- Description:
- Úvod: Efekt kyanoakrylátových lepidel byl opakovaně hodnocen u resekcí jater, plic a dalších orgánů, ale minimálně u resekcí ledvin. Cílem bylo zhodnocení užití kyanoakrylátového tkáňového lepidla u otevřené a laparoskopické resekce ledviny. Metoda: Byl hodnocen soubor 32 pacientů, kteří podstoupili resekci ledvin s ošetřením resekované plochy kyanoakrylátovým lepidlem. Dvacet tři pacientů prodělalo laparoskopickou resekci a 9 pacientů otevřenou. Byly hodnoceny operační výsledky, komplikace a onkologické výsledky. Výsledky: Medián doby sledování byl 46 měsíců. Průměrná doba byla u otevřené resekce 104 minut a u laparoskopické 157 minut, krevní ztráty byly 250 a 184 ml. Nebyla zaznamenána žádná závažná operační komplikace. Ve sledovaném souboru nebylo zaznamenáno úmrtí ani recidiva nádoru v tříletém pooperačním sledovaném období. Závěr: Hemostáza kyanoakrylátovým lepidlem Glubran II se jevila jako účinná a bezpečná u resekce ledvin. Na druhou stranu zvyšuje náklady na operaci a není nezbytná. Klíčová slova: parciální nefrektomie − laparoskopická resekce ledvin − kyanoakrylátové tkáňové lepidlo, Introduction: The effect of cyanoacrylate glues was repeatedly evaluated in resections of liver, lungs and others organs, but minimally in kidney resection. The aim of our study was to evaluate the use of cyanoacrylate tissue glue in open and laparoscopic partial nephrectomy. Method: We evaluated a cohort of 32 patients who underwent partial nephrectomy with treatment of the resected area using cyanoacrylate glue. Laparoscopic resection was done in 23 patients, while 9 patients underwent an open procedure. Surgical results, complications and oncologic results were assessed. Results: The median follow-up duration was 46 months. The mean time of open partial nephrectomy was 104 minutes and that of laparoscopic resection was 154 minutes; blood losses reached 250 ml and 184 ml, respectively. No serious complications occurred, and all patients survived the 3-year follow-up without any tumor recurrence. Conclusion: Hemostasis using the cyanoacrylate glue Glubran II was effective and safe. On the other hand, it increased the costs of the surgery and was not necessary. Key worlds: partial nephrectomy − laparoscopic resection of kidney − cyanoacrylate tissue glue, and J. Košina, J. - Description:
- Cardiac surgery is inseparably linked to the activation of innate immunity cells recognizing danger signals of both endogenous and exogenous origin via pattern recognition receptors such as TLR receptors. Therefore, we followed by flow cytometry TLR2 and TLR4 expression on blood monocytes and granulocytes of patients who underwent coronary artery bypass grafting using beating heart surgery (off-pump, n = 34), with use of standard cardiopulmonary bypass (CPB), (on-pump, n = 30), and miniinvasive CPB (mini on-pump, n = 25), respectively, before, during surgery, and up to 7th postoperative day. TLR2 and TLR4 expression both on monocytes and granulocytes was significantly diminished already at the end of CPB being highly significantly decreased at the end of surgery in all patients' groups. TLR2 and TLR4 expression reached preoperative value at the 1st postoperative day being significantly higher at the 3rd postoperative day. Using intracellular staining we found the peak of TLR2 and TLR4 expression inside of monocytes and granulocytes at the first postoperative day in a subgroup of on-pump patients. In conclusion, TLR2 and TLR4 expression is significantly modulated in patients undergoing coronary artery bypass grafting as a part of adaptive homeostatic mechanisms induced by major surgery. The very surgical trauma is responsible for TLR2 and TLR4 modulation. Surprisingly, cardiopulmonary bypass itself was little contributing to the modulation of TLR2 and TLR4 expression. and J. - Description:
- Mediterranean spotted fever (MSF) in Bulgaria shows increasing severity, especially in patients over sixty years. As T lymphocytes are the primary effector cells in MSF and are altered by the ageing process we aimed to specify the changes in their numbers in elderly patients. We performed a clinicoepidemiological study, haematological and biochemical analysis of 132 patients of 60+ and 30 patients between 19 and 57 years with MSF. We investigated the cell immunity of 20 patients of 60+ using immunofluocytometry. The control group consisted of 10 younger patients and 10 healthy individuals of 60+. MSF showed more unfavourable course in the elderly. We found T-lymphocytes depletion in all patients with MSF. Activated T lymphocytes were increased in elderly patients with MSF. We found significant differences between the number of activated T lymphocytes in elderly patients and healthy elderly persons. The observed T-lymphocytes depletion in MSF corresponded to the disease severity. The increase in the activated T lymphocytes in patients over 60 years contributes to an adequate immune response., Margarita Gospodinova, and Literatura

- Description:
- In this article we solve the non-standard situation that arose after publishing our paper "Crustal deformations in the epicentral area of the West Bohemia 2008 earthquake swarm in central Europe" (Schenk et al., 2012). Horálek and Fischer wrote a statement regarding our publication, sent it to specialists interested in research in the West Bohemia swarm area, and questioned the reliability of the seismic data used in our work. Since the statement regarding the reliability of our work was not directly sent to us we are using this journal to return to professional discussion regarding our results. In this paper we review scientific arguments made in their statement and provide review of various studies on West Bohemia tectonics and related seismicity., Vladimír Schenk and Zdeňka Schenková., and Obsahuje bibliografii

- Description:
- This review considers factors affecting the flight capacity of carabid beetles and the implications of flight for carabids. Studies from the Dutch polders in particular show that young populations of carabids consist predominantly of macropterous species and macropterous individuals of wing-dimorphic species. Also populations of wing-dimorphic carabid species at the periphery of their geographical range contain high proportions of macropterous individuals. However, studies from Baltic archipelagos show that older populations of even highly isolated island habitats contain considerable proportions of brachypterous species and individuals. This suggests that macroptery is primarily an adaptation for dispersal and that there exists a mechanism for subsequently reducing the ratio of macropterous to brachypterous species under stable conditions, due to the competitive advantage of brachyptery. Populations in isolated habitats, such as islands and mountains, have high proportions of brachypterous species. Many macropterous species do not possess functional flight muscles. Species of unstable habitats, such as tree canopies and wet habitats, are mostly macropterous. Brachypterous species tend to disappear from disturbed habitats. There is uncertainty regarding the extent to which carabid dispersal is directed and how much passive. Both Den Boer and Lindroth recognized that mostly macropterous individuals of macropterous and wing-dimorphic species disperse and found new populations, after which brachyptery tends to rapidly appear and proliferate in the newly founded population. It is most likely that the allele for brachyptery would arrive via the dispersal of gravid females which had mated with brachypterous males prior to emigration. Whilst many studies consider wing morphology traits of carabid beetles to be species-specific and permanent, a number of studies have shown that the oogenesis flight syndrome, whereby females undertake migration and subsequently lose their flight muscles by histolysis before eventually regenerating them after reproducing, has been reported for a growing number of carabid species. Wing morphology of carabid beetles clearly offers strong potential for the study of population dynamics. This field of study flourished during the 1940's to the late 1980's. Whilst a considerable amount of valuable research has been performed and published, the topic clearly holds considerable potential for future study., Stephen Venn., and Obsahuje bibliografii
