Akútna inverzia maternice je závažná pôrodnícka komplikácia, pri ktorej je perakútne ohrozený život rodičky. Pri inverzii maternice dochádza k rôznemu stupňu poklesu tela maternice. Príčinou inverzie je takmer vždy nesprávny postup vedenia III. doby pôrodnej. Okamžitou diagnostikou a neodkladnou adekvátnou liečbou sa minimalizuje materská morbidita a mortalita. Prezentujeme praktický postup riešenia tejto komplikácie, ktorý sme zostavili na základe skúseností s tromi prípadmi kompletnej inverzie maternice a literárnych údajov., Acute uterine inversion is severe obstetric and life threatening complication. Cíl: Přezkoumání benefitů metody klokánkování pro nedonošené novorozence, to znamená najít dostatek důkazů pro opodstatnění používání metody klokánkování na Jednotce intenzivní péče pro novorozence. Metodika: Vyhledávání validních indikátorů pozitivního vlivu metody klokánkování prostřednictvím metody praxe založené na důkazech. Pro získání relevantních zdrojů byly využity licencované databáze a volně přístupné databáze - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Výsledky: Analýza výsledků prokázala jednoznačně pozitivní vliv metody klokánkování na nedonošené novorozence umístěné na Jednotce intenzivní péče, a to zejména v oblasti vnímání bolesti u invazivních výkonů, efektivního kojení, zvýšení váhového přírůstku, delšího a klidnějšího spánku, pozitivnější interakce matka – dítě, udržení nebo zvýšení tělesné teploty, zkrácení doby hospitalizace, růstu do délky, růstu obvodu hlavičky a aktivace centrální nervové soustavy. Závěry: Výzkumné studie (n = 34) a jeden systematický přehled ukazují pozitivní vliv metody klokánkování na nedonošeného novorozence umístěného na Jednotce intenzivní péče v oblastech biopsychosociálních. V podmínkách českého zdravotnictví chybí vypracované platné standardy pro využívání metody v praxi., Objective: To review benefits of kangaroo care method for premature babies, it means finding enough evidence to justify using the kangaroo care method in the Intensive care unit for newborns. Methods: Searching for valid indicators of a positive influence method of kangaroo care through method evidence-based practice. To obtain relevant sources were used licensed databases and freely available databases - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Results: Analysis of results showed a clearly positive impact of the kangaroo care methods on premature infants placed in intensive care, especially in the perception of pain in invasive procedures, effective breastfeeding, increased weight gain, longer and quieter sleep, more positive interactions of mother - child, maintaining or increasing body temperature, reduced hospitalization, growth in length, head circumference growth and activation of the CNS. Conclusions: The research studies (n = 34) and one systematic review have shown a positive influence of the kangaroo care methods on premature newborns placed in intensive care in the areas of bio-psycho-social. V práci je představena část projektu „Systém monitorování zdravotního stavu obyvatelstva ve vztahu k životnímu prostředí“, zaměřená na humánní biologické monitorování. Zdůrazněna je nutnost dodržování striktních pravidel celého preanalytického i analytického procesu z důvodu zaručení kompatibility výsledků během dlouholeté studie. Důležitým bodem při interpretaci je posouzení reálných možností laboratoře i vlastní analytické metody. Na základě dlouhodobého monitorování zdravotního stavu obyvatelstva v České republice byly určeny běžné obsahy vybraných prvků (vyjádřené pomocí mediánů) a navrženy referenční hodnoty (95% kvantily) pro sledované toxické prvky. Celkově bylo v projektu MZSO (do roku 2009) analyzováno okolo 6000 vzorků dospělých dárců krve (20–65 let) a 5300 vzorků školních dětí ve věku 8–10 let. Z nalezených hodnot pro Cd, Pb, Hg, Cu, Se a Zn u české profesionálně nezatížené populace vyplývá, že se významně neodlišuje od ostatních evropských států s podobným životním stylem., This study summarized results of human biomonitoring in the Czech Republic concerning trace elements in professionally non-exposed population. The problem of compatibility of long-term observations is discussed. “Normal” and reference values were determined for essential and toxic elements respectively. Altogether about 6000 samples of adults (blood donors, 20–65 yrs) and 5300 samples of children (8–10 yrs) were analysed for Cd, Pb, Hg, Cu, Se and Zn. Ruptured pregnancy in the rudimentary horn of women who have had a vaginal delivery is rare and unpredictable. However, when undiagnosed, this condition could lead to maternal morbidity and mortality. We report a pregnancy at 19 weeks gestation presented with acute abdomen and hypovolemic shock. She was initially thought to have an intrauterine pregnancy with the provisional diagnosis of a ruptured uterus. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%). Women are significantly less affected than men, with the ratio of women to men being 4:1 in the Czech Republic. In terms of risk factors, smoking and alcohol consumption play a crucial role. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. Desmoplastický kulatobuněčný tumor (DSRCT) je raritní onemocnění patřící do skupiny sarkomů. V typickém případě se manifestuje rozsáhlou tumorózní infiltrací břišní dutiny bez evidentního primárního ložiska tumoru. Prognóza onemocnění je stále špatná, 5leté přežití je popisováno asi v 15 % případů i při užití kombinovaných protinádorových léčebných postupů. Uvádíme kazuistiku 34letého pacienta s DSRCT léčeného v našem centru. Zmiňujeme racionální postup v rámci diferenciální diagnostiky měkkotkáňové infiltrace v dutině břišní, podtrhujeme zásadní význam molekulárně genetické analýzy a zaměřujeme se na terapeutické možnosti tohoto zřídka se vyskytujícího onemocnění., Desmoplastic small round cell tumor (DSRCT) is a rare disease from sarcoma group family. Typically, there is an extensive infiltration of abdominal cavity, peritoneal effusion with no evident primary origin of tumor. Prognosis is unfavourable with 5-years overall survival described in 15 % of cases even though multimodal treatment strategy is performed. We introduce case report of 34-years old patient with DSRCT treated in our Oncology center. Reasonable schema of differential diagnosis of soft-tissue expansion in abdominal cavity is mentioned and we emphasize crucial role of molecular genetic analysis. Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%).
