Úroveň řízení lidských zdrojů ve zdravotnictví je jedním z klíčových faktorů, které ovlivňují kvalitu a bezpečí poskytované zdravotní péče. Článek obsahuje výsledky celonárodní studie provedené v lůžkových zdravotnických zařízeních ČR. Cílem studie bylo jak kvantitativně, tak kvalitativně zhodnotit současnou situaci v oblasti řízení lidských zdrojů s důrazem na kompetence lékařského personálu. Výsledky kvantitativní části byly podrobeny křížové analýze za účelem zjistit souvislost úrovně řízení kompetencí se třemi externími motivačními faktory – akreditací, certifikací a akreditací pro postgraduální vzdělávání lékařů. Výsledky studie prokázaly, že všechny tři typy externích motivačních faktorů souvisejí s vyšší kvalitou řízení kompetencí lékařů v lůžkových zdravotnických zařízeních ČR., The level of human resource management of the healthcare staff is one of the key factors influencing the quality and safety of the healthcare services provided by such staff. The following article contains the result of the national study carried out in the Czech Republic. The goals of the study were to both quantitatively and qualitatively evaluate current situation of the management of competencies (privileging) in inpatient healthcare facilities in the Czech Republic, with focus on privileging of physicians. The quantitative findings from individual facilities were then cross-analyzed with 3 external motivational factors - accreditation, certification, teaching status – to see whether those facilities appraised by one or more external systems mentioned above have obtained higher score in the quantitative part of the study corresponding to more sophisticated system of privileging of physicians. All three examined types of external appraisal showed statistically significant correlation with higher score in the level of sophistication of privileging of physicians. The Purpose of the study: define the indications and contraindications to sphincterpreserving operation in lowerampullar cancer of rectum. The Materials and methods: In coloproctological branch of National Oncological Scientific centre (NOSC) in 20052009 years 142 patients were performed radical operations, 64 (45,1%) patients were performed sphincterpreserving operations abdominoanal resection of rectum (AARR), medium age of patients 59,8 years, beside 78 (54,9%) patients were performed abdomeno perineal extirpation of rectum (APER), medium age 54,2 years. The Results: Studied results of the morphological research of removed preparation by histological structure of tumors and by form of the growing, depending on locations of the tumors on toothed line. The remote postoperative results studied beside 51 (79,7%) patient with AARR and beside 63 (80,1%) patients with APER. The Findings: sphincterpreserving operations (SPO) of the rectum can be the operation of the choice in treatment of high differentiated adenocarcinoma (HDA) and medium differentiated adenocarcinoma (MDA) in stages T23 in localization of the lower pole of tumors, not below 1 centimeter of toothed line (TL) and carries combined character, in location of the lower edge of tumors on toothed line in patients with HDA MDA in 4% and 2,2% cases are shown that completing intrasphincternal resections of rectum, in connection with particular aggressiveness LDA question about choice of the method of the treatment under their localizations directly on TL must be solved in favour of complete APER, in exophytic tumor located onto and above TL is indication for sphincterpreserving operation in type of AARR, indices recurrencefree and not metastatic period, oneyear deathrate, threeyear survivability after SPO at inferior ampular cancer of rectum has not an essential distinction from results after APER., A. Aim of investigation: to study immediate and remote results of standard and extended lymphodissection in patients with rectal cancer. Material and methods: 132 patients with rectal cancer were performed radical surgery with lymphodissection in D2 and D3 volume according to a height of tumor location. In immediate postoperated period there were noted complications in 4 patients that made up 3,03%. Observation time was 5 years. Tumor recurrence was reported in 5 patients (3,8%). In remote period metastases were revealed in 8 patients (6,6%). 