Úvod: Primární omentální torze je raritní nález při operaci pro podezření na náhlou příhodu břišní. Přichází hlavně u dětí. U dospělých je častější sekundární příčina. Kazuistika: Uvádíme dva klinické případy, kdy příčinou bolestí břicha v pravém horním kvadrantu byla primární torze cípu omenta. Potíže u obou dívek vznikly po zvýšeném nitrobřišním tlaku, po silném kašli a po gymnastickém cviku. Primární torze omenta obvykle napodobuje apendicitidu s klinickým nálezem v pravém dolním kvadrantu břicha, a to zejména u obézních dětí. V našich dvou případech se ale jednalo o normostenické dívky, které měly bolesti v pravém horním kvadrantu až mezogastriu. Závěr: Primární torze omenta je velmi vzácná příčina náhlé příhody břišní, což potvrzuje i výskyt tohoto onemocnění v našem souboru, kdy jsme za 15leté období zaznamenali pouze dva případy, což odpovídá 0,17 % provedených apendektomií. Tato hodnota je srovnatelná s literárně uváděnými údaji., Introduction: Primary omental torsion is a rare finding in cases of suspected acute abdomen. It is more common in children. Secondary omental torsion is typical for adults. Case report: The authors describe two cases where pain in the right upper abdominal quadrant was caused by primary torsion of the omental corner due to increased intraabdominal pressure: after a strong cough in one girl and after a workout in the other. Primary omental torsion usually mimics acute appendicitis with clinical findings in the right lower abdominal quadrant, especially in obese children. However, our two cases describe normosthenic girls with pain in the right upper abdominal quadrant up to the mesogastrium. Conclusion: Primary torsion of the omentum is a very rare cause of acute abdomen, which is also confirmed by its incidence in our group of patients where only two cases were seen during a 15-year period, which corresponds to 0.17% of all appendectomies performed in our department. Perkutánní nefrolitotomie (PCNL) u pacientů s angiomyolipomem představuje skutečný chirurgický oříšek. V tomto klinickém scénáři hrozí vyšší riziko krvácení, zejména v případě, že je přístup veden skrze samotný tumor. Ve třech prezentovaných případech, kdy byli pacienti s angiomyolipomem léčeni pomocí PCNL, nedošlo k výskytu pooperačních ani peroperačních komplikací (včetně krvácení) a výkony nevyžadovaly krevní transfuzi., Percutaneous nephrolithotomy (PCNL) in the setting of coexisting AML presents a rare surgical challenge. Akutní subdurální hematom (ASDH) je nejčastější typ traumatického nitrolebního krvácení s vysokou morbiditou a mortalitou. Ve většině případů je nezbytné akutní operační řešení, vzácně je možné menší asymptomatické ASDH sledovat, jejich resorpce je většinou dlouhodobá, často s vytvořením chronického subdurálního hematomu (CSDH). Bylo popsáno pouze několik případů promptní resorpce ASDH, ale někteří autoři tvrdí, že je jejich incidence podhodnocena. Prezentujeme případ traumaticky vzniklého ASDH u mladé alkoholičky, u níž došlo k téměř kompletní resorpci hematomu za cca 20 hod., Acute subdural hematoma (ASDH) is the most common type of traumatic intracranial hemorrhage associated with high morbidity and mortality. The majority of cases require acute surgery. Monitoring may rarely be sufficient in smaller asymptomatic ASDH. Their resorption is usually prolonged and often associated with formation of chronic subdural hematoma (CSDH). Prompt resorption of hematomas have been described in a few cases only but some authors argue that their incidence is underestimated. We present a case of traumatic ASDH in a young alcoholic patient, in whom a follow-up CT of the brain performed about 20 hours later revealed almost complete resorption of the hematoma. V našem článku prezentujeme případ podkovovité ledviny s jedním sběrným systémem a jediným zkříženým močovodem a pyelolitiázou. V anglickém jazyce byly dosud celosvětově uveřejněny pouze čtyři publikace popisující případ podkovovité ledviny s jediným zkříženým močovodem. Prezentujeme náš postup při léčbě třicetiletého pacienta s touto vzácně se vyskytující vrozenou malformací a objemnou nefrolitiázou, uvádíme přehled dostupné literatury a nabízíme možná vysvětlení., The present study reports a horseshoe kidney with a single collecting system and a crossed single ureter associated with pyelolythiasis. An association of a crossed single ureter and a horseshoe kidney has only been mentioned in four previous reports in the available English literature worldwide. Mozkový absces vzniká přímým šířením hnisavého procesu z