In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. Hepatoblastom je nejčastější primární nádor jater dětského věku, u dospělých jedinců jde o mimořádně vzácné onemocnění se špatnou prognózou. Hepatoblastom u dospělých je v literatuře popisován pouze formou kazuistických sdělení. Popisujeme případ 26letého muže s inoperabilním hepatoblastomem, který byl na našem pracovišti léčen chemoterapií, chemoembolizacemi a dalšími metodami intervenční radiologie 11 let., Hepatoblastoma is the most common primary hepatic malignant tumor in children. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Histiocytóza z Langerhansových buněk je vzácné onemocnění, vznikající proliferací Langerhansových dendritických buněk, jehož etiologie je nejasná. Vyskytuje se převážně v dětském věku. Vysoké procento pacientů má projevy nemoci v oblasti hlavy a krku. Nejčastěji postiženým orgánem je lebka, zejména temporální kost. V organizmu se může nacházet jedno nebo více oseálních i extraoseálních ložisek, nebo je postižen jeden nebo více orgánů. Klinické projevy imitují řadu jiných chorob, proto je diagnostika často obtížná a protrahovaná (1). Otorinolaryngolog se nejčastěji setkává se symptomy podobnými chronické externí otitidě, akutní mediotitidě, mastoiditidě, dermatitidě nebo onemocnění probíhá pod obrazem nádoru. V této práci přinášíme kazuistiku dospělého pacienta, který vyhledal ošetření pro otoreu nereagující na standardní léčbu., Langerhans´cells histiocytosis (LCH) is a rare disease, in which the proliferation of Langerhans´ cells is believed to be causative, affecting mainly children. The etiology still remains unclear. The head and neck region is frequently involved, the skull being the most often diseased site, especially the temporal bone. It may affect also another organs and systems with single or multiple osseal or extraosseal lesions. LCH can be challenging to diagnose with it´s varied presentations, which may resemble another common diseases. Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress. Hyperviskózní syndrom u pacientů s monoklonálními gamapatiemi vzniká jednak na podkladě patologicky zvýšené hladiny monoklonálního imunoglobulinu, jednak vlastnostmi monoklonálního imunoglobulinu. Vysoká hodnota paraproteinu způsobí nárůst viskozity séra, což vede k rozmanitým klinickým projevům, které označujeme jako hyperviskózní syndrom. Tento stav řadíme mezi akutní stavy v hematologii, neboť pacienta ohrožuje fatálními komplikacemi (např. cévní mozková příhoda, kardiální léze, respirační selhání, ...). Po jeho potvrzení by měla být co nejdříve provedena výměnná plazmaferéza, současně je nutné zahájení léčby základního onemocnění. U pacienta s diabetes mellitus 1. typu je výrazný, opakující se nesoulad mezi vyšší glykémii v plazmě nalačno a glykovaným hemoglobinem. Příčinou snížených hodnot glykovaného hemoglobinu je zkrácená životnost erytrocytů způsobená hemolýzou po podání léku dapsonu. Vhodnější metoda ke sledování dlouhodobé kompenzace diabetes mellitus je v tomto případě stanovení fruktosaminu, jelikož koncentraci fruktosaminu neovlivňuje hemolýza., There is significant recurrent discrepance between elevated fasting plasma glukose and glycohemoglobin in a patient with diabetes mellitus type 1. The fall in glycohemoglobin results is due to short life span of erythrocytes as a product of dapson-induced hemolysis. Víno pijeme nejméně 10 000 let nebo déle, v našem regionu již od doby keltské a germánské. Pokus používat víno k léčbě začíná v dávnověku, avšak teprve v současné době byl prokázán jeho příznivý účinek v prevenci infarktu myokardu, cévní mozkové příhody, diabetes mellitus a hypertenze. Pití velmi dobře působí také v pokročilém věku. Nejlepší účinky má každodenní pití při večeři, u mužů 20–40 g alkoholu, u žen polovina této dávky., We have been drinking wine at least for 10 000 years or longer, in our region already from Celtic and Germanic times. Favorable effects on prevention of myocardial infarction, stroke, diabetes and hypertension were demonstrated recently, following attempts to use alcohol for treatment in ancient times. Jako light chain escape fenomén (LCE) je označována změna biologického chování nádoru u pacientů s mnohočetným myelomem (MM). Při diagnóze jsou v séru přítomny kompletní monoklonální imunoglobulin (mIg) a monoklonální volné lehké řetězce (mFLC), při relapsu nebo progresi onemocnění dochází k prudkému vzestupu koncentrace pouze mFLC, obvykle při stabilní nebo klesající koncentraci mIg, společně s těžkým orgánovým postižením. Pro časnou diagnostiku LCE u nemocných s MM je doporučováno sériové vyšetřování FLC. LCE byl popsán v literatuře zatím pouze u pacientů po hematologické terapii. V tomto sdělení prezentujeme kazuistiku pacienta vstupně hodnoceného hematologem jako doutnající myelom, bez nutnosti hematologické terapie. Domníváme se, že vývoj nálezu odpovídal kritériím LCE., The light chain escape phenomenon is characterized as a change in tumor biological movement in patient with multiple myeloma. Complete monoclonal immunoglobulin molecules (mIg) together with monoclonal free light chains (mFLC) are present in diagnosis, only mFLC great increase is observed in relapse or disease progression together with heavy organ impairment whether mIg concentration is usually stable or decreasing. Serum FLC monitoring is recommended for early diagnostics LCE phenomenon in patients with MM. To this time LCE phenomenon was described only in patients after hematological therapy.