5year total survival rate was 78,5±4,2% at stage II. 5year total survival rate was 56,4±3,7% at stage III. Comparing the results of standard and extended lymphodissection it was noted statistically authentic increase of 5 year survival rate in patients at stage III. The article analyses a surgical treatment of 506 patients taking inpatient treatment in the Center for colon proctology. All the patients underwent a comprehensive examination including radiological and tool test methods in addition to conventional clinical and laboratory ones. On the basis of the complex study, indications, volume and terms of surgery performance were determined. Performance of preand postoperative management of patients is substantiated in details. Postoperatively, 28 (5.5%) had postoperative wound pyesis in the region of the previous colostomy. In the postoperative period 2 (0.4%) deaths of patients were caused with peritonitis which occurred due to incompetence of anastomosis sutures. Rehabilitation of stomed patients is one of challenges in colorectal surgery. Positive results treatment can be obtained only at carrying out of a comprehensive outpatient and inpatient preoperative preparation including recommendations in diet, colostome care, hydrogymnastics of the disconnected part of the intestine and prevention of infectious complications., Isakulov T. Vztahy mezi všeobecnými sestrami a lékaři jsou zejména v posledních letech předmětem zvýšeného zájmu. Existující problémy lze na jedné straně vyjádřit jako specifický odraz strukturálních změn ve společnosti jako celku, na druhé straně jako odraz změn v systému zdravotní péče. Ve zdravotnictví vyspělých zemí dochází ke změně společenského postavení všeobecných sester a lékařů. Všeobecné sestry získávají vyšší vzdělání, další kompetence a zvyšují si své postavení v současném systému zdravotní péče. Bez řádně provedených empirických výzkumů nelze všeobecně hovořit o napětí nebo až konfliktnosti ve vztahu lékař –všeobecná sestra. Na základě reprezentativního výzkumu lékařů a všeobecných sester v České republice bylo cílem zjistit názory (v daném článku lékařů) na problematiku vzájemných vztahů, předávání si kompetencí, společně s řešením konfliktů ve vzájemném vztahu. Na závěr článku (viz příloha) je vytvořena strategie k podpoře týmové spolupráce sester a lékařů., Recently, the relationships between nurses and doctors have become the centre of attention. On one hand, the existing problems can be expressed as a specific reflection of structural changes in the society; on the other hand, they reflect the changes in the health care system. In the developed countries, the social status of general nurses and doctors has been changing. Nurses obtain higher education and competences and improve their position in the current health care system. The expressions “stressful” or even “conflicting” relationships between doctors and nurses should not be used without a reliable previous empiric research. On the basis of a representative research performed on doctors (in the particular article, performed by doctors), the opinions on the problems of mutual relationships, delegation of the competences and dealing with conflicts of the mutual relationships was searched. Komunitní zdravotní péče, zvláště ve vztahu k ošetřovatelským službám, je klíčem ke zlepšení zdraví a tělesné i duševní pohody všech lidí. Cílem výzkumu bylo zjistit, jaké jsou představy odborné a laické veřejnosti o možnostech působení sester v komunitní péči a podrobněji analyzovat jednotlivé oblasti této péče z hlediska uplatnění sester v nich. V rámci výzkumu byly předmětem zkoumání tři cílové skupiny – lékaři ČR, sestry ČR a občané ČR. Do výběrového souboru bylo zařazeno 1007 lékařů, 1005 všeobecných sester a 2022 občanů. Tito respondenti byli vybráni náhodným výběrem pomoci kvót. Následující článek je věnován interpretaci výsledků výzkumu všech cílových skupin – lékařů, sester i občanů z hlediska jejich názorů, týkajících se předpokladů a překážek, které vidí v začlenění sester do komunitní péči v České republice. Odborná i laická veřejnost vidí možnosti uplatnění sestry v komunitní péči v různých oblastech. Péče o seniory je nejvíce preferovanou oblastí ve všech skupinách. Nejdůležitějším předpokladem pro samostatnou práci sestry v komunitní péči je její předchozí klinická praxe, odpovídající zaměření komunitní péče. Hlavní překážky, které brání realizaci samostatné péče komunitních sester uvádí shodně odborná i laická veřejnost: nejasnost financování, finanční zátěž, obtížná spolupráce se zdravotními pojišťovnami, otázky legislativy – uzákonění a ustanovení kompetencí sester v ČR. Lékaři i sestry se shodují v tom, že činnost komunitních sester by měl financovat především stát, menší