Diabetes mellitus 2. typu je heterogenní onemocnění, které vyžaduje personalizovaný přístup k léčbě s cíli přizpůsobenými možnostem a schopnostem pacienta a jeho dalším onemocněním tak, aby se zabezpečila dobrá kompenzace diabetu bez rizika hypoglykemických příhod a vývoje nebo progrese pozdních diabetických komplikací. Doporučení pro léčbu diabetu zařazují do 2. linie terapii bazálními inzuliny jako jednu z možností, a to ihned při selhání terapie metforminem a dietou. Nová generace bazálních inzulinových analog přináší svým profilem účinku a vlastnostmi zcela novou kvalitu do terapie diabetu. Originální Kocherův přístup byl publikován opakovaně v letech 1892−1907. Jednalo se o přístup vedený v intervalu mezi m. extensor carpi ulnaris a m. anconeus. Po subperiostálním uvolnění zevního postranního vazu, kloubního pouzdra a začátku extenzorů od laterálního epikondylu, jejich odtažení ventrálně a obdobném uvolnění m. anconeus od distálního humeru a jeho odtažení dorzálně byl široce zpřístupněn loketní kloub. Dnes je popisován v učebnicích v různých modifikacích, které s originálním popisem spojuje pouze fakt, že preparace probíhá v tzv. Kocherově intervalu mezi m. extensor carpi ulnaris a m. anconeus. Často se proto hovoří o tzv. limitovaném Kocherově přístupu. V článku je popsána naše modifikace Kocherova přístupu, kterou používáme především u zlomenin hlavice a krčku radia, některých zlomenin distálního humeru, dále u ireponibilních luxací loketního kloubu a některých luxačních zlomenin loketního kloubu. Kožní řez probíhá v linii spojující laterální epikondyl humeru a hranici mezi proximální a střední třetinou ulny. Po rozevření okrajů rány identifikujeme silnou, bíle svítící společnou fascii extenzorů. Po jejím protětí a roztažení identifikujeme interval mezi m. extensor carpi ulnaris a m. anconeus. Oba svaly jsou odděleny řídkým vaskularizovaným tukovým vazivem, po jehož rozhrnutí je patrná typická šlacha zesilující horní polovinu předního okraje m. anconeus. V této fázi je výhodné uvolnění začátku m. extensor carpi ulnaris od laterálního epikondylu. To usnadní odtažení svalu ventrálně, m. anconeus odtáhneme lehce dorzálně. Na rozdíl od originálního Kocherova přístupu neuvolňujeme m. anconeus od laterálního epikondylu humeru. Odtažením svalů obnažíme anterolaterální plochu kloubního pouzdra a identifikujeme průběh komplexu ligamentum collaterale laterale (LCL). Protnutí pouzdra provádíme řezem podél předního okraje LCL, a to od laterálního epikondylu až po lig. anulare radii včetně. Artrotomie provedená před LCL šetří úpon tzv. lig. collaterale laterale ulnare na ulnu a zachovává tak stabilitu lokte. Pokud potřebujeme preparovat více distálně a obnažit krček radia, musíme protnout i část m. supinator. V tom případě převedeme nejdříve předloktí opatrně do maximální možné pronace, čímž se přesune canalis supinatorius a v něm probíhající r. profundus n. radialis více vpřed a sníží se tak riziko poranění nervu. Po protětí a roztažení pouzdra se objeví anterolaterální plocha capitulum humeri a capitis radii. V této fázi je výhodná flexe v loketním kloubu 90–100°, kdy ochabne přední část pouzdra a získáme lepší přehled v kloubu. Kloubní pouzdro je nutné uvolnit od distálního humeru společně s extenzory začínajícími od laterálního epikondylu humeru. Tím výrazně zlepšíme přehlednost v přední části kloubní dutiny. Při uzávěru rány musí být provedena velmi pevná sutura společné fascie extenzorů. Ta je významným a ne vždy doceněným stabilizátorem laterální části loketního kloubu. Rozšířená varianta Kocherova přístupu spočívá v odklopení m. anconeus proximálně. Je indikována u některých luxačních zlomenin proximálního předloktí, tj. zlomenin hlavice radia a celé proximální ulny. Po ozřejmění m. anconeus v celém rozsahu je tento sval uvolněn od diafýzy ulny a celý překlopen proximálně. Sval zůstává připevněn svým krátkým proximálním okrajem k laterálnímu epikondylu humeru a k olekranu. Nehrozí tak poranění nervově cévního hilu svalu, neboť motorický nerv vstupuje do svalu středem jeho horního okraje. Po odklopení svalu je zpřístupněna kromě zevní plochy kloubního pouzdra i zevní plocha proximální