63letý pacient, hypertonik, celoživotní kuřák, byl na kliniku TRN FN Plzeň přijat pro tumor pravé plíce zjištěný pro bolesti v pravém hemitoraxu. Na základě stážování byla stanovena diagnóza adenokarcinomu pravé plíce s generalizací do obou plic a levé nadledviny, st. IV. Byla zahájena chemoterapie Paclitaxel (200 mg/m2) – Carboplatina (AUC) – Avastin (15 mg/kg) v rámci studie SAIL firmy Roche s velmi dobrou tolerancí. Po 3. cyklu chemoterapie byla potvrzena parciální remise dle CT hrudníku, dále pokračováno ve stejném schématu chemoterapie s Avastinem do 6. cyklu. Kontrolní CT hrudníku prokázalo další regresi tumoru. Vzhledem k velmi dobrému efektu podávané léčby bylo dále pokračováno v monoterapii Avastinem v třítýdenních intervalech zatím do 22 cyklů. Po 22 měsících léčby s Avastinem bylo dosaženo trvající parciální remise nálezu dle PET/CT trupu, pacient je klinicky zcela bez potíží, neomezen v běžném životě, tedy s velmi dobrou kvalitou života (KS 90 bodů), bez vedlejších nežádoucích účinků Avastinu. Chemoterapie již nemá potenciál pro další prodlužování přežití, je tedy potřebné hledání nových léčebných strategií. Avastin jako inhibitor angiogeneze vede poprvé k prodloužení celkového přežití nad 12 měsíců. Významné prodloužení přežití bez progrese onemocnění (PFS) umožňuje delší přežití bez zhoršení symptomů., A 63-year-old hypertensive patient, a life-long smoker, presented to the Department of Tuberculosis and Respiratory Disease of the Plzeň Teaching Hospital with a right-lung tumour detected due to pain in the right hemithorax. Based on staging, a diagnosis of stage IV right lung adenocarcinoma with generalization into both lungs and the left adrenal gland was made. Chemotherapy with Paclitaxel (200 mg/m2) – Carboplatin (AUC) – Avastin (15 mg/kg) was initiated within the SAIL study of the Roche company with very good tolerance. Following the third chemotherapy cycle, partial remission was confirmed based on CT of the chest; the same chemotherapy regimen with Avastin was continued for six cycles. Follow-up CT of the chest revealed further tumour regression. Given the very good effect of the treatment administered, monotherapy with Avastin was continued in three-week intervals for 22 cycles so far. Following 22 months of Avastin treatment, sustained partial remission was achieved according to PET/CT of the trunk; the patient is clinically free of complaints, without limitations in everyday life, thus with a very good quality of life (KS of 90) and free of adverse effects of Avastin. Chemotherapy has no more potential for further extension of survival; therefore, new therapeutic strategies have to be looked for. As an angiogenesis inhibitor, Avastin leads to an extension of overall survival beyond 12 months. Optimální léčba lokalizovaného karcinomu prostaty není přesně definována. Je možné použít radikální prostatektomii, zevní radioterapii, brachyterapii nebo v některých případech taktiku aktivního sledování. Intersticiální brachyterapie je jedním z přístupů, který se v posledních letech velmi rychle rozvíjí vlivem rozvoje ultrazvukem navigované technologie, nových radioizotopů a dokonalejšími plánovacími systémy. Je vynikající metodou k dodání vysoké dávky do prostaty při velmi nízkém procentu vážných komplikací. Brachyterapie může být provedena technikou trvalého zavedení radioizotopů do prostaty (permanentní aplikace) nebo technikou vysokodávkové dočasné brachyterapie. Obě metody je možné a v některých případech i vhodné kombinovat se zevní radioterapií. Nejlepší technika brachyterapie však není známá a její výběr závisí značně i na zkušenostech pracoviště. Kromě léčebných výsledků je velmi důležité znát i možné nežádoucí účinky léčby s ohledem na kvalitu života pacientů., The optimal treatment for localized prostate cancer has not been clearly defined. It is possible to use radical prostatectomy, external radiotherapy, brachytherapy or, in some cases, the strategy of active surveillance. In recent years, interstitial brachytherapy has been advancing rapidly due to the development