část respondentů si myslí, že financovat činnost těchto pracovníků by měla organizace, ve které pracují a třetím nejrozšířenějším názorem je financování krajem. Lékaři České republiky mají převážně zájem o případnou spolupráci se sestrami, působícími v oblasti komunitní péče a tuto spolupráci by přivítali. Ze strany sester však v současném období převládá v České republice nezájem o případnou práci v oblasti komunitního ošetřovatelství, který lze vysvětlit především nejasným financováním v této oblasti., The goal of the study was to ascertain the ideas of professional and lay public on the possibilities of work of nurses in community care and to analyze in more detail the individual areas of such care from the perspective of use of work of nurses in them. Three target groups consisted of 1007 physicians, 1005 general nurses and 2022 citizens. The respondents were selected by random selection with the help of quotas. Both professional and lay public sees the opportunities of use of work of the nurse in community care in different areas. Care for seniors is the mostly preferred area in all groups. The most important precondition for independent nurse’s work in community care is her preceding clinical practice corresponding to the focus of the community care. The main obstacles to implementation of independent care of community nurses are stated coincidently by professional and lay public: unclear funding, financial burden, difficult cooperation with health insurance funds, legislation questions – codification and establishment of nurse competences in the Czech Republic. Cílem práce je identifikace nejvlivnějších aktérů působících v českém zdravotnickém systému a posouzení šance na úspěšné prosazení vybraných reformních opatření z pohledu podpory těchto nejvlivnějších aktérů. Autoři se domnívají, že podpora nejdůležitějších hráčů může být jednou z podmínek pro úspěšnou zdravotní reformu a přijetí odpovídající zdravotní politiky. V první části článku, představují výsledky empirické studie uskutečněné na jaře roku 2009. Autoři shromáždili na základě řízených rozhovorů a dotazníkového šetření data, která dále analyzovali. Výsledkem jejich práce bylo posouzení deseti nejvlivnějších hráčů podle jejich relativního vlivu, vyjednávacího potenciálu a broker potenciálu. V druhé části práce autoři na základě volebních programů šesti nejsilnějších politických stran vymezili 23 nejčastěji se v koncepcích opakujících navrhovaných opatření a identifikovali reakce jednotlivých aktérů k těmto opatřením. Byly nalezeny tři oblasti (opatření), které shodně podporují všichni zkoumaní aktéři a bylo identifikováno 13 opatření, která aktéři celkově podporují i když každý jinou měrou. Autoři zkoumají též míru uspokojení zájmů jednotlivých aktérů. Vymezují tak oblasti charakterizované určitou mírou konsenzu ale i oblasti charakteristické antagonistickými zájmy., The aim of the paper is to find out the most powerful groups in the Czech healthcare arena and to determine their position towards selected reform measures. Authors believe that the support of the most important players can be one of the conditions for a successful health (reform) policy implementation. In the first part of the paper, authors represent results from an empirical study effectuated during spring 2009. Managed interviews with elite representatives of selected interest groups were conducted; followed by an electronic questionnaire. Both parts of this research consisted of structured and semi-structured questions that gathered information about influence reputations, informal communications, coalitions, and other strategic behaviour. Úvod: Jednou z charakteristických vlastností malígnych nádorov je zakladanie vzdialených metastáz, pričom práve pľúca bývajú často miestom metastázovania. Torakochirurgické odstránenie metastáz je podľa publikovaných údajov akceptované ako jedna z možností liečby. Cieľom tejto štúdie bolo vyhodnotiť údaje súvisiace s metastázami v pľúcach a s ich odstránením, zistiť prežívanie pacientov. Metodika: Retrospektívne autori vyhodnotili 61 pacientov, ktorí sa podrobili chirurgickému odstráneniu metastáz z pľúc na Chirurgickej klinike JLF UK a MFN v Martine. Výsledky: Najčastejším primárnym tumorom bol kolorektálny karcinóm (41 %). Solitárne ložisko malo 50 % chorých. Priemerný vek operovaných bol 51,7 roka a priemerný disease-free interval (DFI) 40,3 ± 36,5 mesiacov. Najčastejším operačným výkonom bola klinovitá resekcia z torakotómie (54 %). Pooperačné komplikácie sa vyskytli u 4,5 % pacientov. Pooperačná mortalita bola 0 %. 