Podstatou jeho mechanizmu účinku je výrazné snížení kardiální a renální sympatické baroreflexní odpovědi na úrovni I1-imidazolinových receptorů s následným snížením aktivity sympatiku. Účinnost a snášenlivost rilmenidinu byly prokázány v randomizovaných srovnávacích studiích u starších pacientů, hypertoniků s diabetem a metabolickým syndromem i u pacientů s renálním selháním. Indikací pro podávání rilmenidinu je léčba esenciální hypertenze u dospělých pacientů všech věkových kategorií.