okolí, metastaticky nebo v souvislosti s kraniocerebrálním poraněním anebo operačním zákrokem. Léčba spočívá v kombinaci navigované punkce a evakuace abscesu s cílenou antibiotickou terapií. Méně častá je radikální exstirpace abscesu. Absces bývá doprovázen kolaterálním edémem vyžadujícím v některých případech antiedematózní terapii. Uvedená léčba by měla v současné době vést k úplnému vyléčení. Ve svém sdělení popisujeme ojedinělý případ nemocného se solitárním abscesem mozku, u kterého došlo po punkci k progresi kolaterálního edému s následnou klinickou deteriorací. Akutně provedená dekompresivní kraniektomie vedla k úplné úpravě klinického stavu. Po ústupu edému byla provedena kranioplastika. Abscesové ložisko po punkci a antibiotické terapii zcela vymizelo. Původcem abscesu byli peptostreptokokus a fusobakterium. Použití dekompresivní kraniektomie v terapii mozkového abscesu je výjimečné a v písemnictví je popisováno sporadicky., Brain abscess is caused by direct dissemination of inflammation from surrounding structures, remote infectious sources or is associated with brain trauma or neurosurgical procedures. Treatment includes surgical aspiration and intravenous antibiotics. Excision of the lesion is sometimes indicated. Antiedematous therapy is necessary in cases of brain oedema. At the present time, the majority of patients with brain abscesses survive and recover completely. Here we describe the case of a patient with brain abscess, whose condition deteriorated after aspiration of the lesion. The deterioration was due to progression of collateral brain oedema. After urgent decompressive craniectomy, the patient recovered completely. A cranioplasty was performed when the oedema disappeared. The abscess resolved completely following aspiration and antibiotic therapy. Fusobacterium and peptostreptococus were identified as the causative pathogens. Decompressive craniectomy has very rarely been applied in the treatment of brain abscesses and is reported sporadically. Polytrauma je jednou z hlavních příčin mortality lidí v produktivním věku. Nejdůležitější při léčbě polytraumatu je rychlá aktivace záchranného systému. Při závažných poraněních je klíčový primární transport do specializovaného pracoviště – traumacentra. Naše kazuistické sdělení se zaměřuje na dva případy polytraumatizovaných pacientů, u kterých došlo v průběhu léčení k relativně vzácným situacím. V prvním případě se jedná o vzácné poranění slinivky břišní, které si vyžádalo pankreatoduodenektomii. V druhém případě o poranění jater ošetřené pravostrannou lobektomií se zřídkavou komplikací. Rovněž poukazuje na nezbytnost multioborové spolupráce v léčbě polytraumatizovaného pacienta., Polytrauma is one of the leading causes of mortality in people at productive age. Prompt activation of the rescue system is most important in the treatment. In cases of severe injuries, primary transport to a specialized hospital – trauma center is crucial. Our report is focused on two cases of polytraumatized patients whose treatment was associated with relatively rare situations. One of the patients suffered a pancreatic injury that required pancreaticoduodenectomy. The other patient had a liver injury, which was treated with right lobectomy with a rare complication. Ruptura aneuryzmatu Valsalvova sinu je vzácné, ale závažné srdeční onemocnění, které může ohrozit pacienta na životě. Včasná diagnostika a správná léčba může výrazně ovlivnit prognózu pacienta. V naší kazuistice popisujeme případ 44letého muže přijatého na Interní kardiologickou kliniku FN Brno pro typické bolesti na hrudi s nespecifickými změnami při elektrokardiografickém vyšetření a s pozitivním troponinem T. Vstupní transthorakální echokardiografické vyšetření odhalilo aneuryzma nekoronárního sinu a komunikaci mezi aortou a pravou síní, která byla potvrzena i CT angiograficky. Pacient byl poté odeslán do kardiochirurgického centra, kde byla následně provedena resekce aneuryzmatu a uzavřena komunikace mezi aortou a pravou síní záplatou z perikardu. Při vzniku aneuryzmatu nelze vyloučit podíl metabolického účinku kortikoidů, které pacient dlouhodobě užíval. Z uvedené kazuistiky plyne, že pečlivé klinické vyšetření a neinvazivní vyšetřovací metody zůstávají stále základním diagnostickým postupem. Early diagnosis and proper therapeutic treatment can significantly affect the patient’s prognosis. We describe the case of a 44‑year–old man admitted to the Cardiology Department of the University Hospital Brno with typical chest pain and non‑specific changes on electrocardiogram