Předmětem této práce je objektivně posoudit dostupná výzkumná data týkající se bezpečnosti biologické terapie v těhotenství a kojení pacientek s idiopatickými střevními záněty. Biologická léčba se zdá být v graviditě bezpečná bez zvýšeného rizika vrozených malformací. Dostupné klinické údaje naznačují, že účinnost infliximabu a adalimumabu v dosažení klinické odpovědi a udržení remise těhotných pacientek převažuje teoretické riziko ohrožení plodu těmito léky. Terapie anti-TNFα preparáty by měla být, pokud je to možné, ukončena na konci 2. trimestru pro transplacentární přestup a z toho vyplývající riziko pro plod. Užití infliximabu a adalimumabu je pravděpodobně slučitelné s kojením., The aim of this article is to objective review available research data regarding the safety of biological therapies during pregnancy and breastfeeding in women with inflammatory bowel disease. Biological therapies appear to be safe in pregnancy, as no increased risk of malformations has been demonstrated. Available clinical results suggest that the efficacy of infliximab and adalimumab in achieving clinical response and maintaining remission in pregnant patients might outweigh the theoretical risks of drug exposure to the fetus. If possible, anti-TNF therapy should be stopped by the end of the second trimester due to transplacental transfer and potential risk for the fetus. Úvod: Radioimunoterapie patří mezi rychle se rozvíjející klinické metody. Často je nejvhodnější možností terapie u pacientů s relabujícím lymfomem, případně u starších pacientů. Vývoj nových typů imunokonjugátů patří proto v posledních letech mezi klíčová témata ve vývoji radiofarmak. Metody: Imunoglobulin G M75 byl značen metodou elektrofilní jodace radionuklidem 125I, který je optimální pro vývoj metody značení a také pro stanovení vlastností značeného preparátu (vazebná aktivita, imunospecifita). Výhody 125I jsou zejména jeho dlouhý poločas (59,408 dne) a nízká radiační zátěž. Zachování vazby aktivního místa na antigen bylo testováno metodou ELISA, koncentrace byla stanovena modifikovanou metodou podle Bradfordové. Fyziologická distribuce byla testována na myších CD1-Foxn1nu. Výsledky: Stanovená kumulace značené protilátky v nádorové tkáni byla > 7 % podané radioaktivity, vazebná aktivita protilátky byla první den po značení vyšší než 90 % ve srovnání s vazebnou aktivitou neznačené protilátky. Závěr: Připravený radioimunokonjugát IgG M75 s 125I si zachovává dostatečnou schopnost vazby antigenu, a má proto potenciál v terapii tumorů exprimujících karbonátdehydratasu. V práci je představena část projektu „Systém monitorování zdravotního stavu obyvatelstva ve vztahu k životnímu prostředí“, zaměřená na humánní biologické monitorování. Zdůrazněna je nutnost dodržování striktních pravidel celého preanalytického i analytického procesu z důvodu zaručení kompatibility výsledků během dlouholeté studie. Důležitým bodem při interpretaci je posouzení reálných možností laboratoře i vlastní analytické metody. Na základě dlouhodobého monitorování zdravotního stavu obyvatelstva v České republice byly určeny běžné obsahy vybraných prvků (vyjádřené pomocí mediánů) a navrženy referenční hodnoty (95% kvantily) pro sledované toxické prvky. Celkově bylo v projektu MZSO (do roku 2009) analyzováno okolo 6000 vzorků dospělých dárců krve (20–65 let) a 5300 vzorků školních dětí ve věku 8–10 let. Z nalezených hodnot pro Cd, Pb, Hg, Cu, Se a Zn u české profesionálně nezatížené populace vyplývá, že se významně neodlišuje od ostatních evropských států s podobným životním stylem., This study summarized results of human biomonitoring in the Czech Republic concerning trace elements in professionally non-exposed population. The problem of compatibility of long-term observations is discussed. “Normal” and reference values were determined for essential and toxic elements respectively. Procesy probíhající v ráně při hojení na sebe vzájemně navazují, ovlivňují se a podléhají poměrně složitému řízení. Obtížně se hojící a chronické rány, jejichž hojení neprobíhá podle základního schématu koagulace-zánět -prohferace-reparace, zůstávají obvykle po delší dobu ve fázi zánětu, což s sebou přináší dysregulace na různých úrovních. Hojení se účastní jak buněčné struktury (neutrofily, lymfocyty, mastocyty, monocyty, makrofágy, fibroblasty, keratinocyty, endotel), tak mediatory, tedy cytokiny s podtřídou růstových faktorů a proteázy, zejména skupina matrixových metaloproteáz a jejich tkáňových inhibitorů. Opakovaně trauma nebo ischemic vede ke zmnožení buněk, které ve větší míře produkují prozánětlivé cytokiny. Stoupá množství proteáz a objevuje se nedostatek inhibitorů. Tato nerovnováha prolonguje hojení, způsobuje odbourávání základní hmoty a umožňuje zmnožení bakterií, které dále proces zpomalují. Pochopení souhry faktorů může přispět k ovlivnění pochodů v ráně a k jejímu zhojení., Wound healing processes create a very complex system of connected reactions which influence each other. Chronic and hard-to-heal wounds do not follow the basic pattern consisting of coagulation-inflammation-proUferation-reparation. They stay in the inflammatory phase for longer time. This condition can cause different levels dysregulations. Both cell structures (as neutrophils, lymphocytes, monocytes, macrophages, mastocytes, fibroblasts, keratinocytes, endothel) and mediators (as cytokines with the subgroup growth factors and proteases, esp. matrix metalloproteases plus their tissue inhibitors) are involved into the healing. Repetitive trauma or ischaemia leads to a presence of a higher number of cells producing more proinflammatory cytokines. Proteases amount increases concurrently with the lack of inhibitors. This dysbalance prolongs the healing itself. Intercellular matrix degradation speeds up while enhanced bacterial burden slows the healing down. The larva of the Palaearctic Renocera pallida (Fallén, 1820) is confirmed as a predator of small species of bivalve molluscs of the family Sphaeriidae. To date only the larvae of three Nearctic Renocera species (and larvae of two other species of Sciomyzidae in two genera) are known to have the same food preference. The life cycle, biology, larval feeding and behaviour are described for the first time and compared with that of the Nearctic Renocera. The systematic position and biology of Renocera in general are discussed. Descriptions of the egg, second and third larval instars and the puparium of R. pallida are presented, the main features of the egg and larvae are illustrated by scanning electron micrographs.
