo devadesátidenní klinický výsledek dle mRS hodnocen jako dobrý (mRS 0-2). Tříměsíční mortalita byla ve sledovaném souboru 14 %. Závěr. Kombinovaná revaskularizační terapie akutní iCMP sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie je bezpečná a efektivní. Okamžité provedení mechanické tromektomie po iniciální intravenózní trombolýze vede k výraznému zkrácení rekanalizačního času, který je rozhodující pro dobrý klinický výsledek u pacientů intervenovaných pro akutní ischemickou cévní mozkovou příhodu., Aim. Ischemic stroke is today one of the leading causes of mortality and morbidity and affects even a younger population of working age. A key factor influencing prognosis after ischemic stroke is early recanalization of close cerebral artery and reperfusion in the affected brain tissue. The aim of our study is to evaluate the safety and effectiveness of re-vascularization combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy in the treatment of patients with acute ischemic stroke. Methods. Consecutive non-selected acute ischemic stroke patients with baseline CTA or MRA documented occlusion of middle cerebral artery (MCA) (segment M1-2) or distal part of internal carotid artery (ICA) or basilar artery (BA) and treated between June 2010 and April 2012 at our stroke center were included in the study. All patiens were treated by combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy by Solitaire stent. Stroke severity was assessed using National Institutes of Health Stroke Scale (NIHSS), 90-day clinical outcome using modified Rankin Scale (mRS). Results. Fifty enrolled patients (mean age 66.8 ? 14.6 years) had baseline median NIHSS 18.0 (8-32). Overall recanalization (TICI 2A, B and 3) was achieved in 94% of patients, complete (TICI 3) in 72% of patients. The mean time from stroke onset-to-maximal recanalization was 244.2 ? 87.9 min. Symptomatic intracerebral hemorrhage occurred in 6% of patients. The median mRS score at 90 days was 1 and 60% patients had good outcome (mRS 0-2). Overall 3-month mortality was 14%. Conclusion. Combined revascularization therapy of acute ischemic stroke consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy is safe and effective. Cíl studie: Klinicky afunkční adenomy (KAA) představují zhruba 25–30 % všech tumorů hypofýzy. Neurochirurgická operace nádoru bývá často neúspěšná a zůstává po ní nádorové reziduum. Účinek následné biologické léčby je závislý na přítomnosti transmembránových receptorů v nádorových buňkách. Naším cílem bylo určit expresní profil somatostatinových receptorů (SSTR1–5) a estrogenového receptoru 1 (ER1) u 69 vzorků KAA. Materiál a metody: Vzorky adenomů se uchovávaly v RNAlater Tissue Protect až do doby zpracování v laboratoři. RNA jsme izolovali Trizolem a přepsali do cDNA pomocí SuperScriptu III. Exprese jednotlivých receptorů byla stanovena metodou kvantitativní real time PCR. Získané výsledky jsme přepočítali na housekeepingový gen pro beta-glukuronidázu. Výsledky: U vyšetřovaného souboru 69 afunkčních adenomů byly kromě SSTR4 a SSTR5 exprimovány všechny zbývající vyšetřované receptory. Mediány relativních hodnot byly: 63,8 % pro SSTR1, 55,4 % pro SSTR2, 19,6 % pro SSTR3, 2,6 % pro SSTR4, 9,2 % pro SSTR5 a 75,4 % pro ER1. Závěr: Variabilní exprese jednotlivých podtypů SSTR a ER1 může ovlivnit účinnost biologické léčby KAA. Proto by určení expresního profilu sledovaných receptorů mělo předcházet zahájení biologické léčby KAA somatostatinovými analogy či modulátory estrogenových receptorů., Objective: Non-functioning pituitary adenomas (NFA) comprise about 25–30% of all pituitary tumors. Transsphenoidal neurosurgery, the treatment of choice, is often unsuccessful and frequently leaves tumor remnants. The effect of subsequent biological treatment is mediated by transmembrane receptors in the adenoma cells. Our aim was to determine the somatostatin (SSTR1–5) and estrogen receptor 1 (ER1) expression profile in 69 specimens of NFA. Material and Methods: Adenoma samples were submerged in RNAlater Tissue Protect and transported frozen to the laboratory. The RNA was isolated by Trizol Reagent and transcribed to cDNA by SuperScript III. The expression of the receptors was determined by quantitative real time PCR. Results were normalized to the beta-glucuronidase housekeeping gene. Results: With exceptions of SSTR4 and SSTR5, all other subtypes of receptors were expressed in the