ulny. Další postup, tj. incize pouzdra a revize kloubu, probíhá stejně jako u limitovaného Kocherova přístupu., The original Kocher approach was published several times in the 1892−1907 period. It extends in the interval between the extensor carpi ulnaris and the anconeus and consists in subperiostal release of the lateral collateral ligament (LCL), joint capsule and origin of extensors at the lateral epicondyle and their retraction anteriorly, and a similar release of the anconeus from the distal humerus and its reflection posteriorly. This provides an extensive approach to the elbow. Today this approach is described in the textbooks in various modifications that have little in common with the original description except for the fact that dissection is made in the so called Kocher interval between the extensor carpi ulnaris and the anconeus. Therefore it is often called a limited Kocher approach. The study describes our modification of the Kocher approach that we use primarily in fractures of the head and neck of the radius, in certain fractures of the distal humerus, and also in irreducible dislocations and certain fracture-dislocations of the elbow. The incision is made along the line connecting the lateral epicondyle of the humerus and the border between the proximal and middle thirds of the ulna. The incision is pulled open and the strong, white opalescent common extensor fascia incised in order to identify the interval between the extensor carpi ulnaris and the anconeus. The two muscles are separated by thin vascularized fatty connective tissue which is split in order to expose a typical tendon reinforcing the upper half of the anterior margin of the anconeus. In this phase it is beneficial to detach the origin of the extensor carpi ulnaris from the lateral epicondyle. It facilitates retraction of the extensor carpi ulnaris anteriorly and of the anconeus slightly posteriorly. In contrast with the original Kocher approach, we do not release the anconeus from the lateral epicondyle of the humerus. The muscles are retracted to expose the anterolateral surface of the joint capsule and to identify the course of the LCL complex. The capsule is incised along the anterior margin of LCL, starting from the lateral epicondyle up to and including the radial annular ligament. Arthrotomy performed anterior to LCL spares the insertion of the lateral ulnar collateral ligament on the ulna and, consequently, preserves the elbow stability. If dissection more distally is required in order to expose the radial neck, part of the supinator must be incised as well. In such case the forearm is first carefully pronated as much as possible, as a result of which the canalis supinatorius including the deep branch of the radial nerve will move anteriorly, thus reducing the risk of injury to the nerve. The capsule is incised and opened, revealing the anterolateral surface of the head of humerus and radial head. In this phase it is beneficial to flex the elbow to 90−100 degrees, when the anterior part of the capsule will get flabby and allow a better visualization of the joint. The joint capsule must be released from the distal humerus together with extensors originating at the lateral epicondyle of humerus. This will considerably improve visualization of the anterior part of the joint cavity. During wound closure the common extensor fascia must be firmly sutured, as it is a significant but often underestimated stabilizer of the lateral part of the elbow. The extended option of the Kocher approach consists in retraction of the anconeus proximally. It is indicated in certain fracture-dislocations of the proximal forearm, i.e. fractures of the radial head and the entire proximal ulna. After dissection of the whole anconeus, this muscle is detached from the ulnar shaft and entirely reflected proximally. The muscle remains attached by its short proximal margin to the lateral epicondyle of humerus and to olecranon. This eliminates the risk of injury to the neurovascular hilus of the muscle, as the motoric nerve enters the muscle in the middle of its upper border. Retraction of the muscle exposes both the lateral