Stenóza cervikálního kanálu může vést ke kompresi krční míchy a představuje nejvýznamnější mechanický faktor v patofyziologii cervikální spondylogenní myelopatie. Míšní tkáň je však vůči kompresi poměrně odolná a k rozvoji manifestní myelopatie dochází až při vyšším stupni komprese a v kombinaci s dalšími patofyziologickými faktory, zejm. dynamickým faktorem a zevním traumatem. Asymptomatická spondylogenní komprese krční míchy (ASCCC) je velmi častým nálezem zvláště v populaci vyššího věku nad 50 let. Objektivizace a kvantifikace cervikální stenózy a komprese krční míchy jsou však nejednotné a nejsou jasně stanoveny prediktory progrese do manifestní myelopatie a s tím související indikace případné preventivní chirurgické dekomprese. Přehled obsahuje nejčastěji používané metody stanovení cervikální stenózy a cervikální míšní komprese pomocí zobrazovacích metod. Nativní radiogram a výpočetní tomografie mají význam v objektivizaci cervikální stenózy, zatímco dominantní metodou objektivizace míšní komprese je magnetická rezonance (MR). Z parametrů, které nejvíce korelují s klinickou manifestací komprese, jsou uváděny příčná plocha míchy v místě komprese a přítomnost intenzitních změn míšního signálu. Z novějších zobrazovacích modalit je nejcitlivější MR zobrazení tenzorů difúze. Z prognostických faktorů rozvoje manifestní myelopatie u ASCCC jsou obecně akceptovány přítomnost manifestní radikulopatie a abnormita motorických a senzomotorických evokovaných potenciálů. Význam zobrazovacích metod v predikci rozvoje manifestní myelopatie a benefit provedení preventivní dekomprese ve skupině nemocných s asymptomatickou kompresí a vyšším rizikem rozvoje manifestní myelopatie bude nutné ověřit dalšími studiemi., Cervical spinal stenosis may lead to cervical cord compression and represents the most important mechanical factor in pathophysiology of cervical spondylotic myelopathy. Medullar tissue is, however, rather resistant to compression and development of symptomatic myelopathy occurs only when higher degree stenosis is present and in combination with other pathophysiological factors, mainly dynamic compression and trauma. Asymptomatic spondylotic cervical cord compression (ASCCC) is a quite frequent finding in older population above the age of fifty. However, reliability of the methods used to verify and quantify cervical stenosis and cervical cord compression is low and clear predictors of development of symptomatic myelopathy and related indication of potential preventive surgical decompression in ASCCC have not been determined yet. The overview discusses the most frequently used methods to establish cervical spinal stenosis and cervical cord compression using imaging methods. Radiogram and especially computed tomography are important for verification of cervical spinal stenosis, while magnetic resonance imaging (MRI) is a preferable method to detect cervical cord compression. The cross‑sectional spinal cord area and T2 MRI spinal cord hyperintensity are among the parameters considered to be the most closely correlated with clinical manifestation of spinal cord compression. Among newly introduced imaging modalities, MRI diffusion tensor imaging seems to be the most promising one. The presence of symptomatic radiculopathy and abnormality of motor and somatosensory evoked potentials are among generally accepted predictors of symptomatic myelopathy. The importance of imaging methods as predictors of symptomatic myelopathy development as well as benefits of preventive surgical decompression in ASCCC individuals with high risk of developing symptomatic myelopathy is to be established in future studies. Dysfunkce autonomního nervového systému patří k častým klinickým projevům roztroušené sklerózy (RS). Incidence autonomní dysfunkce (AD) u pacientů s RS se v různých studiích pohybuje mezi 16 a 80 % a jednoznačně se zvyšuje s délkou trvání RS a progredující disabilitou. Může se však projevit v kterékoli fázi demyelinizačního onemocnění a dokonce může být jeho prvním klinickým projevem. AD významně ovlivňuje kvalitu života pacientů s RS a přispívá k celkové disabilitě. Přesto postižení autonomního nervového systému patří mezi stále relativně poddiagnostikované symptomy tohoto onemocnění. Nejčastějším a nejzávažnějším typem AD u pacientů s RS je narušení funkce kardiovaskulárního a/nebo urogenitálního systému. Méně častá je dysfunkce gastrointestinální, sudomotorická či narušení pupilomotoriky. Autonomní (především kardiovaskulární) dysfunkce u pacientů s RS také úzce souvisí s rozvojem únavy. Cílem tohoto sdělení je poskytnout přehled jednotlivých typů AD