examined tumor specimens. The median relative quantification values were: 63.8% for SSTR1, 55.4% for SSTR2, 19.6% for SSTR3, 2.6% for SSTR4, 9.2% for SSTR5, and 75.4% for ER1. Conclusion: Variable expression of somatostatin and estrogen receptors could influence the effectiveness of the biological treatment. V databázi Globocan 2008 (IARC) bylo za rok 2008 z celosvětového počtu 12,663 milionů nových nádorů odhadováno 2,445 milionů v EU, ze 7,565 milionů zemřelých téměř 1,235 milionů v EU a z pětileté prevalence 28,8 milionů asi 6,617 milionů v EU. Početnější prevalence se týkala ve světě 18 % nádorů prsů, 11 % kolorekta, 11 % prostaty, 6 % plic, 5,5 % žaludku, 5,5 % cervixu, 4 % měchýře a 4 % dělohy, v EU 20 % ZN prsů, 14 % kolorekta, 18 % prostaty, 5 % plic, 4 % melanomu, 3 % ledvin, 3 % dělohy a 3 % NH lymfomu. V roce 2030 mohou počty nových nádorů dosáhnout ve světě 21 milionů případů, v EU 3 miliony, v ČR 93 tisíc (dg. C00-D09). Z předběžných celkových výdajů 288 572 mil. Kč na zdravotnictví za rok 2011 uhradily zdravotní pojišťovny 217 653 mil., z toho 19 217 mil. na onkologii. V letech 1990–2011 vzrostly celkové náklady na zdravotnictví o 257 534 mil. Kč s podílem 7,58 % na hrubém domácím produktu. Průměrné výdaje VZP na léčbu jednoho nádoru v roce 2010 dosáhly 19 030 Kč s nejvyššími náklady u novotvarů mízní a krvetvorné tkáně, mozku a CNS, trávicího ústrojí, úst a hltanu, dýchacích cest. Výkonnost globální ekonomiky nedosahuje potřebných prostředků na zdravotnictví. Český veřejný dluh překračuje 1 680 miliard Kč v době dokončení tohoto článku. I když celkové výdaje obyvatel na zdravotní péči rostou, zdá se, že jeho pokračující zadlužování zvýší také dluhy nemocnic a zdravotních pojišťoven. Dostupné počty nádorů a výdajů na zdravotnictví a jejich zvyšování potvrzují nutnost důsledné prevence rakoviny, efektivní přidělování a využívání prostředků na zdravotní péči., In 2008 (Globocan 2008; IARC), 12.663 million new cancer cases occurred worldwide and, of them, 2.445 million in the EU. Here preliminary total health care expenditures in 2011 were 288,572 million CZK; of these, health insurance companies covered 217,653 million, with 19,217 million per one cancer treatment. Between 1990 and 2011, the total health expenditures increased by 257,534 million CZK, accounting for 7.58 % of the gross domestic product. In 2010 the General Health Insurance Company paid on average 19,030 CZK for the treatment of one cancer case, with the highest expenditures for neoplasms of the lymphoid and haemotopoietic tissues, brain and central nervous system, digestive organs, oral cavity and pharynx, and respiratory organs. The global economic performance cannot provide enough resources for public health care. The Czech public debt exceeded 1,680 billion CZK at the time this article was written. Although out-of-pocket money paid by cancer patients for treatment is growing, the continuing public indebtedness is likely to increase debts of hospitals and health insurance companies. Z 99 378 nových karcinomů kolorekta (KRK) u mužů a 78 977 u žen, evidovaných v registru nádorů ČR v letech 1976–2005, bylo 14 860 (15 %) KRK u mužů a 11 142 (14,1%) u žen spojeno s výskytem jiných zhoubných novotvarů (ZN). Po 6 504 primárních KRK u mužů následovalo 7 406 ZN, po 4 342 KRK u žen 4 798 následných ZN. Naopak 8 356 následným KRK u mužů předcházelo 10 152 primárních ZN, 6 800 KRK u žen předcházelo 7 829 ZN. Počet vícečetných KRK se zvýšil ze 141 v roce 1976 na 1 534 v roce 2005. V letech 1996–2005 dosáhly primární KRK 57 % časných, 30 % pokročilých a 13 % neznámých stadií, následné KRK 43,5 %, 34 % a 22,5 %. Vícečetné KRK byly u mužů nejpočetnější ve věku 50–69 let, u žen 70–79 let. Průměrná doba vzniku následného novotvaru po primárním KRK u mužů byla 5,1 roku, u žen 5,4 roku. Po primárních KRK byly nejpočetnější ZN GIT, kůže, pohlavních orgánů a močového systému. Ze synchronních ZN byla téměř polovina nádorů trávicích cest. Z 12 204 následných ZN bylo 19 % zjištěno v pokročilých stadiích z toho asi polovina ZN trávicích cest. Víc než třetinu primárních nádorů před 17 981 následnými KRK tvořily karcinomy kůže. U 1 715 (26,4 %) mužů a 1 206 (27,8 %) žen, přežívajících v roce 2007 s primárním KRK, byl vyšší podíl časných stadií než s následným KRK; naopak u 6 646 (79,5 %) mužů a 5 291 (77,8 %) žen, zemřelých v roce 2007 s následným KRK, byl vyšší podíl pokročilých stadií