surface of the joint capsule and the lateral surface of the proximal ulna. Vojenská fakultní nemocnice Praha Česká republika patří v celosvětovém měřítku mezi země s velmi častým výskytem a vysokou úmrtností na kolorektální karcinom. Důležitou roli v boji s tímto typem nádoru hraje sekundární prevence, tedy vyhledávání přednádorových lézí - adenomových polypů, které jsou následně odstraněny, a tím je přerušena cesta vzniku nádoru. Z populačních statických dat vyplývá, že riziko onemocnění tímto nádorem výrazně stoupá s věkem, a to od 50 let. Proto v ČR probíhá od roku 2000 program screeningu sporadického kolorektálního karcinomu. Osobám starším 50 let, které dosud neměli žádné příznaky, je nabízen test na okultní krvácení do stolice (TOKS). V případě jeho pozitivity následuje kolonoskopické vyšetření. Nově se provádí kolonoskopické vyšetření i bez předchozího TOKS u osob starších 55 let. Další screeningové programy zahrnují sigmoidoskopii, kombinaci testu na okultní krvácení a sigmoidoskopie a virtuální kolonografii. Nicméně účast cílové populace na tomto programu je stále nedostatečná, zejména ve srovnání se screeningem karcinomu mammy, prostaty a děložního čípku. Proto je naší snahou doplnit stávající metody o takové vyšetření, které by mělo stejnou efektivitu ve vyhledávání přednádorových lézí, ale bylo přijatelné ve větším rozsahu. Takovým vyšetřením se může stát kolonická kapslová endoskopie (kapslová kolonoskopie)., On a worldwide scale, the Czech Republic is among the countries with very high rates of and high mortality from colorectal cancer. Secondary prevention, i. e. identifying precancerous lesions – adenomatous polyps that are subsequently removed, thereby inhibiting tumor formation, plays an important role in fighting this type of tumor. Population statistical data suggest that the risk of disease with this tumor type significantly increases with age, namely from the age of 50 years. Therefore, a screening program for sporadic colorectal cancer has been under way in the Czech Republic since the year 2000. Persons over 50 years of age who have been asymptomatic so far are offered fecal occult blood test (FOBT). If the test result is positive, a colonoscopic examination follows. A colonoscopic examination is newly performed even without a previous FOBT in persons older than 55 years. Other screening programs include sigmoidoscopy, a combination of fecal occult blood test and sigmoidoscopy, and virtual colonography. However, participation of the target population in this program is still inadequate, particularly when compared to the screening for breast, prostate, and cervical cancers. Hence, it is our effort to add to the current methods a test that would have similar effectiveness in detecting precancerous lesions, but would be acceptable on a larger scale. Kolorektální karcinom u dětí je velmi vzácné onemocnění. Léčebné výsledky jsou neuspokojivé a celkové přežití dětských pacientů s kolorektálním karcinomem je nízké. Pacienti mají často histologicky nepříznivý typ nádoru a často jsou diagnostikování až v pokročilém stadiu onemocnění. Podáváme přehled současných názorů na kombinační léčbu a na postavení fixních kombinací v léčbě hypertenze podle doporučení ESH/ESC a ČSH z roku 2013. Mezi nejčastěji doporučované dvojkombinace patří blokátor renin-angiotenzinového systému (ACEI nebo sartan) a blokátor vápníku, dále blokátor renin-angiotenzinového systému a diuretikum a blokátor vápníku a diuretikum. V roce 2014 se na českém trhu objevila fixní kombinace ACE inhibitor (perindopril), blokátor vápníku (amlodipin) a diuretikum (indapamid). Ve studii the PIANIST u 4 731 nedostatečně kontrolovaných hypertoniků fixní trojkombinace perindopril, amlodipin a indapamid vedla ke snížení krevního tlaku o 28,3/13,8 mm Hg a k dostatečné kontrole hypertenze u 92 % nemocných. Výhoda fixních kombinací je především ve větší komplianci nemocného, a tím v lepší kontrole hypertenze. Trojkombinaci k dobré kontrole tlaku potřebuje asi třetina hypertoniků., We present an overview of the present views on combination treatment and on fixed combinations in the treatment of hypertension according to guidelines