2 – ročné prežívanie pacientov po chirurgickej kompletnej aj nekompletnej resekcii bolo 59 %, 3 – ročné prežívanie 48 %, medián prežívania bol 33 mesiacov. Diskusia: Výskyt solitárnych ložísk sa pohybuje od 48 % do 75 %. Pacienti s viacpočetným postihnutím mali štatisticky signifikantne horšie prežívanie ako pacienti so solitárnou metastázou (3 – ročné prežívanie 37 % verzus 61 %, p = 0,03 %). DFI sa pohybuje v rozmedzí od 31,4 do 41 mesiacov. Pacienti s DFI 36 a viac mesiacov mali štatisticky nesignifikantné dlhšie prežívanie ako pacienti s DFI menej ako 36 mesiacov. Záver: Chirurgické odstránenie pľúcnych metastáz je dnes bezpečná a potenciálne kuratívna metóda spojená s nízkym rizikom komplikácií a s dobrou efektivitou., Introduction: One of the characteristic properties of malignant tumors is the foundation of distant metastases, whereby lungs are often the place of these metastases. Thoracic surgical removal of metastases is according to the published information acceptable as one of the possibilities of treatment. The aim of this study was to evaluate data relative to lung metastases and their removal, to find out the living of the patients. Methods: Authors retrospectively evaluated 61 patients, which submitted surgical treatment of lung metastases on Surgical Clinic JLF UK and MFN in Martin. Results: The most common primary tumor was colorectal carcinoma (41 %). 50 % of patients had solitary metastasis. The average age of operated was 51,7 and average disease-free interval (DFI) was 40,3 ± 36,5 months. The most common operative performance was wedge resection from thoracotomy (54 %). Post – operative complications occurred in 4,5 % of patients. Post – operative mortality was 0 %. Two years survival of patients after complete or incomplete surgical resection was 59 %, three years survival was 48 %, median of survival was 33 months. Discussion: The occurrence of solitary metastasis is between 48 % and 75 %. Patients with multiple metastases have statistically significantly worse survival as patients with solitary metastasis (3 years survival 37 % versus 61 %, p = 0,03 %). DFI is variable from 31,4 to 41 months. Patients with DFI 36 and more months had not statistically significantly longer survival as patients with DFI less than 36 months. Východisko: AL-amyloidóza je nejčastějším typem amyloidózy v západních zemích. Ve srovnání s ostatními plazmocytárními dyskraziemi má nejhorší prognózu – ta se však v posledním desetiletí se zlepšuje a současně uváděné přežití pacientů jsou přibližně 4 roky. Vysokodávkovaná chemoterapie s podporou kmenových buněk (ASCT) vede relativně často k dosažení kompletní remise, v úvodních studiích však byla zatížena vysokou mortalitou. Navíc multicentrická randomizovaná studie neprokázala vyšší efekt ASCT proti konvenční léčbě – proto je úloha tohoto postupu stále nejasná. Soubor pacientů a metody: Během poslední dekády byla na našem pracovišti provedena ASCT u 14 pacientů s AL-amyloidózou. Předtransplantační dávky melfalanu, volené podle dříve publikovaného „risk-adapted“ systému, jsme ještě dále modifikovali, respektive redukovali. Výsledky: Transplantační mortalita byla 7,1%. Orgánovou odpověď dosáhlo 57 % nemocných včetně 21 % kompletních remisí (KR). Hematologickou odpověď jsme pozorovali u 58 % pacientů z 12 hodnocených. Střední doba přežití je 77 měsíců, medián doby bez progrese 31 měsíců. Závěr: ASCT s redukovanou dávkou melfalanu je podle našich zkušeností účinná a relativně bezpečná možnost léčby AL- -amyloidózy., Purpose: AL-amyloidosis is the most frequent form of amyloidosis in western countries. The prognosis is the worst among other plasma cell dyscrasias but it appeared to be improved during last decade – currently reported survival is approximately 4 years. High-dose therapy with stem cell support (ASCT) could yield a high rate of complete remission. however, transplant-related mortality (TRM) was high in the early studies. Moreover, a multicenter randomized trial did not confirm the superiority of ASCT and thus the role of this approach remains still unclear. Patients and methods: Fourteen consecutive patients with AL-amyloidosis were treated using own “strict risk-adapted“ dosed melphalan conditioning within last 10 years in our center. Results: TRM was 7,1%, organ response was 57%, including 21% complete remission. Hematologic response was 58% in twelve evaluable patients. Median overall survival was 77 months, median progression-free survival 31 months.
