Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Purpose: This study aimed to evaluate prostate volume changes and prostate motions during radiotherapy.Methods: In 2010, twenty-five patients were treated for prostate cancer by external beam radiotherapy with implanted fiducial markers. Coordinates of three gold markers on kilovoltage images were calculated daily. Volume changes in target structure were observed through changes in intermarker distances. Differences in patient position between laser-tattoo alignment and gold marker localization were evaluated. Intrafraction motion was assessed by measuring marker displacement on kilovoltage images acquired before and after fraction delivery. Results: Prostate shrinkage was observed in 60% of patients. The average shrinkage was 7% of the prostate’s initial volume. Corrections after laser-tattoo alignment remained mostly below 1 cm. The difference between marker centroid position on the actual images and the planning images was 2 ± 1 mm on average. The extension of intrafraction movements was 7.6 ± 0.2 mm on average. Conclusions: In our retrospective study, the possibility for prostate volume changes during radiotherapy was revealed. V letech 1959–2006 bylo u českých mužů evidováno 81 884 nových karcinomů prostaty (KP) s nárůstem z 12,3 na 96,1 na 100 tis. mužů. Úmrtnost se zvýšila z 21,3 na 27,2 na 100 tis. mužů. Podíl KP ze všech nádorů se zvýšil u incidence ze 7,1 % na 13,2 %, u mortality z 6,7 % na 8,9 %. V letech 1989–2005 vzrostla prevalence KP z 98 na 408,5 na 100 tisíc mužů a dosáhla téměř 20 500 žijících. Z 11 183 nemocných s vícečetnými KP bylo evidováno 4 608 primárních a 6 575 následných případů, představujících 17,4 % ze všech evidovaných KP v letech 1976–2005. Z primárních KP bylo léčeno 89,7 % duplicit a 10,3 % multiplicit. Počet primárních KP vzrostl na 282 v roce 1996 a pak klesal, následných KP trvale rostl na 705 v roce 2005. Téměř polovina KP se vyskytla ve věku 70–79 let a třetina ve věku 50–69 let. Poměr 1 115 (21,3 %) synchronních a 4 121 (78,7 %) metachronních následných novotvarů byl 1:3,7 s průměrnou dobou vzniku následného novotvaru 4,1 roku. Z 5 236 následných novotvarů po primárních KP byly nepočetnější nádory kůže 28,6 %, trávicí 26,7 %, močové 17,8 % a respirační 10,9 % soustavy. Z 5 613 novotvarů před KP jako druhou diagnózou, bylo 35,3 % nádorů kůže, 21 % trávicí, 19,6 % močové a 8,7 % respirační soustavy. Z nemocných s primárním KP k 17. 10. 2007 žilo 22,4 % a zemřelo 77,6 %, s následným KP žilo 31,6 % a zemřelo 68,4 % mužů. The most frequent from 5,613 neoplasms before PC as the second diagnosis there were 35,3 % cancers of skin, 21 % gastrointestinal, 19,6 % urinary and 8,7 % respiratory tract. Desmoplastický kulatobuněčný tumor (DSRCT) je raritní onemocnění patřící do skupiny sarkomů. V typickém případě se manifestuje rozsáhlou tumorózní infiltrací břišní dutiny bez evidentního primárního ložiska tumoru. Prognóza onemocnění je stále špatná, 5leté přežití je popisováno asi v 15 % případů i při užití kombinovaných protinádorových léčebných postupů. Uvádíme kazuistiku 34letého pacienta s DSRCT léčeného v našem centru. Zmiňujeme racionální postup v rámci diferenciální diagnostiky měkkotkáňové infiltrace v dutině břišní, podtrhujeme zásadní význam molekulárně genetické analýzy a zaměřujeme se na terapeutické možnosti tohoto zřídka se vyskytujícího onemocnění., Desmoplastic small round cell tumor (DSRCT) is a rare disease from sarcoma group family. Typically, there is an extensive infiltration of abdominal cavity, peritoneal effusion with no evident primary origin of tumor. Prognosis is unfavourable with 5-years overall survival described in 15 % of cases even though multimodal treatment strategy is performed. We introduce case report of 34-years old patient with DSRCT treated in our Oncology center. Reasonable schema of differential diagnosis of soft-tissue expansion in abdominal cavity is mentioned and we emphasize crucial role of molecular genetic analysis. Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%). Centrum pro tyreoidální onkologii a endokrinní orbitopatie Nádory štítné žlázy vycházejí z folikulárního epitelu štítné žlázy, jsou hormonálně dependentní, vyskytují se 5x častěji u žen než u mužů ve všech věkových skupinách, obvykle dobře vychytávají jod. Klinicky se může projevovat jako lokálně rostoucí uzel ve štítné žláze, nádorová infiltrace spádových krčních lymfatických uzlin, vzdálené plicní a kostní metastázy. Nádory tlustého střeva a konečníku patří v České republice k nejčastějším zhoubným nádorům jak v populaci mužů, tak v populaci žen. Více než 50 % nově diagnostikovaných kolorektálních nádorů je zjištěno v pokročilém stadiu vývoje a zhruba 20 % má v době stanovení diagnózy založeny vzdálené metastázy. Játra jsou nejčastější orgánovou lokalizací metastáz kolorektálních karcinomů. Kurativní resekce metastáz je primárně možná pouze u malé části pacientů. Proto systémová léčba, chemoterapie a v posledních letech biologická léčba hrají významnou úlohu v léčbě metastazujícího kolorektálního karcinomu. Cetuximab (Erbitux) je monoklonální protilátka specificky namířená proti EGFR receptoru nádorových buněk, včetně buněk kolorektálního karcinomu. Inhibicí aktivity EGFR receptoru a jeho signální kaskády může Erbitux zastavit další růst nádorových ložisek a navodit jejich regresi. Přestože je cetuximab účinný i jako monoterapie, jeho účinek se zvyšuje v kombinaci s chemoterapií, opírající se jak o irinotekan, tak o oxaliplatinu. Přidání cetuximabu k chemoterapii navíc pomáhá zvrátit refrakternost nádorových buněk vůči chemoterapii samotné. V této kazuistice popisujeme případ staršího pacienta mužského pohlaví, u kterého byl cetuximab nasazen v době, kdy předcházející chemoterapie (režimy FOLFOX4 i FOLFIRI) nedokázala zabránit progresi onemocnění. Léčba cetuximabem v kombinaci s režimem FOLFIRI dosáhla významné regrese metastatického postižení a při pokračující udržovací léčbě cetuximabem samotným zůstává pacient v klinické remisi, která již trvá déle než 15 měsíců. Pacient dlouhodobou léčbu cetuximabem vcelku dobře toleruje, hlavním nežádoucím účinkem léčby je kožní akneiformní reakce, která však nebrání v pokračování terapie., In the Czech Republic, colorectal cancer is one of the most common malignancies both among men and women. More than 50 % of newly diagnosed colorectal cancers are found in an advanced stage and about 20 % have distant metastases at diagnosis. The liver is the most common site of metastasis from colorectal cancer. Curative resection of metastases is primarily feasible only in a small proportion of patients. Thus, systemic therapy, chemotherapy and, more recently, biological therapy play a significant role in treating metastatic colorectal cancer. Cetuximab (Erbitux) is a monoclonal antibody targeted specifically at the EGFR receptor of tumour cells, including those of colorectal cancer. By inhibiting the activity of the EGFR receptor and its signal cascade, Erbitux can stop further growth of tumour foci and induce their regression. Although cetuximab is also effective as monotherapy, its effect increases in combination with chemotherapy involving both irinotecan and oxaliplatin. Moreover, adding cetuximab to chemotherapy helps to reverse the refractoriness of tumour cells to chemotherapy alone. The present case report describes an elderly male patient who was started on cetuximab after previous chemotherapy regimens (FOLFOX4 and FOLFIRI) had failed to prevent the progression of disease. Treatment with cetuximab in combination with the FOLFIRI regimen resulted in a significant regression of metastatic disease and, while being continued on maintenance therapy with cetuximab alone, the patient has remained in clinical remission for more than 15 months now. Ve sdělení autoři prezentují kazuistiku duplicity primárních mozkových nádorů. Její výskyt je velmi vzácný, udává se méně než 1:1 000 000. U 69letého pacienta byl diagnostikován frontotemporálně vpravo lokalizovaný multiformní glioblastom způsobující klinickou symptomatologii a dále klinicky němý drobný meningeom v oblasti levého mostomozečkového koutu. Multiformní glioblastom byl subtotálně resekován z kraniotomie a drobný asymptomatický meningeom byl ponechán k observaci., The authors present a case report of duplicity of primary brain tumours.