of ESH/ESC and ČSH from 2013. The most frequently recommended dual combinations include a blocker of the renin-angiotensin system (ACEI or sartan) and a calcium channel blocker, and further a blocker of the renin-angiotensin system and a diuretic and a calcium channel blocker and a diuretic. In 2014 a fixed-dose combination of an ACE inhibitor (perindopril), a calcium channel blocker (amlodipine) and an diuretic (indapamide) appeared on the Czech market. Within the PIANIST study including 4 731 insufficiently controlled hypertensives, a fixed-dose triple combination of perindopril, amlodipine and indapamide led to a decrease in blood pressure by 28.3/13.8 mm Hg and to a sufficient control of hypertension in 92 % of patients. The advantage of fixed combinations primarily consists in greater compliance of patients and thereby in a better control of hypertension. Přestože je implantace kloubních náhrad metodou volby pro řadu postižení pohybového aparátu a u diabetiků je indikována podle stejných principů jako u pacientů bez diabetes mellitus, je implantace endoprotézy u diabetiků sdružená s vysokým rizikem perioperačních komplikací. S přihlédnutím k velmi obtížné a náročné léčbě hluboké infekce kloubní náhrady hraje klíčovou roli prevence komplikací. Předpokladem adekvátní předoperační přípravy a správně vedené perioperační a pooperační péče o pacienty s diabetes mellitus je těsná spolupráce ortopeda, diabetologa a anesteziologa, kteří musí ve své praxi respektovat v předkládaném sdělení prezentovaná specifika svých pacientů diabetiků., While a joint implant is the method of choice for numerous locomotor disorders and it is indicated for diabetic patients based on the same rules as for patients without diabetes mellitus, a joint implant in diabetic patients is associated with a high risk of perioperative complications. Considering a very difficult and demanding treatment of a deep infection of a joint replacement, the prevention of complications plays the key role. Léčba kolorektálního karcinomu (KRK) vyžaduje multidisciplinární přístup s účastí gastroenterologa, chirurga, patologa, onkologa a radiologa. Možnosti léčby jsou závislé na stadiu nemoci, celkovém stavu pacienta a také molekulární charakteristice nádoru. V rámci předoperační péče se léčebné postupy u karcinomu konečníku mírně liší od léčby karcinomu tlustého střeva. V případě metastatického KRK jsou možnosti léčby jednotné, použití kombinace chirurgické a cílené systémové léčby. Každá léčebná modalita KRK je zatížená rizikem komplikací. Proto je nezbytné s každým pacientem probrat prospěch a rizika navrhované léčby. Prezentujeme dvě kazuistiky pacientů s KRK, u kterých léčba vedla k dosažení remise i za cenu významných komplikací., Treatment of colorectal cancer (CRC) requires a multidisciplinary approach involving a gastroenterologist, surgeon, pathologist, oncologist and a radiologist. Treatment options depend on the stage of the disease, the overall condition of the patient and the molecular characteristics of the tumor. In the context of preoperative care, treatment procedures for cancer of the rectum is slightly different from the treatment of the colon cancer. In the case of metastatic CRC, there are unified treatment options, using a combination of surgical and targeted systemic therapy. Each treatment modality is at risk of complications. Therefore it is necessary to discuss the benefits and risks of the proposed treatment with each patient. Podávání imatinib mesylátu (tyrozinkinázový inhibitor – TKI – 1. generace) se stalo standardní léčbou pacientů s chronickou myeloidní leukemií (CML), prokazatelně navozuje dlouhodobé odpovědi a prodlužuje čas do progrese onemocnění. European LeukemiaNet (ELN) v současnosti pro běžnou praxi doporučuje trvalou léčbu pro pacienty, kteří odpovídají optimálně na léčbu TKI standardní dávkou. Trvalé užívání TKI však může přinášet různé problémy. Zvyšující se počet pacientů odpovídá velmi rychle na cytogenetické úrovni a poté dále dosahuje redukce hladiny BCR-ABL1 transkriptu pod detekční možnosti i vysoce senzitivních molekulárně genetických metod. Sledování takto dlouhodobě stabilizovaných