After a cardiovascular event, patients and their families often have to cope with numerous changes in their lives, including the consequences of the disease or its treatment on their daily lives and functioning. Coping poorly with both physical and psychological challenges may lead to an impaired quality of life. Sexuality is one aspect of the quality of life that is important for many patients and partners that may be adversely affected by a cardiac event. The World Health Organization defines sexual health as “…a state of physical, emotional, mental and social well-being in relation to sexuality; it is not merely the absence of disease, dysfunction or infirmity. Sexual health requires a positive and respectful approach to sexuality and sexual relationships, as well as the possibility of having pleasurable and safe sexual experiences …” The key points can be summarized as follows:Safe timing of a return to sexual activity after a cardiac event, as it has been addressed by the American Heart Association, and decreased sexual activity among cardiac patients is frequently reported.Rates of erectile dysfunction (ED) among men with cardiovascular disease (CVD) are twice as high as in the general population.ED and vaginal dryness may appear 1–3 years after the onset of angina pectoris, however only a small percentage of those with sexual dysfunction seek medical care.Routine assessment of sexual problems and sexual counseling should, therefore, be a part of effective management by physicians, nurses, and other healthcare providers., and Špinar J., Vítovec J.
Srdeční selhání patří mezi závažné zdravotní problémy tohoto století vedoucí zejmena k častým rehospitalizacím, snižuje též kvalitu života a zhoršuje prognózu nemocných. Při progresi chronického srdečního selhání dochází často k narůstající neschopnosti srdce splňovat metabolické nároky většiny orgánů. Zatímco důsledky srdečního selhání na renální funkce (kardiorenální syndrom) byly v literatuře mnohokrát popsány, komplexní popis dopadu srdečního selhání na jaterní funkce (kardiohepatální syndrom) včetně klinického průběhu, biochemických profilů a histologických nálezů v naší literatuře stále chybí, podobně jako popis významu užití laboratorní kontroly jaterních funkcí jakožto prognostického ukazatele u pacientů se srdečním selháním., Heart failure is a major health problem of this century, with frequent repeated hospitalizations, deteriorated quality of life and worsened prognosis. Progression of heart failure is often characterized by an increasing inability of the heart to meet the metabolic demands of most organs. While the consequences of heart failure on renal function (cardiorenal syndrome) have been described many times, a comprehensive description of the impact of heart failure on liver function (cardiohepatic syndrome), including clinical presentation, biochemical profiles, and histological findings is still lacking in our literature, as is a description of the use of liver function tests as a prognostic marker in patients with heart failure., and Ludka O., Hlásenský J., Špinar J.
Kombinační léčbu více řídíme podle komorbidit pacienta a můžeme ji lépe pacientům šít na míru., In recent years heart transplantation has become an established treatment method for patients with severe heart failure. This treatment involves life-long immunosuppressant therapy with a combination of drugs. Classical immunosuppression is achieved with cyclosporine or tacrolimus, mycophenolate mofetil and corticosteroids. In the last few years, new regimes of immunosuppressive drugs have been used in post-heart transplant patients suffering from renal dysfunction or allograft vasculopathy: a combination of low doses of calcineurin inhibitors with low doses of mTOR inhibitors or calcineurin-free regimes, which are not so nephrotoxic. There has been an effort to withdraw corticosteroids from all regimes after the first year and to have life-long therapy based on a combination of two drugs. Combination therapy is tailored according to the patient’s comorbidities., and Špinarová L., Vítovec J.
Tato infekce byla úspěšně zaléčena vorikonazolem a pacientka je nyní 28 měsíců po srdeční transplantaci ve velmi dobrém klinickém stavu bez známek aktivní infekce., Orthotopic heart transplantation is an established method of treatment for patients with end‑stage heart failure, which improves the survival of these patients. The use of immunosuppressive therapy comes at the price of a number of potential risks, the most serious being infectious complications. Our case report describes a patient with heart failure on the basis of primary amyloidosis who underwent cardiac transplantation. Hemato‑oncological curative treatment was planned after transplantation, but has not been applied due to the complete remission of the primary disease. In the early post‑transplant period, the patient experienced a number of complications, the most serious being invasive pulmonary aspergillosis. This infection was successfully controlled using voriconazole, and 28 months after heart transplantation the patient is now in good clinical condition with no signs of active infection., and Honěk T., Krejčí J., Němec P., Adam Z., Žampachová V., Feitová V., Špinarová L.