of ultrasound-guided techniques, new radioisotopes and more advanced planning systems. It is an excellent method for high-dose delivery within the prostate with a very low rate of serious complications. Brachytherapy can be performed by permanent implantation of radioisotopes into the prostate or by using high-dose rate temporary brachytherapy. Both methods can and, in some cases, should be combined with external radiotherapy. However, the best technique for brachytherapy has not been determined and the selection largely depends on the expertise of the staff. Autoři prezentují sestavu 76 nemocných radikálně operovaných pro karcinom hlavy pankreatu a analyzují pooperační komplikace a přežívání. Materiál a metodika: V období 1/2006–9/2010 podstoupilo 76 nemocných potenciálně kurativní hemipankreatoduodenektomii pro adenokarcinom hlavy pankreatu. Pacienti jsou prospektivně sledováni, diskutovány jsou komplikace a přežití. Výsledky: 30denní pooperační mortalita je 2,6 %. Na základě TNM klasifikace 11 % mělo stadium I, 78 % stadium II a 3 % stadium III. Nejzávažnější komplikací byla insuficience pankreatojejunální anastomózy (6,5 %) a opožděná evakuace žaludku (11,8 %). Ve stadiu I přežívá 1., 2., 3. a 4. rok 82 %, 52 %, 35 % a 35 % operovaných. Závěr: Resekce hlavy pankreatu je bezpečná operace s nízkou mortalitou, ale neuspokojivým dlouhodobým přežíváním. Toto je zřejmě způsobeno agresivní biologickou aktivitou nádoru a pozdní diagnostikou., The authors refer seventy six patients radically operated for pancreatic head carcinoma and analyzed postoperative complications and survival. Material and Methods: During 1/2006–9/2010 76 patients underwent potentially curative hemipancreato-duodenectomy for adenocarcinoma of the head of pancreas. The patients are prospectively followed, complications and survival are discussed. Results: The 30-days postoperative mortality was 2,6 %. According to TNM classification 11 % of patients had stage I, 78 % stage II and 3 % stage III. Most severe complications were leak from pancreato-jejunal anastomosis (6,5 %) and delayed gastric emptying (11.8 %). The survival analyses showed 1,2,3 and 4 years survival as follows: 82 %, 52 %, 35 %, 35 % respectively. Conclusion: Resection of the pancreatic head is a safe procedure with low mortality but disappointing long-term survival. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. V letech 1959–2006 bylo u českých mužů evidováno 81 884 nových karcinomů prostaty (KP) s nárůstem z 12,3 na 96,1 na 100 tis. mužů. Úmrtnost se zvýšila z 21,3 na 27,2 na 100 tis. mužů. Podíl KP ze všech nádorů se zvýšil u incidence ze 7,1 % na 13,2 %, u mortality z 6,7 % na 8,9 %. V letech 1989–2005 vzrostla prevalence KP z 98 na 408,5 na 100 tisíc mužů a dosáhla téměř 20 500 žijících. Z 11 183 nemocných s vícečetnými KP bylo evidováno 4 608 primárních a 6 575 následných případů, představujících 17,4 % ze všech evidovaných KP v letech 1976–2005. Z primárních KP bylo léčeno 89,7 % duplicit a 10,3 % multiplicit. Počet primárních KP vzrostl na 282 v roce 1996 a pak klesal, následných KP trvale rostl na 705 v roce 2005. Téměř polovina KP se vyskytla ve věku 70–79 let a třetina ve věku 50–69 let. Poměr 1 115 (21,3 %) synchronních a 4 121 (78,7 %) metachronních následných novotvarů byl 1:3,7 s průměrnou dobou vzniku následného novotvaru 4,1 roku. Z 5 236 následných novotvarů po primárních KP byly nepočetnější nádory kůže 28,6 %, trávicí 26,7 %, močové 17,8 % a respirační 10,9 % soustavy. Z 5 613 novotvarů před KP jako druhou diagnózou, bylo 35,3 % nádorů kůže, 21 % trávicí, 19,6 % močové a 8,7 % respirační soustavy. Z nemocných s primárním KP k 17. 10. 