pacientů navodilo myšlenku, že někteří pacienti by mohli být považováni za „funkčně vyléčené” a mít prospěch z eventuálního cíleného přerušení léčby TKI. Tato myšlenka je v současné době intenzivně studována na úrovni klinických studií po celém světě. V této oblasti však před námi stále stojí mnoho nevyřešených otázek. Především jsou to otázky týkající se definice „fukčního vyléčení” CML, výběru vhodných pacientů pro přerušení léčby, managenentu dispenzarizace pacienta po vysazení TKI a znovuzahájení léčby, dojde-li k relapsu onemocnění., Imatinib mesylate, the first tyrosine kinase inhibitor (TKI) approved for the treatment of chronic myeloid leukemia (CML), became the standard of care, induces durable responses and prolongs event-free survival and progression-free survival. However, if TKI therapy is required indefinitely, then this ongoing drug exposure raises its own problems. Long-term follow-up indicates that an increasing proportion of patients treated with TKI have prompt cytogenetic response and achieve reductions in BCR-ABL1 transcripts to a level that is undetectable by very sensitive molecular genetic methods. Observation of the stability of such responses over long follow-up suggests that such patients may be “functionally cured“ of CML and thus potentially eligible for TKI therapy discontinuation. At present, therapy discontinuation in CML is not recommended in routine practice and is under active investigation in several clinical studies worldwide. Přes veškeré pokroky v léčbě chronických gastrointestinálních onemocnění u části pacientů nedosahuje léčba dlouhodobé optimální efektivity. Proto se řada pacientů uchyluje ke komplementární a alternativní medicíně (complementary and alternative medicine – CAM). Z různých typů CAM je pacienty s GIT onemocněními preferována především homeopatie, akupunktura a v neposlední řadě i fytoterapie, pod kterou lze řadit i léčebné užití Cannabis sativa. Patofyziologická báze terapeutického efektu léčebného konopí není doposud zcela objasněná. Studium působení kanabinoidů v organizmu je zájmem mnoha vědců z mnoha odvětví medicíny a farmakologie od začátku 20. století s významným vzestupem zájmu v 80. letech 20. století. Objev kanabinoidních receptorů (1988) a endogenních molekul aktivujících tyto receptory (1992) vedl k objevení endokanabinoidního systému. Farmakologická modulace endogenního kanabinoidního systému nabízí nové terapeutické možnosti v léčbě mnoha onemocnění a symptomů včetně onemocnění GIT, zahrnující nauzeu, zvracení, kachexii, IBS, Crohnovu chorobu a některá další onemocnění. Kanabinoidy jsou atraktivní terapeutickou možností – ovlivňují mnoho symptomů s minimálními vedlejšími účinky. Zkušenosti pacientů s GIT onemocněními ukazují, že užití kanabis je efektivní a pomáhá v případech, kdy selhává standardní terapie., Despite all the progress achieved in the treatment of chronic gastrointestinal diseases, in some patients the treatment does not reach long-term optimum effectiveness. Therefore a number of patients have turned to complementary and alternative medicine (CAM). Of the different types of CAM patients with GIT diseases tend to prefer in particular homeopathy, acupuncture and not least phytotherapy, where therapeutic use of cannabis may also be included. The pathophysiological basis of therapeutic effect of curative cannabis has not been fully clarified so far. Many scientists in many fields of medicine and pharmacology have been engaged in the study of effects of cannabinoids on the body since the beginning of the 20th century with the interest significantly increasing in the 1980s. The discovery of CB receptors (1988) and endogenous molecules which activate these receptors (1992) led to the discovery of the endocannabinoid system. Pharmacological modulation of the endogenous cannabinoid system offers new therapeutic possibilities of treatment of many illnesses and symptoms including the GIT disorders, including of nausea, vomiting, cachexia, IBS, Crohn’s disease and some other disorders. Cannabinoids are attractive due to their therapeutic potential – they affect a lot of symptoms with minimum side effects.