než s primárním KRK. Z uvedených dat se odhaduje, že z 8 265 KRK hlášených v roce 2010 bylo téměř 1 700 (21 %) vícečetných KRK, z toho 260 primárních a 1440 následných případů. Každé páté onemocnění KRK bylo spojeno s výskytem dalšího ZN. I když je obtížné odhadnout počet vícečetných případů mezi 53 340 přežívajícími s KRK v roce 2010, jejich pokračující trend zvyšuje zátěž diagnostických a léčebných kapacit., Among the total of 99,378 new colorectal cancers (CRCs) in males and 78,977 in females, based on the Czech Cancer Registry in 1976–2005, there were 14,860 (15 %) CRCs in males and 11,142 (14.1 %) in females associated with the other malignant neoplasms (MNs). The primary 6,504 CRCs in males were followed by 7,406 subsequent MNs, the primary 4,342 CRCs in females by 4,798 subsequent MNs. On the contrary before the subsequent 8,356 CRCs in males were registered 10,152 primary MNs, 6,800 CRCs in females 7,829 MNs. The number of multiple CRCs increased from 141 in 1976 to 1,534 in 2005. In 1996–2005 the primary CRCs reached 57 % of early, 30 % advanced and 13 % unknown clinical stages, respectively the subsequent CRCs of 43.5 %, 34 % and 22.5 %. The occurrence of all multiple CRCs were highest in the age group of 50–69 years in males, and of 70–79 years in females. The average interval between primary CRCs and subsequent MNs was 5.1 years in males and 5.4 years in females. The most frequent MNs following primary CRCs were cancers of the gastrointestinal tract, skin, genital organs and urinary tract. Nearly half of synchronous MNs after primary CRCs were cancers of the gastrointestinal tract. There were 19 % cases diagnosed at advanced stages from the total 12,204 subsequent MNs, one half of those in patients with gastrointestinal cancers. More than one third of primary MNs preceded by 17,981 subsequent CRCs were cancers of the skin. Among 1,715 (26.4 %) males and 1,206 (27.8 %) females survived up to 2007 with primary CRCs, there were higher early stages than with subsequent CRSc; on the contrary among 6,646 (79.5 %) males and 5,291 (77.8 %) females, died up to 2007 with subsequent CRCs, were higher advanced stages than with primary CRCs. By the above mentioned data is estimated that among a total of 8,265 CRCs registered in 2010 were nearly 1,700 (21 %) multiple CRCs, i.e. 260 primary and 1,440 subsequent cases. Every fifth CRC was associated with the occurrence of the other MNs. Úvod: Nedostatečný zbytkový objem jater po resekci (FLRV) je hlavní příčinou primární neresekability jaterních nádorů. Embolizace větve portální žíly (PVE) s aplikací hematopoietických kmenových buněk (HSC) může být způsob, jak významnou měrou zvýšit počet nemocných s dostatečným FLRV, a tím rozšířit resekabilitu primárně chirurgicky neřešitelných nádorů jater. Metodika: U dvanácti nemocných s FLRV < 30 % byla provedena PVE na straně nádoru s následnou aplikací HSC do druhostranné neembolizované větve vena portae. Výsledky: PVE s aplikací HSC proběhly bez komplikací. U všech nemocných došlo do 4 týdnů k dostatečnému nárůstu FLRV. Radikální resekce jater byla provedena u osmi nemocných s metastázami kolorektálního karcinomu. U dvou nemocných současně se zvýšením FLRV došlo i k významnému nárůstu objemu nádoru, resekce jater nebyla možná. U jednoho nemocného těžké nitrobřišní srůsty zabránily jaterní resekci. Jeden nemocný je čerstvě po aplikaci HSC. Celkové přežívání nemocných po dobu jednoho roku a tří let je 66,7 %, resp. 33,3 %. Medián přežití bez recidivy je 6 měsíců. Závěr: Kombinace PVE s aplikací HSC se jeví jako nadějná metoda pro zvýšení operability jaterních nádorů., Introduction: Insufficient future liver remnant volume (FLRV) is the main cause of liver tumors unresectability. Portal vein embolization (PVE) with application of hematopoietic stem cells (HSC) can be a matter how to increase resectability of liver tumors. Method: PVE with application of HSC were performed in 12 patients with primary unresectable liver tumors. Results: The procedures were without complications. After 4 weeks the sufficient FRLV grew. Radical liver resection was performed in 8 patients. Tumor progression was pattern in 2 patients and in one patients the sever intraabdominal adhesions were cause of inoperability. One patient is just after PVE and HSC application. Overall survival in one and three years is 66.7 %, resp. 33.3 %. Median of progression free survival is 6 months.