2007 žilo 22,4 % a zemřelo 77,6 %, s následným KP žilo 31,6 % a zemřelo 68,4 % mužů. Vzhledem k výskytu KP ve spojení s dalšími novotvary není u české populace zatím dostatečná intervence ke zdravému životnímu stylu také vzhledem k očekávanému počtu 42 tisíc mužů, žijících s KP v roce 2015., In Czech men there were registered 81,884 new prostate cancers (PC) during 1959–2006, increased from 12.3 to 96.1 per 100,000 men. The mortality increased from 21.3 to 27.2 per 100,000 men. The share of PC of all cancers increased from 7,1 % to 13.2 % in incidence and from 6.7 % to 8.9 % in mortality. The most frequent from 5,613 neoplasms before PC as the second diagnosis there were 35,3 % cancers of skin, 21 % gastrointestinal, 19,6 % urinary and 8,7 % respiratory tract. From patients with primary PC survived 22,4 % and died 77,6 %, from subsequent PC survived 31,6 % and died 68,4 % men up to 17. 10. 2007. Desmoplastický kulatobuněčný tumor (DSRCT) je raritní onemocnění patřící do skupiny sarkomů. V typickém případě se manifestuje rozsáhlou tumorózní infiltrací břišní dutiny bez evidentního primárního ložiska tumoru. Prognóza onemocnění je stále špatná, 5leté přežití je popisováno asi v 15 % případů i při užití kombinovaných protinádorových léčebných postupů. Uvádíme kazuistiku 34letého pacienta s DSRCT léčeného v našem centru. Zmiňujeme racionální postup v rámci diferenciální diagnostiky měkkotkáňové infiltrace v dutině břišní, podtrhujeme zásadní význam molekulárně genetické analýzy a zaměřujeme se na terapeutické možnosti tohoto zřídka se vyskytujícího onemocnění., Desmoplastic small round cell tumor (DSRCT) is a rare disease from sarcoma group family. Typically, there is an extensive infiltration of abdominal cavity, peritoneal effusion with no evident primary origin of tumor. Prognosis is unfavourable with 5-years overall survival described in 15 % of cases even though multimodal treatment strategy is performed. We introduce case report of 34-years old patient with DSRCT treated in our Oncology center. Reasonable schema of differential diagnosis of soft-tissue expansion in abdominal cavity is mentioned and we emphasize crucial role of molecular genetic analysis. Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%). Ve sdělení autoři prezentují kazuistiku duplicity primárních mozkových nádorů. Její výskyt je velmi vzácný, udává se méně než 1:1 000 000. U 69letého pacienta byl diagnostikován frontotemporálně vpravo lokalizovaný multiformní glioblastom způsobující klinickou symptomatologii a dále klinicky němý drobný meningeom v oblasti levého mostomozečkového koutu. Multiformní glioblastom byl subtotálně resekován z kraniotomie a drobný asymptomatický meningeom byl ponechán k observaci., The authors present a case report of duplicity of primary brain tumours. It occurs very rarely with an incidence reported to be 1/1 000 000. A 69-year-old patient was diagnosed with a glioblastoma multiforme localized frontotemporally on the right which caused clinical symptomatology and with a clinically silent, small meningioma in the left pontocerebellar angle. Histiocytóza z Langerhansových buněk je vzácné onemocnění, vznikající proliferací Langerhansových dendritických buněk, jehož etiologie je nejasná. Vyskytuje se převážně v dětském věku. Vysoké procento pacientů má projevy nemoci v oblasti hlavy a krku. Nejčastěji postiženým orgánem je lebka, zejména temporální kost. V organizmu se může nacházet jedno nebo více oseálních i extraoseálních ložisek, nebo je postižen jeden nebo více orgánů. Klinické projevy imitují řadu jiných chorob, proto je diagnostika často obtížná a protrahovaná (1). Otorinolaryngolog se nejčastěji setkává se symptomy podobnými chronické externí otitidě, akutní mediotitidě, mastoiditidě, dermatitidě nebo onemocnění probíhá pod obrazem nádoru. V této práci přinášíme kazuistiku dospělého pacienta, který vyhledal ošetření pro otoreu nereagující na standardní léčbu., Langerhans´cells histiocytosis (LCH) is a rare disease, in which the proliferation of Langerhans´ cells is believed to be causative, affecting mainly children. The etiology still remains unclear. The head and neck region is frequently involved, the skull being the most often diseased site, especially the temporal bone. It may affect also another organs and systems with single or multiple osseal or extraosseal lesions. LCH can be challenging to diagnose with it´s varied presentations, which may resemble another common diseases.