and positive troponin T. Transthoracic echocardiography revealed a non‑coronary sinus aneurysm and communication between the aorta and the right atrium, which was confirmed by computed tomography. The patient was referred to the cardiothoracic surgery centre and the aneurysm was resected. The communication between the aorta and the right atrium was closed via pericardial patch. The aneurysm formation can be caused by the metabolic effects of corticosteroids which the patient had been using for a long time. This case report suggests that careful clinical examination and noninvasive methods remain an essential diagnostic procedure. Ortotopická transplantace srdce je zavedenou metodou léčby pacientů v terminálním stadiu srdečního selhání, která zlepšuje přežívání těchto pacientů. Cenou za podávání imunosupresivní léčby je řada potenciálních rizik, jedněmi z nejzávažnějších jsou infekční komplikace. Kazuistika popisuje případ pacientky se srdečním selháním na podkladě primární amyloidózy, která podstoupila srdeční transplantaci. Po transplantaci byla v plánu kurativní hematoonkologická léčba, která však vzhledem ke kompletní remisi základního onemocnění nebyla uskutečněna. V časném potransplantačním období pacientka prodělala řadu komplikací, z nichž nejzávažnější byla invazivní plicní aspergilóza. Tato infekce byla úspěšně zaléčena vorikonazolem a pacientka je nyní 28 měsíců po srdeční transplantaci ve velmi dobrém klinickém stavu bez známek aktivní infekce., Orthotopic heart transplantation is an established method of treatment for patients with end‑stage heart failure, which improves the survival of these patients. The use of immunosuppressive therapy comes at the price of a number of potential risks, the most serious being infectious complications. Our case report describes a patient with heart failure on the basis of primary amyloidosis who underwent cardiac transplantation. Hemato‑oncological curative treatment was planned after transplantation, but has not been applied due to the complete remission of the primary disease. In the early post‑transplant period, the patient experienced a number of complications, the most serious being invasive pulmonary aspergillosis. Spinální arteriovenózní malformace jsou skupina vzácných onemocnění, jejichž klinické projevy mohou být velmi rozmanité. Optimální léčebný postup je nutno vždy volit individuálně a pacienty směřovat do specializovaných center. Cílem sdělení je uvést přehlednou klasifikaci SAVM, jejich diferenciální diagnostiku a terapii a dále prezentovat dvě kazuistiky. První kazuistika se zmiňuje o případu 27letého muže, u kterého se rozvinuly silné bolesti bederní páteře s dominující meningeální symptomatikou v klinickém obraze. Na MR hrudního úseku páteře (Th11/12) byla nalezena AVM zásobovaná hypertrofickou radikulopiální tepnou. Na specializovaném pracovišti byla provedena úspěšná embolizace AV zkratu. Druhá kazuistika uvádí případ 41leté ženy, které vznikla při lyžování náhlá prudká bolest za krkem. V neurologickém obraze došlo k rozvoji inkompletní léze míšní s kvadruparézou s levostrannou akcentací do těžkého stupně s hranicí čití ve výši C1. Na provedeném MR a MRAG krční míchy a následně na DSA vertebrálních tepen byla detekována AVM zásobovaná levou arteria spinalis posterior. Terapeuticky byla indikována elektivní endovaskulární embolizace a pacientka byla ošetřena na specializovaném pracovišti ve Francii., Spinal arteriovenous malformations are a group of rare diseases with highly diverse clinical manifestations. The optimal therapeutic practice should always be selected individually and patients should be sent to specialized centres. The objective of this paper is to provide a structured classification of spinal AVMs, their differential diagnosis and therapy, and to present two case reports. The first report is concerned with a case of a 27-year-old man who developed severe pain in the lumbar spine with predominantly meningeal symptomatology. An AVM fed by a hypertrophic radiculopial artery was found on the thoracic spine (Th11/12) MR. A specialized centre performed successful embolization of an arteriovenous shunt. The second report presents a case of a 41-year-old woman, who felt sudden sharp pain in the neck while skiing. Neurologically, she developed an incomplete spinal cord lesion with quadriparesis with left-sided accentuation of severe intensity and sensation threshold at C1. The MR and MRAG of the cervical spinal cord and the following DSA of vertebral artery detected AVM fed by the left posterior spinal artery.