u pacientů s RS a možností jejich diagnostiky., Autonomic nervous system dysfunction (AD) represents a frequent clinical presentation of multiple sclerosis (MS). According to published studies, the incidence of AD in MS patients ranges between 16 and 80% and gradually increases with the length of the demyelinating disease and with the progression of disability. However, AD can occur in any phase of MS and can even represent its first symptom in some patients. Autonomic dysfunction has a significant negative impact on the quality of life in MS patients and contributes to overall disability. Even so, autonomic nervous system dysfunction is frequently underdiagnosed in patients with multiple sclerosis. Cardiovascular and urogenital dysfunction are the most frequent types of AD in MS patients with the highest impact on their clinical status and the quality of life. Less frequently, gastrointestinal, sudomotor or pupilomotor dysfunction can be found. Autonomic (in particular cardiovascular) dysfunction in multiple sclerosis is closely related to fatigue, another frequent clinical symptom in MS patients. The aim of this paper is to sum up the spectrum of symptoms of autonomic dysfunction in multiple sclerosis and its diagnostic tools. Lumbální spinální stenóza je klinicko‑radiologický syndrom, nejnovější definice zohledňují klinickou manifestaci i anatomické změny (zúžení páteřního kanálu). Za nejvhodnější neinvazivní vyšetření k potvrzení anatomického zúžení páteřního kanálu nebo přítomnosti komprese nervových kořenů u pacientů s klinickým podezřením na lumbální spinální stenózu je považována magnetická rezonance bederní páteře. K přesnějšímu hodnocení radiologického nálezu byla definována řada kvantitativních parametrů popisujících rozměry páteřního kanálu, nicméně korelace těchto parametrů s klinickým obrazem není uspokojivá. Perspektivním postupem ke zvýšení výtěžnosti radiologického vyšetření se jeví zohlednění dynamického aspektu lumbální spinální stenózy a důraz na tíži komprese nervových a cévních struktur v páteřním kanálu s využitím morfologického popisu durálního vaku a jeho obsahu., Lumbar spinal stenosis is a clinical-radiological syndrome; a recent definition covers both the clinical manifestation and the anatomic abnormality (narrowing of the spinal canal). Magnetic resonance imaging is suggested as the most appropriate non-invasive test to confirm the presence of anatomic narrowing of the spinal canal or the presence of nerve root impingement in patients with clinical suspicion of lumbar spinal stenosis. Many numerical parameters describing the dimensions of spinal canal have been defined for more precise assessment of radiological images; correlation between these radiological parameters and clinical symptoms, however, is unsatisfactory. To improve the usefulness of radiological imaging in patients with lumbar spinal stenosis, it seems to be meaningful to take into account the dynamic aspect of lumbar spinal stenosis and to assess neural and vascular tissue impingement based on morphology of the dural sac and its content. Roztroušená skleróza je časté neurologické autoimunitní onemocnění, které postihuje nejvíce pacienty ve věku od 20 do 40 let. V tomto období se dostává do popředí otázka plánování těhotenství. U pacientů s roztroušenou sklerózou je popisován určitý trend ke snížené plodnosti. Její příčinou jsou jednak sexuální dysfunkce, které patří mezi relativně časté symptomy tohoto onemocnění, jednak předpokládaný vliv dalších faktorů, např. hormonální dysbalance, přidružených onemocnění a specifické terapie roztroušené sklerózy. Důležitou roli hrají také psychosociální vlivy ovlivňující plodnost i následnou terapii. Cílem tohoto sdělení je popis faktorů, které se mohou podílet na ovlivnění ženské i mužské plodnosti u pacientů s roztroušenou sklerózou, a možnosti terapie neplodnosti metodami asistované reprodukce a jejich vliv na následný průběh roztroušené sklerózy. During these decades patients are usually planning pregnancy. In patients with multiple sclerosis fertility seems to be slightly impaired. Several factors might contribute to this condition. It is mainly sexual dysfunctions, which belong to comparatively common symptoms of the disease, hormonal alteration, concomitant (autoimmune) disease and specific therapy of multiple sclerosis. Psychosocial factors have an influence on fertility and its therapy. The aim of this review is description of factors potentially interfering with fertility in multiple, the current applications of assisted reproductive techniques and its influence on course of multiple sclerosis.