The conifer needle scale, Nuculaspis abietis (Schrank) emerged as an important pest of conifers in the Kelardasht region of Mazandaran province, Iran, in the late 1990's. This pest feeds on conifer needles and twigs causing needle drop and branch desiccation. Its discovery in Kelardasht in Mazandaran Province necessitated a local quarantine of conifers in the genera Picea, Abies, and Pinus. We studied the life history and ecology of this scale on Norway spruce, Picea abies, under laboratory and natural field conditions. The complete life cycle of females required 206.4 ± 5.7 days in the laboratory (25 ± 1°C, 65-75% RH, 14L : 10D) and 315.7 ± 9.3 days under field conditions. In Kelardasht, numbers of adult males and females peaked in mid-June and early May, respectively, and numbers of first and second instar nymphs in mid-July and early September. Lifetime fecundity was estimated to be 57.3 ± 5.1 eggs and 54.0 ± 4.4 nymphs per female under laboratory conditions. The sex ratio ranged from 59% female for second instar nymphs to 71% female for adults. Aspidiotiphagus citrinus (Crawford) (Hymenoptera: Aphelinidae) was found naturally parasitizing the scale and overwintering in the larval stage on second instar nymphs. First generation adult wasps emerged in spring from overwintered second instar nymphs to parasitize 64.75% of first instar scales. Second generation wasps emerged from early September to mid-October and parasitized 19.75% of second instar scales, for a cumulative parasitism rate of 84.5%. and Arash RASEKH, J.P. Asthma is a complex disease with a variable course. Efforts to
identify biomarkers to predict asthma severity, the course of
disease and response to treatment have not been very successful
so far. Biomarker research has expanded greatly with the
advancement of molecular research techniques. An ideal
biomarker should be suitable to identify the disease as well the
specific endotype/phenotype, useful in the monitoring of the
disease and to determine the prognosis, easily to obtain with
minimum discomfort or risk to the patient. An ideal biomarker

Unfortunately, at the moment, an ideal biomarker doesn’t exist and
the overlap between the biomarkers is a reality. Using panels of
biomarkers could improve probably the identification of asthma
endotypes in the era of precision medicine.
Obesity is a strong cardiometabolic (CM) risk factor in children. We tested potential CM risk in obese/overweight children and the effect of an intensive lifestyle intervention using newer CM markers: atherogenic index of plasma AIP [Log(TG/HDL-C)], apoB/apoAI ratio and a marker of insulin resistance HOMA-IR. The participants (194 girls, 115 boys, average age 13) were enrolled in an intensive, one-month, inpatient weight reduction program. The program consisted of individualised dietary changes and the exercise program comprised aerobic and resistance training. Anthropometrical and biochemical parameters in plasma and CM risk biomarkers - (AIP, apoB/apoAI ratio and HOMA-IR) were examined before and after the intervention. AIP and HOMA-IR significantly correlated with BMI while apoB/apoAI ratio did not. Only AIP and HOMA-IR showed systematic increases according to the level of obesity by BMI quartiles. Lifestyle intervention significantly improved anthropometrical and biochemical values and the biomarkers too. The response of lipid parameters to the intervention was considerably higher in boys than in girls. The children were stratified into three risk categories according to AIP, where 13.8 % of boys and 5.3 % of girls fell into high risk category. The monitored biomarkers may complement each other in the prognosis of CM risk. AIP was strongly related to obesity and to lipid and glycid metabolism, while the relationship of the apoB/apoAI ratio to obesity and glycid metabolism was not significant. Primary graft dysfunction (PGD) is a life-threatening complication among heart transplant recipients and a major cause of early mortality. Although the pathogenesis of PGD is still unclear, ischemia/reperfusion injury has been identified as a predominant factor. Both necrosis and apoptosis contribute to the loss of cardiomyocytes during ischemia/reperfusion injury, and this loss of cells can ultimately lead to PGD. The aim of our prospective study was to find out whether cell death, necrosis and apoptosis markers present in the donor myocardium can predict PGD. The prospective study involved 64 consecutive patients who underwent orthotopic heart transplantation at our institute between September 2010 and January 2013. High-sensitive cardiac troponin T (hs-cTnT) as a marker of minor myocardial necrosis was detected from arterial blood samples before the donor’s pericardium was opened. Apoptosis (caspase-3, active + pro-caspase-3, bcl-2, TUNEL) was assessed from bioptic samples taken from the right ventricle prior graft harvesting. In our study, 14 % of transplant recipients developed PGD classified according to the standardized definition proposed by the ISHLT Working Group. We did not find differences between the groups in regard to hs-cTnT serum levels. The mean hs-cTnT value for the PGD group was 57.4±22.9 ng/l, compared to 68.4±10.8 ng/l in the group without PGD. The presence and severity of apoptosis in grafted hearts did not differ between grafts without PGD and hearts that subsequently developed PGD. In conclusion, our findings did not demonstrate any association between measured myocardial cell death, necrosis or apoptosis markers in donor myocardium and PGD in allograft recipients.
