A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Kvalita ţivota je ovlivněna zdravotním stavem, který zahrnuje fyzické, psychické a sociální charakteristiky. Největším zdravotním, ale i společenským problémem současné epochy nejsou nemoci akutní, ale nemoci dlouhodobé, chronické a trvalé. Výzkum se zaměřuje na oblast sexuality u pacientů s hypertenzí, s důrazem na holistické pojetí lidské osobnosti, hlubší vnímání a proţívání sexuálního ţivota, sexuálních potřeb pacientů. Cíl: Cílem kvalitativního šetření bylo zjistit, zda léčba hypertenze ovlivňuje sexuální potřeby pacientů. Metodika: Výzkumný soubor tvořilo 20 respondentů, výběr zkoumaného vzorku byl záměrný, byli to pacienti, kteří se léčili pro hypertenzi minimálně 1 rok ve Fakultní nemocnici Ostrava. Výzkum byl proveden pomocí kvalitativní metody, sběr dat byl proveden rozhovorem, který byl nahráván na diktafon v příjemném, nerušeném prostředí. Ve výzkumu byla pouţita metoda barvení textu s definovanými 4 kategoriemi a podkategoriemi. Výsledky: Z analýzy rozhovorů vyplynulo, ţe u některých pacientů antihypertenzní terapie negativně ovlivnila jejich sexuální potřeby. Byl zjištěn výskyt sexuálních dysfunkcí, u ţen se jednalo o sníţený sexuální apetit, sníţenou lubrikační schopnost, delší interval k vyvrcholení a u muţe byl objeven problém s erekcí. Závěr: V rámci subjektivní spokojenosti s vlastní sexualitou u pacientů, kteří se léčí s arteriální hypertenzí je důleţitá znalost vedlejších účinků a následná edukace s vyuţitím výsledků vědeckých studií domácích i zahraničních odborníků., The quality of life is influenced by health condition, which include physical, psychological and social features. The biggest, not only medical but also social problem nowadays are not acute diseases but long-term diseases, chronical and permanent. The research is concentrated on the sphere of sexuality among patients with hypertension, with stress on holistic concept of human being, deeper perception and experiencing of sexual life and sexual needs of patients. Aim: The aim of qualitative research was to find out how the treatment of hypertension influences sexual needs of patients. Methods: Researched sample was formed by 20 respondents. The choice of researched sample was intentional. They were patients who were treated for hypertension minimally one year in University Hospital Ostrava. The choice of researched sample was intentional. The research was done by qualitative method, collection of datas was done by interview, which was recorded on voice recorder in a pleasant, by nothing disturbed place. In the research was used method of coloring the text with 4 defined categories and subcathegories. Results: From the analysis of interviews was clear that at some patients antihypertension therapy influenced their sexual needs in a negative way. There was found out the occurrence of sexual dysfunctions. Among women it was lower sexual appetite, lower lubricative ability, longer time for climax and among men was found the problem with erection. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. Activated hepatic stellate cells (HSC) are a major source offibrous proteins in cirrhotic liver. Inducing or accelerating their apoptosis is a potential way of liver fibrosis treatment. Extracellular matrix (ECM) surrounding cells in tissue affects their differentiation, migration, proliferation and function. Type I collagen is the main ECM component in fibrotic liver. We have examined how this protein modifies apoptosis of normal rat HSC induced by gliotoxin, cycloheximide and cytochalasin D in vitro and spontaneous apoptosis of HSC isolated from CCl4-damaged liver. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Cíl studie: Na základě soudobých studií podat přehled o hlavních biologických funkcích lipoproteinů o vysoké hustotě (HDL), zejména ve vztahu k reverznímu transportu cholesterolu a k jejich protizánětlivému působení. Popsat pravděpodobné mechanismy vzniku dysfunkčních prozánětlivých HDL a uvést některé parametry asociované s tímto procesem. Podat přehled o laboratorních metodách stanovení kvantitativních, strukturních a funkčních vlastností HDL, včetně moderních metod založených na hmotové spektrometrii. Typ studie: přehledový článek Závěr: V případě systémového zánětu a/nebo oxidačního stresu dochází ke vzniku dysfunkčních HDL akumulujících oxidanty. Probíhá strukturní modifikace apolipoproteinu AI, což vede mimo jiné k inhibici reverzního transportu cholesterolu a tvorbě prozánětlivých HDL. Na ztrátě příznivých vlastností HDL se podílí modifikace proteinové i lipidové složky HDL. Izolované měření HDL cholesterolu nemusí u některých syndromů bezvýhradně korelovat s mírou kardiovaskulárního rizika a v některých situacích je vhodné stanovit markery mapující strukturu a funkci HDL., Objective: On the basis of recent studies to make an overview concerning the crucial biological functions of high density lipoproteins (HDL), with emphasis on the role in reverse cholesterol transport and their antiinflammatory traits. The aim was to describe probable mechanisms of dysfunctional proinflammatory HDL formation with introduction some of associated parameters. To present laboratory methods for determination of quantitative, structural and functional qualities of HDL, including advanced mass spectrometry techniques. Study design: review Conclusion: In the case of systemic inflammation and/or oxidative stress, the formation of dysfunctional HDL accumulating oxidants takes place, including apo AI structural modifications. This process can lead to the inhibition of reverse cholesterol transport and proinflammatory HDL generation, among others. Loss of beneficial qualities results from modification of both lipid and protein components of HDL. Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Cíl. Ischemické cévní mozkové příhody (iCMP) jsou stále jednou z nejčastějších příčin mortality a morbidity postihující i mladší populaci v produktivním věku. Klíčovým faktorem ovlivňujícím prognózu po iCMP je včasná rekanalizace uzavřené mozkové tepny s obnovením perfuze v postižené mozkové tkáni. Cílem práce bylo zhodnocení bezpečnosti a efektivity kombinované revaskularizační terapie sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie u pacientů s akutní iCMP a uzávěrem mozkové tepny. Metoda. Do souboru pacientů byli zařazeni všichni po sobě jdoucí pacienti s akutní iCMP a prokázaným uzávěrem střední mozkové tepny v úseku M1-2 nebo distální části arteria carotis interna nebo uzávěrem arteria basilaris, a to v období od června 2010 do dubna 2012. Všichni nemocní byli léčeni kombinovanou revaskularizační terapií, tzn. iniciální intravenózní trombolýzou a přímo navazující mechanickou trombektomií systémem Solitaire. Iniciální stupeň neurologického deficitu byl stanoven pomocí NIHSS skóre. Výsledný klinický stav byl zhodnocen po 90 dnech pomocí modifikované Rankinovy škály (mRS). Výsledky. Do souboru bylo podle kritérií zařazeno 50 pacientů (průměrný věk 66, 8 ? 14,6 roku) se vstupním mediánem NIHSS skóre 18,0 (8-32). Rekanalizace (TICI 2A, B a 3) bylo dosaženo u 94 % pacientů, kompletní rekanalizace (TICI 3) u 72% pacientů. Průměrný rekanalizační čas (čas od začátku symptomů do dosažení maximální rekanalizace) byl v našem souboru 244,2 ? 87,9 minut. Symptomatické intracerebrální krvácení se vyskytlo u 6 % pacientů. Medián mRS po 90 dnech byl v našem souboru 1, 60 % pacientů mělo devadesátidenní klinický výsledek dle mRS hodnocen jako dobrý (mRS 0-2). Tříměsíční mortalita byla ve sledovaném souboru 14 %. Závěr. Kombinovaná revaskularizační terapie akutní iCMP sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie je bezpečná a efektivní. Okamžité provedení mechanické tromektomie po iniciální intravenózní trombolýze vede k výraznému zkrácení rekanalizačního času, který je rozhodující pro dobrý klinický výsledek u pacientů intervenovaných pro akutní ischemickou cévní mozkovou příhodu., Aim. Ischemic stroke is today one of the leading causes of mortality and morbidity and affects even a younger population of working age. A key factor influencing prognosis after ischemic stroke is early recanalization of close cerebral artery and reperfusion in the affected brain tissue. The aim of our study is to evaluate the safety and effectiveness of re-vascularization combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy in the treatment of patients with acute ischemic stroke. Methods. Consecutive non-selected acute ischemic stroke patients with baseline CTA or MRA documented occlusion of middle cerebral artery (MCA) (segment M1-2) or distal part of internal carotid artery (ICA) or basilar artery (BA) and treated between June 2010 and April 2012 at our stroke center were included in the study. All patiens were treated by combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy by Solitaire stent. Stroke severity was assessed using National Institutes of Health Stroke Scale (NIHSS), 90-day clinical outcome using modified Rankin Scale (mRS). Results. Fifty enrolled patients (mean age 66.8 ? 14.6 years) had baseline median NIHSS 18.0 (8-32). Overall recanalization (TICI 2A, B and 3) was achieved in 94% of patients, complete (TICI 3) in 72% of patients. The mean time from stroke onset-to-maximal recanalization was 244.2 ? 87.9 min. Symptomatic intracerebral hemorrhage occurred in 6% of patients. The median mRS score at 90 days was 1 and 60% patients had good outcome (mRS 0-2). Overall 3-month mortality was 14%. Conclusion. Combined revascularization therapy of acute ischemic stroke consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy is safe and effective. Cíl studie: Klinicky afunkční adenomy (KAA) představují zhruba 25–30 % všech tumorů hypofýzy. Neurochirurgická operace nádoru bývá často neúspěšná a zůstává po ní nádorové reziduum. Účinek následné biologické léčby je závislý na přítomnosti transmembránových receptorů v nádorových buňkách. Naším cílem bylo určit expresní profil somatostatinových receptorů (SSTR1–5) a estrogenového receptoru 1 (ER1) u 69 vzorků KAA. Materiál a metody: Vzorky adenomů se uchovávaly v RNAlater Tissue Protect až do doby zpracování v laboratoři. RNA jsme izolovali Trizolem a přepsali do cDNA pomocí SuperScriptu III. Exprese jednotlivých receptorů byla stanovena metodou kvantitativní real time PCR. Získané výsledky jsme přepočítali na housekeepingový gen pro beta-glukuronidázu. Výsledky: U vyšetřovaného souboru 69 afunkčních adenomů byly kromě SSTR4 a SSTR5 exprimovány všechny zbývající vyšetřované receptory. Mediány relativních hodnot byly: 63,8 % pro SSTR1, 55,4 % pro SSTR2, 19,6 % pro SSTR3, 2,6 % pro SSTR4, 9,2 % pro SSTR5 a 75,4 % pro ER1. Závěr: Variabilní exprese jednotlivých podtypů SSTR a ER1 může ovlivnit účinnost biologické léčby KAA. Proto by určení expresního profilu sledovaných receptorů mělo předcházet zahájení biologické léčby KAA somatostatinovými analogy či modulátory estrogenových receptorů., Objective: Non-functioning pituitary adenomas (NFA) comprise about 25–30% of all pituitary tumors. Transsphenoidal neurosurgery, the treatment of choice, is often unsuccessful and frequently leaves tumor remnants. The effect of subsequent biological treatment is mediated by transmembrane receptors in the adenoma cells. Our aim was to determine the somatostatin (SSTR1–5) and estrogen receptor 1 (ER1) expression profile in 69 specimens of NFA. Material and Methods: Adenoma samples were submerged in RNAlater Tissue Protect and transported frozen to the laboratory. The RNA was isolated by Trizol Reagent and transcribed to cDNA by SuperScript III. The expression of the receptors was determined by quantitative real time PCR. Results were normalized to the beta-glucuronidase housekeeping gene. Results: With exceptions of SSTR4 and SSTR5, all other subtypes of receptors were expressed in the examined tumor specimens. The median relative quantification values were: 63.8% for SSTR1, 55.4% for SSTR2, 19.6% for SSTR3, 2.6% for SSTR4, 9.2% for SSTR5, and 75.4% for ER1. Conclusion: Variable expression of somatostatin and estrogen receptors could influence the effectiveness of the biological treatment.
